Maryland CDL Requirements

The State of Maryland requires all CDL drivers to have a valid DOT medical card, unless the type of commerce you drive qualifies as ‘Excepted’.

  • Non-Excepted Interstate (NI)
    You must meet the Federal DOT medical certificate requirements.
  • Non-Excepted Intrastate (NA)
    You must meet the Maryland state medical certificate requirements.

 

CDL Self-Certification

Submit the following two documents:

  1. Driving Certification for Maryland Commercial Driver’s License holders – download the form (DL-330) here from Maryland Motor Vehicle Administration.
  2. Copy both sides of your valid DOT medical card, along with the waiver/exemption if required. Enlarge the copy to ensure that all information is legible.

Medical Certificate Update

You are responsible for knowing when your medical card will expire and for providing MVA with a copy of your new Medical Examiner’s Certificate. You must maintain a valid Medical Examiner’s Certificate on record with MVA at all times.

The Self-Certification Form must be completed and submitted each time a new medical card is submitted to the MVA. Be sure to send both sides of the Medical Examiner’s Certificate (medical card) and any accompanying Waiver/Exception or Skills Performance Evaluation (SPE).

You may used the options described above – in-person, mail, fax or email – to submit these documents.

Special Information Updates Pertaining To ALL STATES

MEDICAL EXAMINERS MUST CONTINUE TO ISSUE MEDICAL CERTIFICATES TO ALL DRIVERS

The FMCSA has requested that all medical examiners continue providing drivers with a paper copy of the Medical Examiner’s Certificate – Form MCSA-5876, so that Commercial Driver’s License (CDL) holders can provide a copy to the State licensing agency, and the non-CDL drivers can provide the documentation to their employers and Federal and State enforcement officials.

The Agency is currently completing efforts to put into place an electronic system to enable medical examiners to transmit the medical certificate information from the National Registry system to the State licensing agencies.
Until that system is completed, which compliance date according FMCSA is June 23, 2025, medical examiners must continue to issue paper copies of the medical certificates to drivers who pass the medical exam.

ALL DRIVERS MUST CONTINUE TO CARRY A PAPER COPY OF THEIR MEDICAL CERTIFICATE

Commercial Drivers – You still need to carry a paper copy of your medical card, until FMCSA announces that this is no longer necessary.

    My DOT card expires 8/12/23. If I get a physical in July when does the new card take affect?

    I have a CDL Class A M License and my DOT card is going to expire on June 7 12023 and my question is I had Heart Surgery on April 3 2023 and I just got my Physical done May 25 2023 and my DOT Doctor told me that she would put me on pending with the Registry but my DOT card is going to run out in June. She said she needs a medical form from my Doctor stating I’m Physically fit to Drive a Commercial Truck so if I get that Form could I go back to her by June the 3rd and get my Physical again so I can go to the MVA and turn my DOT card in.

      Yes, if the CME feels that you are physically qualified to return to work, you’ll be good to go.

    I send my medical card back in January but i didn’t realize it didn’t went through. I uploaded my medical card February 19 and medical card expired February 24. I got message saying it will take 10 days to process. Am i no longer be able to drive?

      If you are holding a valid medical card in your possession, then you are good to go. If there are any questions, all you need is to provide this information about the slow input at the DLD..

    I was recently hired to drive school bus I no longer have medical insurance and have high blood pressure and diabetes I’ve been treated for both in the past… Is it wise for me to go forward without having a primary care physician?

      The states guidelines are more lenient than at the federal level. But even at the state level for your medical card, the CME will need to have your high blood pressure and diabetes under control.

    Hey I’m wondering about how to go about getting a d.o.t card with a provisional license is that possible because it’s holding me up from getting a job and yes I’m over 21 years old. How can I get one?

      There should not be any issues get the medical done. If there are provisions on the license then the medical examiner will take them into consideration. He should then provide you with what you need for the state.

    Got my physical recently and the examiner only gave me one year instead of two. He cited meds. but I’ve been one the same ones for years. Any comment
