Missouri CDL Requirements

All Missouri CDL holders are required to certify the type of commercial operation they’re engaged in.

Interstate Non-Excepted
You must meet the Federal DOT medical certificate requirements.

Intrastate Non-Excepted
You must meet the Federal DOT medical certificate requirements.

Under the federal regulations, all CDL holders are required to self certify to a single type of commercial operation. Based on that self-certification, you may need to provide a current medical examiner’s certificate and show any variance (FMCSA waiver) you may have in order to obtain or keep your CDL.

CDL holders required to have a “certified” medical status who fail to produce and keep up-to-date their medical examiner’s certificate with the Department will become “not-certified” and they may lose their CDL.
CDL retesting may not be required unless you voluntarily downgrade your license or let your CDL license remain expired for more than six months.

Submitting your Medical Certificate to Missouri License Office

When requested, submit a copy of your valid DOT medical certificate.

Special Information Updates Pertaining To ALL STATES

June 23, 2025:  FMCSA electronic reporting requirements for DOT Medical Certificates will be fully enforced.

See detailed information at FMCSA Medical Examiner Certificate Reporting Requirements for CDL Drivers

CDL Drivers

The good news:
• You no longer have to go through the hassles of updating your medical card with your SDLA.
• You no longer have to carry your medical card with you.

Is there ‘not so good’ news?
Safeguard your CDL!  How does the new DOT Medical Certificate reporting process affect your CDL?

Non-CDL Drivers

The electronic reporting requirements do not affect non-CDL drivers.
• The medical examiner must give you a physical MEC Form MCSA-5876.
• You should keep your medical certificate available at all times whilst driving a CMV.
• You will still need to provide a copy of the MEC to the companies you work for.

I am a 50yr old man in decent shape, however a couple years ago I had to have a total hip replacement on my right side. Other than that I have a plate in my right forearm from a break years ago. My son is urging me to become a licensed heavy machine operator specifically to run a crane. Will my hip replacement or the small metal plate in my right forearm, the ulna, (the pinky finger side of my forearm) keep me from passing the MODOT physical examination? Any advice is greatly appreciated. I am 50 and having to play catch up with my finances before retirement. Sincerely, worried worker!

In most states, simply maintain a valid medical card. Do confirm this with your local drivers license department.

Multiple states allow you to upload a CDL Medical Card online, but Missouri requires in-person visits ONLY.

Why is MoDOT behind the times. It’s especially inconvenient for drivers out months at a time or home so few days just to stand in line at their local DMV.

My point is time is precious and MoDOT is behind.

      @ Spanky
Since cdl’s are state issued, you need to check with your local drivers license department. Most states require a valid medical card to hold a valid cdl, driving or not.

DOT/FMCSA has extended their grace period until February 26, 2022. Your company should know this. Send them to the DOT/FMCSA website so they can read it for themselves. The information is also available on our website from DOT/FMCSA.
