Wisconsin CDL Requirements

Federal regulations require that all CDL holders self-certify their category of commercial operation by January 30, 2014.

The State of Wisconsin requires all CDL drivers to have a valid DOT medical card, unless the type of commerce you drive qualifies as ‘Excepted’.

Interstate Non-Excepted
You must meet the Federal DOT medical certificate requirements. You are required to submit your Fed Med card to the DMV and keep it updated.

Intrastate Non-Excepted
You must meet the Federal DOT medical certificate requirements. You are not required to submit your Fed Med card to DMV.

 

Self-Certify Online With Wisconsin DMV

  • Click this link to Self-certify Online.
    Print a copy of your application. Note: The copy is not valid to present to law enforcement or as proof you meet the commercial medical standards. The actual medical certificate and information on your record are the true verification that you meet these standards.
  • If you self-certify Interstate Non-Excepted, then you must submit your Fed Med card.
    Upload File types accepted are PDF, JPG, GIF or PNG (not to exceed 5 MB).

If you are not able to upload the file, then you may submit a copy of your medical certificate to the DMV by mail or fax.

Although most certificates are added to your record in two to three business days, it may take up to 10 business days to complete this process.

Special Information Updates Pertaining To ALL STATES

MEDICAL EXAMINERS MUST CONTINUE TO ISSUE MEDICAL CERTIFICATES TO ALL DRIVERS

The FMCSA has requested that all medical examiners continue providing drivers with a paper copy of the Medical Examiner’s Certificate – Form MCSA-5876, so that Commercial Driver’s License (CDL) holders can provide a copy to the State licensing agency, and the non-CDL drivers can provide the documentation to their employers and Federal and State enforcement officials.

The Agency is currently completing efforts to put into place an electronic system to enable medical examiners to transmit the medical certificate information from the National Registry system to the State licensing agencies.
Until that system is completed, which compliance date according FMCSA is June 23, 2025, medical examiners must continue to issue paper copies of the medical certificates to drivers who pass the medical exam.

ALL DRIVERS MUST CONTINUE TO CARRY A PAPER COPY OF THEIR MEDICAL CERTIFICATE

Commercial Drivers – You still need to carry a paper copy of your medical card, until FMCSA announces that this is no longer necessary.

    if i just hold a Class D license but have my federal medical certificate do i have to register it with the dmv?

      Best to confirm with your local drivers license department. In our state it is not needed or required for a lesser class, class D, to report a medical card.

    Does a class h truck with cld being driven into Wisconsin for repair require a dot number since it’s not in commercial use and that’s the only reason it’s being driven across the border?

    I just received my new med card and the examiner linked it to the DMV, my company thinks that Im not allowed to drive until the DMV shows the link, is that true

      Most states are a few days to a couple weeks behind in getting data input to their system. If you have a valid medical card in hand, you are fine to drive.

    If they want me to carry my med card with me at all times why did they stop making them wallet size? Where am I supposed to keep it now?

      If off for a temporary disability, can an expiring fed med card recertification be delayed until healing of the injury?

        If you are out with injuries and your med card expires, you will need to wait until fully released to return to work before you are eligible to do the med cert exam. You may want to talk with your local DLD for guidance through their process, so your cdl is not downgraded.
