Missouri CDL Requirements
All Missouri CDL holders are required to certify the type of commercial operation they’re engaged in.
Interstate Non-Excepted
You must meet the Federal DOT medical certificate requirements.
Intrastate Non-Excepted
You must meet the Federal DOT medical certificate requirements.
Under the federal regulations, all CDL holders are required to self certify to a single type of commercial operation. Based on that self-certification, you may need to provide a current medical examiner’s certificate and show any variance (FMCSA waiver) you may have in order to obtain or keep your CDL.
CDL holders required to have a “certified” medical status who fail to produce and keep up-to-date their medical examiner’s certificate with the Department will become “not-certified” and they may lose their CDL.
CDL retesting may not be required unless you voluntarily downgrade your license or let your CDL license remain expired for more than six months.

MEDICAL EXAMINERS MUST CONTINUE TO ISSUE MEDICAL CERTIFICATES TO ALL DRIVERS
The FMCSA has requested that all medical examiners continue providing drivers with a paper copy of the Medical Examiner’s Certificate – Form MCSA-5876, so that Commercial Driver’s License (CDL) holders can provide a copy to the State licensing agency, and the non-CDL drivers can provide the documentation to their employers and Federal and State enforcement officials.
The Agency is currently completing efforts to put into place an electronic system to enable medical examiners to transmit the medical certificate information from the National Registry system to the State licensing agencies.
Until that system is completed, which compliance date according FMCSA is June 23, 2025, medical examiners must continue to issue paper copies of the medical certificates to drivers who pass the medical exam.
ALL DRIVERS MUST CONTINUE TO CARRY A PAPER COPY OF THEIR MEDICAL CERTIFICATE
Commercial Drivers – You still need to carry a paper copy of your medical card, until FMCSA announces that this is no longer necessary.

I tried renewing my med card today. I am on a cpap and I wasn’t with in the guidelines required for usage. The Doctor won’t sign off on my med card. What should I do? I plan on trying to use the cpap better, but my med card expires in a week.

CME’s are being monitored just like you and the CME needs to see proper compliance with the unit.
Your examiner needs to see a thirty day proper compliance before he can do any thing. Check with him and ask if there is anything he can offer to help you out in this situation.
My husband got a letter in the mail today saying his CDL has been downgraded due to a valid medical certification not being on file withe the DMV. What does he have to do to get it reinstated? Just go to the DMV and give them a copy of it or is it more complicated than that?

Check with your local DLD to make a confirmation. You should only need a valid medical certificate and possibly a fee to have it re-instated.
What did it take to get this reinstated? I’m in the same boat
I have been through this 3 times in Missouri with the Department of Revenue you have six months from the time they tell you it has expired to get a valid physical on file you need to take it up to the DMV along with proof of Who You Are and they will upgrade it right then and submitted to Jeff City
I had a Radom DOT inspection upon they discovered I was a cancelled CDL carrier. I received a ticket but the officer said I need to see county clerk to update my medical card bc that may have been the issue. After digging through mail after a death in the family I was indeed giving a letter indecating to see circuit court judge of the county you reside within 30 days bc of the med card. Which had only been 7. Before leaving the county clerks office I asked if this meant I was updated in system and could legally drive a commercial vehicle, in which she said yes go on.
After further research and newly discovered mail does this sound correct? Could I not possibly be valid she just misunderstood?

If you have a newly validated medical card in your pocket, then you are good to go. But the county clerk does not issue you a new medical certificate, so unless you have had a recent medical certification examination, you are NOT good to go.
my missouri cdl medical card will expire. i do not need a class a anymore. will i be able to get a class e license without any testing?

I don’t know what state you live in, but in Missouri if you go to the license office and tell them you want to voluntarily surrender your class A for a standard license they will do so for a slight fee. When your med card expires, if they see you have self downgraded the med card is void and nothing more is needed. But check with your local office to make sure.
After 6 months your license will automatically drop down to an E without penalty. If you voluntarily give up your CDL in the state of Missouri and later want it back at some point you will have issues to deal with. Your best bet is to renew it as is let the the physical expire and at that point it will automatically be an E from there on without penalty.

Your best bet is to check with your state Drivers License Department and ask them.
Each state is different and this website is focused on Federal regulations.
If a person was to let their dot medical certificate expire will they lose their cdl license if they aren’t driving for anyone at that time that requires a cdl.

If you have a CDL, it may have been downgraded by now. You’ll need to check your status with your state DMV.
Not exactly you can keep your CDL even though you’re not working at this time by getting a new physical card you can go to any doctor that does DOT physicals and get one all you have to do is submit it to the DMV and they will upgrade your license and send the new form to Jeff City if they ask you can tell them that you are just between jobs or whatever the case may be they don’t usually ask.

A DOT medical card is valid nationwide so you can get your medical card in any state, no matter which state you have your CDL.
Make sure you send a copy of your new medical card to Missouri License Office to update your CDL record.

When a CDL driver is grandfathered for an intrastate medical certificate, what action must he do to meet the current regulations. Must he certify with the Department of Revenue to be Excepted Intrastate or Intrastate Non-Excepted? Must he have a variance document and if so what form would be used.

As a grandfathered CDL holder all you have to do is get a new DOT medical certificate and do your state self certification. That should be all you need.
