Wisconsin CDL Requirements
Federal regulations require that all CDL holders self-certify their category of commercial operation by January 30, 2014.
The State of Wisconsin requires all CDL drivers to have a valid DOT medical card, unless the type of commerce you drive qualifies as ‘Excepted’.
Interstate Non-Excepted
You must meet the Federal DOT medical certificate requirements. You are required to submit your Fed Med card to the DMV and keep it updated.
Intrastate Non-Excepted
You must meet the Federal DOT medical certificate requirements. You are not required to submit your Fed Med card to DMV.

Print a copy of your application. Note: The copy is not valid to present to law enforcement or as proof you meet the commercial medical standards. The actual medical certificate and information on your record are the true verification that you meet these standards.
- If you self-certify Interstate Non-Excepted, then you must submit your Fed Med card.

MEDICAL EXAMINERS MUST CONTINUE TO ISSUE MEDICAL CERTIFICATES TO ALL DRIVERS
The FMCSA has requested that all medical examiners continue providing drivers with a paper copy of the Medical Examiner’s Certificate – Form MCSA-5876, so that Commercial Driver’s License (CDL) holders can provide a copy to the State licensing agency, and the non-CDL drivers can provide the documentation to their employers and Federal and State enforcement officials.
The Agency is currently completing efforts to put into place an electronic system to enable medical examiners to transmit the medical certificate information from the National Registry system to the State licensing agencies.
Until that system is completed, which compliance date according FMCSA is June 23, 2025, medical examiners must continue to issue paper copies of the medical certificates to drivers who pass the medical exam.
ALL DRIVERS MUST CONTINUE TO CARRY A PAPER COPY OF THEIR MEDICAL CERTIFICATE
Commercial Drivers – You still need to carry a paper copy of your medical card, until FMCSA announces that this is no longer necessary.

Can you be taking a prescribed prescription of hydrocodone-acetaminophen and still obtain a fed med card.

The medication will trigger questions about why you take these meds. With proper documentation from your prescribing doctors and a medical release form, maybe.
It is not just the medication that is a question, but the physical, mental and emotional condition you are in also.

My license doesn’t expire until 2024. My fed med expired July 2021. Got letter today that my school bus endorsement expired. Does that mean cdl is expired or just that part? Also do I need to get a whole new license? No longer driving bus.

Your cdl and endorsements are all state issued, so check with your local drivers license department for your best answer.

Most states do require a medical card for any driver who operates a vehicle used for commerce. Best check with your local drivers license department for your best answer.

I have a potential employee that is legally blind in one eye. Will he pass the WI DOT physical? Does WI have an exemption form that needs to be completed?

There are exemption forms from the state. As a legal driver they will need to operate within a limited range, usually. Check with your local DLD for proper directions in this case.

It will depend on your class drivers license and what you are transporting. Check with your state drivers license department.
