Illinois CDL Requirements
Illinois law requires all CDL drivers to have a valid DOT medical card, unless the type of commerce you drive qualifies as ‘Excepted’ (EI) or (EA).
- Non-Excepted Interstate (NI)

Medical Certificate Update
If you are a non-excepted interstate (NI) driver, you must provide the Secretary of State’s office with all updates of your Medical Certificate and/or Medical Variances prior to the corresponding expiration date(s).

Special Information Updates Pertaining To ALL STATES
MEDICAL EXAMINERS MUST CONTINUE TO ISSUE MEDICAL CERTIFICATES TO ALL DRIVERS
The FMCSA has requested that all medical examiners continue providing drivers with a paper copy of the Medical Examiner’s Certificate – Form MCSA-5876, so that Commercial Driver’s License (CDL) holders can provide a copy to the State licensing agency, and the non-CDL drivers can provide the documentation to their employers and Federal and State enforcement officials.
The Agency is currently completing efforts to put into place an electronic system to enable medical examiners to transmit the medical certificate information from the National Registry system to the State licensing agencies.
Until that system is completed, which compliance date according FMCSA is June 23, 2025, medical examiners must continue to issue paper copies of the medical certificates to drivers who pass the medical exam.
ALL DRIVERS MUST CONTINUE TO CARRY A PAPER COPY OF THEIR MEDICAL CERTIFICATE
Commercial Drivers – You still need to carry a paper copy of your medical card, until FMCSA announces that this is no longer necessary.

My Illinois dot physical card expired last year i was not driving commercial vehicle so I didn’t bother renewing my dot physical card now that I wanna drive again can I go just get a physical card or do I have to do the test and everything all over again ?

You will need to check with your local drivers license department to confirm and/or get additional guidance.
In my CDL physical today the nurse was going over the health questions that they always do. When she asked me within the last two years have I any illegal substance, I said yes and told her marijuana. The doctor then came in and saw me signed off and gave me my updated dot medical card and said see you in two years. Do I have anything to worry about?

can a person with one eye drive a dot pick up truck? he would not have a cdl but has to have a dot pshical to drive a company pick up truck.

As long as the CME understands the states guidelines and variances, and the state allows him/her to drive now, then they should be fine.
I have a question. I have had lung surgery. The first I lost my upper lobe on left side. The 2nd was an elloeser flap. Will I pass a physical if I don’t have problems breathing? Thank You!

The CME will have questions and may request additional documentation. If all else is normal, you should be fine.
Recd letter that CDL and Driver’s license is cancelled due to not having medical card on file. Does this mean to start all over from the start to obtain reinstatement?

In most cases all you need do is get your medical card updated and registered. But confirm with your local DLD.
Can a person with bad or no vision in one eye drive a tractor trailer NE in Illinois and I’d there an age limit as too old? Thank you

You will have difficulty passing the medical for a class A cdl. You would need a vision waiver from the FMCSA, which takes time and a special examiner to ride with you to confirm your driving abilities. Age is only an issue if there are additional conditions which may cause restrictions in your ability to operate a commercial motor vehicle.

My CDL has been expired because of omedical….how long can I be out before I have to start all over again?
