Oklahoma CDL Requirements
The Oklahoma Department of Public Safety (DPS) records your medical certification status and the information on your medical examiner’s certificate on your Commercial driver’s license system (CDLIS) record.
All Oklahoma CDL holders must provide Self-Certification information to the Oklahoma Department of Public Safety regarding the type of commercial motor vehicle operation they drive or expect to drive.
If you are operating Interstate Non-Excepted, you will also be required to submit a current medical examiner’s certificate to obtain a “certified” medical status as part of your driving record.
If you do not provide DPS with your self-certification and your medical examiner’s certificate and any required variance document DPS will notify you that you are no longer medically certified to operate a CMV in Non-Excepted Interstate commerce. DPS will then remove all your CDL privileges from your license.
Submit Your Self Certification Affidavit and Medical Certificate to Service Oklahoma
Submit the following two documents:

- Copy of your valid DOT medical card. Enlarge the copy to ensure that all information is legible.

Keep your Medical Certificate current with Oklahoma DPS
If you self-certify Non-Excepted Interstate and submit a medical certificate, you are required to keep a valid medical card on file with Oklahoma DPS.
If you fail to keep your medical examiner’s certificate up to date, Oklahoma DPS will notify you that you are no longer medically certified to operate a CMV in non-excepted Interstate commerce. DPS will then remove all your CDL privileges from your license.

Special Information Updates Pertaining To ALL STATES
MEDICAL EXAMINERS MUST CONTINUE TO ISSUE MEDICAL CERTIFICATES TO ALL DRIVERS
The FMCSA has requested that all medical examiners continue providing drivers with a paper copy of the Medical Examiner’s Certificate – Form MCSA-5876, so that Commercial Driver’s License (CDL) holders can provide a copy to the State licensing agency, and the non-CDL drivers can provide the documentation to their employers and Federal and State enforcement officials.
The Agency is currently completing efforts to put into place an electronic system to enable medical examiners to transmit the medical certificate information from the National Registry system to the State licensing agencies.
Until that system is completed, which compliance date according FMCSA is June 23, 2025, medical examiners must continue to issue paper copies of the medical certificates to drivers who pass the medical exam.
ALL DRIVERS MUST CONTINUE TO CARRY A PAPER COPY OF THEIR MEDICAL CERTIFICATE
Commercial Drivers – You still need to carry a paper copy of your medical card, until FMCSA announces that this is no longer necessary.

That will require checking with your local drivers license department for their process.
I had a class A since 2007. In December 2021 I needed the star in the upper right corner. I was told I would have to self surrender my class A and obtain a class d here in Oklahoma. I had until 08/31/2023 on my class A. I want my class A back. But must I retake all the test.

Does not make sense. Most people were simply given/sent a new license with the star on it. Would recommend you go to the drivers license department and speak with a supervisor regarding this situation.
Is there somewhere we can check online for current self certification in Oklahoma. I had a to emailed in from state facility in earlyJanuary and it’s now March and my employer is still not seeing this updated in the system

Is there a grace period in Oklahoma on med exam cards.

End of February to get it done and registered.
I had my CDL for almost 20yrs. In 2019 I could not pass my DOT physical because of high blood pressure, so I let my CDL go. Since then I have gotten a new DOT physical and everything is good. Will I have to take the tests again to get my CDL back?

Where is the expiration date located on my Medical Certificate
Do I understand it correctly that if a CDL driver in Oklahoma claims only intrastate driving that a medical card is not required? Is this true?

That may or may not be true. You want to confirm your answer with your local drivers license department. Since each state is different and operates on differing guidelines and requirements, then they are the ones you need to talk to.
