Missouri CDL Requirements
All Missouri CDL holders are required to certify the type of commercial operation they’re engaged in.
Interstate Non-Excepted
You must meet the Federal DOT medical certificate requirements.
Intrastate Non-Excepted
You must meet the Federal DOT medical certificate requirements.
Under the federal regulations, all CDL holders are required to self certify to a single type of commercial operation. Based on that self-certification, you may need to provide a current medical examiner’s certificate and show any variance (FMCSA waiver) you may have in order to obtain or keep your CDL.
CDL holders required to have a “certified” medical status who fail to produce and keep up-to-date their medical examiner’s certificate with the Department will become “not-certified” and they may lose their CDL.
CDL retesting may not be required unless you voluntarily downgrade your license or let your CDL license remain expired for more than six months.

MEDICAL EXAMINERS MUST CONTINUE TO ISSUE MEDICAL CERTIFICATES TO ALL DRIVERS
The FMCSA has requested that all medical examiners continue providing drivers with a paper copy of the Medical Examiner’s Certificate – Form MCSA-5876, so that Commercial Driver’s License (CDL) holders can provide a copy to the State licensing agency, and the non-CDL drivers can provide the documentation to their employers and Federal and State enforcement officials.
The Agency is currently completing efforts to put into place an electronic system to enable medical examiners to transmit the medical certificate information from the National Registry system to the State licensing agencies.
Until that system is completed, which compliance date according FMCSA is June 23, 2025, medical examiners must continue to issue paper copies of the medical certificates to drivers who pass the medical exam.
ALL DRIVERS MUST CONTINUE TO CARRY A PAPER COPY OF THEIR MEDICAL CERTIFICATE
Commercial Drivers – You still need to carry a paper copy of your medical card, until FMCSA announces that this is no longer necessary.

We can do about 99% of everything online. Why can’t we send a copy of our Med Certificate via a website, or email, or whatever? It’s a nuisance to have to go to the drivers license office, sit in line for 30 or more minutes, for a 30 second issue.

Good question and most states will allow you to do so online. Some states are very touchy about the time lines. Check with your states DLD you may be able to do so online.

What’s the next step? I know Missouri is the Show Me state. How do we get some action going?
This stuff is clear as mud. Maybe someone can advise. If I want to purchase a bucket truck that weighs 32,000 lbs. I will need a class B CDL
If I am going to drive only intra-state in Missouri and not for hire, will I need a Medical card? According to federal regulations I read no. But I can’t figure out what Missouri is saying here concerning the medical card?

It is hard to answer. Most states require a valid medical card to maintain a valid cdl, period. Your state should have the best answer for you.
I have a d.o.t card but i dont have my cdl or work for a company but i would like to renew mine so if i do work for someone who require it my would be current. So my question can i renew it on my own without working for a company

Yes you can.
I’m have a yearly exam with my heart doctor So why does the CDL doctor tell me I need a stress test every 2 years to keep my CDL card. That is very expensive test that should not be required if you are under the care of a heart doctor.

It’s a FMCSA requirement not the examiners request.
i sent my DOT medical card in to the address for the DMV. POB 200 Jeff City, MO.
apparently that is not the correct procedure

Take a valid medical card to your local DLD, once a copy goes on file, in most cases, your back in business.
You present your Medical Card to where ever you renew your CDL (License Bureau). Cost around $3.00

Check online for your states DLD, they may have a way to get it registered without returning to the state. Check with your local DLD for the best answer.
I think it’s 60 days. In the past I have received a letter about my medical card. When I let it lapse (not by choice) I had 60 days to recertify or drop my license to an intrastate (Missouri only) CDL.. I think its called a “K” endorsement. Otherwise the license is dropped down automatically.
I have an FAA class 3 medical to fly airplanes which is difficult to obtain and much more stringent than the DOT medical. Why will that not serve as medical certification for my CDL? This is frustrating that these two massive governmental agencies can’t work together to save taxpayers like me time and money.

In order to submit your Medical Certification WITHOUT going to their office, you must submit a Form 5463 and your medical certification. You can fax it to 573-526-7365 or email it to dlbmail@dor.mo.gov

All medical cards expire at midnight of the expiration date, no grace periods.

Please site our error.
You might want to check with your state in the state of Missouri your license will be downgraded to a regular operator license if your medical card expires you have 6 months to submit a new medical card before your CDL expires. I’m sure Missouri is not the only state that does this
