Missouri CDL Requirements
All Missouri CDL holders are required to certify the type of commercial operation they’re engaged in.
Interstate Non-Excepted
You must meet the Federal DOT medical certificate requirements.
Intrastate Non-Excepted
You must meet the Federal DOT medical certificate requirements.
Under the federal regulations, all CDL holders are required to self certify to a single type of commercial operation. Based on that self-certification, you may need to provide a current medical examiner’s certificate and show any variance (FMCSA waiver) you may have in order to obtain or keep your CDL.
CDL holders required to have a “certified” medical status who fail to produce and keep up-to-date their medical examiner’s certificate with the Department will become “not-certified” and they may lose their CDL.
CDL retesting may not be required unless you voluntarily downgrade your license or let your CDL license remain expired for more than six months.

MEDICAL EXAMINERS MUST CONTINUE TO ISSUE MEDICAL CERTIFICATES TO ALL DRIVERS
The FMCSA has requested that all medical examiners continue providing drivers with a paper copy of the Medical Examiner’s Certificate – Form MCSA-5876, so that Commercial Driver’s License (CDL) holders can provide a copy to the State licensing agency, and the non-CDL drivers can provide the documentation to their employers and Federal and State enforcement officials.
The Agency is currently completing efforts to put into place an electronic system to enable medical examiners to transmit the medical certificate information from the National Registry system to the State licensing agencies.
Until that system is completed, which compliance date according FMCSA is June 23, 2025, medical examiners must continue to issue paper copies of the medical certificates to drivers who pass the medical exam.
ALL DRIVERS MUST CONTINUE TO CARRY A PAPER COPY OF THEIR MEDICAL CERTIFICATE
Commercial Drivers – You still need to carry a paper copy of your medical card, until FMCSA announces that this is no longer necessary.

Hi. I have question before I’ll get my dot. I’m 26 women and I was have a surgery with across my left eye 3 years ago, but my vision in this eye didn’t change ( I almost lost my vision in this eye). On my right eye I have perfect vision on my second eye. So my question is if I have this issue whit my vision it’s gonna disqualified me as a CDL driver? And sorry for my English I’m not from here.

FMCSA vision requirements are 20/40 or better with both eyes and each eye individually. Now that can be with and/or without corrective lenses. If you fall below those levels, either with both eyes or each individual eye, you cannot pass the DOT examination.
Just went to dmv to pay my $2 or 3 I guess to register my medical card which I have line was out the door what a sh** show ! F the dmv and the federal regulators this can and should be fixed
To all that are out there and hold a commercial license, due to all the accidents involving commercial driver’s due to medical condition, we should welcome the idea of having this type of regulation to carry a valid DOT physical card, to maybe know the health condition of a driver. it is always the unknown condition of just bad health.
I know, I do feel a litter better about the commercial driver next to me on the highway then I did a few years ago.
The down fall like anything, a person can still hide a major medical condition.

That is a good point… then why don’t police, fire fighters, or bus drivers have to comply. It’s nothing more than a money grab, and more regulations to follow.
I am a farmer in southeast Missouri, I stay within 150 miles of home to delivery our product. I have a class A drivers license do we have to have a medical card?

Most states are requiring that to maintain a valid class A drivers license, you need to have a valid medical card.
Check with your local drivers license department and see if they have anything different to say.
Call FOR 573 7514509. I think you do even though it sounds like “intrastate” being in SE MO if you leave the state even a mile you will.
In Missouri, what are the guidelines to be grandfathered in for Intra state, which would not require carrying the medical card?

Yes.
There is a certain radius for Intrastate, you can pick up a CDL book at the license office and it will tell you that radius if you go beyond that radius or if you step 1 mile out of the state you will need a CDL might want to check with the Department of Revenue.
If I drive a box truck but only a have class F license do I have to pass a Dot physical?

Most likely. If you drive a vehicle for commerce, you need a medical card. Double check with your local drivers license department.
If I drive a cargo van for delivery with class E? Do I have to have a DOT medical card in Missouri?

A Class E is a commercial driver’s license that qualifies a person to drive a vehicle commercially, so yes, you have to have a medical card.
Class E in MO is a basic operator license. Generally you do not need a DOT card however depending on what size cargo van you may need it. Call DOR 573 751 4509
I tried renewing my med card today. I am on a cpap and I wasn’t with in the guidelines required for usage. The Doctor won’t sign off on my med card. What should I do? I plan on trying to use the cpap better, but my med card expires in a week.

CME’s are being monitored just like you and the CME needs to see proper compliance with the unit.
Your examiner needs to see a thirty day proper compliance before he can do any thing. Check with him and ask if there is anything he can offer to help you out in this situation.
My husband got a letter in the mail today saying his CDL has been downgraded due to a valid medical certification not being on file withe the DMV. What does he have to do to get it reinstated? Just go to the DMV and give them a copy of it or is it more complicated than that?

Check with your local DLD to make a confirmation. You should only need a valid medical certificate and possibly a fee to have it re-instated.
What did it take to get this reinstated? I’m in the same boat
I have been through this 3 times in Missouri with the Department of Revenue you have six months from the time they tell you it has expired to get a valid physical on file you need to take it up to the DMV along with proof of Who You Are and they will upgrade it right then and submitted to Jeff City
