Special Information Updates Pertaining To ALL STATES
MEDICAL EXAMINERS MUST CONTINUE TO ISSUE MEDICAL CERTIFICATES TO ALL DRIVERS
The FMCSA has requested that all medical examiners continue providing drivers with a paper copy of the Medical Examiner’s Certificate – Form MCSA-5876, so that Commercial Driver’s License (CDL) holders can provide a copy to the State licensing agency, and the non-CDL drivers can provide the documentation to their employers and Federal and State enforcement officials.
The Agency is currently completing efforts to put into place an electronic system to enable medical examiners to transmit the medical certificate information from the National Registry system to the State licensing agencies.
Until that system is completed, which compliance date according FMCSA is June 23, 2025, medical examiners must continue to issue paper copies of the medical certificates to drivers who pass the medical exam.
ALL DRIVERS MUST CONTINUE TO CARRY A PAPER COPY OF THEIR MEDICAL CERTIFICATE
Commercial Drivers – You still need to carry a paper copy of your medical card, until FMCSA announces that this is no longer necessary.

If my medical card expires how long do I have to renew? Will this cause me to lose my CDL?
How do I report a failed DOT random test to the State of New Mexico. Is there a specific form that must be completed by the MRO and DER. I have looked all over your website and can not find it.
Respectfully,

@Paula
In most cases the positive random test results are given to the employer. It then becomes the responsibility of the employer to have the employee go through a substance abuse program and is given another chance. He would be tested more routinely and more frequently, demonstrating proper compliance. Don’t know that it is your responsibility to report it to anyone else. If you feel that you must report this incidence, then check with the local state level DOT and drivers license department and see what they have to say. This would be a state issue not a federal issue.
Got shut down due to my updated medical card not attach or matching with my class A. The card and drivers license are current, but does not match in computer system. Could I resolve this with the state Mvd over phone?
@William
Call you local MVD to find out what they need. Only they can help you.
Hello I have a cdl class A full endorsements went for my re- certification on my medical card pass for only 3 months due to hypertension this is resolve now but went to go back for a I year certification and could not get a appt in time due to clinic saying they’re were under staff for this process so my time elapsedI have not been driving for three years have drove a tanker for 11 and a half years with no incidents my medical card has expired 7/15/2015 but my cdl is good till end of this year is there any thing I can do or what can I do to get this resolve so I can go back to work ? Thanks for your response sir!
@Robert
If you can’t get into the original examiner, then go to another one.
If your BP is within limits, you will get a one year card from them.
You only get one three month card to get your BP under control, so make sure you’re ready before you go in for the DOT examination.
Do I need to send in a copy of all three pages of my examination report? What about a copy of the actual card? How much time do I have to send it after I get it done. If I send it through the mail how will I know if they got it? Do they send a receipt? Can I take pictures ofth and email them. If so is it the Helpdesk Email that I send it through?
@Dawn
Sorry we cannot find any additional information to what is already listed above.
Yes your best option is to go through Helpdesk email: NMCDL.HelpDesk@state.nm.us
I am working for a company out of Indiana. I got a DOT physical and drug screen through their doctor. I also filled out the self certification form. Do I still need to send a copy if the doctors certification to DMV?
@Brett
Yes, you need to send a copy of your medical card, and self-certification form to your state DMV.
I have a driver that has just informed me that his regular doctor visit has resulted in his diagnosis, with diabetes, (insulin recommended) and high blood pressure. I know the regulations on the hbp. but not sure on the diabetes. Is there any concerns of my driver loosing his cdl. or being declined a medical clearance. Other than the hbp not being controlled.
thank you in advance.
@Paul
It will depend on the over-all diagnosis.
If insulin diabetic, he will have to go through the FMCSA to acquire a insulin waiver. NO Driving during that process.
HBP he will need to have his BP below 140/90 to be able to pass the DOT physical.
He might get away with diet and medications (oral) for treatment of his diabetes. Right now it’s about getting it all under control.
I have looked though as many regulations as I can find and don’t se an answer to this question. If I get a medical card in one state then move to another state six months later do I need to renew my medical card? The reason I ask is on the bottom of the medical card it lists your cdl drivers license number and issuing state. will the new state have a problem with the cdl numbers being different?
@Carl
Overall there is no real problem.
Once you have your new state issued drivers license, you have a couple of choices.
1. You could go back to the original DOT examiner and ask if he could issue you a new medical certificate that matches your new state license, but keeps the old date of expiration. – OR –
2. You could do nothing and just be willing to explain to a DOT officer that you moved following your last examination, (try to keep an invalid copy of your old drivers license will be a big help).
But there is not a law or regulation that makes it mandatory for the driver’s license and medical card to match, but it does reduce questions.
