Vermont CDL and DOT Medical Card Requirements

Vermont CDL Requirements

If you hold a commercial driver’s license (CDL) or are applying for a CDL, you are required to certify the following information with Vermont Department of Motor Vehicles:

  • The type of driving you are engaged in (interstate or intrastate) and
  • Whether or not you are subject to or exempt from the Federal DOT medical examination requirements.

Interstate Non-Excepted
You must meet the Federal DOT medical certificate requirements.

Intrastate Non-Excepted
You must meet the Federal DOT medical certificate requirements.

All CDL holders must continue to carry all medical certification documents with them at all times even after completing the self-certification process with DMV.

 

Submit Your CDL Medical Certification to Vermont DMV

Submit the following two documents:

  1. CDL Medical Certification – download the form here from Vermont DMV.
  2. Copy of your valid DOT medical card (not the long form). Enlarge the copy to ensure that all information is legible. If your medical examiner’s certificate is only valid with a vision, diabetes or a skills performance evaluation waiver granted by FMCSA, you must also provide a copy of your waiver.

Keep Your Medical Certificate Current With Vermont Agency of Transportation

You must ensure that your medical documentation on file with DMV remains current and accurate. Failure to maintain current medical documentation on file with DMV may result in a suspension and/or downgrade of your CDL.

You may submit your new medical certificate by mail, fax, or email as described above.

Special Information Updates Pertaining To ALL STATES

MEDICAL EXAMINERS MUST CONTINUE TO ISSUE MEDICAL CERTIFICATES TO ALL DRIVERS

The FMCSA has requested that all medical examiners continue providing drivers with a paper copy of the Medical Examiner’s Certificate – Form MCSA-5876, so that Commercial Driver’s License (CDL) holders can provide a copy to the State licensing agency, and the non-CDL drivers can provide the documentation to their employers and Federal and State enforcement officials.

The Agency is currently completing efforts to put into place an electronic system to enable medical examiners to transmit the medical certificate information from the National Registry system to the State licensing agencies.
Until that system is completed, which compliance date according FMCSA is June 23, 2025, medical examiners must continue to issue paper copies of the medical certificates to drivers who pass the medical exam.

ALL DRIVERS MUST CONTINUE TO CARRY A PAPER COPY OF THEIR MEDICAL CERTIFICATE

Commercial Drivers – You still need to carry a paper copy of your medical card, until FMCSA announces that this is no longer necessary.

    I have my CDL can I keep it without my med card? I don’t plan on driving for a year.If I change my mind do I go update my med card?

      Most states require a valid medical card to maintain a cdl. If you let the medical card lapse for a period of time you may need to re-test. Best to confirm with your local DLD for guidance.
