Maryland CDL Requirements

The State of Maryland requires all CDL drivers to have a valid DOT medical card, unless the type of commerce you drive qualifies as ‘Excepted’.

  • Non-Excepted Interstate (NI)
    You must meet the Federal DOT medical certificate requirements.
  • Non-Excepted Intrastate (NA)
    You must meet the Maryland state medical certificate requirements.

 

CDL Self-Certification

Submit the following two documents:

  1. Driving Certification for Maryland Commercial Driver’s License holders – download the form (DL-330) here from Maryland Motor Vehicle Administration.
  2. Copy both sides of your valid DOT medical card, along with the waiver/exemption if required. Enlarge the copy to ensure that all information is legible.

Medical Certificate Update

You are responsible for knowing when your medical card will expire and for providing MVA with a copy of your new Medical Examiner’s Certificate. You must maintain a valid Medical Examiner’s Certificate on record with MVA at all times.

The Self-Certification Form must be completed and submitted each time a new medical card is submitted to the MVA. Be sure to send both sides of the Medical Examiner’s Certificate (medical card) and any accompanying Waiver/Exception or Skills Performance Evaluation (SPE).

You may used the options described above – in-person, mail, fax or email – to submit these documents.

Special Information Updates Pertaining To ALL STATES

MEDICAL EXAMINERS MUST CONTINUE TO ISSUE MEDICAL CERTIFICATES TO ALL DRIVERS

The FMCSA has requested that all medical examiners continue providing drivers with a paper copy of the Medical Examiner’s Certificate – Form MCSA-5876, so that Commercial Driver’s License (CDL) holders can provide a copy to the State licensing agency, and the non-CDL drivers can provide the documentation to their employers and Federal and State enforcement officials.

The Agency is currently completing efforts to put into place an electronic system to enable medical examiners to transmit the medical certificate information from the National Registry system to the State licensing agencies.
Until that system is completed, which compliance date according FMCSA is June 23, 2025, medical examiners must continue to issue paper copies of the medical certificates to drivers who pass the medical exam.

ALL DRIVERS MUST CONTINUE TO CARRY A PAPER COPY OF THEIR MEDICAL CERTIFICATE

Commercial Drivers – You still need to carry a paper copy of your medical card, until FMCSA announces that this is no longer necessary.

    I am trying to email one of my Drivers medical Cert to the Email that I have always emailed them to.and its kicking back with a message that the email only accepts messages from people in its organization or on its allows senders list. and you email isn’t on the list. How am I supposed to send Med Certs?

      Yeah, me too! tried to email it as I always do and got the same kick back that you did. I guess our options are to fax or postal mail. or, take to a MVA office.

    I just updated my CDL physical address, but the address and new License number does not match with DOT Medical Card that hasn’t expire yet. It expires in 5 months. Do I need to update my medical card address? If so how?

      You could carry your old license for the next few months, as proof. Otherwise, a new medical examination, but a bit early.

    My DOT examine was on 5/13 and due to expire 5/29. I thought I was fine but didn’t know I had to report 90 days sleep data as I never have before and have been on the CPAP over a year. I found out I was not in compliance now my dot has expired. Im required to now give 14 days sleep data to get a 30 DOT but what about my CDL which will be downgraded to a C class. Do I have time to get the data in before my license reflects a C license and still get my CDL class back?

      You have time, states are not that fast as to downgrade your cdl in a few weeks. If there are any questions from the state, just explain the situation, you’ll be fine.

    I was given a 90 day card because by blood pressure was elevated.Must I take another full cdl recertifacation after my blood pressure is under control.

      If the CME has your card in a holding pattern, then he will complete the physical and give you the remainder of the one year card. If not, you will need to do a new medical certification examination.
