Nebraska CDL Requirements
The State of Nebraska requires all CDL drivers to have a valid DOT medical card, unless the type of commerce you drive qualifies as ‘Excepted’.
Interstate Non-Excepted
You must meet the Federal DOT medical certificate requirements.
Intrastate Non-Excepted
You must meet the Federal DOT medical certificate requirements.
Effective January 1, 2012 , all individuals holding a Nebraska Commercial Driver’s License (CDL) are required to certify to the DMV what type of operation they engage in no later than January 30, 2014. In addition, if you operate in Interstate Non-Excepted, you are required to provide the DMV with a copy of your Medical Examiner’s Certificate and keep current with the DMV.
Drivers who cannot meet these requirements or do not wish to retain their CDL can visit any driver licensing office and downgrade their CDL to a Class O (car license). This downgrade must take place by the required deadline in the letter. Failure to provide the appropriate documentation or to downgrade to a Class O license by the required deadline in the letter will result in the cancellation of your CDL.
If you have kept your address current with the DMV, a letter will be sent to you from the DMV between January 1, 2012 and January 30, 2014, requesting you to complete a Self Certification Categories Certificate. You will be required to return this form and any accompanying documentation to the DMV main office by the required deadline in the letter.
If you do not receive a letter from the DMV by December 1, 2013, follow the process below:

- Copy of your valid DOT medical card (Medical card only, not the long form.) Enlarge the copy to ensure that all information is legible.

Keep Your Medical Certificate Current with Nebraska DMV
If you certify as Category A: Interstate and Non-Excepted, it will be your responsibility to keep your medical examiner’s certificate current with the DMV at all times. Please be aware that the Nebraska DMV will not be sending you reminders to keep this information current.
In the future, submit your renewed medical examiner’s certificate (medical card – not long form) to the DMV prior to the expiration of your current medical examiner’s certificate. If you fail to do so, your Commercial Driver License will be cancelled.

MEDICAL EXAMINERS MUST CONTINUE TO ISSUE MEDICAL CERTIFICATES TO ALL DRIVERS
The FMCSA has requested that all medical examiners continue providing drivers with a paper copy of the Medical Examiner’s Certificate – Form MCSA-5876, so that Commercial Driver’s License (CDL) holders can provide a copy to the State licensing agency, and the non-CDL drivers can provide the documentation to their employers and Federal and State enforcement officials.
The Agency is currently completing efforts to put into place an electronic system to enable medical examiners to transmit the medical certificate information from the National Registry system to the State licensing agencies.
Until that system is completed, which compliance date according FMCSA is June 23, 2025, medical examiners must continue to issue paper copies of the medical certificates to drivers who pass the medical exam.
ALL DRIVERS MUST CONTINUE TO CARRY A PAPER COPY OF THEIR MEDICAL CERTIFICATE
Commercial Drivers – You still need to carry a paper copy of your medical card, until FMCSA announces that this is no longer necessary.

We have a driver that his DOT Physical card expires on 9-28-23. He went to get his physical done on 9/21/23 . He hasn’t been feeling well and due his breathing they would not do the physical and told him he has 45 day grace period from 9/28/23. we had never heard that and so I called the DMV and she said it is a 60 day grace period. He can drive for up to 60 days from 9/28/23 without an updared DOT Physical. Is this TRUE? I just don’t want to tell our driver the wrong information

States are running behind on getting med cards on file. Some states have no grace period, like CA, and other have 45 to 60
days grace. If he is stopped by a federal DOT officer, there may be no grace period. Go by what your state is saying and you should be alright.
Is there a way that the NE CDL self-certification form can be fillable and e-mailed online?

It’s a good question for NE drivers license department.
I’m out on workman’s comp. Due to a work related accident. I just received a notice in mail that my physical card expires in 4 days. In the condition surgery has put me in I couldn’t pass the physically able part of requirements. After I complete my physical therapy it won’t be a problem. Is there a grace period so I don’t hav to start from scratch? (Rotary cuff )

There is no grace period and each state has a process for situations like yours. Talk with your local drivers license department (DLD) for guidance.

Are you required to carry the long form in your truck as well as your Medical Card??

We believe that, by law, you must carry your medical card while driving any vehicle. The long form is not a mandatory. The last statement on the long form indicates that the medical card is sufficient. If in doubt, take pictures of your long form on your phone.
I had my CDL canceled because my medical physical had expired,I was out of the country at the time.After diving truck for 30 yrs I had to go through the entire license process again .
I would hope in the future the Great State of Nebraska would consider dropping the license to a class O instead. Thanks for your consideration.
I had a medical card about 4 years ago with a vision waiver and could only drive within nebraska. Is that waiver still available or am I sol??

The waiver would have come from your states drivers license department. Best to check with them for confirmation.
