Indiana CDL and DOT Medical Card Requirements

Indiana CDL Requirements

All Indiana CDL holders are required to be medically qualified to drive a CMV by the means of a valid DOT medical certificate.

  • Non-Excepted Interstate 
    You must meet the Federal DOT medical certificate requirements.
  • Non-Excepted Intrastate 
    You must meet the Indiana state medical certificate requirements and endorsements.

 

CDL Self-Certification

The self-certification process requires you to certify to the Bureau of Motor Vehicles the type of commercial motor vehicle you operate, or intend to operate, as well as acknowledge that you are required to meet the medical requirements for that CDL.

Do a Self Certification online.

  • You will need to create a “myBMV” account, if you do not already have one.
  • You do not need to submit your medical certificate when you self certify. This information is already on file from when you where issued your CDL.
    You may use the automated system to verify that your medical certificate is on file by calling 888-692-6841, option number one (1).

 

Medical Certificate Renewal

To keep your CDL valid, you are required to keep a valid medical card on file with Indiana BMV. Submit the completed Medical Examination Form (long form) and the Medical Examiner’s Certificate:

Special Information Updates Pertaining To ALL STATES

MEDICAL EXAMINERS MUST CONTINUE TO ISSUE MEDICAL CERTIFICATES TO ALL DRIVERS

The FMCSA has requested that all medical examiners continue providing drivers with a paper copy of the Medical Examiner’s Certificate – Form MCSA-5876, so that Commercial Driver’s License (CDL) holders can provide a copy to the State licensing agency, and the non-CDL drivers can provide the documentation to their employers and Federal and State enforcement officials.

The Agency is currently completing efforts to put into place an electronic system to enable medical examiners to transmit the medical certificate information from the National Registry system to the State licensing agencies.
Until that system is completed, which compliance date according FMCSA is June 23, 2025, medical examiners must continue to issue paper copies of the medical certificates to drivers who pass the medical exam.

ALL DRIVERS MUST CONTINUE TO CARRY A PAPER COPY OF THEIR MEDICAL CERTIFICATE

Commercial Drivers – You still need to carry a paper copy of your medical card, until FMCSA announces that this is no longer necessary.

    It says above that “All Drivers have to carry a paper copy of their CDL Physical”. Do they have to carry the whole physical or just the last page? Is it ok to shrink and laminate the last page?

      It is always best to carry/have available for inspection, a copy of your medical. If you shrink it, it must be legible to the officer with his naked eye. Generally, just a copy of your medical card is sufficient. The last statement on the bottom of the medical examination form indicate that all the driver needs to have available, if an officer request, is a copy of your medical card. Take a picture and have it on your phone for backup.

    My physical examinations do if I don’t have it done before the end of the month how long do I have before my license comes invalid

      Since your cdl is a state issued document, this question will be best answered by your local DLD. Our recommendation is to get it done and get it into the state ASAP.

    If I take a new yearly DOT physical and forget to provide documentation for my CPAP, can I still drive on my old, not yet expired DOT physical until I get the documentation turned in?

      It will depend, if the CME inputs your new information to the registry, then your old card is invalid. If he does not, then your old card is still valid.

    I failed a drug screen but have completed all the necessary steps to get my CDL back except for the RTD negative test, which I think must be completed before I renew my medical license. My medical license expires soon and I am unable to pass a drug screen for my medical exam at this given moment. If I let my medical license expire for a couple weeks to ensure I am able to pass a drug screening, would I completely lose my CDL or just have to make some type of payment since it is past due?

      Most likely you will not lose your cdl by waiting to pass the drug screen. Confirm with your local DLD.

    If your CDL medical card expires and your doctor can’t sign off on it for a year (due to medical issues) what happens to your license? Is it still valid? Does it expire? Do you have to go through the entire testing process again? It’s hard to find information about this. Can you direct me to the best place.

      You will be medically disqualified, but do not lose your license. You cannot drive a truck but you can continue driving a passenger vehicle. Need to verify how long you can be medically disqualified before you lose your CDL endorsement. Mine was for three months and I did not need retesting.

      After the date on the physical gets older than 30 days, it will revert your class a to an operator….after a year you will have to retest.

    I’m currently out side the country and my DOT experies in May of this year
    Do I need to renew the medical card before the expiration date or I can get an extension

        There is no extension, so best to get the medical card before hand. Then it is up to your state as to how long before they downgrade your cdl. Confirm with your local DLD.

      Since this is not class A cdl, you will fall under the states guidelines and variances. best answer will come from your local DLD. You may be restricted to intra-state only, but that is only a maybe. Check with your DLD.

    If your CDL medical card expires and you go to the doctor to renew, can they extend the Medical card for 45 days for a re-examine. Or is your medical card expired on the date it says.

      There is no extension, card is invalid at midnight of the expiration date. A new medical certification card has a new date of expiration.

      There is a 30 period after the date…..the DMV always sends a warning letter
