Utah CDL Requirements

All CDL holders have to certify driving type, and if required you must keep a valid medical card on file with Utah DMV. Your self-certification of driving type tells DMV which qualifications you must meet to hold a CDL.

Non-Excepted Interstate
You must meet the Federal DOT medical certificate requirements and have a Medical Examiner’s Certificate.

Non-Excepted Intrastate (Utah only, requires K Restriction)
You must meet the Utah medical requirements. You must have a Utah Intrastate Waiver card and a Medical Examiner’s Certificate.

Intrastate “K” Restriction
The Utah Medical Waiver Program considers medical conditions in specific categories. For more information see Intrastate Waiver Program. The intrastate only (K) restriction is sufficient to indicate the driver has met the State of Utah medical guidelines for the commercial license.  To inquire about more information about the Intrastate Waiver Program, call at 801-965-4437.

 

Submit your CDL Self-Certification and Medical Certificate to Utah DLD

Submit the following two documents:

  1. Commercial Driver License Medical Self Certification – download the form (CDL 42) here from Utah Department of Public Safety.
  2. Copy of your valid DOT Medical Examiner’s Certificate (MCSA-5876) and, if applicable, your Utah Intrastate Waiver card.  Enlarge the copy to ensure that all information is legible.

Medical Certification Update

You are responsible for knowing when your Medical Examiner’s Certificate will expire and for providing Utah Drivers License Division with a copy of your new Medical Examiner’s Certificate.

If your DOT medical card expires, and you do not submit a renewed medical certificate your driver license will be downgraded.

If the status of your driving condition changes since you submitted a previous CDL Self-Certification, then you must also submit a new Self-Certification form.

You may submit a copy of your new Medical Examiner’s Certificate (MCSA-5876) together with a Commercial Driver License Medical Self Certification form (CDL 42) to Utah Drivers License Department by mail, fax, email, or in person as described above.

Special Information Updates Pertaining To ALL STATES

MEDICAL EXAMINERS MUST CONTINUE TO ISSUE MEDICAL CERTIFICATES TO ALL DRIVERS

The FMCSA has requested that all medical examiners continue providing drivers with a paper copy of the Medical Examiner’s Certificate – Form MCSA-5876, so that Commercial Driver’s License (CDL) holders can provide a copy to the State licensing agency, and the non-CDL drivers can provide the documentation to their employers and Federal and State enforcement officials.

The Agency is currently completing efforts to put into place an electronic system to enable medical examiners to transmit the medical certificate information from the National Registry system to the State licensing agencies.
Until that system is completed, which compliance date according FMCSA is June 23, 2025, medical examiners must continue to issue paper copies of the medical certificates to drivers who pass the medical exam.

ALL DRIVERS MUST CONTINUE TO CARRY A PAPER COPY OF THEIR MEDICAL CERTIFICATE

Commercial Drivers – You still need to carry a paper copy of your medical card, until FMCSA announces that this is no longer necessary.

    Why is my CDL card due at a different time than my driver license in my birthday? It would be a lot easier to remember it is due on my birthday.

    This needs to be changed on the website for Utah. CDL holders still have to send in renewed MECs but only have to send in the self-certification form once unless there is a change to the type of certification or a change to the license:

    Medical Certification Update
    You are responsible for knowing when your Medical Examiner’s Certificate will expire and for providing Utah Drivers License Division with a copy of your new Medical Examiner’s Certificate. Every time you update a DOT medical certificate you must also turn in the self-certification form.
    If your DOT medical card expires, and you do not submit a renewed medical certificate your driver license will be downgraded.

      Highly unlikely. May depend on class cdl you hold. Class A, no. Lesser class cdl is under the states requirements.

    If I’m going for a physical appointment for a class D driver license. Will they suspend my license for having Thc in my system even though im 30 days clean but still show as positive for thc

      There is no drug testing in the DOT physical. Only if the employer request an additional drug screen could this be an issue. 30 days clean and you should be clear.

    What if you fell on hardship and barely getting back on your feet the DMV told me I have time the 28th of February 22 to renew it I got a job but want to keep my CDL will Utah extend the grace period

      You have until the end of February to get your card into the federal registry. States are different and may give a small grace period. Check with your state for clarity.

    I’m not currently driving but have kept my class A just in case. My DOT card expired without me realizing, is there a grace period in Utah? If so how long do I have? I can’t find that answer anywhere. Thank you.

      Depending on how long your medical card has been expired, you may just need to get a new card and get it registered with the state. Do it now.
