Indiana CDL Requirements
All Indiana CDL holders are required to be medically qualified to drive a CMV by the means of a valid DOT medical certificate.
- Non-Excepted Interstate
You must meet the Federal DOT medical certificate requirements. - Non-Excepted Intrastate
You must meet the Indiana state medical certificate requirements and endorsements.
CDL Self-Certification
The self-certification process requires you to certify to the Bureau of Motor Vehicles the type of commercial motor vehicle you operate, or intend to operate, as well as acknowledge that you are required to meet the medical requirements for that CDL.
Do a Self Certification online.
- You will need to create a “myBMV” account, if you do not already have one.
- You do not need to submit your medical certificate when you self certify. This information is already on file from when you where issued your CDL.
You may use the automated system to verify that your medical certificate is on file by calling 888-692-6841, option number one (1).
Medical Certificate Renewal
To keep your CDL valid, you are required to keep a valid medical card on file with Indiana BMV. Submit the completed Medical Examination Form (long form) and the Medical Examiner’s Certificate:

MEDICAL EXAMINERS MUST CONTINUE TO ISSUE MEDICAL CERTIFICATES TO ALL DRIVERS
The FMCSA has requested that all medical examiners continue providing drivers with a paper copy of the Medical Examiner’s Certificate – Form MCSA-5876, so that Commercial Driver’s License (CDL) holders can provide a copy to the State licensing agency, and the non-CDL drivers can provide the documentation to their employers and Federal and State enforcement officials.
The Agency is currently completing efforts to put into place an electronic system to enable medical examiners to transmit the medical certificate information from the National Registry system to the State licensing agencies.
Until that system is completed, which compliance date according FMCSA is June 23, 2025, medical examiners must continue to issue paper copies of the medical certificates to drivers who pass the medical exam.
ALL DRIVERS MUST CONTINUE TO CARRY A PAPER COPY OF THEIR MEDICAL CERTIFICATE
Commercial Drivers – You still need to carry a paper copy of your medical card, until FMCSA announces that this is no longer necessary.

I moved from Florida and was working for county government. All government agencies are exempt from the physical requirement in Florida. Planning on doing government roadwork here and was wondering if Indiana requires physicals for government class A drivers?

You will need to check with Indiana DLD to confirm the need or no need for your situation.
I fell in December of 2018 and suffered head trauma. In March of 2019 had a seizure due to the head injury. I’ve been on medication and seizure free. The state of Indiana requires for you to be seizure free for one year. Well it’s been a year got my medical card took my Dr. release disclosed everything to the examiner. Took my card to the bmv got a call that I need an exemption form from FMCSA . So I call for the fifth time four were previously to make sure I didn’t need a document or anything. So the answer was no , tell them my problem so it gets passed on to the medical safety dept. I will receive a e-mail which I did saying since I’m intrastate I fall under state regulations. Federal has nothing to do with it. Well I take a copy of two separate e-mails saying it’s state regulations so they say feel a affidavit saying I’m a intrastate driver and that will clear up the confusion and I will receive a letter which I got saying I need a exemption form from FMCSA. I know it’s a friggin joke. So back to the bmv they call back to the cdl and say oh the problem is my vision test wasn’t filled out go to the examiner and bring it back. So yes I have my card with me show to young lady as she tells the lady I’m looking at it now it’s written in I can hear oh yeah I see it. Well now it’s wait on another letter with information as to what they need. Yes your right it’s going to say we need a exemption form from FMCSA. Someone help please.

It’s a state thing and FMCSA is not really involved in this issue. See if you can go higher up in supervisors with your state bmv.
I live in Indiana and I had a physical prior to starting new employment in construction operating heavy equipment. I was only granted a 3 month because of risk of sleep apnea. My 3 months expired 3 months ago. Can I just go get a new exam? My employer won’t allow me to work without it.

Yes, you can go and get a new medical. If you are not a class A driver, let the examiner know.
I live in Indiana and am not currently working driving a CMV in ANY Capacity.
Do I still need to maintain a Current DOT Physical in order to keep the CDL I just had surgery a week ago and due to finances was not able to get it taken care of prior to the surgery.
I am being caught by a technicality due to the fact I was prescribed a narcotic pain killer ON A POST OP TEMPORARY BASIS that is seemingly threatening to screw me up due to it being in my medical records.

Check with your local DMV to determine the process you will need to go through to maintain your cdl. Once you are released, without restrictions or limitations, get a new medical card. Your local DMV should be able to guide you through the process. And your meds are temporary, so it is unlikely that they will be an issue, once you have been released to return to work.
I just got my Indiana Medical Examiner’s Certificate approval for my DOT card. What is my next step to secure my DOT Medical Card from the BMV and how much does it cost? I intend on driving a vehicle just over the 10,000 lbs wt category. Thank you.

If you got the medical certificate/card, then you have what you need. You may need to send a copy to your BMV. Check with your states BMV for best instructions.

You will need to confirm with your local DLD for the states variances regarding your situation. Then when you go for the medical card examination you’ll have the need documentation ready.

Check with your local DLD. You would fall under the states variances more so than FMCSA. If you need a medical card according to the state, then make sure the examiner understands your states laws and variances.
I have a 12000 GVWR truck in Indiana and do not require DOT numbers because we only haul our own property within the state. Does Indiana require my drivers to get a DOT physical? The Federal gov’t says you need a DOT physical if you are a “commercial driver” involved in “interstate” commerce with GVWR over 10000lb.

You need to check with your local drivers license department for the best answer. Most states are wanting a valid medical card, but some do not require one for situations like yours.

Hello I’m in in Indiana I just took my cdl physical yesterday for class B with air brakes I had leg injuries but I function ok the doctor approved me I told them about the cholesterol meds I was taking except I didn’t tell them about my hydrocodone script I was planning to maybe not take them and was not taking them during the day thought I might drop them because of the stigma with them. Except I rely appreciate being able to take them after the day is over to relieve the days pain and be able to function at home and sleep pain free at night how do I go back and
have them added I don’t want to be dishonest i can get a letter from my doctor. And what repercussions can I expect getting a job I function very well on them I don’t have any of the so called side effects from them they genuinely just take the pain away regards 🙂
If your company does random drug testing you are toast. Hydrocodone is a class 1 controlled substance. It is a derivative of codeine which shouldn’t be a big deal but it is.
My license has been downgraded causemy medical card expired. I just went and got a new physical and passed they now send it to the state on there own how long will it take for my license to be upgraded I’m in indana.

Just the medical certificate (but having a copy of your long form isn’t a bad idea for backup – take a photo of it on your smartphone if you have one!)
I carry CDL licenses in Inpls in…I just took my DOT phycial n was informed that bcuz my BMI was 2 points over I have to have a sleep study…. I was given a 90 day card to have the test done…my problem is having hard time finding a clinic also not sure of the cost n if insurance will pay…? Is what can I do if I can’t get the test done???

BMI only is not a condition that requires a sleep study.

You’ll have to check with your local drivers license department. They may allow you, while you’re not driving, to change your self-certification to “intrastate excepted” so you don’t have to have a current medical card. Or they may have a period of time where you can downgrade your license, then reinstate your CDL again later. Only they can give you they details of how they can help you with their state process.

You need to have an interstate CDL medical card and drivers license. A lot depends on what class driver you are and why you have been limited to intra-state only.

I live in Indiana, I went for a physical in may and only had 1mo. report for my sleep apnea, he said I needed 3 mo. So he gave me a 3mo. card and said get a 3 mo. Report and bring it back before this expires and he will give me the full year card w/o retaking the physical, I did and now they say the dot changed this and now I have to retake the physical, sounds like they make this up as they go, are they correct?

Based on the new FMSCA forms, there are no extensions. Each time is a new physical exam and a new certification that overrides the previous one. Your new card will be a full year starting from your new exam date.
I went for a dot CFL physical and told them I had mild neorupathy in my feet. It’s so mild I don’t take any medicine for it. The doc told me I was disqualified to get a CD’s. Is there an appeal process or can I look for a different Doctor?

Go get a second opinion elsewhere. Have your primary care doctor fill out a medical release form explaining your situation and have that in hand before your next DOT exam.
I live in TN which does not require a dot medical card for driving a box truck under 26000 lbs. I am going to work for a company across the state line in KY and the company is based in Indiana. Ky will not grant me a waiver because I live in TN and tells me to contact TN Dept of Licensing. TN tells me they can’t issue a waiver because they only require a dot medical card for CDL drivers which I will not be. Which state rules and what must I do to get a waiver that TN says I don’t need?

Sounds like your driving category will now be considered “interstate” rather than “intrastate”.
Interstate drivers fall under FMCSA regulations and need a federal medical card. You will also need to self-certify your driving category.
Not sure what waiver you are talking about. But, if you need a waiver for medical qualification, then as an interstate driver you’ll need a Federal waiver.
You should still be able to drive with a TN license, so talk again with the folks at TN drivers license department about self-certifying “interstate”.
