Medical Condition – Medical Release Opinion

One of the complications of getting your medical certificate is if you have a medical condition that may pose a risk to safety while operating a commercial motor vehicle.

How does this play into the FMCSA DOT physical exam?

The medical examiner has a responsibility to ensure that a commercial driver does not have any medical condition which could impact that driver’s ability to meet all the physical and mental demands of the job, now or for the period for which the medical card will be issued.

Here’s what the DOT doctor is checking for:

  • Have you had a medical condition which poses a risk to safety?
  • Do you have symptoms that may indicate an undiagnosed condition?
  • Do you currently have a diagnosed condition?
  • Is the condition being treated?
  • Is the treatment effective, and safe, for you to drive a CMV?
  • Is the medical condition stable, and safe, for you to drive a CMV?

If a medical condition is a concern, the medical examiner must evaluate whether the condition is a risk for incapacitation:

  • Is the onset of incapacitation symptoms so rapid as to interfere with safe driving?
  • Is the onset so gradual that you may be unaware of diminished capabilities?

Medical Release Opinion

To ensure that the medical examiner can complete the DOT physical exam at your appointment you should bring a Medical Release Opinion letter from your treating physician. You can download instructions and a letter template here, or refer your treating physician to this page for the necessary information.

If you do not have the necessary documentation with you, for the DOT physical exam, the medical examiner may have to temporarily disqualify your medical certificate. You will need this documentation before you can be reconsidered for qualification for a medical certificate.

Medical Conditions

These are the medical conditions listed on the FMCSA long form for which you should have supporting documentation for the medical examiner:

Illness or injury within the last 5 years • Head/Brain injuries, disorders or illnesses • Seizures, epilepsy • Eye disorders or impaired vision (except corrective lenses) • Ear disorders, loss of hearing or balance • Heart disease or heart attack; other cardiovascular condition • Heart surgery (valve replacement/bypass, angioplasty, pacemaker • High blood pressure • Muscular disease • Shortness of breath • Lung disease, emphysema, asthma, chronic bronchitis • Kidney disease, dialysis • Liver disease • Digestive problems • Diabetes or elevated blood sugar controlled by diet or pills or insulin • Nervous or psychiatric disorders e.g. severe depression • Loss of, or altered consciousness • Fainting, dizziness • Sleep disorders, pauses in breathing while asleep, daytime sleepiness, loud snoring • Stroke or paralysis • Missing or impaired hand, arm, foot, leg, finger, toe • Spinal injury or disease • Chronic low back pain • Regular, frequent alcohol use • Narcotic or habit forming drug use

Role Of A Commercial Driver

It’s important that your primary care physician, who is treating you for the medical condition, also understands the physical, mental, and emotional demands of your job, as well as the responsibilities, work schedule and job stresses you have to deal with.

These are extraordinary factors that a family practice physician or specialist does not encounter when dealing with their general patients. These factors must be considered in the doctor’s Medical Opinion Release letter. Qualification For Your Medical Certificate

The DOT doctor, aka certified medical examiner, has been certified for evaluating the special circumstances of a commercial driver.

The Medical Opinion Release letter from your primary care physician helps the DOT doctor to evaluate your medical condition in view of the safety risk in driving a commercial vehicle. However, the decision to qualify or disqualify you for a medical certificate rests with the DOT doctor, not your primary care physician.

    I had a brain injury back in 2018 and was given a medical card at the end of that year. I went to renew my card and was told that I failed. Can I get one from the same place that gave it to me.

      Yes, you can always request a second opinion. The injury will need supporting documentation and a medical release from your doctors, but you should be OK. The CME needs the medical documentation and releases for risk factors and decision making process’s.

    I have diabetic retinopathy. I have filled out all necessary forms for insulin dependent diabetic and supplied other forms required from my retina specialist. The examiner informed now that due to the retinopathy I can’t get a medical card. That the condition disqualifies me. I tried looking for it on the internet like she told me but can’t find it. Can you tell me if this is true or where I can find this information? Plus is there anything I can do to get an exemption. I’ve been driving at my present job now for 6 months but in jeopardy of losing my job. Thanks for your help.

      Of course your condition is raising questions. According to the DOT/FMCSA guidelines:
      • Vision Annual Evaluation Checklist.
      o Exemption requires evaluation by an ophthalmologist or optometrist.
      o The driver diagnosed with diabetic retinopathy is required to have an eye examination by
      an ophthalmologist.
      It requires the CME to read and understand the guidelines and do further evaluation and request additional information from your physicians, if needed.
      Can be re-certified once a year, if all other areas are within normal limits.

    You, DOT Physical Doctors all assume that all CDL drivers drive Class A Tractor trailers and that we all drive 12 to 14 hour days. Well That’s not true and I feel that I’m being scrutinized and discriminated against for Holding a Class B and only driving sometimes 4 to 6 hours a day. I ‘am a CPAP user and have used one for over 10 years now and it has done wonders for me But, every year and not every 2 years because of WI. Law, I have to take a DOT Physical because you think that I’ll be falling a sleep behind the wheel. You assume, you have no proof of this and yet I’m still being scrutinized. If all you DOT Doctors feel this way then Why not test all drivers for sleep APENA. People that drive Cars, Trucks, Motorcycles, etc.. God knows that I have seen people on the roads that could probably use it the way they drive. Maybe yourself. I agree, we all want safer roads. But why just punish just the few that don’t drive Class A’s. I’m tired of this political B.S.

      Well Mike we will say that there are those of us who understand the differences between DOT/FMCSA class A guidelines and state regulations and variances. Unfortunately too many individuals saw an opportunity to become CME’s and still don’t understand the differences. Best bet when you go in for your next medical card examination, explain to the examiner that you are a class B driver and that you fall under the states variances, not FMCSA. If the examiner does not understand the difference, get up and leave.

        Thank You for your output on my concern regarding this situation. Wouldn’t it be nice if they had a form asking you on what kind of class that your applying for to avoid this. Also the clinic that I go to requires payment upfront before any testing is preformed, thus just getting up to leave because he doesn’t recognize the differences between state and federal guidelines would be costly of over $100.00. Suppose I could ask if the examiner understands the difference between the two before the exam. Thanks for your output.

    My husband had a stroke in 2018. We live in Ohio. He needs a dot physical to drive for a company he is to start with in April. He went to get a dot physical a few days ago and they told him that he needs to take the call test both written and driving over again since his stroke. He has waited the one year and he has all the letters from neuro and cardiology and even pulmonologist and pcp saying he is good to drive, etc. he keeps getting the run around and they have told him twice when they slap this big boom in front of him that he is not meeting the guidelines to get his dot physical done..??? But yet when we ask for specific and where and who to talk to no one can seem to help ?!

      If the state is saying he needs to redo the cdl testing, written and driving, then his issue is with the state. Go up the supervisors chain within the state drivers license department for further assistance. It sound like he can provided the medical review officer with the proper documentation, so do not see where the holdup would be. A CME can perform the DOT/FMCSA medical certification examination without a cdl license.

        All this BS to keep a CDL I been out of work for over year because my heart doctor will not sign one paper till I pay Thousands of dollars for test been a driver for 43 years Bo not no what to do

          My husband’s cardiologist signed a cardiac release exemption and concentra is rejecting it and disqualifying him regardless. Am I correct that he is entitled to an exemption due to cardiologist making the exemption?

            Get a second opinion from another CME. Not sure what class drivers license or exemption you may be dealing with. Unless your cardiologist is FMCSA certified, then his exemption must be cleared by another DOT/FMCSA certified medical examiner.

    Had a mini stroke in aug of 2019 then again oct 14 2019 i was cleared by my doctor to return back to work my job wants me to renew my dot physical the mro says i have wait a year ive read up on it says recommend but not required what can i do to really find out

      Two strokes in 2 months, not good. Nearly all CME’s are going to hold to the one year wait following the last stroke. The risk is far too high for the examiners to try to justify a new medical card. If you drive intra-state only, you may fall under the states variances, check with your state drivers license department for their guidelines and regulations on this issue.

    I have been a truck driver 2 years before the start of the cdl program i have been the same size for the last 15 years last week went in to the same Dr. i went to last year he knew my size and medical history told me last year bring paper from My DR. did what he said i went in he took my $100 he ran me through the exam then he hit me neck size and over weight then here it comes the sleep Apnea study i cant afford to go through the study and dont want to be a gov lab rat i think we are being scam looks like my career is over

      Go elsewhere and get a second opinion. Neck size and bmi are not part of DOT/FMCSA requirements, they may be a small indicator of issues, but not requirements.

    I have been in recovery for 2 years in a methadone maintenance program and am currently going through a slow taper to detox out. I just got a job offer where I have to take a dot physical. I’m not required to get a cdl as I will just be driving a cmv basically like a uhaul. Does methadone disqualify me?

      Check with your local drivers license department before going for the medical card. Most CME’s are doing examinations for class A drivers and you would not pass the requirements. But, your state variances may allow you to get a medical card. Find an examiner that understands the difference between class A, federal, and state variances.

    I have a class A cdl but I drive a pickup truck for work everyday, is there a form so I don’t have to take physical for medical card? Or do I need to just get class D license?

      Most states require a valid medical card to maintain a class A license. There is no form, unless it is from the state drivers license department. Check with your state first.

    I am taking Lamictal and Amitriptyln. I needed a Letter of Clearance from my doctor for the medical examiner for the company. My doctor gave clearnance to drive. I submitted to my doctor and it wasn’t good enough. He told me exactly what needs to be in the letter and a list what needs to be answered for the clearance to drive. Again, gave it to my doctor and she did as requested what the medical examiner asking for. It still wasn’t good enough for the medical examiner. The medical examiner keeps changing changing what’s he’s requesting. This is the third time I have submitted what he has requested AGAIN for letter of clearance STILL wasn’t good enough.The first time when he reviewed the letter he said exactly, “I need your doctor to give clearance to drive a commercial vehicle. My doctor forfilled his request. STILL not good enough. He wants more medical terminology in the letter for clearance to drive when received the frist letter almost 3 months ago and did not tell this additional information. I been waiting for almost three months to work at this company. My doctor has delivered on his demands and depleted all the questions answered. She cannot do anymore for me. I feel discrimnated against when I am completely competent of obtaining a CDL. Can I apply to other trucking companies or will this issue be on my record for DOT. I have my clearance from my doctor.

      Get a second opinion from another examiner. You have the right to obtain a second opinion anytime you feel the examiner is being unfair or unreasonable.

      The CME is going to question the medication and its reasons for use. Most likely the examiner is going to want a medical release form from the prescribing doctor.

    I have a question, my medical examiner didn’t give me my medical card for my CDL because my DIABETES / SUGAR LEVELS / LIB blood work results came back high and my current medical card expires in 2 day . Would they allow me some time to get on a diet to bring it down to their requirements without loosing my CDL License ?

    Hello, I drive a semi locally and I was diagnosed with TIA just over a year ago and was told by my company I had to be off a year with no further incidents before I could return to work. The is up and when I went last month for my DOT test the Dr. looked at my release form and diagnosis and said it was a stroke and I would have to wait 5 years. He said the Neurologist had to be trained in the DOT regulations. Is this true and should I get a second opinion. I would think the neurologist knew what he was talking about!

      Your neurologist does know neurology, but may have no idea about the DOT/FMCSA guidelines. Go get a second opinion with your medical release form in hand from your neurologist. If all your test results indicate a good recovery, then the one year wait may be sufficient, it will be up to the CME. Explain that you drive locally, only, and have had no issues.

    Hi, you have a great website and it’s sources of information like this that help me learn about DOT physicals. I’m looking for a new psychiatrist to get a second opinion before I take the physical. My former doctor was prescribing me anti psychotic medication. So I have medical records that say I have bipolar or schizophrenia. I’m not exactly sure what they wrote down because I was never told what the illness was I was just given medication. I’m currently waiting for my medical records to be given to me so I can see what they say. So my question is, after I get a new psychiatrist can I get a new diagnosis and a medical release opinion letter that says I am stable without medication? I’m asking because I don’t have any symptoms and I can hold a job fine. But the last time I tried the medication it made me very drowsy and it was hard to make it through the day. I feel much better without the medication and i really don’t want to go back on any.

    I had my prostate removed due to cancer 4 weeks ago and I need to take a dot medical exam before returning to work in 3 weeks. I am now incontinent and can not give a on demand urine sample at my exam. It all leaks out on its own and I cant fill my Blatter full enough to give a sample. Also will incontinence disqualify me from driving? I ware a depend and it controls leakage just fine.

      Don’t try to take the DOT medical certification examination until you are fully released, without restrictions, from your doctor care. The DOT examination does require a very small amount of urine to check for blood, protein and sugar, so a very small amount is all that is necessary. The situation does not disqualify you.

      Contact a medical supply store for something called a Texas Catheter. It is a rubber-like condom that has a plastic connection on the end to hook up to a drain bag. You can also find a special drain bag at the same place to connect to the condom catheter that has straps to hold it on your leg, under clothing, not noticeable. The drain bag has a petcock opening in the bottom so that you can fill the sample jar from the drain bag. Google ‘Texas Catheter kit’ for pictures and suppliers.

    I have bipolar II disorder and take Geodon and Welbutrion to control it. I want to drive, but the examiner at the location the CDL school sent me to seemed confused about my circumstance and after he researched online denied me because of the Geodon. My psychiatrist didn’t see a problem with me driving on the medication when I brought it up. Could this be something my psychiatrist or primary care could sign off on and be re-examined?

      The medications are not a disqualifier for a medical card. What’s required of you is to provide recent medical documentation from your prescribing doctors. Have a medical release form from your prescribing doctor too.

    Hello I had brain tumor removal surgery in August 2017. I got my recertification last July. However, I have recently started at a new job. Went to the medical center for my newest medical card. Now the examiner is saying that she cant give me a medical card until 2 years post surgery. Never had any seizures, no medications and have been in remission since last April (2018). My oncologist and primary doctor has signed off. What gives?

      FMCSA guidelines recommend a 2 year wait following your surgery. With proper documentation and additional information from your doctors, the CME could reduce the wait from 2 to 1 year.

    I took a dot physical for my union job and was told because I didn’t include I was taking Suboxone it was a automatic fail. I do go to a doc and I’m on a very low dose because I’ve been weening down. The drug has really changed my life and now they say I need a doc note saying I’m completely off this medication. I got a taper plan from my doc to come off but it’s much quicker do to me not working. I have mortgage and bills. The examiner did say me not listing it on my dot form was the major violation not necessarily Suboxone. I gave them note showing 2 week taper plan from my doc with my current dose but even that she said may not be good and couldn’t give me clear info how to resolve this. My boss also said he received email saying not only did I fail but i was a danger behind the wheel which he announced in front of all my coworkers. I thought some of this stuff was confidential. Anyway I would appreciate any help figuring out what I need to do next.

      The CME needs to know why your on the medication, his job is to look for any possible risk factors for both you and the public at large. Check with your companies human resources department regarding the confidentiality issue, your boss is out of line.

    I am a disabled veteran I am currently taking two medications, 1st is Bupropion HCL 150mg for depression and the 2nd is Divaprolex 500mg for mood swings, I have been on these medications for over three years and have had no side effects and neither cause drowsiness for me. If my VA doc fills out the paper work and gives me the head nod do you think I will be able to obtain my CDL A or is this going to be a disqualifier? Thank you for any input.

    I had a kidney transplant in Sept 2016 and have a letter from my doctor stating that I had a successful Kidney transplant and I can return to full duty immediately with no restrictions.
    On my current long form medical certificate, certified by a FMCSA medical examiner, it is written that I, “Meet standards, but periodic monitoring required for Other – Renal Transplant *can not drive within 12 hours of taking insulin for steroid induced hyperglycemia”. My medication and medical history is fully documented on my long form.

    It is now April 2019 and I recently obtained my CDL class A. I was about to be hired by a company until my medical information was reviewed by the company, at which point they dismissed me as a candidate because “I had a kidney transplant”.

    Q: My dream job is in this industry but am I not able to drive because I had a kidney transplant in 2016? Maybe it has to do with my medication? Please explain what the issue is, and how long until I can work as a driver.

      Sounds like a company policy and not a medical disqualification via FMCSA. The situation and condition are going to raise questions with the CME and will need supporting medical documentation, be prepared.

      Since you are going to be driving intrastate only, you fall under your states variances. Check with your local drivers license department regarding your medical card requirements.

      Hello! I just bought a CPAP machine because the doctor/DOT said I have to. How can I obtain a one year card. I am just trying to start CDL school. I have a 3 month card only right now.

        Just before the end of your three months. get a reading off your CPAP machine showing a 70% compliance and you should be good to go.

    I have a torn rotter cuff , should I not consider driving a 18 wheeler cause steering might become painful , the doctor did not do any thing to it , just stuck his scope in my shoulder & looked around ,& told me it looked like a mess ,& 3 of 4 tendons are torn , another doctor said that he would like to bring two ends of the muscle together by sewing a piece of human skin between the ends ,do you think I should not consider getting my CDL license after all ,abandon the idea of becoming a truck driver.

      At present you can not fulfil the duties of a commercial motor vehicle operator. Once shoulder repair is completed and fully recovered, without restrictions, then you may continue your journey.

    If I have Nephrotic syndrome that is being treated with rituximab once every four months, would that disqualify me from getting my DOT Medical card. I could bring all the necessary paperwork from my doctor. Don’t need the CDL for my new job, but want to make sure I will qualify

    My husband has been refereed for a sleep study by a CME because his BMI is 39 and his neck is 18 inch. He had to get the referral from his primary physician and his primary physician said he does not need a sleep study. So now how does my husband get his DOT card for longer than 3 monyhs?

    Question.so i had my physical dine a couple days ago.i had put my medications down on the form (narco.for chronic pain in my leg dur to having metal rod and pins in leg.and my ambians do to hard falling asleep at night..i had ti take a driver medication form to my primary doctor to fill out.he did n i go back soon to turn the paper in ..will i be denied my medical card.

      For a class A, interstate driver, you are not going to qualify. As a lower classed license holder, you would fall under the states variances. You may try intra-state only cdl.

    What’s up doc! It will be 1yr in about two months that I had a loss of concisness at work.im a class a driver here in california.i started seeing a neurologist as soon as the episode happened! And what it was exactly is unclear.i had an eeg it was clean of seizure activity.i had never had a seizure in my life nor in my 20 yr career as a professional driver.my doctor had put me on a mild presription.the dmv had sent a letter to me saying I’m no longer on medical probation! When I called the dmv they told me I may never get back my cdl.how can I fight the dmv on this!or am I missing something

      I have narcolepsy without cataplexy and have been treating for the last 7 months. No accidents or even history of problems behind the wheel even before being diagnosed. I’m being told I can’t obtain a DOT card for CDL with S endorsement because of my diagnosis because I’m a “hazard” to other drivers. Why am I able to hold my regular drivers license with no record of driving issues but not able to obtain my CDL? I’m tired of disability telling me I don’t qualify because my symptoms aren’t severe enough yet employers say my diagnosis alone is too severe for employment.

        You may have a couple of options. Start with your local drivers license department and ask if you can obtain a cdl with your diagnosis. If yes, then ask if you are restricted to intra-state only or could you drive interstate. Interstate, class A, will be difficult with your diagnosis, but with good medical documentation from your doctors and a medical release form from them you expand your possibilities. Intra-state only cdl falls more under the states variances and not DOT/FMCSA. So you would need to find a CME that understands the differences and knows your states variances.

    I drove 33 years grandfathered in pre CDL, Now on permanent disability in 2010. Renewed my CDL in 2015 for 5 years. Now my CDL license shows inactive because of DOT Medical exam not being taken. If on permanent S.S. disability would my disability be denied and stopped if getting a DOT exam? In worst case scenario if I had to resume driving again I wouldn’t have my CDL , is there any options? Thank you

      Best check with your local DLD. As a class A driver, you will not pass a DOT medical certification examination while on permanent dis-ability. Your state DLD should present you with the other options.

      Hi. My question is a bit off the topic here, however, I’d appreciate any feedback!!
      My boyfriend Joe is required to get this letter from his doctor, listing all of the medications he is prescribed. (No biggie, we all understand the reasonings behind that)
      Yet, when he handed it in to administration, she called him back, claiming the letter was “no good” as it was missing “certain words/verbiage”.
      The issue we have with this is, now the document is late and has to pay an additional $50 fee. Not to mention the time it’s going to take to get to the doc office r which is NOT CLOSE. He goes to school 6 nights a week and has a full time day job. C’mon you all know the gig! Lol
      Isn’t it the employers responsibility to get in touch with the doctor office?? Or at least waive the fee, considering she didn’t bother to mention anything about these words that needed to be in the letter???
      Lastly, I’m shocked a psychiatry office, that’s been in business 39 years, doesn’t have particular templates/documents etc. approved by HIPPA, ODC, Medical board etc…
      Seems so unprofessional! Also, she said to him on his way out “you have to pay cash directly to me” ALTHOUGH HE ALREADY PAID THE PROPER AUTHORITIES
      Also, pardon my ignorance. I work in computer software, and all of this is so new to me!!!!

        You need to have your medical card exam done by a CME that understands the difference between a class A interstate driver and a intrastate only driver. FMCSA guidelines and your states driver variances are different state to state, so check your states information from the drivers license department.

    What if your come and primary dr are in the same building? Can the come and your primary care exchange information with your concerns and to resolve any issues ? In order pass the dot physical if any issues arises and are taken care. Between the driver and pcp and cme during the time to get resolved if any medical issues are found and to be corrected?

      Your medications are going to cause questions and will need supporting documentation for the DOT examiner to move forward. Make sure you have a medical release form from your doctors and from your prescribing doctors explaining your need and usage so the examiner can make an informed decision.

    My doctor prescribed Methocarbamol daily and said it should be O K to drive on if taken two hours before I drive. He also prescribed Meloxicam for times I am off duty. He is treating inflammation in my neck and shoulder. I’m confused! Everything I have read says Metocarbamol is a big no,no? He felt that Meloxicam is more worrisome and may have more side effects. From what I read Meloxicam is acceptable and Metocarbol is not. Can I use either one of these medications and would they show up as a banned substance in a urine sample?

      If you are being treated with these medications and suffering from neck and shoulder pain, then you are not qualified to drive at the moment. Both of these medications are red flags for a DOT examiner, so be prepared to answer questions and provide supporting documentation.

    Okay I have been driving for 5 years now. I was diagnosed with ADD and bi polar disorder when I was in my early teens. I took meds for a short time durning that time. Then stopped taking them I recently went back to the doc and he confirmed again the diagnoses of bi polar and put me on Lithium. I have contuined to drive a truck since being diagnosed and on tretmant and have no ill effects. I am gonna be switching company’s here shortly and will have to take another physical. My question is will my being bi polar and the fact I am on Lithium disqualify me from get my medical card cause I have been driving for 5 years now and havent had any issues

    From Idaho – Last year I only got recertified for 1 year as my blood pressure was 137/89. – (White coat syndrome & waited 2 hours to see the dr.. I was ticked)

    Just went to get recertified and again blood pressure was 140/82. I see my Dr. regularly and we’ve discussed this issue as I DO NOT have high blood pressure ever at her office. I usually run 117/82-84. Dr. told me I have to come back 3 months and if its not lower at his office, I will not be certified any longer. No options.. Is this correct??

      We do tend to get a bit excited when going to drs office, since about 1995ish every dot doc is an ALARMIST to the extreme, see, I know and trust what you’re saying. Really do.
      I was on a hiatus in 2012 and 13, I had my 30 yrs and millions logged, I wasn’t thinking and hadn’t kept up with all the SISSY ARSE RULE CHANGES in ref to physicals and CDL in my pocket. In 2014 I strolled into a license agency to renew my classA CDL and this examiner, we’ve ( my family) seen for years, asked for my medical card, I told her it lapsed and unless a good local job opens, I’m partially retired. She didn’t say hey, go get your medical, no, she kept and downgraded me to operator. Strange stuff,

      If he wants to drive class A interstate, FMCSA says no ICD’s. Intra-state or a lesser drivers license class, then he falls under the states variances and may be able to drive. Check with the local drivers license department for an answer.

    I had a presumptive TIA (Mini-stroke) 25 years ago and a CME has denied my physical until I get a release form from a neurologist. I haven’t had any in 25 years and even got through the Marine Corps and other DOT physicals with it being known without any release form. Is there a statute of limitations for requiring a release from a neurologist?

      The CME should only request information to support his final decision to either pass or fail you on the DOT medical examination. You should be able to have a medical release form completed by your doctors, stating that they feel it would be OK for you to drive on a full time basis without any restrictions. Once you have the medical release forms completed, make sure you take a copy with you for the DOT examination.

    I am a Certified Diabetes Educator; have recently helped a pt bring his A1c down from >10% to well controlled and stable per home BG testing (not long enough yet to repeat A1c). He was started on basal insulin and tapering down with add-on SGLT-2i pills; will likely get off the Lantus insulin, so I don’t think my pt has to worry about his desired INTRAstate CDL in Oregon.

    However, the DOT certified medical examiner (not realizing the patients DM care plan since his initial exam) has totally dumped this on the PCP that he is to provide an insulin waiver as one of the various things on the pts “to-do list” for various health issues; letters from specialists etc.

    My question– saying the pt actually DID need to stay on low dose basal insulin– WHICH DOCTOR IS RESPONSIBLE FOR ACTUALLY SIGNING THE “INSULIN WAIVER” (quoting the ODOT med examiner’s words in his summary letter to the pt’s to-do’s)? Is it the primary care provider (or endocrinologist if one were part of the care team) or the ODOT physician? We can’t seem to get the answer from anyone.

    We (myself and PCP who work in the same clinic) can’t locate any “insulin waiver” form anywhere, and it seems that the DOT physician should be supportive in making what is needed clear. Is it just a letter from the PCP indicating the pt has stable DM and low risk of hypoglycemia on his current regimen which includesl insulin, or does the PCP have to sign some ODOT/DOT official form? Does this signing put the signer at risk of their medical license should an adverse event occur?

    Seems to me the DOT provider should be the one that signs any waiver, as they are the one certifying after they obtain info from providers who know, evaluate and treat the pt.
    I appreciate your help.

      The waiver would come from the FMCSA/DOT and is signed by the medical examiner that is in charge of the patient. The CME simply needs to see the waiver, which has to be presented at the time of the DOT examination. FMCSA has made it sound like the medical community is at risk by signing a patient off. This is due to wording, but is not reasonable by any standard. The PCP has the best information since they have the longest relationship with the driver and the CME should only request additional information to support his final decision making process based on FMCSA guidelines and regulations.

    so i had an unprovoked siezure 1 year ago today i stepped away from my garbage man job due to it because i didnt know what the future would bring now ive had clean eegs and no siezures prior or since should i be worried about failing my DoT physical now that im trying to go back to my job since i have my PCP clearing me and backing me? i live in illinois and i know some rules vary state to state i just know it is what im good at im 26 and without the job ill lose everything i have

    I have a question regarding my husband. He had a mild heart attack in 2012, there was zero damage and a stent was placed. He was under a cardiologist’s care for 2 years and released to go be monitored by our GP. 2 years ago when he went in to renew his card, he was surprised with a requirement to take a stress test, which he did and passed. He’s coming up on needing to take one again this year, and we’re wondering if there is any way that we can get a waiver to not to have to go through the expense of this ever again. Who would we contact? He tried calling NDOT who told him to call the Dr who does his exams who told him to call NDOT.

      CDL and medical certificates are two different things. You need a valid medical certificate to maintain a valid class A drivers license. Oxycodone disqualifies you according to the FMCSA.

        You can have your doctor sign a release that it does not affect you’re being able to drive. I have been on Oxycodone for 7 years and hold my Class A and FedMed. I have to do a physical every year though.

    Had a recent physical to get medical card renewed unfortunately gotnit denied due to a recent questionable TIA in November. 2weeks after this incident went to neurologists as instructed by Dr. she gave me the all clear to return to work as well as Drive with no neurological deficit.
    I had taken release form to physical and unfortunately the PA-C would not release me due to diagnosis of TIA on assessment from neurologist. Pa-C advises me that neurologist needed to change diagnosis on assessment to clear me for my renewal which has now expired on 1/15. Mind you I had also gone back to neurologist within the past week to do another assessment on me with no sign of any sort of TIA symptoms. And again wrote a formal letter to the PA-C stating that she has released and cleared me of working and driving without limitations. Unfortunately yet again PA-C would not renew cause of this whole TIA diagnosis previous. I had also gone to see a chiropractor who had diagnosed me with a pinched nerve in my neck/shoulder which would have given me the symptoms of an apparent TIA. I had also given that to the PA-c with no avail as well. Of course this non renewal of my dot medical card has cost me my job because of this. And I don’t feel that waiting a year to be able to try to renew is ludicrous. Any other options? Please help

      Get full clarification from the neurologist, did you or did you not have a TIA? If you did then you are held back by the FMCSA guidelines, and the PA-c is correct, and they make the final judgment. The wording of the FMCSA guidelines can be gray in some areas, have them re-read the FMCSA guidelines and see if there is any areas of question.

He has a letter from his physician that says “no tx needed and “not engaged in an ongoing medications tx.” regarding the PTSD dx. The DOT said that this was not good enough and he would not approve him. What do you recommend

      If your husband has any supporting information from his treating physician, then we don’t see a problem. Ask the CME what further information would be required. In some cases the examiner is looking for a release form from the treating psychologist.

    I last saw my doctor on 12/06/2017, he was pleased with my progress.
    When I next see him on 01/03/2018, I’m going to try to go back to work.
    I’ll have had 30 days of physical therapy, my last day is 12/29/2017.
    What form(s) do I need to satisfy my employer in AR., and the state of AR?

    I am 73 and diabetic but not taking insulin. I had my physical exam done at Concentra on Nov. 15th and the Medical examiner gave me this letter (clearance form) to be signed by my doctors at the VA, particularly for my heart and knees. I just did my stress test at the VA and I was given a go signal to go back to work and even provided me with her notes and reports that I could show to DOT medical examiner. I showed the note/report but it was insufficient and told me to have the letter signed by my cardiologist. I have yet to hear from her on the letter.

    Re my knees, the medical examiner saw me limping and asked me to bend, squat and touch my toes and passed the test, too. I have bilateral osteoarthritis of knees. I could walk without difficulty and pain. I don’t have a VA specialist for my knee since there was no work to be done). My PCP found no problem that will hinder me from driving a truck.

    I have been going to VA for my check-ups and has all my medical records and test results. My problem is my Primary Care Physician is at the VA and wouldn’t sign the DOT letter for my knee because it’s functional work and I should go see another doctor.

    Your advice will be greatly appreciated.

    I had a very small heart attack in 2000 with minimal damage that does show up on ECG…that resulted in a CABG of the LAD using the Mammary artery to bypass…I am completely symptom free and have been since then…and am on no meds…So, my question is…Can I still qualify for a 2 year DOT medial card?

    Hi my name is Bill I’m trying to obtain a physical for commercial driver’s license I take no medications have only one medical condition I am on dialysis I have no restrictions other than going to dialysis 3 days a week is there a way around getting a physical card for somebody on dialysis

      You will be restricted to “intrastate only” and with proper documentation from your primary physician you shouldn’t have any problems. You fall under state variances, not FMCSA.

      No form of dialysis it’s acceptable I’m going through dialysis myself if you get lucky enough to get a physical card you’ll be slipping through the cracks if you’re able to get a kidney transplant that’s probably your best option then he’ll be able to return to work

    Hello all,
    I slipped on ice in the parking lot 5 and a half years ago. The doctor talked me into removing a minor amount of blood off of the back of my brain. Since that surgery, I have taken no meds, had no issues, and passed fed med card stuff every year(due to my original Dr. Examiners request).
    I stopped in last summer, my Dr was busy, so some crazy pants lady examined me, I passed everything as usual, then she read through med history, and went berzerk, shutting down my cert, demanding I get cleared by a neurologist, then telling me I would only be cleared for 3 months at a time!!
    Is this true? Also, since it has been over 5 years now(almost 6), and I have had zero issues, do I still need some silly neurological clearance?(when I passed the first four years after the surgery no problem)…

    two years a go i went to company paid doctor who looked at my neck and said i need to take apnea test. so i did, as I believe everyone has it to a degree as mine is very slight. now with all the uproar and confusion I have my DOT physical coming up and wondering if I went to another doctor with a good bill of health would I need to still produce proof of cpap usage

      If you’re not using a CPAP machine now then you should not have a problem as long as you do not present with any health conditions that may indicate you have sleep apnea. BMI and neck measurements are not to be use as sole indicators for sleep apnea testing.

        I take a low dose 1/2 pill of 0.5 Zanex daily for last 20 years. Since I started driving a truck 6 years ago and am on the road for long periods of time my doctor writes my prescription for 2 pills daily so don’t have to come in every month. On my recent med card exam I was told this is inexcusable. What are my options.

    We live in Oklahoma, my husband has held a CDL for 20+ years he recently applied for another driving position and naturally they sent him for a drug test and although he still has a current DOT card not due to expire til October they want this particular facility to issue a DOT card…. He had a reverse shoulder replacement back in 2015 but has since returned to work and also had his orthopedic surgeon send his release to the medical examiner and the Nurse Practioner told him he needed to go to the FMCSA SPE website and print off an application and get a list of doctors to do a skilled performance evaluation… In the meantime she sent a fail to the prospective employer. What is he to do… He has gone and renewed his DOT card with absolutely no problem since.

      Unless there are restrictions in operating the shoulder, an SPE evaluation is unnecessary.
      Go and get a second opinion from another medical examiner.

    class A: II had under gone a couple of surgeries within a 4 mo span. It took the doctors almost a year to find out why I was in so much pain. I was given a rx of the lowest dose of a pain medicine. I NEVER took my medicine when I had to work. A dot pysical dr looked at my record and freaked out because of the length of had been prescribed medication and how many times I COULD take. He disqualified me. He had requested a letter from my primary and that she was in compliant. That would not satisfy him. He never even drug tested me. What do I do now? Can I go to another dot and provide this information and get certified?

        I’m in the same boat I’m prescribed pain medicine for many years and the doctor told me that I have to have a primary doctor write a note for the prescription he gives me and he took my CDL license and that I can’t have this medication without a doctor stating that I’m not risk behind the wheel and my doctor won’t do that so the federal government took my CDLs even though I passed ma DOT physical he said He could not sign my DOT card cuz my doctor would not right a letter Stayton I wasn’t at risk

    I have a ky medical waiver. Can I get my full dot medical card back to drive out of state if my heart attack was over 5 years ago and not having any problems. No longer taking any meds.

    I take subsolv (suboxone) and have for some time now. I do not have a cdl and only drive a small box truck. I went to get my med card and the dr gave it to me but i have to get a waiver. Do i need this waiver since I only drive small vehicles and don’t need a cdl? Thanks for your input and I live in KY and only drive in state.

    July 2016 i collapsed at an outside event on a 100 degree day, My heart stopped. After a week in the hospital i was not diagnosed with any condition or illness and while not required an ICD was heavily suggested to prevent it again. Keep in mind the temperature and i didnt eat or drink anything since 6 the night before. On top of that no blood work or temperature was recorded before getting hooked up at the hospital. (after being on an IV in the ambulence) Everyone ive spoken to thinks it was heat stroke. Ive since gotten the doctor to turn the implant off completely and its only monitoring my heart now. I should be able to pass a medical exam now right?

      It will be very questionable. FMCSA regulations state that any class A driver with an ICD is disqualified as an OTR driver. With proper documentation from your cardiologist you should be able to do intra-state driving. We suspect that most CME’s are going to hold you back until the unit is removed completely, but it is worth a try with a medical release form and proper documentation.

    I am uunder pain management and im interviewing for a management position but have to get a DOT card because about 25% Of my job will be driving. I am prescribed percocet, i only take them when i know i won’t have to drive for any reason. Am I disqualified from getting DOT card???

    What is the federal guideline for evaluating a client with cerebral palsy. I would appreciate any direction with this question.

      We would have to start with, what class drivers license does this driver have?
      Some medical exams vary from state to state. This driver may fall under state variance that allow him to pass within the state’s regulation and not FMCSA.

    I had a motorcycle accident back in September and lost use of my right arm. I drove interstate commerce. I was denied disability and need to get back to trucking (which I have done for 20 years). I was told by my driver’s license center here in PA that a FMCSA doctor can sign off on me. I was also told by a representative at a congressmans office which talked to the federal boy’s. Is this correct? Can I drive and not have to do a SPE?

      Class A, interstate driver, no, you will need the SPE before you would be re-qualifed to drive.
      It is not up to the CME to make that decision, he needs documentation from the other examiners before he can make his final decision.

    I am a CME and recently examined someone who has been on Suboxone as well as Xanax. The combination is not recommended in the first place. Suboxone is used in place of Methadone which is an absolutely excluded drug but I can not find reference Suboxone in official site. Lastly the pt told me he only takes Xanax three time weekly, but, I checked on State online Narcotic Site and found that he fills Rx for 60 every month. So, he is either taking it twice daily or it is being diverted some how.

    Thoughts would be appreciated, especially if you can reference official recomendations

      Pointing you in the direction of a reference will be difficult since FMCSA removed their guideline some 27 months ago for site updates.
      So it really becomes your call based on any and all available documentation from the driver’s primary doctor.
      Even though suboxone is used for a methadone replacement, it is also used as a pain reliever. So you want to contact the primary doctor, or have a medical release form from the doctor explaining the drivers need, usage and length of time using this medication. Do the same thing for the Xanax.
      In the end it will be your call on whether the driver should or should not be driving based on all of the available documentation presented to you via the driver. Is he safe to drive and is he fit for duty? – That is the final decision you have to make. And it is the driver’s responsibility to provide the information to your office.

        I would like to solely comment in the driver’s defense solely on the suboxone. Not the xanax! I will say I was on pain killers for around 2 years. I will say the only thing that has saved my life and has given me a second chance is suboxone. Their is research to show long use to opiates can damage the brain and for the rest of our life we will always crave opiates every few months. I can say I came off pk’s for over 3 months and I still felt withdrawal symptoms everyday. I found suboxone through an illegal source. However later as I found a doctor who has prescribed me for about 2 years. I do not feel like I am on pk’s and have’t for a very long time. I forget to take it actually. Suboxone also blocks the receptors.So let’s say I found pk’s and took the entire bottle. I wouldn’t feel any high. Suboxone has never made me feel like I am drugged or that high I have completely forgotten. I am normal and forget about it. Unlike methadone that has made me sleepy. Suboxone has never and I mean never made me feel tired or drugged after the first few days you are a normal guy.

    I have a difribulator for almost a year now . Have made full recovery , doc will release me to go back to work when I’m ready . With the difribulator and a good ejection fraction is there any possible waiver to obtain to renew med. card ?

      Once fully released, make sure you have a medical release form from your doctor, and your ejection factor report.
      A lot will depend on the type of driver you are. FMCSA has a set of very strict rules regarding class A Interstate driver and defib units.
      So a lot will depend on the type of driver class that you are. We are not aware of a waiver that is available for a class A Interstate driver. If you are class A, then you will want to contact the FMCSA directly for final verification. Good luck.

      Yes, as long as there is not a defib. unit attached you can continue to drive, once fully healed, medical release in hand and ready to return to work.
      The CME may want to see further testing results so be prepared to answer any questions and be able to get any additional information that he may request.

    I was just told by a doctor at the clinic where I went to get a DOT physical, that, because I take Vyvanse, she can’t issue me a DOT card (in Tennessee) even if I have my doctor fax a medical release form, because it has some ingredients in it that are stimulants. I’ve had a DOT card for 17 years, and the last three times I had a physical, I was taking Vyvanse and listed it. Why now? Why this doctor? It’s going to cost me a great job!

      We recommend that you go get a second opinion and make sure you start with the medical release form in hand. If you have been on these medications for some time, then you need to explain and have your doctor explain why and how often and for what condition you are taking these meds.
      FMCSA seems to be on a witch hunt at the moment and a lot of the CME’s are simply not willing to take any possible risk that would be questioned.

    Ive had a 3month medical card due to blood pressure,the physician im seeing has me on medication,they say 140/90,is the desired target,im hovering around 150,s/100,s,can I get another 3month while under Dr,s care,or can I go somewhere else,I need to get back to work.

      You get a one time three month card. If you cannot get your blood pressure under the 140/90, then you are disqualified until you do get it below the FMCSA requirement. If you go somewhere else, be aware that the one time card is in the FMCSA data base and you would flag yourself as doctor shopping. Work with your doctor to get it under control.

    We live in Florida my husband went for his DOT exam today, he is diabetic and provided all labs to DOT examiner, as well as a letter from the eye doctor indicating no vision issues as a result of being diabetic. The DOT examiner measured my husband neck and said that his BMI was 32, at which time told him he has to go for a sleep study. This seems to be some type of ongoing tactic for a lot of the DOT examiners. I reviewed my personal medical insurance and he does not meet the medical necessity qualifications for the sleep study. What are his options? The DOT examiner gave him a 3 month clearance. Does he have to return to that DOT examiner or can he go somewhere else? This just does not seem fair to have to pay out of pocket for this very expensive test.

      Some time before your last medical card expires, go somewhere else for a second opinion. No new rules regarding sleep study testing, bmi and neck measurements. Once diagnosed with so, then there are regs, but not in the FMCSA prior to that diagnosis. Second opinions are worth the time.

        It has gotten to the point where many of these “medical examiners” are making choices they normally would not have in the past. I do not know what is driving these decisions but it is getting extremely tiresome to deal with some of these examiners.

          As a CME I can tell you at least 70% of the drivers I have required to get tested for OSA do have the condition. To help with costs I require a screening overnight SaO2 which is offered free of charge in our community. If that comes back positive an apnea link is only $190. Medicare takes that as definitive enough to qualify for treatment. With a positive screening , insurance companies then cover costs also.

    3 years ago, I caught an infection (sepsis) that destroyed my mitral valve. I had the valve replaced with a tissue valve. The cause of the valve replacement was purely due to the infection, there is no other vascular or heart disease. They gave me a pacemaker right afterwards, to coordinate my intake and output heartbeats. 5 weeks later, my heart healed electrically, and the pacemaker was turned down to the point that it no longer paces. My heart has been doing all the work on its own for nearly 3 years since, and they expect that to continue to be the case. I am on no medications at all except 25mg of Losartin per day for high blood pressure. My cardiologist says I’m not pacemaker dependent (I get it checked every 6 months, and it always shows that it hasn’t been used), and that I am good to go to drive or to do whatever line of work I want with no restrictions. I’m about to have a DOT physical for a CDL. I have no other medical issues, but I’m nervous! Should the release form you provide be good enough? I’ve downloaded it and am planning on taking it to my cardiologist to fill out.

      Just make sure you have a medical release from from your doctor explaining your particular situation.
      If there are no other issues, then you should get a one year medical card. The CME may request a stress test result from within the past two years, so if you have one, take it with you to the examination. Good Luck

    I work for an ambulance service. I had a black out spell in September of this year. I went to see a neurologist and he stated that all of my tests come back completely normal. He told me that I’m not allowed to drive for 6 months though because of the one time episode. I am just wanting to know if this is true or not? My job does not require me to have a CDL, my regular drivers license is all that is required for my job. So, am I allowed to drive the ambulance or not?

      It’s not the cdl that is in question with you, it’s the medical certificate.
      As a class A driver running interstate you would be disqualified to drive.
      As a class D/standard drivers license, you fall under the states rules and regulations. Check with your local drivers license department to make the final decision.

    1)I was advised earlier this year I have an A1C of 6.1. I was given metformin as preventative measures and to help lose weight. I stopped them about 1 1/2 months later. Will I need a dr release form?

    2)I have a thumb (right hand and I’m right handed) that cannot bend all the way, only slightly. It has been this way from birth.I have never had any problems performing any tasks. Will this disqualify me? Do I need a release letter?

    3)I had asthma since age 4/5, and used to have many issues as a child. I had a nebulizer machine and inhalers. I have not had any issues with it in the last few years, nor do I have any current meds. Will I need a release form?

    4)I have only been seeing my current dr since February this year. Last dr was about 2 years or so prior. Will the release form (if needed) be creditable/valuable from the current dr?

    5)Does the examiner need to be advised if you have HSV1 or 2 and take med’s (rarely) for it? Do I need a release form?

      1. Yes.
      2. No, as long as you have a good grip and grasp you should be fine.
      3. Medical release form would put the CME’s mind at rest and have an emergency inhaler as a backup, just in case.
      4. Should be OK if the new doctor is functioning off of your older doctors records. If not working off old records, make sure he has a working knowledge of your physical condition and get him a job description for a driver. (check FMCSA for that)
      5. Should not be an issue, but a medical release would not hurt.

    I have recently went and passed my DOT physical examination and obtained my 1 year card for a pr employment opportunity. On my paper work I put that I take Tyenol 3 and Lysinpril. They sent out my drug test and it came back positive for codeine. The medical Examiner called me up and asked if I had a prescription for this drug and I told him that I did. He asked for me to have my DR fax over a paper for the prescription on what it is etc. I usually only take this is in the evening and doesn’t require e me to take it all the time. I have already started my training with the company prior to this drug test other than the actual driving part. Will this now stop me from getting this job or should the documentation from my Dr be enough? Im worried that Examiner is going to hold me back from driving for this company.

      Should not be a problem as long as you can support the medication use with a properly prescribed RX. You could also have your doctor complete a medical release form for you explaining the medications usage and dosage.

      I had a brain surgury 10 months ago i have not gotten any seizures i have no issues what so ever. My dot was denied right after surgery when my dot expired. Would i be able to get a dot after almost a year?

    Iw as just diagnosed with hemiplegic migraines which have caused some sensory deficits on the right side of my body. I have loss of strength and cant tell hot/ cold or sharp/ dull and other issues. Nuero doesnt see anything at this time that will prevent me from hopefully making a full recovery but if I dont I am worried I wont be able to work for my company anymore becasue we are regulated by the DOT but we dont use CDL unless we are over the road drivers. Any help would be greatly appreciated. Also i had tremors during this episode that would last 30 minutes and leave me stroke like. When the migraine is beginning I have been losing strength on the right side.

    Hi my name is Jimmie,

    I moved from Colorado to Kansas a little over 1 year ago and had a doctor that I saw for everything I needed her for. I find myself here in Wichita Kansas and had to have a referral to a pain clinic concerning my ongoing pain I have in my back and shoulders. I recently got a job driving a bus on a contingency basis providing I can pass all the CDL training they are offering. I explain to them the only problem I will have here that I did not have in Colorado is the little PA from the pain clinic will probably not fill the form out for me.

    I have taken Percocet 10/325 and a muscle relaxer for years. Of course I would not take the short acting meds at all except for after my day ends and only 6 hours before. I can certainly see the risk in taking these kinds of medicines but I am tolerant to them and they do not impair my judgment as they do some people.

    This is going to be my livelihood as I have a hard time with other kinds of work. I am disabled and on Social security disability income but recently gat a letter from them stating I can test my ability to work. Driving does not bother me like other kinds of work. Any suggestions would be appreciated.

      Get a medical release from your original doctor explaining your medications and their needs. That should help the CME make an informed decision. Also check and see if your driving is going to be over the road or intra-state only. Intra-state only falls under state variances and not FMCSA/OTR driving.

    I’ve had controlled lupus for 14 years and I take medication everyday I’m on hydroxycloroquine twice a day that doesn’t cause drowsiness and I’m on 10-325 hydrocodon as needed for lupus pain I don’t have to take it every day and it doesn’t cause drowsiness or blurred vision or anything like that with the stop me from getting my CDL?

      The medications are going to cause questions.
      The CME doing the medical examination is going to need supporting documentation from your prescribing doctor. Have the prescribing doctor explain the length of time that you’ve been on this medication, and what if any side effects you’ve experienced.
      It’s still up to the CME to make the final determination.

    My husband had 3 strokes on 09/06/2016, has fully recovered but we are both worried about his CDL. He’s been cleared from his PCP, NEURO, and CARDIO to return to work full duty.
    With their clearances is there still a possibility he wouldn’t be cleared with a DOT physical?

    I have Trigeminal nerve damage since 1980. The pain never ends… I take four Norco and two oxycontin daily to control the pain so I can have a life and work. I’ve been own these meds for about ten years now. I have never mentioned it on my DOT forms, I’m afraid to open up a major can of worms now and lose my job.

    Has anyone out there been approved for a DOT cert on these meds? I have a non CDL driving job but I do need a DOT physical each year. (i’m on blood pressure meds) I’m thinking just keep my mouth shut and keep going as is and hope I never get asked for a drug test urine.

    Does anyone know a DOT Doctor on Long Island, NY that would give me a DOT cert with these meds if I get the proper forms from my neurologist?

      Fast acting narcotics are OK Aa long as you take it after work or 6 hours before your shift . time release is not allowed at all. It’s better to be honest then get caught for falsifying. Good luck wish there were more research done on how safe these are with people who are tolerant

        Thanks for your reply Bella. I’m really getting conflicting answers for this question. I read a lot that if I just get a letter from my Dr stating that I am under his care and that I have no serious side effects from the meds that I will be ok.

        Now you say that the time released meds (oxycontin) is no good. This is the first I’m hearing of this. I wish the Dr would give his opinion. I get 5 norco a day, most days I only need 4. If I say I only take it after work that may be a red flag. I seriously don’t know what to do.

    If I had a very mild stroke rececently,but have made a full recovery(no neurological or physical deficits) from the stroke am i still disqualified for 1 year from being allowed to drive? Is it possible to receive a variance if a board certified neuroligist gives me a clean return to work statement?

    I have been diagnosed with heart failure and HB. i had a heart procedures on right and left heart catheterization. I am currently on carvedilol and lisinopril. My cardiologist says I will not pass the DOT physical. I need to recertification for my job. My cardiologist is giving me 4 weeks to see if the medicine is working. With a medical release form from him will I get cleared? I only have 5 months until retirement. Thanks

    I am recently a dialysis patient. My kidney doc says my kidneys work, just not enough on their own. Is dialysis alone,enough to disqualify me. I passed every test but when I mentioned dialysis to the examiner, he immediately failed me.

      You will be limited to driving intra-state only, but can have a cdl and work within the state. You want to confirm with your local drivers license department as to what the policy of the state is for you to continue to drive within the state only.

    I have some medical issues issues that I am concerned might throw up some red flags for the CME:

    1. I have had a history of major depression for the last 15 years, but it has been well controlled by taking an SSRI (was fluoxetine, now is sertraline and bupropion).

    2. I had surgery for diverticulitis a few years ago, which was completely resolved. Since then, I have had a couple of recurrent ventral hernias. They have been repaired, and I had mesh installed to help prevent future ones. I have some weakness in the abdomen as a result (I cant really do sit-ups) but can function OK in everyday situations.

    3. I was diagnosed with psoriasis and psoriatic arthritis a couple of years ago, but it has been well controlled by taking bi-weekly shots of Humira.

    I am at the point now where I am only exploring the possibility of becoming an OTR / long haul driver, and would love to know if this might disqualify me before I spend thousands on driver training.

      Long before you need to spend any serious money the trucking school should ask for a valid medical card.

      To get started on that you want to have medical releases from your prescribing doctors explaining your situation and need for the medications that you are taking. The meds for your depression and the condition are going to raise questions. So when you get your medical release from your doctor make sure he explains that you have been on these meds for a while and that you have no residual side effects from their usage. All conditions are stable and your medication usage is also stable.
      With these supporting information in hand you will have done all you can to be pro-active.

        Defibrillator is a disqualifier for class A interstate drivers, even with the doctors note.
        But you may be able to driver cdl intra-state only and be just fine. Check with your local drivers license department and ask them for full verification.

    I am going through a school for cdl. I went in for a d.o.t test i let them know i used to take ambian but i have stopped all my meds. She disqualified me. My dr said he would fax them a letter, i didn’t realize i was going to need one. They wont take it and they say its gonna be 6 months before i can take another physical. I didn’t test positive or anything. What do i do now?

      Get a medical release from your doctor and then go somewhere else for a second opinion. If you have been cleared from this medication usage, then you should not have any issues with proper documentation in hand with a new CME.

    Who does the skill performance evaluation the CME? I was born with a hand missing there are NE restrictions on my class d licence I can drive a manual transmission car and want to get my class a will that be difficult or do I just pass a test? And I have worked my entire life no liability I can do pretty much anything anyone else can do.

      You want to check with your local drivers license department to see who they recognize as a SPE doctor. That way you get it right the first time. The CME does not do this test.

    Hello and thank you so much for any help you can give me. My name is Brandon and i have mild ADHD. I am prescribed 20mg of Adderall twice daily. I am about to go to a company sponsored CDL training and i am very concerned on whether i will be cleared by the medical examiner. I am planning on taking a letter from my family doctor who has been my doctor for the last 6 years. He says it will in no way impair my driving. My concern is if i am not cleared by the companies medical examiner i will have to pay back 7000 dollars that i do not have. The information online is very vague and unhelpful. I leave in a week and any information would be a great help at this point. If necessary i am willing to discontinue the medicine. What is the best way to move forward? I want to drive over the road more than anything!

    I was told gabapentin is an automatic disqualifier for obtaining a cdl class b DOT licensure from the doctor at my physical testing facility. if i have an explanation from my neurologist, does that allow me to qualify?

      As long as the CME understands why and for what purpose you are taking this medication, then it is up to him to make the final decision. A medical release form from your neurologist will be a big help, but not a sure fact to be DOT qualified.

    Im a driver and i also have worked on trucks and trailers for over 30 yrs i broke my neck in 96 went back in 09 passed a driving school 4.0 average taught the instructors a few things they never knew when i first started driving i trained. Myself to urinate be4 i got in the truck if I had the urge to go again i could make it go away till i was done. I’ve been urinating at 4am for the last 35yrs like clockwork wake up out of a dead sleep and can go right back to sleep then i cant go until late-night everyone says they can urinate any time they want i cant turn it on i can turn the urge off …
    I went for a urine sample told them i had to go real bad my appointment was at 9am they made me wait till 9:45 and handed me the cup my mind and body went into drive mode that ended it i drank water was there 5 more still couldn’t go closing time at 5pm still couldn’t go.
    Said i failed to test they said i had to take a shy bladder test next day i went the dot dr the next day gave me a reg exam like you take to get your med card he said i failed to test and the only thing DOT will except was renal shut down for failing to give a sample if i had renal shut down id be almost dead and shy bladder test please tell me what is done in the test ? I Read in the DOT reg. The Dr is supposed to give me the test within 5 days they said i had to take it the next day no test just a reg exam the next day ….can you explain test and was i right about the 5 days

      Was this a test for your DOT medical card or for a drug screen? One can be redone at anytime, but no medical certificate until you perform the urine test. The other is a pass/fail based on your ability to urinate and the test is sent out to the lab.

    I am a Independent transport driver of empty vehicles. (Etc) new Rvs, busses, sometimes cdl units that require a Class b cdl. I do not transport any kind of passengers. I recently had to have a Pacemaker for a electrical issue that I was not aware of. I have until July 21 2017 before it expires. My question is do I have to get a new CDL Physical and Medical Card. MY cardiologist said I could drive in two weeks. I have notified the company I deliver for and they have no issues with a letter from the Cardiologists.

      If the pacemaker install is new, then yes you need a new medical certificate to keep your commercial drivers license valid. Get a medical release form from your cardiologist and take it with you. You should be good to go if all else is OK.

      Well if you need the sleep study, based on true necessity, then you should be able to talk to your CME and explain your situation.
      Since there were no new rules regarding OSA, you may want to get a second opinion from another CME. Also check with your primary care physician and get an opinion from him too.

    I have chronic kidney disease. I have been a driver for 17 years. Passed all my medcards no problem, went in March and passed, received my year. Went in and tested again (medcard) for a new job a couple weeks ago and failed. High amounts of proteins in my urine. They gave me a 3 month medcard and said I need to come back in 3 months. If I get a release signed from my doc stating I’m just fine to drive,will I receive my year medcard back? How does that work? I am fine to drive, I am not on dialysis, just on meds, but have never had an issue. Completely clean driving record, again for 17 years. Its my livelihood. thanks.

      Since your condition is progressive and you would be best served by driving intra-state, you really need to check with your state drivers license department and see what the state variances are for your situation. A medical release form would be very helpful for any CME so he has the best and most up-to-date information to base his final decision on.

        So if I get a release from my doc stating I’m good to go, will I still be able to pass my medcard? I don’t see any reason for me to be on disability because I can perform my duties just fine, never had an issue. Seems stupid for the government/taxpayers to pay for me to be on disability if I can still work…stupid actually, I want to work.
        I just need to know if my doctor provides the necessary documents, I can still drive. I don’t think now researching it, intra-state is an option here where I am employed. So a release is the option. Thanks for your time and answers.

          If your doctor is certified to do DOT examinations and he wants to pass you off as OK to drive, then that decision making process is up to him.
          As an interstate driver in stage 4 renal failure, not sure any CME is going to pass you off without a lot of further testing and supporting information from your primary doctor. It’s in the FMCSA guidelines that your issue resides, not in your doctors opinion. He is really good at what he does, but it’s the FMCSA guidelines that will question this.

    I have a question I’ve had 2 dot physicals and passed both of them I had a liver kidney transplant 4 years ago no problems at all first physical had to have clearance from doctors and they passed me for one year then when time was up I had my second one do e they said they don’t need another letter from doctor and passes me that one still has 1 year on it but got new job they sent me to a place blood pressure was 143/98 never had blood pressure problems at all I have a cdl a she said I need to get her a letter saying I’m able to work another clearance letter and blood work and did not even give me 3 months can they do that it’s for a new job

      All depends on what the company policies are.
      So if you want to pass this examiner’s physical, then you will need to jump through her hoops. Not real sure why they would hold you from work when you are already working, but a medical release form from your doctor with a copy of your last blood workup should be satisfactory. You will only be good for one year at the time due to the liver transplant, but the blood workup is for confirmation that all is well with you.

      With a medical release form from your cardiologist with an expected prognosis in hand at the time of the examination and you should be pretty good to go. Your condition is not an instant disqualifier, but has to be supported with additional documentation from your cardiologist.

    Hello I recently had a dot physical where I was required to undergo a sleep study, turns out I have very mild case of sleep apnea. Well I used the crap and got my medical card back. 4 months later I switch jobs and the CME denied me my medical card and said I have to do the cpap compliance again, why? Also the pulmonary Dr stated I no longer need the therapy…… What can I do???

      Compliance is one thing and redoing a sleep study is different.
      Just get a recent readout from your unit demonstrating a 70% compliance on a daily basis and you should be good to go.

        Some ME’s are now wanting 80% compliance on a sleep study…..call ahead!! i just went in with a 73% and failed even though the DOT only requires 70%. This in the state of GA where they are allowing the ME’s to make a decision on what your results should be. Talk about a crap shoot!

    I had torn my rotator cuff and had surgery to repair three major tears.After a year of therapy,one of the repairs let loose again so they had to go back in and repair it again.I was finally released with permanent restrictions (No overhead lifting and no lifting over 20# on my right side).My employer sent me to update my medical card in case they needed a back up driver and the examining Dr. told me that I couldn’t get a DOT card with any physical restrictions unless my Ortho released the limitations.Was wondering if there was any kind of waiver or restricted type CDL that could help.Any info would be greatly appreciated…

      You want to have your orthopedist explain to the CME, on a medical release form, why you have the present restrictions and if you can bring in a job description from work, that will also be a big help. Since it appears that you do not drive on a full time basis, the CME needs to understand what your situation is and what your capabilities really are.

    I just got disqualified/placed on hold from the examiner because I have a mild case of cerebral palsy on my left side. He said i should get a clearance from my physician or a orthopedist, I believe. I can drive fine. I just would like to know the steps to pass and get my card. He also said something about going through a practice run of things truckers do to see if I can handle it, which I was confused about. He said that i should be able to get it. He wasn’t that friendly or helpful. I do appreciate it all the help and advice. Thanks again

      You will need a medical release form from your doctor and your neurologist. Not sure how extensive your condition is or whether you would need a skills performance evaluation. Your CME should be able to make that determination after he sees your medical release forms. FMCSA is looking over all the CME’s shoulders and their main issue is going to be safety. So get the information that is needed and go back with the forms in hand and ask more questions regarding the need for the SPE.

        Please, tell us more. It’s so complicated to figure out steps to do.
        Before I attend CDL driving school or do any steps to driving career I must be sure that I will get medical approvement.

        When I asked my neurologist about it he said that he does not give me any restrictions and instructor, who is going to test me must decide on the account of how I do on Driving Test.

        Should I take some paper with that statement from my neurologist? Are there some form for it? Is my doc in charge to give me this statement or I must go to some special doctor?

        What is CME?

          Yes, you should make sure you can be medically qualified before you attend CDL school.
          CME is “certified medical examiner” – the person who you need to see for a DOT physical exam to see if you qualify for a DOT medical card.
          You should take a medical release form from your neurologist to your DOT physical exam, so the CME has good information to help with the qualification decision.
          The CME makes the qualification determination, not the neurologist, so good information is important.

            When I went to u.s. Xpress I was given a one year physical but they did not hire me because I was on Adderall a few days before from an old prescription

            The doctor there knew I had cerebral palsy he gave me a one year instead of a two-year like the chiropractor but did not get hired because of the Addral or at least that’s what they said

    Hi i’m 26yrs old i recently applied for a truck driving job and was bused to a cdl school and was given a dot physical, the CME asked if i had any scars i said yes and told him it was from doctors putting in a tube to drain fluid from my brain the only issue i had that required them to put in the tube was my right hand shook i havent had any issue or seizures which i know goes with that type surgery since 94 when the surgery was done. I passed everything else on exam but was never given the medical card the doctor held on to it until i talked to the school to see what companys i could work for but i was told no company would hire me. My question is will this stop me from obtaining my cdl.

      No! Maybe the CME is looking for more information before he moves forward in your case. He would need a medical release form from your primary doctor explaining that you have never had seizures and that the surgery was to remove excess fluids. The more information that you can provide to the CME the better. He may feel that the risk factors are too high without further information.

    Last December 2014 I was involved in (MVA) where a fuel tanker pulled out of a re-fueling depot and ran into the side of my tandem Mack. I was cleared of the at-fault which was obvious. My problem is I tore 3 of the 4 rotator cuff tendons off the bone in my dominate arm and ruptured my bi-ceps tendon as well all of which I have been in therapy for since Not recovered yet or released from treating surgeon. I also tore three ligaments and the cartilage in two places of my dominant hand and wrist too of which I just currently had my first of three surgeries to correct the issues stated. I’m on pain management 10mg of oxicontin 2x a day and 900 mg of neurontin 3x a day to resolve the pain issues until all my surgeries are completed. My dilemma, my medical card is up next month and I’m sure I will fail because of the above. I live in Pa. and was told I will have to downgrade to a box EI on Pa.’s self-certification form DL-11CD (2-13) until I can pass a medical exam from this accident I was told I have to fail and then downgrade. Is this correct info or is there more or less I have to do? Thanks you guys do us drivers a great service!! John

      You are not in a position to even take the DOT examination because you have not been released and you’re set up for more surgery. So go down and check with the local drivers license department and see what they have to say. You should be able to downgrade for a short time, given your circumstances, and then re-enstate your CDL once you can pass the DOT physical examination again.

    I had an AAA done on 09/29/2015 and just recently went through orientation for a carrier and passed the physical. The physician assistant that did the physical tore up my dot card and said I needed a letter stating from my surgeon that I pose no threat to myself or others around me if I drive a commercial vehicle … I also have to wait three months after the surgery before I can qualify.. Is this correct information ????

    2012 – a friend of mine who’s been a long time Tractor Trailer Class A in good standing driver, was tricked into visiting a mental health institute by his sociopathic brother – stating its “go there and get checked out or leave my house for good”, so not wanting to cause trouble he went unaware that his brother had called ahead stating my friend was saying he’s “hearing voices and waving a gun around” and when my friend got there the doc said here this will ease any anxiety you might have during this interview and gave him two pills to take – so not wanting to ruffle feathers he gullibly took the pills- it was haldol that immediately rendered him a zombie and he was then held for a month unknowingly. He’s not schizo, never was, and isnt on meds and I wish I was in state when this happened to have helped him. My questions is does he HAVE to put this incident on the forms for his DOT med card renewal while he’s trying to get it reversed?

      It would be best to be honest. If he was ultimately released without further incidence, then a note/medical release form from the treating physician should be all that is needed.

      You’ll need information from your neurologist along with a medical release from him. Drivers on seizure medication are going to have a tough time due to the medications and the seizures. It’s mostly a safety issue. But the information should be helpful for the examining CME to make the best decision across the board given your situation.

      It is a progressive condition, so make sure you take a medical release form from your doctor explaining what level your condition is in. That way the CME has the available information to make the best decision. In and of itself Lupus isn’t an instant disqualification.

      Due to the effects and possible side effects of this drug, you will need to have been on the medication for a good period of time and you will need a medical release form from your prescribing doctor. With a proper explanation from the doctor, you may pass the DOT medical examination. But it will require supporting paper work from your doctors.

    I recently had to undergo a sleep study to determine if I had sleep apnea. it turns out I do and I’m now under treatment of a cpap machine. I recently got a citation for speeding so I decided to check my mvr. here’s where I find out my cdl is canceled supposedly indefinitely because I failed to report medical/disability but I’ve had follow ups with my dot doc and everything else is going well. so I’m confused as to what this could mean

    My boyfriend twisted his ankle and the doctor put him in a cast for a few weeks. No breaks, just tendon damage. He has a year before his next dot physical is due, but his company is saying that when he’s released to go back to work, dot regulations state he has to get a new dot physical. He’s upset because they cost $ and he still has a year. I just want to verify this. Can you point me in the right direction for answer or help verify. Thank you.

      FMCSA states that any injury that takes a driver from behind the wheel, for any reason, must have a new DOT physical before returning to driving. Check FMCSA website. It’s not about the time left on the medical card, it’s about the ability to perform the duties of a CMV operator.

    I had one of my disks in my back herniate a few years ago. When I got it checked out they told me 2 of my disks were messed up. one had herniated awhile before which cause the other one too. I never received any surgery. I got the steroid shots done in it. I usually only take motrin for the pain if it causes me problems. In the military it has caused problems because of wha I do. I do not feel it would effect anything as a driver. Would this cause me to fail MY CDL medical exam?

      FMCSA will have a ton of questions and so will the DOT. Too many of the wrong type of side effects with this medication. DOT will, most likely, have a very negative outlook on the medication in the possession of an OTR driver on the road.

    Hello,
    I am 22, about to be 23. When I was a year old I had heart surgery to sew up a hole, and had a stroke during the surgery, but it hasn’t put any affect on my daily life. When I was about 9 or 10 I had a nocturnal seizure and was put on medication for a few years and stopped before i got my licences and haven’t had any recorded since then. (added) Scratch that, I have had 1 recorded since then, meaning I went to a hospital, and one where I could have had one but didn’t go to the hospital for it. And they all happened within a five year span.
    I don’t believe my sleeping patterns have changed any and don’t believe I’ve had any since then, and the ones I have had have been strictly while sleeping. Is this gonna affect me passing my dot physical. I also haven’t had a PCP for a few years now. Will I have to give my full medical history while I’m there? Thank you and have a good day.

      You are going to need a medical release form from a neurologist before any CME is going to do any DOT examinations. Because the first question is “are you now or have you ever had a stroke or seizure?”

    I had a triple bypass done 2 months ago. I am trying to get everything together to take to my medical examiner to get it all done. I had a Q stress test before I started cardiac rehab. Is that the type stress test that is acceptable to allow me to receive my medical exam card back?

      Make sure you have a medical release form from your cardiologist along with the test results before you go for your next DOT medical examination. You should be fine if you are ready to return to work, once the cardiologist releases you.

        My wife had a triple bypass done in 2016 now there is a leaky heart valve that is not allowing full blood flow to one chamber of the heart her doctor it treating it with meds but took her CDL which she has had for over 30 years. The job she has now for the last 5 years is truck mechanic and a CDL is required so now with out a CDL she is endanger of loosing her job . The CDL is so she can road test and move trucks she works on! What options does she have?

          Check into intra-state only with your local drivers license department to verify, but we believe that would allow her to maintain a valid medical card and her only restriction would be that she could not drive a class A vehicle outside of your states borders.

    I had PCI 15 years ago, all went well and have been stable ever since. I have elevated blood pressure, but it is stabilized with medication. I do the Nuclear Stress Test every 2 years and achieve good results. Did the nuclear stress test last year and took the results along with a clearance letter from my cardiologist. I completed the exam this year with satisfactory results, but the medical examiner temporarily disqualified me until I provided them with:

    All medical records and results
    Current echocardiogram results within the last 1 year
    Current exercise or nuclear stress test within the last 2 years
    Current medications and treatments
    Clearance letter from cardiologist

    When I read the FMCSA web site I see I need the stress test, I have that.
    Annual Medical Examination, I’m there for that.
    Current medications, listed on Medical Exam form.
    Letter from cardiologist, I have that

    It took will be 10 days before my appointment to get a Echocardiogram!
    Why the echocardiogram all of a sudden and all medical records when I am asymptomatic for 15 years and have what FMCSA lists that I need after a PCI?

      Your CME is trying to work with the FMCSA and is requesting everything that the FMCSA guidelines recommend. You have everything that is required for the DOT examination, but your CME is looking to cover their a** for any possible safety risk factors.

        The FMCSA is some what over doing these medical conditions. I feel that a medical card is not necassary in order to drive.
        It’s like a second license to drive, and the cdl is worthless to obtain.
        It’s just my opinion, don’t get me wrong safety is a concern. Leave that up to the cdl holder who knows there health better than the CME Does.

    I applied for a job and they did a background check and then called me to go do a drug test so I did. I told the office doing the drug test that I was on Narco from my doctor and had the hard copy from pharmacy. The office doing the drug test told me to show it to my employer so I did and they made copies and said their shouldn’t be a problem. It has been three days and haven’t heard nothing from employer or the doctor. And now it’s the weekend. I was wondering how long it takes for them to get back with me and will I be able to get the job or can they deny me a job because of the medication.

      With the medications that you are taking, there will be issues.
      The job drug test will go to the medical review officer for further evaluation. This can take a few days to a week before you get results. The MRO should be trying to get a hold of you for a copy of the prescription. One way or the other someone is going to get in touch with you.

    I have type two diabetes and I am currently on insulin to help control it. I have never had any accidents or injuries that have resulted from my diabetes. However I will be taking my first DOT physical next month and I was told that diabetics usually don’t pass. My diabetes has been heavily monitored by an endocrinologist for well over a year now. What can I do to prepare for my physical? Is there any specific paper work that I need?

      As an insulin dependent diabetic, you are going to need a medical release form from your doctor.
      And even with that, you may pass the examination, but will have to go through the FMCSA waiver process to drive as a diabetic on insulin. This process may take a few months to get through. It will also depend on what type of driver and what class license you are going for. But as an over-the-road driver, you will need a diabetes waiver before you can drive OTR.

          If your doctor is on the NRCME, then his will hold up, but if he is not a CME on the National Registry, then the CME’s card is the only valid medical certificate. Check with the FMCSA regarding the reasoning.

    I had a frontal lobe brain tumor removed a year ago in sept of 2014. I was off work for 2 1/2 months recovering. I was cleared to go back to work with no restrictions by the nuero surgeon, the nuerologist and my companies safety director. I was 1 year into a 2 year physical at the time of the procedure. I have been working since released to go back to work. my current 2 year physical expires august 31 of 2015. the CME told me, after I passed the physical, that she could not certify me because I wasn’t to be driving for 2 years post op. the nuero surgeon is livid regarding this as I have had no seizures and follow up mri showed no tumor. I read above that you had stated that a later date passed physical would supercede a failed physical. so I can go to another doctor and get a passing physical to send in? please advise. thank you

      If you have been completely released from the neurologist and neurosurgeon and have had no physical, mental, emotional symptoms and there is no seizures to report and you are not on any seizure medications — Then with proper documentation you should be able to pass the DOT certification.
      The CME may hold you to a one year card to continue to monitor your situation, but you would be able to continue driving. You should take all of the available information and the medical release from your doctors with you to the next examination.
      If you pass the examination, then yes it will supersede the non-qualified exam report.

    My husband has aortic aneurysm and he has been treated/seen by a doctor for the last 3 years. It is checked every year to see if it has grown. Per his doctor, they may never operate, it all depends on if the aneurysm grows or not. Will this diqualify him from driving in NYS? They will not operate based on its current size so can the doctor complete the “Medical Release Opinion” and will the DOT Doctor accept this?

      The CME will need a medical release from your cardiologist and a given size of the aneurysm.
      The FMCSA has a guideline that will be followed, so depending on the size of the aneurysm and the doctors release, you should be OK.

          FMCSA uses a series based on likelihood of additional problems. Starting in the range of 4 mm in size. That’s why you want to have your doctor confirm on his medical release information the size and likelihood of any possible changes within a given time frame. The CME may choose to limit the length of your medical certificate so that he is kept abrest of any changes in your situation or condition. The guidelines are just that, guidelines and the ultimate bottom line is safety to both the drivers and the public at large.

    I got my ICD implanted almost a year ago because of SCA. It have never fired, and haven’t heard a peep from it. I do not have a heart problem. I had SCA only because of low potassium. Right now I am not taking any meds. If I get the ICD deactivated or removed, will I be qualified to drive commercial again?

      If you get the unit removed, then there should not be much in the way of questions, just a medical release from your cardiologist. Not sure about just having it de-activated. Again a medical release form would need to explain your condition and situation. With the unit removed, you should be able to return to driving.

    I am a doctor, but not a CME (I have no desire to be a CME).

    I treat a lot of patients with acoustic neuromas and dizziness.

    Can you help point me to some resources that provide some guidance regarding how to counsel these drivers?

    My understanding was that an acoustic neuroma meant that a driver could no longer drive– should be removed from duty at the time of diagnosis and not allowed to be reinstated unless the tumor has been removed and the individual symptom free for a few years. Is this wrong?

    Just looking for help here. As you know, they don’t teach DOT/CDL stuff in medical school.

      You are correct in your evaluation to remove the driver from operating a commercial motor vehicle.
      The FMCSA has a guideline manual that would support the direction that you are headed.
      Until the tumor has been removed and all symptoms have dissipated, the driver is disqualified from operating a CMV. Time frames can vary to some degree, but the driver must be free from all signs and symptoms, also a medical release form from his surgeon and PCP are going to be needed for the driver to have his medical examination done.
      The best reference is the FMCSA guidelines. That document was available online from the FMCSA.dot.gov website, if they have it up. It has not been available for months as FMCSA was/is in the process of making changes.

      FMCSA is going to put you on a waiting period for the severe head injury. You are looking at a possible one year waiting period before qualifying to do the DOT examination. Since there is the possibility of an unprovoked seizure and those possibilities are high and they do not reduce much with time. The FMCSA makes a very strong recommendation not to certify for at least one year post accident. Even then you will need a medical release form from your neurologist before any CME is going to do a DOT medical certification examination.

    My husband took his DOT physical, but failed it because of his blood pressure. He told the doc he has white coat syndrome and they wouldn’t listen…he needed to get relaxed, but they took it three times and failed him. Then the doctor told him he needed a sleep apnea test and he told them that it isn’t required. Can he go to another doctor, or is it reported that he already failed his test?

      So yes it was reported to the FMCSA as a failed test.
      And that being said, you can go anywhere you like and have the test done again. If you pass this time, it overrides the failed test report and starts a new medical certificate. Just make sure the blood pressure is under control and if you are taking medications for the HBP make sure you take a medical release form from your doctor with you.

    The medical card system has now turned into a political game where you are limited and forced to work at McDonald’s for diseases of-age that may eventually lead to a disability. How about convicting someone for a future crime before they commit it. A 4-wheeler is more likely to run into a ditch after 400 miles of night driving than a 60yo driving a Rig for 1500 miles. But what .. a driver from Mexico is exempt from carrying a Medical Card on his person but enter the USA? Oh wait .. I forgot, there are no diseases in Mexico or Canada. My bad .. forget what I said. I’ll go work at McDonald’s.

    I’m thinking of starting a career in transportation. I’ve passed out 3 times in my life (I’m 33 years old), the most recent time was about 3 and half years ago. I’ve went to the ER when it happens and they just say that I was too tired, dehydrated, and once said that my potassium levels were a little low. They have given me MRIs, X-rays, and echocardiogram (sp?), but ultimately they never did anything else beyond let me rest drink water and gave me potassium pills once. Would this be an issue that I would need to have extensive testing on before trying to obtain a medical card? Thanks.

      You will need to have your primary doctor fill out a medical release form stating your situation and any issues that he has dealt with regarding you passing out. As you can imagine, this is a problem when 80,000 pounds is moving without a driver. If you have the medical release form in hand at the time of the examination, then the CME can make a more informed decision regarding your DOT physical.

        Here in Canada I’m on a 10 year waiting list for a primary care doctor. ER doctors (whose names I’m sure I could never recall) have been the only ones to have seen me about this. Should I try to seek out a private doctor for this? If so would he even be able to give any type of medical release? Thanks.

      Just be aware that aneurysms are normally progressive and can increase in size as life goes on. You want to have radiographs or CAT scans for base line size and then monitor routinely. This may cause your medical card to be limited to one year at the time and you will need a medical release form from your doctor explaining your situation and prognosis.

      A driver who loses consciousness is now a disqualified driver and his medical card is invalid.
      The driver needs to get with his primary care physician and find out what happened and why. From there the issue needs to be corrected, if possible, and then the driver can apply for a new medical card. He will need to have a medical release form completed by his doctor and take the form with him at the time of the new examination.

    My best friend is looking to get her cdl license soon but she’s HIV positive, but is doing great and hasn’t had any issues since diagnosed as a kid. Will she be able to pass a physical or will she be wasting her time. She has been on the same medicine for years and hasn’t had any problems with it. It says it may cause drowsiness so she only takes it at bedtime.

      She should have her medical doctor fill out a medical release form for her. It should explain that her condition has been in play since childhood and that she is on medications that are not a safety issue. Otherwise she should not have any problems.

      Tough one. The FMCSA has decided that defib’s are an instant disqualifier. You could try, but not real sure that it’s going to work. If the driver is traveling intra-state only, then you would want to check with the state DLD and see what their stance on this issue is. In some states where the driver is intra-state only and maybe downgrade to a class B license, might be a chance there. Check with your state DLD for that information.

    My primary care provider is not the doctor I receive medications from that may be in question by the examiner. How should I proceed? Is there another form or will the examiner accept a letter from the doctor? I do not see my primary care wanting to fill out a form based on another doctors plan of care.

      Your primary care provider is not going to fill out a medical release form based on someone else’s treatment.
      So go to the prescribing doctor to get your medical release form filled out by that physician. That is the information that the CME is going to be looking for.

    I went to a doctor two and a half weeks ago, complaining about sleep problems. He prescribed me Trazodone to help me go to sleep. Now I’m trying to get into Prime’s trucking school, and they tell me that I will likely be disqualified if I am taking Trazodone, and need to get a Dr.’s note stating that I’m off the meds before I will be able to pass the medical exam. Is there anything I can do so I can keep taking Trazodone, I feel like a new man already, or do I need to find a new med or get off sleep meds completely?

      You could start with a medical release form from your prescribing doctor explaining the needs and usage of the present medications that you are taking. With a proper explanation and proper usage, you may be able to continue using what is working for you. The big issue is the possible re-actions and /or side effects to a new medication. Or how you may act under a medication that is new to you. You could also ask your prescribing doctor what other medications you could possibly use.

    Hi i have a pacemaker, recived it oct 7 2014. i had a dot physical done to day with a release form and had to have doctors office fax over my medical reasons why and my pacemaker reports, i have to find out tomorrow but i did receive a 2week card till they get the rest of the report to get my 1yr card, told i have to do a one year follow up and dot physical every year. my question is they did not say i may receive a card yet and i want to know if i may get denied. in 8 months i have had this my reading have been good about 11% that the pacemaker worked and 98% left on batteries and is set to not go no less then 60 bpm. i ment on this that 11% of the time the pacemaker was operational in those 8 months.

      This is the normal process for pacemakers and DOT. With all of the proper documentation, you should receive your medical card for one year and you will need to have a recertification done each year thereafter.

    I drive a truck with GVWR under 26k and travel almost exclusively within a 200 mile radius. Because of frequent kidney stones and nerve damage in my shoulder, and a mildly herniated disk in my lower back, I take Percoset up to 40 mg a day. It does not make me sleepy, it actually has the opposite effect most of the time, and I don’t take it while driving just to make sure. I went to get my medical card renewed and they said having Percoset prescribed to me is an automatic disqualifier. They said the only way they will give me my card is if I get a letter from Dr. stating that I will stop taking this medicine and the Dr. will not prescribe me any more. They said it was a DOT rule and they had no control. My quality of life will suffer as I have weaned myself off before and got next to no sleep because of the pain and had to sit with a heating pad and take more ibuprofen than directed just to not be in tears, so not having the Percoset is not an option. I don’t abuse it, I use it as a tool to make my quality of life better.

      This is not a RULE and the CME seems to be over-stepping the CME’s boundaries. You drive and are home every evening and you should not be held to an interstate drivers rules and regulations.
      Get a medical release form from your doctor explaining your situation and issues. That should answer any questions that the CME should have. I would take my next examination somewhere else. Your situation is going to pose questions, but with proper documentation you should be good to go.

    I just recently applied for a new job so part of the pre employment physical was to have a urineanalysis dot doctor said i have high protein in my urine that i need medical clerance from my primary doctor got checked out and my doctor said i have high protein also wont sign off on my clearance did he can do all sort of follow up test… I have one week for the job offer to take of this? Is there anything i can do or im i officially disqualified from getting said job?

      Your medical doctor needs to find out why you have a high protein count. Once he gets a handle on your situation, then all he will need to do is complete a medical release form for you and you should be good to go. But without knowing what is causing the situation, no one is going to sign off on this issue.

    I was involved in an accident on a city bus the brakes failed and I steered away to avoid a fatality and was knocked out due to the impact was taken to the er had an extended hospital stay now they are trying to say it was a syncopal episode the video from the bus shows I was awke and alert until the impact had a dot card removed three days after issued for further records review was released by PCP and CME now off in limbo for maybe one year been misdiagnosed and railroaded been a bus driver 15 yes 1st accident and not my fault!

      Start with the ER doctor and have him write a medical release form stating the facts. Then have your primary doctor do the same thing. This should over-ride the CME’s opinion because he has two other physicians stating the cold hard facts. That should take care of the issue.

        Yes, and you will need to be fully healed with a medical release form from your cardiologist and a recent stress test in hand.
        Your medical certificate will be good for one year at a time and you’ll need a stress test done every two years for you to continue driving.

          Last week I took my husband to renew his CDL now his own doctor says that he has a good bill of health for someone with diabetes however this doctor is looking at his BMI and says that he needs a sleep study. What? His own doctor, my laywer , the federal dot and the ohio dot which I called. All say that their guidelines are not a rule let alone a law so how can these doctors do this and get away with it. I just read the ohio and the federal does not require this. Now this is funny, why this place is going by these guidelines is because some doctor wrote a book saying that it’s dot guidelines for commercial drivers. Isn’t that like going the law and taking matters into your own hands ?

    I live in Nys an just got my physical , been on suboxone for 12 years. Why is it I can take Xanax an klonopin or narcs! But not that? He gave me a med form for my klonopin to be filled out by my physican yet he totally disqualified me for my suboxone. No form nothing. It does not affect me in any way like taking a vitamin. Is there still a waiver I can get ? I am awaiting a call back from my doctor, this doctor was super unreasonable when it came up . Wouldn’t hear me out not one bit . Should I talk to my doctor an then go an hope to see a more reasonable doctor . I was straight up honest an unless I taped off for the next 2-5 months I am screwed… Help please an yes I tried to read the above messages an help but didn’t see any that pertain to my specific predicament if you will.

      You must get your primary prescribing doctor to fill out a medical release form for you to take to the next DOT examiner. Your doctor must explain the dosage and the need for such a medication, as well as the length of time that you have been on these meds, and its side-effects to your driving, if any. Without proper documentation, you are going to be flagged without the DOT examiner’s understanding what is going on.

    On Jan. 19th 2015, I had some tingling in my arm and hand, I went to the local urgent care and the
    ER doctor diagnosed it as a TIA. They did a CT scan and a doppler scan on the arteries in my neck, and a chest x-ray. All of the tests came back negative. ALL my symptoms had subsided within about 20 mins., but since the Dr diagnosed, me with a TIA, she referred me to a neurologist in Lacrosse which when I went to that appt. the neurologist ordered more tests, which all came back negative for an embolic source for my TIA. The CT was neg.for carotid narrowing, the ECHO of the heart showed normal functioning. I also did the DOT occupational therapy driver prescreen evaluation,with the results being above average for my age group. But since I now have this diagnosis of TIA or stroke on my record I can’t even get a DOT DR. to even look at the test results. They won’t even give an appt.for the DOT physical. Is there any way I can get an unbiased DOT DR. to at least look at my situation? One more thing I never passed out or lost consciousness, and was given no medications or therapy for any of this. Thank You Dave

      This is going to be tough, until you can get the ER doctor to rethink his initial diagnosis. It can be done, but you will have to be patient and bring all of the available information to him for further examination. He has to have everything you have to re-think his preliminary diagnosis.

    In the last year, my boyfriend has had 3 dot physicals (new job, coming back from an injury and now another new job). He passed the 1st two with no problem. The third examiner stated he had protein in his urine and wanted further info. We sent her what we had from lab results. He has been diagnosed by his primary with chronic kidney disease and has had it most of his life (he’s in his early 50’s). She has disqualified him from driving stating that he is in kidney FAILURE. She’s telling him that she has derived this from the lab results we gave her but according to the charts I’ve seen, he has not reached that final stage yet. He is stuck…2,000 miles from home with a new company that isn’t sure what they should do with him but are trying to stick by him. He’s a terrific driver. What are our options? Shouldn’t he at least be given something short-term until he can see his primary again?

    I just started a new job driving regionally my company sent me to there DOT Medical EX. I’m Taking a oxycodone 10mg 180 pills a month have for 6 years I broke bones in my neck and back I don’t take them while I’m driving only after work as needed. I Got the letter from my primary DR for him, he would only give me a medical card for 30 days til I changed my script to 45 pills a month did that got another letter from my primary DR. again for him he gave me a medical card good for 90 days so he can check that I am only getting 45 pills a month he has told my work about my prescription what I’m taking an the amount. I’m on paid Leave right now the DOT EX. has to talk to the companies corp. DR. an decide if I am gonna keep my job because the card is only good for 90 days. I work in Utah. Does the Medical EX. have the right to do what he is doing.

        Thank you for your response. How about the other part of my question? Is the Doc allowed to dictate my prescriptions? Is he allowed to determine what pain regiment I am supposed to take? If my doctors have written a letter stating that I am fully capable of driving a semi while taking these medications, that I have taken for over 6 years, is it ethical/ legal for this doc to say NOPE, I say you can ONLY do this. Without ever seeing my medical history, an X-Ray or a CT Scan or anything to determine the type of pain I am in or what my pain management should be? Without even taking into consideration the side effects of taking me off this medication suddenly?

          The CME is not the person to make that determination. His job description is to determine “medical fitness for duty” period. He has over stepped his boundaries and job description.
          Also you want to have your medical doctor complete a medical release form for you, not just a note saying you can drive, that isn’t his job to make the CME’s determination.

My doctor is willing to fill out the medical release form stating I am ok to drive the bus. Will this fly with the motor vehicle department in my state? And if so do i need to do present the release form to anyone other than the lab that draws my random drug tests? Thanks.

Can I be taking this while holding a class A cdl. I have taken this before with no problem. Didn’t realize it could be an issue before ,thought I would check before starting again.

    I am trying to find information for someone that has their CDLs and is in a category A. He was told that he is overweight and will lose his CDLs if he cannot lose 70 lbs before he takes his medical exam to get the medical card. My question, Is there a way to get around this? He only has 18 months until he can retire and would like to keep the CDLs. I have been searching and I have not yet seen anything about there being a weight limit that would cause him to be restricted.

      There is no weight limits on any driver. So someone is blowing smoke up the information tube.
      There may be other conditions that may limit the length of the medical card, but weight is not one of them.

    State of Maryland senate bill effective 10/01/2003 stating. If you are a Maryland NON- Commercial Driver operating a commercial motor vehicle in interstate commerce (within the State of Maryland) and the vehicle weighs 10,001 and up to 26,000 pounds,you are now required to have a valid Federal Motor Carrier (DOT) physical card in your possession. If licensed prior to October 1.2003, have a preexisting Medical Condition, and the onset was prior to 10/01/03, you would be EXEMPT from this requirement for a 20 year period,provided your preexisting condition dos not worsen. You are still required to maintain a DOT physical. This is also found on the Maryland MVA’s Web Site.
    Would this Suggest that I’m Exempt due to my preexisting Medical condition I take a anti-seizure medication due a brain surgery in 2000 and have for the past15yrs

      You need to check the wording on some of your statements. Interstate is not in the state only. Interstate means that you are traveling outside of your home state. Intra-state only is with in the states boundaries and that the cargo is from and staying inside the state boundaries. If the cargo being hauled is leaving the state, then you are driving interstate commerce.
      As far as the medications you are taking, you may, after checking with the state Drivers License Department, be correct as far as the condition and the time frames. Check with your state DLD and confirm. If not just make sure you get your primary prescribing doctor to complete your medical release form for you and take it to the examination. That will answer most of the examiner’s questions.

      Unless you sign some type of medical release form from the company giving them permission to release any of your medical records, then the answer should be NO. And if they did so with out your permission, then they are way outside of HIPPA compliance and you may have a case against them for doing so. Make sure you have all your information together before making any accusations.

One more thing at the time of the surgery in 2000. i turned in my B -CDL’s down graded to a C license – non CDL. The tow trucks are all under 26,0001lb. We Tow for the State Police ,Baltimore co. and Harford Co. the state police are the only one to ask for a dot medical card. I still have the letter from the Maryland Medical advisory board say to go to the local MVA a get your down graded license.

      Start with your local drivers license department.
      You should not be trying to pass a class A interstate test. You are in a class which requires a medical certificate, but not a class A certificate.
      Each state has specific rules and examination requirements that are different than the class A drivers.
      You should not be having any problems, especially since you have been doing this for quite a while. Get a medical release form from your doc and explain to the DOT medical examiner what has been, and what is going on.

      You are also going to test positive on any drug screen, so you will still need your doctor’s prescription and notes.

      FMCSA regulations – You will need to be completely healed and released by your doctor, with a medical release in hand, before you can retake your medical examination. Once you pass a new medical examination, then you are good to go.

    Why do the psychiatrists do not give the clearance letter to people with mental illness, doctor’s office manager say there has been new policy that they are not required to give clearance letter any more basically its up to the doctors to give you a clearance letter or not. My question is what should a mentally ill person do if he\she do not get the clearance letter from the doctor for his\her CDL license? please explain in plain english.

      All of the certified medical examiners, CMEs, are looking for a medical release form for any and all conditions that a driver may have. That request for medical explanation from the primary doctor is only to provide supporting information that the driver is ‘fit for duty’ mentally, physically, and emotionally. That release can come from any of your primary doctors. As far as why your doctor will not provide you with a release is beyond me. I have no explanation and there is no new rule or regulations that point in that direction.

He has hypertension and reads even higher at the doctors office, he has been on drugs to control the BP for the last 3 1/2 weeks but they are still telling him conflicting information at Nextcare where the DOT exam is being done. They keep saying he has to have THREE “good” readings in office?

      Your state should not revoke your cdl due to a shoulder injury, yet you can not qualify as a fit for duty cdl driver.
      Just let the state know what the situation is, and they will have some policy that takes your situation into consideration.

      With a properly controlled conditions such as you describe, you can go for the CDL.
      There will be lots of questions regarding your condition and how it is being controlled. Make sure you take a completed medical release form from your prescribing doctor. That will answer most of the questions that a CME will have regarding your condition and what your physical, mental and emotional states are.

    I just had my DOT physical. They found sugar in my urine but my blood sugar was under 200. They restricted my license to 3 months (in NC). I know it is high but I thought as long as it was under 200 I could get a normal 1 year license. Does this sound correct to you?

      No matter what level your blood sugar is you cannot have sugar in your urine.
      The CME is giving you 3 months to get your sugar under control, you could have been disqualified on the spot.
      Get it under control, go back and have a clean urine sample and they will take you to a one year card.

    I have. been a class b driver for 14 years now just doing inner city driving for a sanitary district…I have been currently seeing a councilor for marriage counseling where she basically diagnosed me with PTSD…she is private practice and has not put this in my transcript…I would like to look into medicating but don’t want to open up a can of worms???? What do you think???

      Check with your doctor. If they feel that the meds will be of help, then check it out. You may need a bit of time off to see how the medication effects you and you will need a medical release form completed by the prescribing doctor for your DOT medical certification examination.

    After cardiac bypass surgery, what information is needed to take with you for a new DOT medical exam. There seems to be some confusion about whether a stress test is needed now or later. One doctor is saying it is not needed until 5 years after the surgery but her associate is saying one is needed now. Both doctors are in the same office & both are certified to do DOT exams. What info is required at DOT exam?

      If you have been taken off the medications and the condition is under control, then you want to tell the CME that that is the case. He may request a note from your primary prescribing doctor stating that you have been taken off the medication. So you could do that in advance of the DOT examination.

    I just got the opinion letter signed by my doctor in regards to BP pills which makes my BP completely normal, and another opinion letter about diabetes. I take pills right now for my diabetes and it makes my glucose completely normal. This is my 1st time getting a DOT physical card will I be limited to 1 year or should I qualify for a 2 year?

      Your medical card will be limited to one year as long as you are taking medications. The DOT and FMCSA want to keep a close monitor on anyone taking medication such as yours.

    I had stoke 5 years ago, I gave up my class A cdl when it was up for renewal 2 years ago and have my regular DL.

    I am still in the business and still cable of driving a semi, but I only have use of one arm. would I be able to get my CDL back if a doctor passed me physically able?

      With your present limitations you would need a medical release form from your doctor and the DOT examiner is going to want a SPE (skills performance examination). Otherwise you would not be granted a class A medical card or license.

    I started treatment for adhd/ depression earlier this year. I take vyvanse in the a.m. along with wellbutrin and adderall in the afternoons. Currently I am a relief driver cdl a tractor trailer and 24ft box trucks, but I started out OTR truckload.My original doc thought it was unsafe for me NOT to be on treatment driving etc. I have become a much better driver since starting treatment, however now I am in need of a new medical cert. and want a new job. My original physician is not covered under my insurance so how do I go about being cleared as an eligible driver on medication? intrastate or interstate? thank you

      Just make sure, before you go for your next DOT exam, that you have your prescribing doctor complete your medical release form for you. That will answer most of the questions that the CME will have about your medications and your present condition.

    I was told today by the fmsca by email that it is not there rule that you must take a year off after a stroke.but that is what is in their guide line book for drs. to use .I had a letter from my nurologst but dot dr would not even look at it.this should not be the case . how would you fight this?.

        I damn near passed my physical and they asked me why I was on Coumadin. I had a stroke in November and as of last week I failed my physical cause of comadin. I got a doctors appointment in 1 week and hope I get better results . Mike

          You are not being held for the use of a properly prescribed medication. You are being held due to the fact that you had a stroke. That waiting period, depending on what type of stroke you had, can vary from 1 to 5 years before returning to work as a driver of a commercial motor vehicle.

            I also had a mild stroke in nov2014 after being out on disability for hip replacement surgery in sept 2014. Now at the end of my 6 month disability. Went for dot phys and nurilogist cleared me for work but dot doc says guidelines tell him 1 year wait. So what do I do for 7 months now ? Unemployment with a $2400 cobra!!! O try for disability n Medicare ? I live in n. J. Thanks Charlie

            Depending on where the stroke was, it could be an even longer wait period.
            You may be able to do intra-state only. At least you could work. Check with your state Drivers License Department to make the final determination.

      Once fully healed and with the primary doctors completion of a medical release form, he should be able to return to work. Once he completely medically fit to safely operate a commercial motor vehicle. This may require a stress test before he tries to return to work.

          Don’t know about your home state. But, according to the FMCSA, you would get a one year medical certificate. Just make sure your medical release form is completed by your primary before you go for your DOT examination.

        I just had heart surgery to stop Afib. I was told by my surgen I was fit to return to work. I had to go get a return phyical at some place our company uses and they said I had to wait a month. It was NOT open heart or in my vein type surgery. I cant find a thing about having to wait a month to return to work.

    Hello, I’m a 38yo male with a pacemaker and a mitral valve replacement as a result of childhood rheumatic fever. I’m currently taking Bystolic and Coumadin. I asked my cardioligist if this would affect me being able to obtain my class A and drive interstate, but he wasn’t sure. Would any of this prevent me from truck driving?

      Until you are very stable on the medications that you are taking right now, you will have trouble passing the DOT medical examination.
      Once stable on the meds and your doctor is willing to state that he feels very comfortable with your medications and your condition, then have him complete your medical release form for you to take to the DOT examiner. That will answer most of the questions that the CME will have. Good Luck

      You will need a medical release form completed by your primary doctor.
      You may also need to have a neurological evaluation to determine the extent of the TBI residuals.
      Medical release first and then try the DOT examiner. He may request the neuro evaluation.

    Is there a liaison that helps with concerns about DOT certification? My husband was offered a job pending his physical and passed the physical with flying colors. However, the CME has temporarily disqualified him based on pain medications. He is legally prescribed these opioid medications by a reputable doctor specializing in pain management who also treats spinal injuries (reason for my husband’s treatment).

    The CME refuses to grant the certification unless my husband stops taking his meds but the process for coming off of six years of pain management is longer than one weekend and my husband needs to start work on Monday. We attempted to explain this and asked if we could provide documentation from his prescribing doctor but the CME wouldn’t entertain the idea. He’s made a broad-based decision with no merit and no evaluation of medical history. This seems unfair at minimum and borderline discriminatory in reality. Do we have any recourse? I’ve been scouring the internet for information and advice.
    Right now our outlook is bleak. It seems the only option my husband has is to stop his medication immediately in order to get his certification from this guy. Doing so is extremely risky and can result in life-threatening detox. The company is only having my husband drive a pickup truck for them but they are insistent on DOT certification. The CME also completely disregardedrour request for temporary certification (90 days) to allow my husband to come off his medication safely.

      The primary question: Did your husband take a medical release form, completed by the prescribing doctor, with him to the DOT examination? That, in and of itself, may help you out immensely. The CME has to have supporting information from the primary prescribing doctor, so see if that will answer the main CME’s questions.

        No, because we didn’t know anything about any of this. When the original CME temporarily disqualified him, we asked about a medical release because we immediately started looking up information and discovered that he could rule in my husband’s favor with a release.

        The CME refused to accept any communication from the prescribing doctor and said that the only way he would qualify my husband was if he immediately stopped taking his medication (which would be very dangerous). Another CME happily certified my husband without even batting an eye at his medications. Will this certification stand if the first CME temporarily DQ’d him?

          This is not the CME position to do such a thing.
          So if you went to another medical examiner, one that understands his position, and he clears your hubby, then you should be good to go.
          If your hubby has a CDL, then this is the medical certificate your hubby submits to your state DMV for self-certification record with his CDL.

    How long will my medical card be good for 6 months or 1 yr in state of florida.

      Once you are fully healed you will receive a one year card from this point on.
      You will also need supporting testing regarding your heart each time you go for your re-examination with the DOT examiner. Some of the testing need only be done every two years, so check with your examiner to see what all you will need starting now. Good Luck.

    I am currently a driving instructor, and I have a student who knows he is suffering from kidney failure, it is to the point of causing him tears.
    He wants to complete the instruction and obtain his CDL for local work at home. I know it is a risk, but he is a good driver. I have researched FMCSR for this and it is rather vague. . .
    My question is; “If he regulated his treatments, once it comes to that, can he still hold a valid CDL and work?”

    I have been licensed as a Commercial Driver for numerous years, I have Type 2 Diabetes and have controlled in the past with diet and oral medications. About 1 1/2 years ago I started on Insulin injections. I work and reside in Georgia now and only drive in the state. I took a new Medical Exam in July of this year and passed all ok and was issued a certificate good for 1 year. I got a job operating a Yard Dog truck here locally in August, they called me in yesterday and told me that to their understanding I am not qualified to operate a Commercial vehicle if I am taking Insulin and are going to lay me off……….Help I need some good answers

      Check your present medical card and determine if the examiner has you as an intra-state only driver.
      Most of the state DMV’s will let you drive intra-state with a state diabetes waiver.
      Check with your state DMV and determine if you can drive intra-state as an insulin dependent driver and what the process is to acquire the state waiver. Good Luck!

      To pass the DOT examination you will probably need to get a permanent pair of hearing aids.
      Once you pass the test using a hearing aid it will be marked on your medical certificate that the driver must wear hearing aids at all times to operate a commercial motor vehicle.

    I was diagnosed with minor sleep apnea while in the military. I’ve been using the CPAP for almost three years now prescribed by my military physician. Next month I retire from the service and I’ll be attending truck driving school to obtain my CDL class A. I was told a medical exam was a part of the class and I’d have to disclose medical information on some form. If I disclose my sleep apnea will I have to participate in another sleep study even though I’ve been compliant and still receiving treatment? I already have a machine. I’m already using it on a constant basis. Are they going to make me do another sleep study? It was a long process to get my sleep study done while in the military and I’d hate to go through it again. This is stressing me out. Is there an investigation done to see if everything on the medical form is truthful? Are they going to dig into my VA/military medical records.

    I have been diagnosed with urgency incontinence and I will be taking Oxybutynin. One side effect of this drug, and an alternative (tolterodine), is “sleepiness.” Does taking either one of those drugs *automatically* disqualify, my CDL-A? And would this be a questionable drug requiring physician intervention/explanation? Thank you.

      Once you have been on these medications for a while and you see how you react to them, then you will know which way to go.
      You will need a medical release form completed by your primary doctor so you can take it with you to do your next DOT physical. But the medical release form should answer the CME’s primary questions. Good Luck

    Last November I had a sleep study done and I was put on a c-pap with oxygen only used at night. I went for my physical today and the PA-C told me that I had to 70% complaints but my doctor says I need 90% to drive a school bus. Is this something the DOT set or the doctor? I know to go back to driving I need a releases form made out but……who is right?

      The DOT medical examiner is following the FMCSA guides, not sure what your doctor is following.
      The main thing is use of the unit should reduce excessive daytime sleepiness and make the driver more alert.
      Another thing to take into consideration is you are a school bus driver and not a full-time over-the-road driver. So check with your state DLD and see what standards they are operating with.

    I am attempting to get my CDL and I have sleep apnea. The DOT Examiner says I need a 30 day compliance report from my CPAP machine, however I just got my CPAP machine last week. Can I get a 30 day card until my compliance report is ready? I really need the job and I am afraid they are going to give it to someone else before my 30 day compliance report is ready. I have passed every hoop but this one and my family desperately needs me to land this job. My Primary Doctor says she is willing to write me a letter saying I have been on it for a week as long as I bring her the card out of my machine supporting the action.

      FMCSA regulation states that you must be on the unit for at least 30 days before a medical card can be written. As a regulation, there is no getting around this for the moment. It’s a safety issue and I don’t think anyone will sign off until the 30 days are met. Good Luck

    Hi! My husband was just presented with a job opportunity that is a blessing to our family. He would need to get his CDL and the only issue at hand is that he’s a diabetic. He takes Metformin and Glimperide currently. He’s taken insulin years ago, but not now–he’s managing with the medication and diet. We didn’t foresee this coming, and he hasn’t been great about checking his levels, but they’ve been stable. I know his doctor will fill out the form for him, but I was curious as to how they check these sugar levels. I know he could be under 200, but I’m not sure if he’s gotten up over that in the last few months-so I don’t know if that shows up as an average in your system–or if they check right there by a finger prick? Thanks in advance for your help–this is all so confusing to me and he Really needs this job!

      As a diabetic, you want your primary doctor to complete a medical release form before he goes for his DOT medical examination. If his condition is stable, according to his primary, then he will be restricted to a one year card, but should be ready to go. Good Luck

    I had a dot about 14 months ago. They put me on warfarin and installed a small filter to “catch” any clots that might go to my heart or lungs. I have had zero issues since. Would any of the above disqualify me from a Class A CDL?

      More questions than answers. Why are you on warfarin? Why did they install a clot catch?
      Without these questions answered, it would become a guessing game to figure out the answer to your question.
      Both of these situations will require a medical release form completed by your primary doctor. That will help the CME make his final decision.

    I went to get my physical renew. for my C D L When the doctor ask what medicine I was taking I told him I was taking Chantix to quite smoking, I was told I couldn’t get my CDL because this is a drug that I can’t take while I am driving a school bus.

      For class A drivers traveling interstate, this may be true. If you are less than a class A driver and driving a school bus, then check with your DMV. I do not believe that this medication is on their “no drive” list.

      This will be tough for a while. He will need to have his neurologist complete a medical release form stating that the driver is physically, mentally and emotionally stable and can be safe behind the wheel of a class A vehicle.
      Brain injuries are required to have a stabilizing wait period before returning to operating a commercial motor vehicle. It is do-able, but may require a little more patience to get it done. Good Luck.

    My son recently had a stroke. We have progress notes from his cardiologist and the neurologist that say they have released him to go back to work. The company sent him to a dot physician to get another physical even though the one he had was still good. Can he qualify for a medical certificate or is there a waiting period for him to go back to work? We brought all of the records we had to the DOT that said he had been released. From what I am reading a cerficate is different than the medical card that the cdl drivers carry. I understand also that the medical examiner can limit the time that the certificate is good for and that he would need to be recertified. When I looked on the fmsca website there was a form for the certificate and a different form for a physical. Do we need to go to the DOT and just request a certificate?

      First, regarding the stroke. There is a one year waiting period from the point of release before he can get a medical card/certificate.
      Second, without knowing what his drivers class is, I can’t tell you what his requirements are going to be. But the examiner should have all of the correct paperwork you would need. So check with them first. Good Luck.

      DOT Physical Forms
      2 forms are needed. The DOT doctor will have the forms, so you don’t need to worry about that.
      1. The Medical Examiner’s Certificate is the same thing as a Medical Card, also known as a DOT Card. The driver must carry this with him/her at all times whilst driving.
      The Medical Examination Report, also known as the Long Form, is completed by the driver and the doctor during the DOT physical exam.

    I have sleep apnea, doctor gave me a 1 month medical card when I went to renew my card contingent upon me bring a copy of my Sleep study. My most recent sleep study was 2 years ago and he will not accept it. My insurance won’t cover cost of another study since I haven’t met my deductible. I have the study that shows the AHI and the read out that shows I’m in compliance with my CPAP.

      Looks like everybody is between a rock and a hard place.
      You could try getting your primary doctor to fill out your medical release explaining your compliance and see if the examiner will accept it, along with any other documentation you may have. Good Luck.

    Hi i took a dot random drug test, the medical reveiw dr called me about a drug i am taking, it is generic adderall, i gave him all the information they needed to confirm it, it was confirmed i have a prescription for it, it is used for adah, they still called my employer don*t think they said name of drug but said i was adah, i drive a transit county bus for mrdd adults, been driving over 25 yrs, never had trouble like this before for random drug tests, they would call, i gave info and was always passed, they never called my employer before, but they called this time, can they do that when they say i passed negative this time and told my employer my personal issues, i live in marion ohio 43302 work for marion county mrdd, thanks

        I don’t know but they have me working at different depts til sept 18 2014 where they are sending me to be evaluated, I think this is wrong, they say something about a 327 code, or maybe some kind of code that started with 3. Well I guess I will go for valuation, I don’t get it but I have to do as they say, thanks, have a good one”

    I am going to have open heart surgery for a bicuspid aortic valve and aortic aneurysm. Will I be eligible to keep my cdl? If not, is there anything I can do to keep it?

      Once you have completely healed and your cardiologist completes your medical release form, you should be able to continue your driving career. You will need a new DOT physical exam. Take the medical release form for the DOT medical examiner.
      Your medical card will probably be good for a year at the time, but you’re still driving. Good Luck.

    I am a disabled veteran with a service connected hearing loss. I have been a truck driver for over 30 years and have had no accidents, tickets or any other problems. My current employer for the past 9 years has recently indicated that they don’t feel it is safe for me to drive. this is 9 years after having no tickets, accidents and getting excellent reviews from them. I went to see an ENT who indicated I am ok to drive as long as I wear my hearing aids. The employer still indicates they don’t feel It is safe for me to drive a truck.

      This sounds more like company policy.
      If you pass the DOT examination with the use of hearing aids, then there is no issue with the federal requirements.
      So check with the company as to what policy they are using. Companies are allowed to have stricter requirements than the federal requirements. If it’s company, then you know where you stand.

    I have stage 4 renal see a kidney specialist on a regular basis and preparing for dialysis.
    I have a class B license in California driving a 18 person transport van for elderly and disabled. No strenuous work about it just pick them up and drop them off at store or day programs.
    I was told once I start dialysis I would lose my class b per the dot dr after jumping through hoops for 3 months to get him to approve me for another year when I went it for renewal.
    My question that I can not get anyone to clearly answer is when I go on dialysis do I lose my class b or can I still drive for a living?

      This is a state related question. If you have an intra-state only license and the state of CA says you can operate the bus and still have dialysis then you are good to go.
      Check with California driver’s license department to confirm.

    I was just denied recertification because it was discovered that I have some osteoporosis in my back. The Dr. who gave me my examination said that in order to be DOT certified, I have to be found able to drive a truck cross-country and load and/or unload any cargo I’m transporting. Is that accurately Interpreting DOT regs? I am employed as a transit bus driver and am not even allowed to help any passenger, who may have fallen, get to their feet. It seems unreasonable to expect me to do more than my current job expects me to do. Thanks for your input!

      If you are a class A driver, then the examiner is correct.
      But if you are not a class A driver and only operate a transit bus, then he is off base.
      Each state has different class driver and different specification for the varying classes. Check with your state DLD and get those specs for your class license and then go get your medical card.

      He has a waiting period of three months and he needs to be fully healed before returning to work.
      Make sure he brings a medical release form, filled out by his doctor, to his next medical examination. He will have to have a new medical card before he can return to work, along with a return to work note from his primary doctor.

    Recently had a drug test that reported out as negative but since I am taking an amphetamine based medication need a medical note from doctor saying safe to drive. How often will I need to provide this note to my employer – how long is it good for? Thanks!

      You will need that note until you are off the meds. But one note, one time, until retested again. The MRO review officer will need to see the prescription each time you are drug tested, until you are off the meds.

    Quick question. Went in for a new dot physical and am told I have to go take another sleep test. I have already taken a test 3 years ago and results came back with no sleep apnea. Am i required yet again to go spend more money and take another test? Or are my results still good?

    hi I am off work right now due to an illness, my question is if I am on warfarin and I have a heart problem and now possibly a lung problem that I am waiting to here back on should I even try to get back a medical card to drive or based on what’s going on with my health should I just not even try to get it back.

      Potentially your conditions sound like they may be taking you off the road. The medications are going to cause problems too. So if you have another choice, I would be investigating those options. Best wishes.

    I recently went in to have my DOT physical, now I won’t lie I have a wrestler type body. I took a sleep study about a year ago and was told I had mild sleep apnea but not cpap required. when I hand the results to the N.P. I was told that those results are useless and per DOT rules you must have a new sleep study done every year and I could not have temporay card too get it done per DOT rules. lost three weeks of pay dealing with it. is this true you cannot have a card while you are getting a sleep study done?

      Sounds like it’s time to go somewhere else for your examination. The NP is kinda overstepping boundaries on this one.
      Get a note from your MD and have him fill out the medical release form for you.
      Then go somewhere else for your examination and take the medical release form to the DOT doctor.

    I am on a blood thinner Xarelto for parasoxmal Afib my PCP and my Cardiologist told me all is fine. according to the tests they do not see a problem. My job told me I cannot be on a blood thinner that I am taken out of service. The doctor told me it is not encouraged to pass people on blood thinners due to it being a federal mandate and ruling. I have searched high and low and have not the found anything which would agree with what he said. I think he is jerking me around. please advise.

      It isn’t so much a mandate or ruling, it’s more about the possible effects of the medication on a driver.
      I would get a written medical release form completed by your primary care physician and take it with you when you get your DOT examination done.
      You will, more than likely, only receive a one year medical card due to your condition and medications. Remember it’s about safety to you and the people around you while driving, nothing personal, just basic guidelines.

      I would make sure you have a medical release form filled out by your primary healthcare provider and copies of your latest testing in hand when I went for the DOT examination. With everything in hand when you walk in the door, you shouldn’t have any real difficulties.

      Not sure of the rules within NV.
      As a class A driver you will have a waiting period following the pacemaker install. But if you are not a class A driver then the wait may be shorter.
      Also, please be aware that once you were no longer able to operate a vehicle, due to surgery, that you need to acquire a new medical card. Or at least that is true with the class A drivers.
      Make sure you check with your state DMV to get the best answers regarding your specific situation.

    I am trying to get my CDL to be a bus driver, I went to see my PCP and got my physical. I am on Effexor for depression, which I have been on for years. And I am also on Suboxone due to a surgery that I had 7 years ago that caused me to have to come off of many pain medications in which my doctors failed to tell me how addictive they would be. Is this going to stop me from getting my CDL.

      I believe, that as long as you are on these medications, you will have difficulty passing your DOT medical examination.
      What I would to is get your medical release filled out by your PCP and have that in hand when you go to take your DOT exam.
      This information helps the examiner make a more informed decision. But the final qualifying decision remains with the DOT examiner.

          I am on Prozac and I have a cdl. It depends on why you take it, the cdl motor vehicle will send you a form after or if it’s detected during drug testing. Or the doctor can write on your physical paper why you take it and you will only pass if the doctor doesn’t think it will be detrimental in your duties, a hazaed during driving and your problem will be evaluated. But I was sent a letter and my doctor sent a reply on why I use it and I am allowed to use it.

      If you have a stroke, you must wait for a period of at least one year from the time of medical release.
      You don’t lose your CDL, just your medical certification for the next year.

      Your company will probably send you to a doctor, theirs when you are well again and if you are cleared to drive you keep it, I was on disability and my physical came up, I lost my endorsements when renewing my license but they won’t take them premantly until after 2 or 3 years you have to ask your motor vehicle agency or call the dot like I did. When you are better and you take new physical papers to doctor and you are cleared then you can just go to motor vehicle and add them back on as long as the time lapsed is not over the amount allowed, you still have a drivers license just not the endorsement until you clear.

    CME folllow Federal Regulations and the guide lines for the Fittness test. I believe CME’s have a huge roll in helping us CDL drivers with stay complient and healthy, just as we the driver has. It isnt like being lic. to drive a regular passenger car. Be healthy and know your job I say!

    I am being sent for a sleep study after my medical exam for a dot medical drivers card. question: can I use the at home sleep study testing device from a certified sleep lab? this way I don’t have to go sleep in a lab.

      This can effect your cdl dramatically, if you fail your sleep test, they will give you a breathing machine, you cannot get one until diagnosed with a sleep study first. If you use it and they determine with another kind of test you are fine to drive they will clear you and you will have to go more frequently for physicals and bring results from sleep study doctor with you saying they cleared you to drive.

    How can a DOT medical examiner be allowed to determine our careers. I think there should be no medical cards to drive truck! That’s our lively hood and now having a DOT saying we might not be able to drive why is that?? Just because we a health issue that can be resolved by our family dr.

      I agree with you. They threaten our livelihood and careers. I am dealing with the threat of my job loss because my Dr who has been seeing me for 15 years now, has told the Dr at the place who did the Physical, that they will not give me a medication to bring my BP down TWO POINTS because it would be too dangerous for me and the DOT Dr that has seen me ONE time, says no that’s not right that it has to be lower. I don’t FEEL good when it’s lower than my normal. There needs to be some regulation protecting people from medical bullying like this.

        It’s because DOT drivers and their health do not only affect them but every single person on the road.

        The profile of the average truck or bus driver:

        Male.
        More than 40 years of age.
        Sedentary.
        Overweight.
        Smoker.
        Poor eating habits.

        Statistically, drivers are:

        Less healthy than the average person.
        More than two medical conditions.
        Cardiovascular disease prevalent.

        Seems like common sense to me.

      I’m thinking it’s the DOT, and they just want safe drivers, so why would you think a person with issues which effect their driving should be driving long hauls with lethal trucks and school buses. Hey I am dealing with issues too now for my breathing, it doesn’t effect my job yet I might lose my med. cert if I cannot get resolve. I’m just as mad but well those are the rules. you might as well just hand out licenses to anyone if you don’t want to follow the rules.

        I feel it’s not just CDL drivers that may or may not have health issues.
        There’s other professions out that require the same amount of responsibilities.
        Think about manufacturing and the auto industry. And may also pose the same health issues. DOT PHYSICAL are a money market and the cost for one. Can range anywhere from 55-250 dollars. Now we all know that the FMCSA will make our lives more complicated than it is already. They have no clue what this business is all about.
