Oklahoma CDL Requirements
The Oklahoma Department of Public Safety (DPS) records your medical certification status and the information on your medical examiner’s certificate on your Commercial driver’s license system (CDLIS) record.
All Oklahoma CDL holders must provide Self-Certification information to the Oklahoma Department of Public Safety regarding the type of commercial motor vehicle operation they drive or expect to drive.
If you are operating Interstate Non-Excepted, you will also be required to submit a current medical examiner’s certificate to obtain a “certified” medical status as part of your driving record.
If you do not provide DPS with your self-certification and your medical examiner’s certificate and any required variance document DPS will notify you that you are no longer medically certified to operate a CMV in Non-Excepted Interstate commerce. DPS will then remove all your CDL privileges from your license.
Submit Your Self Certification Affidavit and Medical Certificate to Service Oklahoma
Submit the following two documents:
- Oklahoma Commercial Driver Licensee Medical Self-Certification Affidavit – download the form here from Service Oklahoma.
- Copy of your valid DOT medical card. Enlarge the copy to ensure that all information is legible.

Keep your Medical Certificate current with Oklahoma DPS
If you self-certify Non-Excepted Interstate and submit a medical certificate, you are required to keep a valid medical card on file with Oklahoma DPS.
If you fail to keep your medical examiner’s certificate up to date, Oklahoma DPS will notify you that you are no longer medically certified to operate a CMV in non-excepted Interstate commerce. DPS will then remove all your CDL privileges from your license.

Special Information Updates Pertaining To ALL STATES
MEDICAL EXAMINERS MUST CONTINUE TO ISSUE MEDICAL CERTIFICATES TO ALL DRIVERS
The FMCSA has requested that all medical examiners continue providing drivers with a paper copy of the Medical Examiner’s Certificate – Form MCSA-5876, so that Commercial Driver’s License (CDL) holders can provide a copy to the State licensing agency, and the non-CDL drivers can provide the documentation to their employers and Federal and State enforcement officials.
The Agency is currently completing efforts to put into place an electronic system to enable medical examiners to transmit the medical certificate information from the National Registry system to the State licensing agencies.
Until that system is completed, which compliance date according FMCSA is June 23, 2025, medical examiners must continue to issue paper copies of the medical certificates to drivers who pass the medical exam.
ALL DRIVERS MUST CONTINUE TO CARRY A PAPER COPY OF THEIR MEDICAL CERTIFICATE
Commercial Drivers – You still need to carry a paper copy of your medical card, until FMCSA announces that this is no longer necessary.

does okla.have avision waivier for cdl.i have 20-225 vision in one eye due to an acident
When I was living in Oklahoma, I had a “K” hold put on my commercial license since I wasn’t driving a truck at the time. I’m getting ready to go back to truck driving, but I’m moving to Washington. Will Washington state release my K hold or do I need to do it in Oklahoma before I move?
can I get a physical from a Nationally registered doctor from another state and report it to Oklahoma with the required forms and still maintain my CDL status?
Yes.
An online website with Your States DPS, would be convenient to reference when checking or confirming whether or not your current years medical certificate is validated. Just saves a phone call or trip to DPS that validates the driver for the current year with no surprises on the job.
Will the CDL holder have to return to home state/Ok to renew med-card, Or at all possible mail something in.

My husband has a CDL with X endorsement. He recently had an ICD put in. What are the Oklahoma disqualifiers and requirements to ask for a waiver for inter and/or intra state? He has been driving for 30 years with no infractions or health problems.

To confirm or deny what you need for the state you want to check with the local state drivers license department. Each state is different and you want to know about your state.
Really needing some info and a way to get passed an issue
I went to test for my cmv license and was denied due to a visual medical issue.
I don’t know what else to do as of now.
I cant pass the initial eye exam due to some vision loss due to lazy eye
thinking about sending in a exemption waiver to the feds
anyone ever know of the success rate in this process?

If you pass everything else on the DOT examination, other than vision, then you want to check with the state and the FMCSA about a vision waiver. As long as you can perform the duties and pass the skills performance portion of the vision test, then you should be good to go. We see plenty of drivers with the exemption, so it is do-able.
I wish that doctors were allowed to mark the medical card expiration date as the last day of the month when you get a physical, so the renewal wouldn’t always creep earlier and earlier in the month. I guess it doesn’t make sense to me why the renewal for the medical card wouldn’t work just like CDL renewal.
