Idaho CDL Requirements
All Idaho CDL holders are required to certify the type of commercial operation they’re engaged in.
Interstate Non-Excepted
You must meet the Federal DOT medical certificate requirements.
Intrastate Non-Excepted
You must meet the Federal DOT medical certificate requirements.
Have a CDL but Not Currently Driving:
If you hold a valid Idaho CDL, but are not actively working, you are not required to maintain a DOT Medical and may certify the driving category as “Excepted Intrastate”. However, should you decide to operate a commercial vehicle again in one of the “non-excepted” categories, you must obtain a current DOT Medical and update your certification by reporting one of the above “actively working” categories before operating a commercial vehicle in a non-excepted category.
CDL Self Certification
All CDL holders must self-certify their commercial vehicle operating category to ITD by July 15, 2013.
Drivers who have not declared a driving category will have CDL privileges withdrawn. It will then be necessary to complete the self-certification process and contact ITD Driver Services at (208) 334-8735 to reinstate Commercial driving privileges.

In Person:
At any County Driver’s License office.
Important Notice: Submitting Your Medical Card
(Information from Idaho Transportation Department – Currently ITD is unable to collect and process DOT Medical Card information.)
CDL and Commercial permit holders who self-certified in a “non-excepted” category will receive a letter from ITD explaining when and where to submit their DOT medical card.
These letters will be mailed to affected drivers in September, 2013. You should not submit a DOT medical card until notified by ITD to do so. Any DOT Medical card submitted before September will be shredded and will have to be submitted again.
After September, 2013, Commercial license and permit applicants who self-certify in a non-excepted category will be required to submit a copy of their DOT medical card at the county DMV when obtaining a CDL or CLP.
Until further notified, Idaho commercial drivers renewing a DOT Physical should continue to provide copies of their DOT Medical card to the Idaho Transportation Department via the methods outlined in the September letter

MEDICAL EXAMINERS MUST CONTINUE TO ISSUE MEDICAL CERTIFICATES TO ALL DRIVERS
The FMCSA has requested that all medical examiners continue providing drivers with a paper copy of the Medical Examiner’s Certificate – Form MCSA-5876, so that Commercial Driver’s License (CDL) holders can provide a copy to the State licensing agency, and the non-CDL drivers can provide the documentation to their employers and Federal and State enforcement officials.
The Agency is currently completing efforts to put into place an electronic system to enable medical examiners to transmit the medical certificate information from the National Registry system to the State licensing agencies.
Until that system is completed, which compliance date according FMCSA is June 23, 2025, medical examiners must continue to issue paper copies of the medical certificates to drivers who pass the medical exam.
ALL DRIVERS MUST CONTINUE TO CARRY A PAPER COPY OF THEIR MEDICAL CERTIFICATE
Commercial Drivers – You still need to carry a paper copy of your medical card, until FMCSA announces that this is no longer necessary.

Tennessee says
I’m a Idaho CDL holder does the state of Idaho have a grace period after your DOT medical card expires
Trucker Docs™ says
@ Tennessee
Officially, there is no grace period. Reality, most states are running a bit behind in getting medical card information into their system, so you may have a couple weeks before the state has an issue with it. DOT officers may not be as forgiving.
Jeff R says
I have a Idaho CDL and no medical card I would like to have my medical card reinstated but I was diagnosed with diabetes Will I still be able to get a medical card
Trucker Docs™ says
@ Jeff R
It will depend on the level or type diabetes you have been diagnosed with. Check with a local CME for guidance.
Indy says
Where do we send our medical clearance to in Idaho. Should we e mail it to Boise DMV?
Judy G says
To my knowledge you take it to your local DMV. That is what the instructions say on my letter telling me to update my medical card.
Elbie m says
I had open heart surgery 11-11-23, got recertification in March. I’m hearing that I have to get a stress test and ecocardigram done every 2 yrs….what Idaho requirements??
Trucker Docs™ says
@ Elbie m
All depends on the class cdl you carry. Class A interstate, stress test every two years, medical card renewal annually. Lesser class cdl’s fall under your states guidelines and variances. Check with your local DLD.
Chris says
Does the address on my commercial driver’s license have to match the one on my medical card?
Trucker Docs™ says
@ Chris
No, but a DOT officer may have some questions about it.
Chris says
But will I get fined?
Gabe says
We do business in Idaho & Oregon with vehicles less than 26,000; do my drivers need medical cards?
Trucker Docs™ says
@ Gabe
Most states require a valid medical card for any vehicle that is used for commerce (make money). Check with your local DLD for confirmation.
Robert C says
I’m in Idaho and My medical card was up 4 months ago when I left my last job. Is there a grace period in which I can just go renew my medical card and have my CDL reinstated or do I need to go through the whole process of getting a CDL again.
Trucker Docs™ says
@ Robert C
This question is going to be best answered by your local drivers license department, since it has to do with your cdl.
Fabian says
My dot medical card expired a couple days ago havent renewed it cause I’m not working at the moment due to wife’s health problems will i get in trouble or lose my cdl?
Trucker Docs™ says
@ Fabian
Most states have a small grace period before you would lose your cdl. Best to get it done ASAP though and get a copy to your local DLD.
Ross the rookie says
So if I have high blood pressure, like 150/110, that disqualify me? I have CDL training in less than a month and if I need to get on meds then I will!
Trucker Docs™ says
@ Ross the rookie
Your present blood pressure will pass for no more than 90 days. That means you will receive a one time 90 day card, allowing you to get your bp below 140/90 or lower. Medication may be necessary, if so get started now.
Mike says
Why can’t you come up with a better card rather that a full size piece of paper?A piece a paper will never last. Why do we even need to carry a card why can’t it be stored in one of your data bases just like every thing else???
Trucker Docs™ says
@ Mike
Take it up with DOT/FMCSA. Take a picture of your card and carry it in your phone. Shrink it down, laminate it, find a wallet size version, be resourceful.
Bill says
The company dot disqufied me in mach but I waited 6 months and saw a doc tor and got my clearance and dot physical do I have to go back to the same do that disqufied me??? Cus the ex company is saying I have to
Trucker Docs™ says
@ Bill
You only have to return to the first DOT examiner if it is the company’s written policy, otherwise your present medical card is valid.
Rodney E. R says
I just completed my physical and i used to fax my copy of the card, now where do send my copy?? Can I still fax it, mail it or do I have to go to the local office??
Trucker Docs™ says
@ Rodney E R
Each state is different, so check with your local drivers license department for your best answer.

@ Kylie
Not that we are aware of from DOT/FMCSA.

Sean Bergener says
The trucks that I drive at work have a GVWR of 24,750 and according to my work I am not required to have a CDL to drive it because of this. Am I still required to have a medical examiner’s card with not having been issued a CDL?? Or is my work required to have us licensed as CDL drivers so that we have to obtain a card?? I know that the laws are really strict and any clarification would help immensely.
Thanks, Sean Bergener
Trucker Docs™ says
@Sean
The need for the medical card is for any driver who is operating a vehicle that is used for commerce. Your state has a drivers classification process that would determine what class drivers license you should have to operate a commercial motor vehicle that is in the 24,750 pound range. Each state is different, so check with your DLD for further information regarding your drivers class of license. You may not need a CDL, but you may need the medical card to keep yourself and the company out of trouble.
