Indiana CDL Requirements
All Indiana CDL holders are required to be medically qualified to drive a CMV by the means of a valid DOT medical certificate.
- Non-Excepted Interstate
You must meet the Federal DOT medical certificate requirements. - Non-Excepted Intrastate
You must meet the Indiana state medical certificate requirements and endorsements.
CDL Self-Certification
The self-certification process requires you to certify to the Bureau of Motor Vehicles the type of commercial motor vehicle you operate, or intend to operate, as well as acknowledge that you are required to meet the medical requirements for that CDL.
Do a Self Certification online.
- You will need to create a “myBMV” account, if you do not already have one.
- You do not need to submit your medical certificate when you self certify. This information is already on file from when you where issued your CDL.
You may use the automated system to verify that your medical certificate is on file by calling 888-692-6841, option number one (1).
Medical Certificate Renewal
To keep your CDL valid, you are required to keep a valid medical card on file with Indiana BMV. Submit the completed Medical Examination Form (long form) and the Medical Examiner’s Certificate:

MEDICAL EXAMINERS MUST CONTINUE TO ISSUE MEDICAL CERTIFICATES TO ALL DRIVERS
The FMCSA has requested that all medical examiners continue providing drivers with a paper copy of the Medical Examiner’s Certificate – Form MCSA-5876, so that Commercial Driver’s License (CDL) holders can provide a copy to the State licensing agency, and the non-CDL drivers can provide the documentation to their employers and Federal and State enforcement officials.
The Agency is currently completing efforts to put into place an electronic system to enable medical examiners to transmit the medical certificate information from the National Registry system to the State licensing agencies.
Until that system is completed, which compliance date according FMCSA is June 23, 2025, medical examiners must continue to issue paper copies of the medical certificates to drivers who pass the medical exam.
ALL DRIVERS MUST CONTINUE TO CARRY A PAPER COPY OF THEIR MEDICAL CERTIFICATE
Commercial Drivers – You still need to carry a paper copy of your medical card, until FMCSA announces that this is no longer necessary.

I moved from Florida and was working for county government. All government agencies are exempt from the physical requirement in Florida. Planning on doing government roadwork here and was wondering if Indiana requires physicals for government class A drivers?

You will need to check with Indiana DLD to confirm the need or no need for your situation.
I fell in December of 2018 and suffered head trauma. In March of 2019 had a seizure due to the head injury. I’ve been on medication and seizure free. The state of Indiana requires for you to be seizure free for one year. Well it’s been a year got my medical card took my Dr. release disclosed everything to the examiner. Took my card to the bmv got a call that I need an exemption form from FMCSA . So I call for the fifth time four were previously to make sure I didn’t need a document or anything. So the answer was no , tell them my problem so it gets passed on to the medical safety dept. I will receive a e-mail which I did saying since I’m intrastate I fall under state regulations. Federal has nothing to do with it. Well I take a copy of two separate e-mails saying it’s state regulations so they say feel a affidavit saying I’m a intrastate driver and that will clear up the confusion and I will receive a letter which I got saying I need a exemption form from FMCSA. I know it’s a friggin joke. So back to the bmv they call back to the cdl and say oh the problem is my vision test wasn’t filled out go to the examiner and bring it back. So yes I have my card with me show to young lady as she tells the lady I’m looking at it now it’s written in I can hear oh yeah I see it. Well now it’s wait on another letter with information as to what they need. Yes your right it’s going to say we need a exemption form from FMCSA. Someone help please.

It’s a state thing and FMCSA is not really involved in this issue. See if you can go higher up in supervisors with your state bmv.
I live in Indiana and I had a physical prior to starting new employment in construction operating heavy equipment. I was only granted a 3 month because of risk of sleep apnea. My 3 months expired 3 months ago. Can I just go get a new exam? My employer won’t allow me to work without it.

Yes, you can go and get a new medical. If you are not a class A driver, let the examiner know.
I live in Indiana and am not currently working driving a CMV in ANY Capacity.
Do I still need to maintain a Current DOT Physical in order to keep the CDL I just had surgery a week ago and due to finances was not able to get it taken care of prior to the surgery.
I am being caught by a technicality due to the fact I was prescribed a narcotic pain killer ON A POST OP TEMPORARY BASIS that is seemingly threatening to screw me up due to it being in my medical records.

Check with your local DMV to determine the process you will need to go through to maintain your cdl. Once you are released, without restrictions or limitations, get a new medical card. Your local DMV should be able to guide you through the process. And your meds are temporary, so it is unlikely that they will be an issue, once you have been released to return to work.
I just got my Indiana Medical Examiner’s Certificate approval for my DOT card. What is my next step to secure my DOT Medical Card from the BMV and how much does it cost? I intend on driving a vehicle just over the 10,000 lbs wt category. Thank you.

If you got the medical certificate/card, then you have what you need. You may need to send a copy to your BMV. Check with your states BMV for best instructions.

You will need to confirm with your local DLD for the states variances regarding your situation. Then when you go for the medical card examination you’ll have the need documentation ready.

Check with your local DLD. You would fall under the states variances more so than FMCSA. If you need a medical card according to the state, then make sure the examiner understands your states laws and variances.
I have a 12000 GVWR truck in Indiana and do not require DOT numbers because we only haul our own property within the state. Does Indiana require my drivers to get a DOT physical? The Federal gov’t says you need a DOT physical if you are a “commercial driver” involved in “interstate” commerce with GVWR over 10000lb.

You need to check with your local drivers license department for the best answer. Most states are wanting a valid medical card, but some do not require one for situations like yours.
