New Jersey CDL Requirements

Federal Motor Carrier Safety Administration requires all New Jersey CDL holders to certify the type of commercial operation they’re engaged in.

Starting in July 2012, when you apply for a CDL you will be required to self-certify as to what type of commercial operation is applicable by signing a CDL Holder Self-Certification.

All current CDL holders should wait until they receive a notice from the MVC before they submit a Self-Certification.

Interstate Non-Excepted
You must meet the Federal DOT medical certificate requirements.

Intrastate Non-Excepted
You must meet the Federal DOT medical certificate requirements. New Jersey regulations require a medical certificate for intrastate drivers.

Submitting your Self-Certification and Medical Certificate to New Jersey Motor Vehicle Commission

Submit the following two documents:

  1. New Jersey MVC CDL Holder Certification – download the form here from New Jersey Motor Vehicle Commission.
  2. Copy of your valid DOT medical card. Enlarge the copy to ensure that all information is legible.

All current CDL holders should wait until they receive a notice from the MVC before they submit a Self-Certification. It must be completed in full and mailed to the MVC in the enclosed self- addressed envelope.

By Mail:
New Jersey Motor Vehicle Commission

PO Box 685
Trenton, NJ 08666

Keep Your Medical Card Current with New Jersey MVC

In accordance with Federal Motor Carrier Safety Administration regulations, New Jersey CDL holders must submit a valid (non-expired) medical certificate to the New Jersey Motor Vehicle Commission.

Failure to submit a valid Medical Certificate or a Medical Variance may result in your CDL being disqualified. The MVC will notify you that you are no longer medically-certified to operate a CMV in non-excepted Interstate or intrastate commerce. If the MVC still does not receive a valid medical certificate, they will remove all your CDL privileges and downgrade your license to a basic driver license.

If your CDL is disqualified, you will not be legally permitted to operate a commercial motor vehicle. If your CDL is disqualified, you may also be required to retest prior to reinstatement of your CMV operating privileges.

Submit your renewed valid medical certificate to New Jersey MVC…

Special Information Updates Pertaining To ALL STATES

MEDICAL EXAMINERS MUST CONTINUE TO ISSUE MEDICAL CERTIFICATES TO ALL DRIVERS

The FMCSA has requested that all medical examiners continue providing drivers with a paper copy of the Medical Examiner’s Certificate – Form MCSA-5876, so that Commercial Driver’s License (CDL) holders can provide a copy to the State licensing agency, and the non-CDL drivers can provide the documentation to their employers and Federal and State enforcement officials.

The Agency is currently completing efforts to put into place an electronic system to enable medical examiners to transmit the medical certificate information from the National Registry system to the State licensing agencies.
Until that system is completed, which compliance date according FMCSA is June 23, 2025, medical examiners must continue to issue paper copies of the medical certificates to drivers who pass the medical exam.

ALL DRIVERS MUST CONTINUE TO CARRY A PAPER COPY OF THEIR MEDICAL CERTIFICATE

Commercial Drivers – You still need to carry a paper copy of your medical card, until FMCSA announces that this is no longer necessary.

Go get a new medical certification card and get it registered with the state. If there are issues, it will be with the state DLD.

Hello, My cdl license is not expiring for a year. But my medical exam is due. I have not received any paperwork from dmv in which I get every year. Should I wait for the forms or just get my exam and send it in? I’m very concerned it usually expires the end of January. Thanks so much

Best to go ahead and get the examination done and get the paperwork completed. Not sure about waiting on the government to do anything.

Don’t wait! My husband never received anything from DMV! He was very sick and in and out of the hospital so he never gave it a thought! 2 weeks ago letter in the mail stating he was stripped of his CDL as of the previous day! We all know how DMV is right now! Don’t wait!

If your husband was in and out of the hospital, then the DMV should take this into consideration and re-instate his cdl, if he is ready to return to work. Most states don’t strip the cdl, they may invalidate, but you have reason for missing deadline. Get it done as soon as he is ready to go to work. Also check with the states drivers license department right now for guidance in this situation.

What happens if they downgrade my CDL license due to not send the medical exam, can I reinstate it later or is not recommended?
I don’t want to have it anymore, but don’t want to lose it completely.
    Thanks for any information.

Is a Medical Examiner’s Card applicable to Non-CDL (Class D) drivers?

If you are operating a vehicle that is used for commerce, then your state may require a medical certificate even though you are a class D driver. Best to confirm with your local drivers license department.

A medical card without a CDL is required for driving any vehicle over 10,000 pounds less than 26000 lbs. Transporting over 8 passengers for compensation, 15 passengers not for compensation including driver in NJ also requires a medical card whether or not CDL is required. NJ has adopted the Interstate rules or Intrastate travel within NJ.

Work with your doctor to get the cancer cleared up. You will need to contact your local drivers license department and explain your situation. They should guide you through their process for maintaining your license while you are under care.

If you are trying to upload the medical card for your boyfriend, then it needs to be sent to the New Jersey drivers license department. Touch base with them, they should be able to help.

Each state is different and regulations are state specific. Most states now require a valid medical card to operate any vehicle that is used for commerce, to make money. Some states for simple delivery cars or pickup trucks and others need to have a cdl. Check with your state drivers license department for the best answer.
