Maryland CDL Requirements
The State of Maryland requires all CDL drivers to have a valid DOT medical card, unless the type of commerce you drive qualifies as ‘Excepted’.
- Non-Excepted Interstate (NI)
You must meet the Federal DOT medical certificate requirements. - Non-Excepted Intrastate (NA)
You must meet the Maryland state medical certificate requirements.
CDL Self-Certification
Submit the following two documents:
- Driving Certification for Maryland Commercial Driver’s License holders – download the form (DL-330) here from Maryland Motor Vehicle Administration.
- Copy both sides of your valid DOT medical card, along with the waiver/exemption if required. Enlarge the copy to ensure that all information is legible.

Medical Certificate Update
You are responsible for knowing when your medical card will expire and for providing MVA with a copy of your new Medical Examiner’s Certificate. You must maintain a valid Medical Examiner’s Certificate on record with MVA at all times.
The Self-Certification Form must be completed and submitted each time a new medical card is submitted to the MVA. Be sure to send both sides of the Medical Examiner’s Certificate (medical card) and any accompanying Waiver/Exception or Skills Performance Evaluation (SPE).
You may used the options described above – in-person, mail, fax or email – to submit these documents.

MEDICAL EXAMINERS MUST CONTINUE TO ISSUE MEDICAL CERTIFICATES TO ALL DRIVERS
The FMCSA has requested that all medical examiners continue providing drivers with a paper copy of the Medical Examiner’s Certificate – Form MCSA-5876, so that Commercial Driver’s License (CDL) holders can provide a copy to the State licensing agency, and the non-CDL drivers can provide the documentation to their employers and Federal and State enforcement officials.
The Agency is currently completing efforts to put into place an electronic system to enable medical examiners to transmit the medical certificate information from the National Registry system to the State licensing agencies.
Until that system is completed, which compliance date according FMCSA is June 23, 2025, medical examiners must continue to issue paper copies of the medical certificates to drivers who pass the medical exam.
ALL DRIVERS MUST CONTINUE TO CARRY A PAPER COPY OF THEIR MEDICAL CERTIFICATE
Commercial Drivers – You still need to carry a paper copy of your medical card, until FMCSA announces that this is no longer necessary.

For a DOT/FMCSA medical certification examination for cdl, class A, holders, these issues are going to raise serious questions. You will need supporting documentation, as well as medical releases, without restrictions, from your doctors.

I live in Montgomery County Maryland and only need a DOT medical exam. If I have a ICD implanted will they automatically deny me a medical card?

Lots depend on what type of driving and what class drivers license you are driving on. As a class A interstate driver, you are disqualified. If it is just to drive a small truck and you need a medical card for the job, then you would fall under the states variances and may be OK with the ICD. Check with your states drivers license department for your best answer.

No, unless a multitude of other symptoms are visible to the examiner and should require a different set of questions to help make that determination.

Just let the CME know in advance and he will not pass. If all he wants to do is hurt people, then he will have to find another way. Or maybe see if you can get him some other kind of help to resolve his issues.
The medical card for a driver with a CDL is expiring. The said driver was involved in an accident & will not be able to pass the medical exam when the card expires…how does this affect the CDL

Depends on what state this is happening in. Best bet is to check with the local drivers license department and ask them. In most cases the CDL is in suspension until the driver has a valid medical card. But check with the state for confirmation.
how do i get a blank copy of the medical examiner’s certificate for my physicial to fill out after my exam? He says that he doesn’t have any or know where to get one. Thanks.

Sounds like the ‘examiner’ you are speaking of is not an FMCSA certified examiner, or he would have these forms and he would be well informed about the process.
If you do not get your certification done through an examiner who is certified through the National Registry, your medical card will not be valid. We recommend you find an examiner who has experience certifying professional drivers.
im applying for Penske and they sent me for a DOT physical they gave me a 3 mo card because my bmi is greater than 40, if I test positive with sleep apnea will I lose my card and job?

They gave you 3 months for a bmi. That is not a FMCSA standard examination. Go somewhere else for the second examination when you need to re-test in 90 days.
