Indiana CDL and DOT Medical Card Requirements

Indiana CDL Requirements

All Indiana CDL holders are required to be medically qualified to drive a CMV by the means of a valid DOT medical certificate.

  • Non-Excepted Interstate 
    You must meet the Federal DOT medical certificate requirements.
  • Non-Excepted Intrastate 
    You must meet the Indiana state medical certificate requirements and endorsements.

 

CDL Self-Certification

The self-certification process requires you to certify to the Bureau of Motor Vehicles the type of commercial motor vehicle you operate, or intend to operate, as well as acknowledge that you are required to meet the medical requirements for that CDL.

Do a Self Certification online.

  • You will need to create a “myBMV” account, if you do not already have one.
  • You do not need to submit your medical certificate when you self certify. This information is already on file from when you where issued your CDL.
    You may use the automated system to verify that your medical certificate is on file by calling 888-692-6841, option number one (1).

 

Medical Certificate Renewal

To keep your CDL valid, you are required to keep a valid medical card on file with Indiana BMV. Submit the completed Medical Examination Form (long form) and the Medical Examiner’s Certificate:

Special Information Updates Pertaining To ALL STATES

MEDICAL EXAMINERS MUST CONTINUE TO ISSUE MEDICAL CERTIFICATES TO ALL DRIVERS

The FMCSA has requested that all medical examiners continue providing drivers with a paper copy of the Medical Examiner’s Certificate – Form MCSA-5876, so that Commercial Driver’s License (CDL) holders can provide a copy to the State licensing agency, and the non-CDL drivers can provide the documentation to their employers and Federal and State enforcement officials.

The Agency is currently completing efforts to put into place an electronic system to enable medical examiners to transmit the medical certificate information from the National Registry system to the State licensing agencies.
Until that system is completed, which compliance date according FMCSA is June 23, 2025, medical examiners must continue to issue paper copies of the medical certificates to drivers who pass the medical exam.

ALL DRIVERS MUST CONTINUE TO CARRY A PAPER COPY OF THEIR MEDICAL CERTIFICATE

Commercial Drivers – You still need to carry a paper copy of your medical card, until FMCSA announces that this is no longer necessary.

    I Had my left leg below the knee amputated. I will drive a semi that is automatic transmission and will not use my left leg in any manner other than exiting or barding the truck. In other words ,this does not effect me in any way. Passing my physical will be NO!!! problem with my prostatic. Am I good to go??? I desperately need to geed back to work.

      Most likely, you will need a Skills Performance Evaluation [SPE] certificate before going for your class A cdl medical certification examination.

    My CDL Recert is due an i can’t pass the eye exam due to having to have cataract surgery. Is there any way to get an extension on my CDL until after the surgery?

      You will need to talk with your local drivers license department and follow their process. Once you explain your situation, they will have guidelines to follow. You will be fine.

    I had shoulder surgery in March. My Dr cleared me with a 20 lbs weight restriction and says I am cleared to drive. I was denied my Medical card because they say I cannot have any restrictions at all. Is this right?
    My employer approves of my weight restriction. I do not have CDL. This is for my chauffeurs license.

      Who ever did your medical certification examination is holding you to a class A cdl standard. You are not to be held to that standard. You fall under the states requirements and not federal standards. Find a medical examiner that understands the difference between state and federal regulations. Check with your local drivers license department for confirmation.

    I have a Indiana cdl and my medical physical has been expired for over a year will I have to retest to get reinstated?

    My cdl physical was due march 20 2024 I had to have open heart surgery on February 6 2024 so wasn’t able to due physical also there is a mandatory 3 month suspension from date of surgery but received 2 letters from state saying my license is suspended and 2nd letter says your medical certification status on the national commercial driver’s license information system (CDLIS) is not-certified and your commercial driving privileges are disqualified my mandatory 3 month suspension ends may 6 2024 can I get reinstated with a new dot physical with all proper heart test that is required plus all proper papers from cardiologist stating I am physically able to resume driving a commercial motor vehicle How much time do I have to do this been told I have 1 year Or do I have to start all over with all testing as if I were obtaining a cdl for first time I’m in Indiana.

      Most likely with a new medical card in hand at the DLD and your driving privileges will be reinstated. Very doubtful they would make you start over given your situation. Don’t depend on some time frame, get the physical as soon as you are ready to return to work.
