CDL Medical Exam Services
DOT Physical Exam ~ $100
Payment options are cash or credit card.
- You will get original copy of medical certificate and a wallet size medical card.
- We will fax a copy of the medical certificate to your employer or Oregon DMV for you.
DOT Drug Testing
We also do DOT drug testing. This is not a part of the CDL medical exam, unless your company requests at the same time.
We take care of your DOT medical card process and get it done right the first time.

Highly recommend you go here for your DOT physical. Very kind, efficient and thorough exam. Staff are wonderful in the waiting area. They make the process very easy. Definitely will return in the future.
A little late writing this…this was my 2nd time visiting Dr. Poon to renew my medical card and he was as great as the 1st visit !!! I would tell everyone to go see him for your medical card…the receptionist is so nice and she even remembered my name !!!!
