Alaska CDL Requirements

Alaska law requires all CDL drivers to have a valid DOT medical card, unless the type of commerce you drive qualifies as ‘Excepted’.

Interstate Non-Excepted
You must meet the Federal DOT medical certificate requirements.

Intrastate Non-Excepted
You must meet the Federal DOT medical certificate requirements.

 

CDL Medical and Self Certifying Verification

If you are applying for an original, duplicate or renewal of your Commercial Driver License (CDL), please take all required forms to your local DMV.

If your CDL is current, you may submit the Self Certification Form and Medical Card in the following manner —
Submit the following two documents:

  1. Medical and Self Certifying Verification – Form 413 – download the form here from Alaska Division of Motor Vehicles.
  2. Copy of your valid Medical Examiner’s Certificate (Medical card only, not the long form.) Enlarge the copy to ensure that all information is legible.

Special Information Updates Pertaining To ALL STATES

MEDICAL EXAMINERS MUST CONTINUE TO ISSUE MEDICAL CERTIFICATES TO ALL DRIVERS

The FMCSA has requested that all medical examiners continue providing drivers with a paper copy of the Medical Examiner’s Certificate – Form MCSA-5876, so that Commercial Driver’s License (CDL) holders can provide a copy to the State licensing agency, and the non-CDL drivers can provide the documentation to their employers and Federal and State enforcement officials.

The Agency is currently completing efforts to put into place an electronic system to enable medical examiners to transmit the medical certificate information from the National Registry system to the State licensing agencies.
Until that system is completed, which compliance date according FMCSA is June 23, 2025, medical examiners must continue to issue paper copies of the medical certificates to drivers who pass the medical exam.

ALL DRIVERS MUST CONTINUE TO CARRY A PAPER COPY OF THEIR MEDICAL CERTIFICATE

Commercial Drivers – You still need to carry a paper copy of your medical card, until FMCSA announces that this is no longer necessary.

    What happens if I no longer use my CDL? I took a job that doesn’t require it and the job doesn’t require drug testing so I was thinking about starting marijuana but I would like to know what will happen to my CDL. Also my medical card is expiring, is it ok to just let it expire because I no longer need it?

      Most state require a valid medical card to maintain a cdl. And if there is no medical card, then the state down grades your cdl to a standard drivers license. Confirm with your local drivers license department.

    If someone’s medical expires do they lose their cdl? and who would they notify that they renewed your medical so that they don’t lose their cdl?

      Most states want a copy of your medical card to maintain your cdl. Check with your local DLD for guidance.

      Most states require a medical card to operate any vehicle that is used in commerce. Not sure about a cdl. Check with your local driver license department for guidance in this situation.

    Can a driver with a non commercial license from Alaska have his medical exam in CA to gain his commercial license in Alaska?

    I have renewed the medical exam in other cases, but just verifying first and initial exam is ok.

      For a class A, yes, because it will be a federal. For a lesser class cdl, it will fall under the states requirements.

    CME’s pass out misinformation in Arizona. First, I was told by Family Medical Walk-in in Prescott Valley that they can only do medical exam if you have an Ariazona license.

    The Conseco on Black Canyon Highway in Phoenix said they cannot do physical if license is expired, even if you have State-issued ID card. If that was true someone getting a CDL for the first time could not get a physical.

    DOT has really made this more difficult. My Primary Care Physician used to do the exam with no problem and put it on my insurance, now it is over $100 if you can find anyone who will do the exam and fill out the form. And I an sue to leave for Alaska to get my new license tomorrow !

      A valid drivers license is all that is needed and you can have your DOT medical examination anywhere in the US, as long as it is a registered CME.

    Actually I was incorrect with my earlier comment about the email being wrong. I just got an email back from the email listed on your site. It does list a different one in the 413 document though. My bad.

    Hi, I have a class B end P. Alaska CDL. My BD is April 2 ,and need renewal this year. I am in Wa.St. And will not be in Alaaska until Early Aug. I have all my papers current. Can I renew on line or over the phone. Thanks

    If my blood pressure is fine and I have no signs of anything else just because I am obese for my height does a physician have the right to draw my blood for a dot medical examiners card to chk for diabetes, thyroid etc…

      The job of the Certified Medical Examiner (CME), is to determine “Medical fitness for duty” to operate a commercial motor vehicle, that’s all.
      He isn’t supposed to make any other diagnosis or treatment protocol calls. So unless there was some special circumstances that presented during your medical examination that prompted him to draw blood, he may be over-stepping his boundaries as a CME for the DOT.
      I do not know the laws in Alaska, there may be some reason for such activity that I am unaware of.

      They test for diabetes through a urine sample. That’s part of the regular testing for a CDL medical exam. They should not be drawing blood at all. That’s not part of the exam. If you are obese however the new rule is that you have to take a sleep apnea test and you have to pass it otherwise if you fail it they will give you an extension on your CDL exam until you’ve worn the pap machine for a certain. Of time and then turned in positive results showing that you use it. And then they will give you your renewed CDL exam
