CDL Medical Exam Services
DOT Physical Exam ~ $100
Payment options are cash or credit card.
- You will get original copy of medical certificate and a wallet size medical card.
- We will fax a copy of the medical certificate to your employer or Oregon DMV for you.
DOT Drug Testing
We also do DOT drug testing. This is not a part of the CDL medical exam, unless your company requests at the same time.
We take care of your DOT medical card process and get it done right the first time.

Nice people and in and out in a reasonable time. My last DOT physical was not nearly as pleasant.
This place is Excellent and I was in and out quickly. My DOT exam was thorough. Dr Poon is professional and I will be sending my employees here for their exams!
A very clean, friendly and inviting medical office. All staff are polite and very considerate. Dr Poon is extremely professional, and keeps you informed of the steps throughout the exam, and also the results.
He takes his time to respond to any questions, and provides a very thorough exam.
Dr Poon was able to see me in short notice, and we spent a very comfortable amount of time going through the exam.
I highly recommend Dr Poon for CDL Medical exams.
