Montana CDL Requirements
Compliant with Montana law, CDL holders are required to certify the type of commercial operation they’re engaged in.
Interstate Non-Excepted
You must meet the Federal DOT medical certificate requirements.
Intrastate Non-Excepted
You must meet the Federal DOT medical certificate requirements.
Failure to keep your medical certification status information current with the state may result in the downgrade of your interstate CDL to a base, Class D license.
Submitting your Medical Certificate to Montana Department of Justice Motor Vehicle Division
Submit the following two documents:
- CDL Self-Certification Classification and Medical Documentation – download the form here from Montana Department of Justice Motor Vehicle Division.
- Copy of your valid DOT medical card. Enlarge the copy to ensure that all information is legible.

Special Information Updates Pertaining To ALL STATES
MEDICAL EXAMINERS MUST CONTINUE TO ISSUE MEDICAL CERTIFICATES TO ALL DRIVERS
The FMCSA has requested that all medical examiners continue providing drivers with a paper copy of the Medical Examiner’s Certificate – Form MCSA-5876, so that Commercial Driver’s License (CDL) holders can provide a copy to the State licensing agency, and the non-CDL drivers can provide the documentation to their employers and Federal and State enforcement officials.
The Agency is currently completing efforts to put into place an electronic system to enable medical examiners to transmit the medical certificate information from the National Registry system to the State licensing agencies.
Until that system is completed, which compliance date according FMCSA is June 23, 2025, medical examiners must continue to issue paper copies of the medical certificates to drivers who pass the medical exam.
ALL DRIVERS MUST CONTINUE TO CARRY A PAPER COPY OF THEIR MEDICAL CERTIFICATE
Commercial Drivers – You still need to carry a paper copy of your medical card, until FMCSA announces that this is no longer necessary.

I have a cdl but I have not had a medical exam for 4 years. I need to renew my license. Can I have a exam and renew my license and retain my cdl? Thank you.

Go get the medical and take your results straight to your local drivers license department. That way, if they have downgraded your cdl, they see that you are trying to be compliant.
I have a CDL that expires in 2024, but my Med card expired in May 2022. If I renew my Med card, is my CDL still valid until the 2024 expiration date?

Can patients see their PCP who is also a CME? Or do they have to see a different CME who doesn’t know the patient because the PCP could be biased?

If your PCP is a CME, then by all means use him/her.
i thought i had another month to renew my dot but it expires today. is there a grace period in mt or will i perminintly loose my cdl, especially if im not driving?

DOT/FMCSA has extended the grace period until the end of August. So you have some time, still recommend you get it done ASAP.
If a Montana cdl driver fails his or her dot physical do they still have to pay for it. Also who wrote the law of no nicotine before a dot physical.

Yes, you pay for the examiners time and talent, not a test. There is no law about no nicotine before testing, but it does keep blood pressure down.
Hi, I was wondering if you know how long a new driver for a CDL has to be on a CPAP machine in order to pass the DOT medical if they are just starting treatment.

30 day minimum and 90 day at the most, with a 30 day readout demonstrating a 70 % compliance.
