Missouri CDL Requirements
All Missouri CDL holders are required to certify the type of commercial operation they’re engaged in.
Interstate Non-Excepted
You must meet the Federal DOT medical certificate requirements.
Intrastate Non-Excepted
You must meet the Federal DOT medical certificate requirements.
Under the federal regulations, all CDL holders are required to self certify to a single type of commercial operation. Based on that self-certification, you may need to provide a current medical examiner’s certificate and show any variance (FMCSA waiver) you may have in order to obtain or keep your CDL.
CDL holders required to have a “certified” medical status who fail to produce and keep up-to-date their medical examiner’s certificate with the Department will become “not-certified” and they may lose their CDL.
CDL retesting may not be required unless you voluntarily downgrade your license or let your CDL license remain expired for more than six months.
Submitting your Medical Certificate to Missouri License Office
When requested, submit a copy of your valid DOT medical certificate.

Special Information Updates Pertaining To ALL STATES
MEDICAL EXAMINERS MUST CONTINUE TO ISSUE MEDICAL CERTIFICATES TO ALL DRIVERS
The FMCSA has requested that all medical examiners continue providing drivers with a paper copy of the Medical Examiner’s Certificate – Form MCSA-5876, so that Commercial Driver’s License (CDL) holders can provide a copy to the State licensing agency, and the non-CDL drivers can provide the documentation to their employers and Federal and State enforcement officials.
The Agency is currently completing efforts to put into place an electronic system to enable medical examiners to transmit the medical certificate information from the National Registry system to the State licensing agencies.
Until that system is completed, which compliance date according FMCSA is June 23, 2025, medical examiners must continue to issue paper copies of the medical certificates to drivers who pass the medical exam.
ALL DRIVERS MUST CONTINUE TO CARRY A PAPER COPY OF THEIR MEDICAL CERTIFICATE
Commercial Drivers – You still need to carry a paper copy of your medical card, until FMCSA announces that this is no longer necessary.

I am a 50yr old man in decent shape, however a couple years ago I had to have a total hip replacement on my right side. Other than that I have a plate in my right forearm from a break years ago. My son is urging me to become a licensed heavy machine operator specifically to run a crane. Will my hip replacement or the small metal plate in my right forearm, the ulna, (the pinky finger side of my forearm) keep me from passing the MODOT physical examination? Any advice is greatly appreciated. I am 50 and having to play catch up with my finances before retirement. Sincerely, worried worker!
@ Worried Worker
You should be fine.
Do Class E drivers need a DOT Medical Card in Missouri?
@ Carlos
You will get the correct answer to your question from your local drivers license department (DLD).
I’m a retired trucker but want to keep my cdl class a what should I have to do?
@ Kent
In most states, simply maintain a valid medical card. Do confirm this with your local drivers license department.
Multiple states allow you to upload a CDL Medical Card online, but Missouri requires in-person visits ONLY.
Why is MoDOT behind the times. It’s especially inconvenient for drivers out months at a time or home so few days just to stand in line at their local DMV.
My point is time is precious and MoDOT is behind.
I have a defibrillator and my doctor said that I could go back to work I need to put who to talk to you. I went and got tested and my fraction rate is 50%
@ Dennis F
Is your doctor a CME on the national registry?
My medical card expired last week, I switched jobs, but is gonna keep my dot license. If im not driving right now do I need to get my physical right away or is there a grace period? Thanks
@ Spanky
Since cdl’s are state issued, you need to check with your local drivers license department. Most states require a valid medical card to hold a valid cdl, driving or not.

DOT/FMCSA has extended their grace period until February 26, 2022. Your company should know this. Send them to the DOT/FMCSA website so they can read it for themselves. The information is also available on our website from DOT/FMCSA.

Does a Class E license require a dot?
@ shawn
You will need to check with your states drivers license department for more information.
No it does not unless you are driving a 24,000 pound dock truck/ straight truck. It is not required if you are just driving a baby dock or a uhaul for personal use i.e. moving. Commercial uses you’ll need your med dot card.
I have a cdl-b. I was issued a temporary d.o.t. medical card, which doesn’t expire until November of this year. I forgot to go to the dmv & submit my temporary d.o.t. card. I got a letter in the mail saying my license has been downgraded, and that my cdl has been cancelled. How do I correct this? Is it as simple as submitting my d.o.t. medical card to the dmv?
@ Ron F
Check with your state dmv. they should be able to help you correct the situation.
