Oklahoma CDL Requirements

The Oklahoma Department of Public Safety (DPS) records your medical certification status and the information on your medical examiner’s certificate on your Commercial driver’s license system (CDLIS) record.

All Oklahoma CDL holders must provide Self-Certification information to the Oklahoma Department of Public Safety regarding the type of commercial motor vehicle operation they drive or expect to drive.

If you are operating Interstate Non-Excepted, you will also be required to submit a current medical examiner’s certificate to obtain a “certified” medical status as part of your driving record.

If you do not provide DPS with your self-certification and your medical examiner’s certificate and any required variance document DPS will notify you that you are no longer medically certified to operate a CMV in Non-Excepted Interstate commerce. DPS will then remove all your CDL privileges from your license.

 

Submit Your Self Certification Affidavit and Medical Certificate to Service Oklahoma

Submit the following two documents:

  1. Oklahoma Commercial Driver Licensee Medical Self-Certification Affidavit – download the form here from Service Oklahoma.
  2. Copy of your valid DOT medical card. Enlarge the copy to ensure that all information is legible.

Keep your Medical Certificate current with Oklahoma DPS

If you self-certify Non-Excepted Interstate and submit a medical certificate, you are required to keep a valid medical card on file with Oklahoma DPS.

If you fail to keep your medical examiner’s certificate up to date, Oklahoma DPS will notify you that you are no longer medically certified to operate a CMV in non-excepted Interstate commerce. DPS will then remove all your CDL privileges from your license.

Special Information Updates Pertaining To ALL STATES

MEDICAL EXAMINERS MUST CONTINUE TO ISSUE MEDICAL CERTIFICATES TO ALL DRIVERS

The FMCSA has requested that all medical examiners continue providing drivers with a paper copy of the Medical Examiner’s Certificate – Form MCSA-5876, so that Commercial Driver’s License (CDL) holders can provide a copy to the State licensing agency, and the non-CDL drivers can provide the documentation to their employers and Federal and State enforcement officials.

The Agency is currently completing efforts to put into place an electronic system to enable medical examiners to transmit the medical certificate information from the National Registry system to the State licensing agencies.
Until that system is completed, which compliance date according FMCSA is June 23, 2025, medical examiners must continue to issue paper copies of the medical certificates to drivers who pass the medical exam.

ALL DRIVERS MUST CONTINUE TO CARRY A PAPER COPY OF THEIR MEDICAL CERTIFICATE

Commercial Drivers – You still need to carry a paper copy of your medical card, until FMCSA announces that this is no longer necessary.

    Can you show me the website or give me a link so I can download the Health form for CDL driver to take to the doctor for my physical? My doctor told me I need to bring this document to him.

      You need to check with Oklahoma DMV for the best answer. Most states just want to see a valid medical card as soon as possible before forcing you to redo cdl.

    I have a class A CDL in Oklahoma. I was stoped in a personal vehicle and discovered my license had expired the day befor. Is there a grace period at all on the license??

    Is it true that a person needs a sleep study for sleep apnea in order to pass the DOT physical? My husband went for his physical and was told that he had to have one done. He does not have any medical issues that put him at risk for sleep apnea.

      Go some place else for a second opinion. There is no mandate for sleep studies by the FMCSA, unless there are a multitude of other existing symptoms.

    I have not been using my license for a little while now. I have a class A cdl but my medical card expired about a year ago. My license itself has not expired but wil I need to re test in order to get my cdl back to active? Or just a medical certification?

      Depending on which state you live in, your cdl may have been downgraded without your knowledge. Check with your local drivers license department to get the best answers.

      If you have a valid CDL, then yes you need a medical certificate. Most states invalidate a drivers CDL if they do not have a valid medical card on file with the state. Check with your local drivers license department to confirm your CDL.

        IF YOU LIVE IN OKLAHOMA AND ONLY DRIVE INTRASTATE NOT INTERSTATE THE KEY WORD IS INTRASTATE YOUR LICENSE MUST HAKE A K RESTRICTION I HAVE HAD MINE FOR YEARS AND ALL I HAVE TO DO IS SIGN A PAPER WHEN I RENEW SAYING I DONT CROSS STATE LINES THEN YOU GET THE K ON YOUR LICENSE
