Oregon CDL Requirements
All CDL holders have to certify driving type, and if required keep a valid medical card on file with Oregon DMV. Your certification of driving type tells DMV which qualifications you must meet to hold a CDL.
- Non-Excepted Interstate (NI)
You must meet the Federal DOT medical certificate requirements. - Non-Excepted Intrastate (NA)
You must meet Oregon state qualification requirements standards.
If you cannot meet the Federal DOT medical qualifications, you may qualify for an Oregon Waiver of Physical Disqualification, acceptable for use in intrastate commerce only.
CDL Self-Certification
You’ll be required to certify driving type each time you renew, replace or upgrade your CDL at a DMV office. You might also be required to certify driving type at the same time you submit a Medical Examiner’s Certificate as a result of a written request from DMV.
Submit the following two documents:
- Self-Certification of CMV Driving Type – download the form (735-7369) here from Oregon Department of Transportation.
- Copy of your valid DOT medical card (medical card only, not the long Waiver of Physical DisqualificationWaiver of Physical Disqualificationform). DOT medical cards are often two-sided or have two parts. Ensure you submit a copy that is complete and legible. Enlarge the copy to ensure that all information is legible.
You may fax a legible copy of your medical certificate. However, unless your CDL is in danger of cancellation, DMV suggests you mail a copy of your certificate. To ensure accuracy, write your name and license number legibly on a separate piece of paper enclosed with the documents.

Medical Certificate Update
If you self-certify Non-Excepted and submit a medical certificate, you are required to keep a valid medical card on file with Oregon DMV.
You are responsible for knowing when your Medical Examiner’s Certificate will expire and for providing DMV with a copy of your new Medical Examiner’s Certificate. You must maintain a valid Medical Examiner’s Certificate on record with DMV at all times. If your Medical Examiner’s Certificate expires before DMV receives a new card, you will be sent a CDL cancellation notice that will be effective 30 days later. However, once your medical certificate expires, you may not legally operate a CMV.
Submit a legible copy of your medical certificate (medical card only, not the long form) to the Oregon DMV:

MEDICAL EXAMINERS MUST CONTINUE TO ISSUE MEDICAL CERTIFICATES TO ALL DRIVERS
The FMCSA has requested that all medical examiners continue providing drivers with a paper copy of the Medical Examiner’s Certificate – Form MCSA-5876, so that Commercial Driver’s License (CDL) holders can provide a copy to the State licensing agency, and the non-CDL drivers can provide the documentation to their employers and Federal and State enforcement officials.
The Agency is currently completing efforts to put into place an electronic system to enable medical examiners to transmit the medical certificate information from the National Registry system to the State licensing agencies.
Until that system is completed, which compliance date according FMCSA is June 23, 2025, medical examiners must continue to issue paper copies of the medical certificates to drivers who pass the medical exam.
ALL DRIVERS MUST CONTINUE TO CARRY A PAPER COPY OF THEIR MEDICAL CERTIFICATE
Commercial Drivers – You still need to carry a paper copy of your medical card, until FMCSA announces that this is no longer necessary.

My drivers are NOT CDL drivers, but do cross over the state line from Oregon to Washington. I have always had them get their DOT card. One of my drivers just had to renew his and came back and told me I need to file it with DMV. I have tried to look this up on line but I am not finding it. Again they are not CDL drivers, they driver truck the size of Penske rentals.

Yes, if they cross state lines they need an “interstate” medical card no matter what class drivers license they have, and that medical card needs to be on file with their license at their local drivers license department.
Does Oregon law require truck companies that require CDL and ODOT physicals to pay for the Physical?

We know that California does, but not Oregon unless this is something new.
Our focus on this website is health related questions, specifically federal.

I retired from driving a school bus and now drive a big 5th wheel trailer which I live in. I would like to keep my Oregon CDL. Do I still need a medical certificate if I am not driving commercially?

Yes, you must keep your medical certification current if you want to keep your CDL.
Oregon may allow you to change your self-certification to an “excepted category” so you don’t need a medical card. You’ll need to check with your local drivers license department to see if they will allow you to do so.
My medical card expired and my dsl is no longer valid. If I get my medical card will my cdl become valid again. How long after a medical card expires does a person have to submit a new medical card before having to redo cdl testing?
i guess nobody is responding to this question.. this is the same question i have.. if you are overdue, can you still get your CDL back.
Do non CDL commercial drivers need to have a medical card in Oregon? If you have a WA driver’s license but are driving mostly in OR where do you need to get you’r medical card?

Yes, you need a medical card if you drive for commercial purposes. If you drive in states other than your home state, you need an interstate medical card.
You can get your medical certification done by a certified medical examiner in any state, no matter where you hold your license.
Our friend just had his DOT physical in Oregon by the employer assigned certified Doctor. The new Dr (the last one just retired and every driver has felt the new Dr is a jerk in general) has been requiring sleep apnea studies to pass the physical even though normal blood pressure and blood work, no obesity but overweight. This seems like a way for the Dr to get more money as the sleep studies are expensive and unnecessary in nearly every case. Dr told him it’s because his neck measurement is 18 inches. His neck measurement was 18 inches when he was in high school as he is a big guy!
Is this really a new direction all DOT certified Dr’s are going requiring the sleep study? Is it required by FMCSA?
My husbands neck is 15 3/4 inches, also the same as in high school, average weight & he will need to renew the medical card next year. If you don’t snore, don’t wake up still tired and never get sleepy or doze off driving this is ridiculous.

There are no BMI/neck size regulations by FMCSA for sleep apnea screening. Overall health conditions are considered by the CME for sleep apnea screening.

I have to ask you I was working for pony express courier express uci distributions and a few other companies in the courier business and I am asking i need an odot medical card for physical work to drive and delivery work in a lot of companies and I am also would like to know what else would be wise to have to be able to go back to work some where I also like being able to say I did a drivers training at pony express for my defensive drivers course and test there are a lot of places doing this on line when pony express needed it physically done. They needed to test me in person not with a computer.
The computer does not drive the car. I am looking to get a job in the courier guard work again and the one place asked for an odot medical card that I do not have at this time where should I go to get this done at I live in Gladstone Oregon can this be done with my own doctor.

This is crazy that a tech has to do all this. We don’t drive but once in a bluemoon and its only for a few miles, its not like we are long haul driving. Come on!
