Oklahoma CDL Requirements
The Oklahoma Department of Public Safety (DPS) records your medical certification status and the information on your medical examiner’s certificate on your Commercial driver’s license system (CDLIS) record.
All Oklahoma CDL holders must provide Self-Certification information to the Oklahoma Department of Public Safety regarding the type of commercial motor vehicle operation they drive or expect to drive.
If you are operating Interstate Non-Excepted, you will also be required to submit a current medical examiner’s certificate to obtain a “certified” medical status as part of your driving record.
If you do not provide DPS with your self-certification and your medical examiner’s certificate and any required variance document DPS will notify you that you are no longer medically certified to operate a CMV in Non-Excepted Interstate commerce. DPS will then remove all your CDL privileges from your license.

Keep your Medical Certificate current with Oklahoma DPS
If you self-certify Non-Excepted Interstate and submit a medical certificate, you are required to keep a valid medical card on file with Oklahoma DPS.
If you fail to keep your medical examiner’s certificate up to date, Oklahoma DPS will notify you that you are no longer medically certified to operate a CMV in non-excepted Interstate commerce. DPS will then remove all your CDL privileges from your license.
Submit a legible copy of your medical certificate (medical card only, not the long form) to Service Oklahoma:

Special Information Updates Pertaining To ALL STATES
MEDICAL EXAMINERS MUST CONTINUE TO ISSUE MEDICAL CERTIFICATES TO ALL DRIVERS
The FMCSA has requested that all medical examiners continue providing drivers with a paper copy of the Medical Examiner’s Certificate – Form MCSA-5876, so that Commercial Driver’s License (CDL) holders can provide a copy to the State licensing agency, and the non-CDL drivers can provide the documentation to their employers and Federal and State enforcement officials.
The Agency is currently completing efforts to put into place an electronic system to enable medical examiners to transmit the medical certificate information from the National Registry system to the State licensing agencies.
Until that system is completed, which compliance date according FMCSA is June 23, 2025, medical examiners must continue to issue paper copies of the medical certificates to drivers who pass the medical exam.
ALL DRIVERS MUST CONTINUE TO CARRY A PAPER COPY OF THEIR MEDICAL CERTIFICATE
Commercial Drivers – You still need to carry a paper copy of your medical card, until FMCSA announces that this is no longer necessary.

Trucker Docs™ says
@ Ronney
Best check with DMV, not us.
Evelyn says
Is it true that a person needs a sleep study for sleep apnea in order to pass the DOT physical? My husband went for his physical and was told that he had to have one done. He does not have any medical issues that put him at risk for sleep apnea.
Trucker Docs™ says
@ Evelyn
Go some place else for a second opinion. There is no mandate for sleep studies by the FMCSA, unless there are a multitude of other existing symptoms.
Craig says
I have not been using my license for a little while now. I have a class A cdl but my medical card expired about a year ago. My license itself has not expired but wil I need to re test in order to get my cdl back to active? Or just a medical certification?
Trucker Docs™ says
@ Craig
Depending on which state you live in, your cdl may have been downgraded without your knowledge. Check with your local drivers license department to get the best answers.
Joel K sr says
Do i need a medical certificate if I only drive intrastate in Oklahoma on my cdl
Trucker Docs™ says
@ Joel
If you have a valid CDL, then yes you need a medical certificate. Most states invalidate a drivers CDL if they do not have a valid medical card on file with the state. Check with your local drivers license department to confirm your CDL.
ALLEN L says

When I was living in Oklahoma, I had a “K” hold put on my commercial license since I wasn’t driving a truck at the time. I’m getting ready to go back to truck driving, but I’m moving to Washington. Will Washington state release my K hold or do I need to do it in Oklahoma before I move?
Jo L says
can I get a physical from a Nationally registered doctor from another state and report it to Oklahoma with the required forms and still maintain my CDL status?
Trucker Docs™ says
Yes.
Bryan K. S. says
An online website with Your States DPS, would be convenient to reference when checking or confirming whether or not your current years medical certificate is validated. Just saves a phone call or trip to DPS that validates the driver for the current year with no surprises on the job.
TOM G says
Will the CDL holder have to return to home state/Ok to renew med-card, Or at all possible mail something in.
How does one go about cleaning up your MVR
Trucker Docs™ says
@Tom
Read the article above: Keep your Medical Certificate current with Oklahoma DPS – you can submit a legible copy of your medical certificate (medical card only, not the long form) to the Oklahoma DPS by mail, fax, or email.
Can’t help you with cleaning up your MVR, you’ll need to contact DPS.

My husband has a CDL with X endorsement. He recently had an ICD put in. What are the Oklahoma disqualifiers and requirements to ask for a waiver for inter and/or intra state? He has been driving for 30 years with no infractions or health problems.
Trucker Docs™ says
@Pam
To confirm or deny what you need for the state you want to check with the local state drivers license department. Each state is different and you want to know about your state.
landon says
Really needing some info and a way to get passed an issue
I went to test for my cmv license and was denied due to a visual medical issue.
I don’t know what else to do as of now.
I cant pass the initial eye exam due to some vision loss due to lazy eye
thinking about sending in a exemption waiver to the feds
anyone ever know of the success rate in this process?
I don’t know what to do any help is very appreciated
Trucker Docs™ says
@Landon
If you pass everything else on the DOT examination, other than vision, then you want to check with the state and the FMCSA about a vision waiver. As long as you can perform the duties and pass the skills performance portion of the vision test, then you should be good to go. We see plenty of drivers with the exemption, so it is do-able.
Brian says
I wish that doctors were allowed to mark the medical card expiration date as the last day of the month when you get a physical, so the renewal wouldn’t always creep earlier and earlier in the month. I guess it doesn’t make sense to me why the renewal for the medical card wouldn’t work just like CDL renewal.
Dennis Smith says
When the change came around I had turned my form in to get a K endorsement on my CDL, license renews in 2016, will I have to provide a medical certificate to renew and keep my CDL or what is the new process.
Trucker Docs™ says
@Dennis
This is going to be a state by state issue, so check with your local drivers license department.

The e-mail address that you have as “ktiger@dps.state.ok.us” is no longer a valid e-mail address and comes back as undeliverable. All correspondence needs to be sent to the “mec@dps.state.ok.us” address only. I learned this after being on hold with the OK DPS for a lengthy period.

My Medical card came due before payday. What happens if it is due 8 days before able to renew? Is there any grace period? Going to renew the medical card but I will be a few days late. Will my CDL be revoked?
scott james says
I did not get a yearly CDL physical in june of 2015, due to illness in my family,and i have been downgraded to a class D. I am currently not working, due to caring for the ill family member, can i get a physical soon and have the downgrade removed?
Trucker Docs™ says
@Scott
Check with your state DLD to make that determination, about re-instating your CDL. Then go get your physical done.
In most states, they are fairly reasonable to what and where the drivers are going through. They may charge a small fee to reinstate your CDL. But each state is different, so check with your local drivers license department.
Kurt says
What is the time limit for registering a new medical card? I just found out by my current employer that my cdl has already been downgraded. I never received a letter. Why can’t the medical facilities which perform the examinations be linked to this database? Eliminate a step and simplify the process. My employer has submitted the required documentation TWICE and still no upgrade. Such an inefficient system is only hurting those who are simply trying to make a living.
Aaron says
I have lived at the same address for 15 yrs and never received one letter from dps or the state on the new cdl extension. We went through alot to just get answers of what was going on. I don’t think we should have to retest if we didn’t know about this medical cert.
david says
the state of oklahoma has said they have sent letters to all cdl drivers is a out right lie. i never got one and have talked to 5 other drive,s that i know and they never got one either.i have my med card that show,s that i,m legal. and i,m just a intrastate driver and by law that,s all i need, i,m not crossing state line,s. and now the fine state of oklahoma is trying to make me retest after having a cdl for 23yrs what a joke.
Robyn Carpenter says
my question is my husband works for the county and has many endorsements on his
CDL and it is time to renew his license. When he worked for other companies he has gone in for dot physical and received a medical card. The County H/R is saying that they are exempt from requiring their employees who have endorsements to have a dot physical or carry a medical card. But common sense is saying this can’t be right. Could you please clear our question up.
Trucker Docs says
@Robyn
County drivers may be exempt. Check with Oklahoma DPS on this. See more information on our blog about the new FMCSA medical certification requirements.
If your husband is able to qualify for a medical card then it makes sense to certify in the commercial category that gives him the most driving opportunities i.e. Interstate non-excepted. In this case he will need a current medical certificate when he renews his CDL.
Hope this helps to clear up your question.
