North Carolina CDL Requirements
No later than January 30, 2014, all CDL holders must provide information to the North Carolina DMV regarding the type of commercial motor vehicle operation they drive or expect to drive.
Drivers operating in non-excepted types of commerce will be required to submit a current DOT medical examiner’s certificate to the NC DMV to obtain a “certified” medical status as part of their driving record.
If you do not provide NC DMV with your self-certification and if required, your medical examiner’s certificate and any required variance document by January 30, 2014 – NC DMV will notify you that you are no longer medically certified to operate a CMV in non- excepted interstate/non-excepted intrastate commerce. You will have 30 days to respond or your entire NC driving privileges will be cancelled.

Submit your Self-Certification and Medical Certificate to North Carolina DMV
You are required to self-certify to a single type of commercial operation on your driver license. If you self-certify to non-excepted interstate or non-excepted intrastate, you must provide NC DMV with a copy of your current DOT medical examiner’s certificate and any applicable variance document.
Submit the following two documents:
- North Carolina Certification for CDL – download the form CDL-5 here from NC DMV.
- A copy of your valid DOT medical certificate. Enlarge the copy to ensure that all information is legible.

Keep Your Medical Card Current With North Carolina DMV
CDL holders required to have a ”certified” medical status who fail to provide and keep up-to-date their DOT medical examiner’s certificate with the NC DMV will become ”not-certified” and may lose their CDL.
If your medical card or variance expires and you do not provide NC DMV with a new one, the DMV will notify you that you are no longer medically certified to operate a CMV in non- excepted interstate/non-excepted intrastate commerce. You will have 30 days to respond or your entire NC driving privileges will be cancelled.
Before your medical card expires, you must have a new medical examination and obtain a new DOT medical examiner’s certificate.

MEDICAL EXAMINERS MUST CONTINUE TO ISSUE MEDICAL CERTIFICATES TO ALL DRIVERS
The FMCSA has requested that all medical examiners continue providing drivers with a paper copy of the Medical Examiner’s Certificate – Form MCSA-5876, so that Commercial Driver’s License (CDL) holders can provide a copy to the State licensing agency, and the non-CDL drivers can provide the documentation to their employers and Federal and State enforcement officials.
The Agency is currently completing efforts to put into place an electronic system to enable medical examiners to transmit the medical certificate information from the National Registry system to the State licensing agencies.
Until that system is completed, which compliance date according FMCSA is June 23, 2025, medical examiners must continue to issue paper copies of the medical certificates to drivers who pass the medical exam.
ALL DRIVERS MUST CONTINUE TO CARRY A PAPER COPY OF THEIR MEDICAL CERTIFICATE
Commercial Drivers – You still need to carry a paper copy of your medical card, until FMCSA announces that this is no longer necessary.

I received a letter stating that my commercial driver license will be cancelled, however I took my Self Certification Form and Medical Examiner’s Certificate sign and dated down to the DMV Headquarters Building 1100 New Bern Av. After my personal experience in submitting my documentation to the NC dot, I want to build a list of other NC CDL holders who have submitted their medical examiner certificates this year and had it rejected or unprocessed due to an invalid administrative claim.
EXAMPLE: Incomplete information, Illegible, not received, etc.
If there are enough CDL holders at risk of losing or their CDL status due to an in-valid NC DOT administrative claim after the jan 30th deadline and has their CDL revoked it is grounds for a class action lawsuit.
I’ve already given the NC Dot Commissioner Kelly J. Thomas an opportunity to respond to this issue.
If an administrative claim by the NC DOT is believed to be in-valid and you have proof it is grounds for a contested case after administrative processes have been exhausted, contact me.
Shoot me your name, your problem with the medical cert process, and any contact info.

I’ll build a list and show this issue to some of my contacts and see what we can do about this nonsense if they can’t get their administrative tasks together by the end of the month.

We spend a lot of time and money keeping our professional driver’s license and it is unacceptable that the incompetency of a few Raleigh, NC administrative personnel would put that entire investment at risk.

I just got my CDL in June 2013. do I still need to do the Self-Certification.

The changes began in February 2012, so you most likely completed the form at the time you got your CDL.
If you’re not sure, you may want to check with your local DMV.
With all the apparent problems contacting the NC Dot concerning Self-Certification, can the appropriate forms be hand-delivered to a NCMV office?

Some states will not accept walk-in forms.
There is no information about whether hand-delivery is okay or not in NC.
Call them – 919-861-3525 – and ask, if you can get through.
With the deadline so close, the feedback we’re getting is that this process has become a nightmare for many drivers, and probably the local DMVs are scrambling too.

Called a local NCDMV office and was advised to scan and e-mail the forms to cdlmedical@ncdot.gov . A confirmation would be given. The person I spoke with said processing would not be any faster if forms were hand-delivered to a DMV office.

In December, I paid extra money to mail the form using Certified Mail so that I would have proof that it was mailed and delivered because I could pretty much guarantee that this would happen. I received the return receipt stamped as delivered on Dec 23rd. Yet, on Jan 6, I received another letter stating that I had not submitted the form. So to recap my experience (and apparently those of others as well):
Mail: Even using certified it didn’t get processed.
Fax: Machine won’t answer.
Phone: I haven’t had anything but a busy signal.
Alternate phone number found on website: Left message, person won’t return call.
Website contact link: Sent message through website – no response.

However, I bet on March 1st everything will be Johnny on the Spot and my CDL will be cancelled at 12:00am that morning putting the employment and livelihood of an untold number of CDL drivers at risk. It is obvious that many are trying to comply with the new requirements. However, how can we do that if we can’t get the required documentation through your system? Please tell me what to do to get this to you!!

We’ve been answering questions for drivers across the nation and it looks like this process is a nightmare for many.
All we can do is try to help with the information we have (as DOTPhysicalDOCTORS is in no way affiliated with FMCSA or any state DMV).
Looks like you’ve done everything possible. Maybe your only other option – If you can’t get to visit the DMV yourself, maybe you have someone who can visit for you with all the appropriate paperwork and proof of submission.
We hope you can get this worked out.

Is there a new physician certification for the physician to be certified as an authorized medical examiner for ncdot for 2014?

Yes. New federal regulations state that starting May 21, 2014, CMV drivers can get medical certificates only from a medical examiner who is FMCSA National Registry Certified.
There are many organizations who offer certification training. Just Google “NRCME training”.
Just renewed my medical card today. The Dr. said they must enroll with the a National Registration now. Their certification # is put on the card.

The Certified Medical Examiner now puts his/her National Registry # on the medical card.
Doctors who conduct DOT physicals, must be certified in the near future to do DOT physical exams. The deadline for this certification / registration for medical examiners is May 21, 2014.
Until the deadline, drivers may still have their DOT physicals done by a medical practitioner who is not yet a registered certified medical examiner.
The number of certified medical examiners is still in the couple of thousands, so for millions of drivers to find a CME after May 21, 2014 is going to be craziness.
That’s why we’ve set up this website – to help both drivers and doctors. We get calls every day looking for a DOT doctor.
If the medical practitioner from whom you’ve been getting you DOT physical is not a CME, encourage them to get certified.
If your doctor is not yet on DOTPhysicalDOCTORS, encourage him/her to be here so other drivers can find them.
Which box should someone who is currently incarcerated check? They will be released in 2014 and do not want to lose their CDL license.
