Indiana CDL Requirements
All Indiana CDL holders are required to be medically qualified to drive a CMV by the means of a valid DOT medical certificate.
- Non-Excepted Interstate
You must meet the Federal DOT medical certificate requirements. - Non-Excepted Intrastate
You must meet the Indiana state medical certificate requirements and endorsements.
CDL Self-Certification
The self-certification process requires you to certify to the Bureau of Motor Vehicles the type of commercial motor vehicle you operate, or intend to operate, as well as acknowledge that you are required to meet the medical requirements for that CDL.
Do a Self Certification online.
- You will need to create a “myBMV” account, if you do not already have one.
- You do not need to submit your medical certificate when you self certify. This information is already on file from when you where issued your CDL.
You may use the automated system to verify that your medical certificate is on file by calling 888-692-6841, option number one (1).
Medical Certificate Renewal
To keep your CDL valid, you are required to keep a valid medical card on file with Indiana BMV. Submit the completed Medical Examination Form (long form) and the Medical Examiner’s Certificate:

MEDICAL EXAMINERS MUST CONTINUE TO ISSUE MEDICAL CERTIFICATES TO ALL DRIVERS
The FMCSA has requested that all medical examiners continue providing drivers with a paper copy of the Medical Examiner’s Certificate – Form MCSA-5876, so that Commercial Driver’s License (CDL) holders can provide a copy to the State licensing agency, and the non-CDL drivers can provide the documentation to their employers and Federal and State enforcement officials.
The Agency is currently completing efforts to put into place an electronic system to enable medical examiners to transmit the medical certificate information from the National Registry system to the State licensing agencies.
Until that system is completed, which compliance date according FMCSA is June 23, 2025, medical examiners must continue to issue paper copies of the medical certificates to drivers who pass the medical exam.
ALL DRIVERS MUST CONTINUE TO CARRY A PAPER COPY OF THEIR MEDICAL CERTIFICATE
Commercial Drivers – You still need to carry a paper copy of your medical card, until FMCSA announces that this is no longer necessary.

My dot medical card is on file with the bmv. I my card is current but I do not have a copy. I need a copy for a job application. How do I get a copy?

Just get in touch with the medical examiners office that you used. By law, they will have a copy and should be able to forward one to you.
If my medical Card Expired will I have to start everything over in Indiana?

It will depend on how long ago your medical card expired. Usually, a few weeks expired requires a new medical card. Months expired may have fees attached to reinstating your cdl. Years, may require starting all over.

What is the wait time when you have had an ICD put in? This is not the pacemaker.

It all depends on the class cdl you hold.
If a CDL driver is newly diagnosed with high blood pressure, does the driver self report that to DOT or what does the employer need to do? I can see there will be more frequent re-certs. But up front, do we (the employer) need to send for a new physical?

If the driver was recently placed on bp medication, then he will need a new medical card. He should be on these meds for at least thirty days and see that they are being effective, before the new DOT/FMCSA medical examination is done.
I have a class a CDL. My medical card is about to run out. Is it OK to just have my family medical doctor submit The medical form. I just had a very complete physical run. Thank you for your help

Unless your primary physician is on the national registry as a certified medical examiner for DOT/FMCSA, then the answer is no.
is there a grace period for indiana cdl physicals from expiration date if so what is it?

You will need to check with your local drivers license department (DLD) for the best answer.
I have been on high blood pressure and warfarin for 14 years getting and have always been granted a 2 year physical certificate.
I just talked to an examiner, while scheduling a new physical and was told I can only get a 1 year certificate, because of the 2 medications I take. My BP runs 120/68 my warfarin has been dialed in for 30 years. Are these new rules?

No, these rules have been in place for years. When the DOT/FMCSA created the Certified Medical Examiners, (CME), the rules and guidelines were already there, but non-CME’s had been very lax or did not understand them. The CME’s have to follow the guidelines or be dismissed from the registry.
Actually no….I have been on high blood pressure medication for the last 15 years, and yes, every year you have to renew. It’s ridiculous because the meds keep your blood pressure under control. But yep….every year you gotta spend your hard earned money to keep someone else employed.
