Q&A

My medical certificate only expires in nine months. What if I recently developed a medical condition that I couldn’t drive. Can I drive truck again now?

What if I’ve have a Myocardial Infarction (MI)?

My medical certificate only expires in nine months. What if I recently developed a medical condition that I couldn’t drive. Can I drive truck again now?

Even if you currently have a ‘current’ medical certificate, you are prohibited from driving a CMV with any medical condition that would be disqualifying or may interfere with the safe operation of a CMV. Once a disqualifying medical condition is resolved, and before resuming operation of CMVs, you are responsible for obtaining re-certification from a Medical Examiner.

What if I’ve have a Myocardial Infarction (MI)?

You will need to be off work for at least two months. You will need to get an Ejection Fraction and Exercise Tolerance Test, and be cleared by your cardiologist before returning to work.  This is a disqualifying medical condition, so once it is resolved, and before resuming operation of a CMV, you are responsible for obtaining re-certification from a Medical Examiner.

What if I’ve have coronary artery bypass surgery?

You will need to be off work for at least three months. You will need to get an Ejection Fraction and Exercise Tolerance Test, and be cleared by your cardiologist before returning to work.  This is a disqualifying medical condition, so once it is resolved, and before resuming operation of a CMV, you are responsible for obtaining re-certification from a Medical Examiner.

What if I’ve have angioplasty surgery or stents?

You may return to work once you’ve been cleared by your cardiologist, which may be as soon as one week after the procedure. You will need to get an Exercise Tolerance Test three to six months after the procedure, and again at least every other year. This is a disqualifying medical condition, so once it is resolved, and before resuming operation of a CMV, you are responsible for obtaining re-certification from a Medical Examiner.

What if I’ve have ventricular tachycardia?

If you have recurring episodes you should be evaluated by a cardiologist to see if the underlying cause of the ventricular tachycardia is disqualifying.

Can I get a DOT medical card if I have a defibrillator?

The defibrillator is a disqualifier for inter-state driving. The defibrillator is designed to correct problems when they occur, but it still leaves a safety risk.

You may be able to drive inter-state, based on your state’s regulations.

What if I take nitroglycerine for angina?

Taking nitroglycerine is, of itself, not a disqualifier. The medical examiner may require an evaluation by your treating physician to make sure that your angina is stable.

    I am a CME who also happens to be a physician assistant with the PCP for the driver I will be certifying. He’s a 74 year old male who underwent CABG 10 years ago. He is asymptomatic. He recently had a stress test which was normal but also had an echo which showed an EF of 30-35%. FMCSA only requires the stress test annually 5 years after the CABG, not the echo, but now I’m not sure what to do with that result since it is less than 40%.

      FMCSA is requiring a 40% LVEF. Best consider the drivers capabilities due to age and medical condition. A medical release from the drivers cardiologist, making sure the cardiologist has a full understanding of the drivers requirements. As a class A driver, he must be able to maintain life on the road and you must consider his possible risk factors.

    My Husband had an episode of AFib and Chronic Congestive heart Failure with an EF of 20%. He had a Cardiac Death. Was put on a life Vest for 4 months. His heart is functioning at a “normal” rate and AFib is staying at bay, with the Beta Blockers, Blood Thinners and other Meds he’s on. They are talking Cardiac Ablation and a Loop device as an implant under the skin. We live in Illinois. With all that said, what are the legalities of him driving again? He does have a current Medical Card and CDL. He is 59

      If he is a class A driver, then there are going to be lots of questions and requested medical documentation. If he has a medical card at this time, then he has already gone through the process. As a lower class license holder he will fall under the states regulations and variances.

    One of our drivers had a heart attack 10 years ago. He is healthy and has no residual issues but when he went for his DOT physical last week the Doc is telling him he has to get a stress test in order for him to clear the driver. Can a DOT Doc require this? I would think a medical release/opinion would suffice?

      DOT/FMCSA request that the medical examiners have a copy of a new stress test every two years, for a class A driver. Outside of class A the driver then falls under the states requirements and variances.

          With a lesser class cdl, not class A, you will fall under the states rules and variances. Check with your local drivers license department.

      You will need supporting medical documentation indicating you have been released to return to work without restrictions or limitations. You will have a three month waiting period and once you pass the DOT/FMCSA medical exam your medical card would be good for one year at the time.

    My 20 yr old son had an LVAD installed 1st week of DEC 2020. He also has a defibrillator, altho it isn’t functioning anymore. Will he be able to obtain a CDL for intra-state driving ? I understand it disqualifies him for inter-state driving? We live in NW Wisconsin

      Not sure about any states cdl requirements, but in most cases he would fall under the states regulations and variances, Check with Wis. drivers license department for the best answer.

    If I had a mild TIA in 2003 went back to work ant then passed a road test given by the state to verify I was qualified to drive a commercial vechicle with my CDL-A license. Can a medical examiner deny my medical card 18 years later if I have had no further stroke problems what so ever.

      The examiner may request additional information from your primary physician, but in and of itself a TIA 18 years ago is not an automatic disqualification.

    Are there updated guidelines on medical clearance for a driver with aortic stenosis who has undergone TAVR? Surgical repair/replacement requires 3mo post operative but with less invasive procedure do drivers still have to be off work 3mo? I have been unsuccessful finding any new guidelines for this procedure.

      To our knowledge there is no new guidelines based exclusively on TAVR. DOT/FMCSA guidelines still recommend a 3 month waiting period. You may try going to a CME with medical documentation, a full medical release to return to work, without restrictions or limitations. And a medical release from the surgeon. An explanation of the TAVR and its ability to be less invasive with a faster recovery. The CME needs to fully understand and use all the available information on hand in his decision making process.
      DOT/FMCSA Recommend to certify if:
      The driver has:
      • Mild aortic stenosis that is asymptomatic.
      • Moderate aortic stenosis that is asymptomatic and the driver has no disqualifying findings and/or conditions.
      • Severe aortic stenosis that has been surgically repaired and meets all aortic valve repair surgical guidelines.

    I recently was diagnosed with atrial fibrillation after a successful cardio version, after second one, I’m in normal rhythm . Do I need to pass a stress test to comply with DOT?

      Once you go through your waiting period, you will need a medical release from your cardiologist. A return to work without limitations or restrictions, but should not require a stress test.

        I am new to performing DOTs and I am wondering about a patient that has Afib, treated with xarelto(so no INR), in regular rhythm, no issues and follows his PCP/cardiologist q6 months. He has been certified yearly for over 7 years with condition. Do I need to require anything from him or just do my physical and certify for one year.

    If I have a 100% clogged carotid artery, is that a disqualifier for my CDL license? The doctor is telling me that they can not operate.

      It creates may questions and since the risk of a stroke are possible, even if it is a very slim possibility, the CME may not qualify you.
      The CME must take you, the driver, and the public at large into his/her risk factor decision making process. It is not a cut and dried decision maker, but will require medical documentation in support of reducing risk factor possibilities.

      Once fully healed and released without restrictions, you will need a medical release form from your cardiologist. The CME may request additional information to help in his decision making process. And your medical card will be limited to one year at the time.

    I had a open heart surgery in Feb 2018 to replace a defective aortic valve. There was also a pacemaker installed subsequently afterwards. I have no problems since. Are the open heart surgery or pacemaker installation disqualifying defects for renewal of my CDL-A?

      Your situation will need to be supported with medical documentation and medical releases, but if all else is within normal limits, then you could be OK. Your medical card will be limited to one year.

    I have a couple of stents and my stress test will not be current when I try to renew in 2021. My cardiologist says a stress test or ETT is not medically necessary and therefore won’t approve insurance billing. Is there any way around the stress test requirement? Or any way to bill insurance when doctor says it’s not medically necessary?

      If you are a class A driver and your last stress test is more than two years old, you will need to have a new stress test done before you go for your next DOT/FMCSA medical certification examination. Let your doctor know that it is needed for your license and your livelihood, by federal regulations.

      DOT/FMCSA for a class A drivers recommend that the driver be asymptomatic, tolerates medications. has a satisfactory exercise tolerance test (ETT). He will also need a resting left ventricular ejection fraction (LVEF) greater than or equal to 40%, and no electrocardiogram ischemic changes.
      2 months waiting period is the guidelines recommendation. Patient needs to have a medical release form from his cardiologist, without restrictions or limitations. And a new DOT/FMCSA medical certification examination.

      FMCSA request a ETT, Exercise Tolerance Test, be completed. If your cardiologist can support your findings with a nuclear stress test, then the CME may take it into consideration. But as a class A driver, they may follow the DOT/FMCSA guidelines to the letter.

    I have hypertrophic cardiomyopathy and have a defibrillator. I was always granted my medical card for one year. This year, the nurse said the rules changed and the card is now only good for 6 months. Is this accurate?

      The CME is weighing the risk factors of your situation against the DOT/FMCSA guidelines and regulations. Within the appendix of the guidelines, there is a section on defibrillators and pacemakers. It is then left up the examiners discretion to decide how long before the medical certificate expires. It’s not a DOT/FMCSA decision, but the examiners.

    I had a heart attack in 2017. Had the stress test and echocardiogram done and returned to work. Do i need the echocardiogram and stress test each year to qualify to keep my CDL?
    I have been scheduling both tests each year and someone told me that I only have to have the stres test done every other year. Is this correct? Or do I need the echocardogram?

    I have a CDL B and drive a school bus. I have had several angioplasty procedures and 6 stents since 2008.
    I also have had an MI 3 years ago. I am required to have and pass a stress test each year and my cardiologist sign a school bus wavier form in order to maintain my school bus license. If I would not pass my stress test will I still be able to keep my CDL B?

    Back in June I was in the hospital for congestive heart and lung failure. I’m anticipating going back to work in November I’ve lost weight and the doctor said my blood test numbers are down to good rates. She has me on bumetanide , Klor-con and metformin she said I was pre diabetic but the meds are just temporary. What should I expect at the dot physical ?

      You will need a full medical release, without restrictions or limitations. A medical release form from your doctors. and the CME may request additional documentation from your doctors. With everything in place, you may be fine, if all else is within normal limits.

    I have been told I have left ventricular hypertrophy and take 100mg atenolol once a day is this disqualifying me from getting my dot med card

      It is going to raise questions by the medical examiner. Depending on the length of time you have been on the medication and how you have responded will be taken into consideration by the CME. He/she may request additional information from your prescribing doctor before making a final decision, if all else is within normal limits.

    I have a Michigan cdl class A. I drive only Intrastate only. I have had a difibrillator installed last year. Can I still pass a Michigan DOT medical exam? And keep my CDL.

    I’m a cmv driver that’s been on medical leave. I had a icd implant since last year I haven’t had any shocks or other problems since my surgery. Will I be able to return to work driving with this icd implant? Can I pass a dot physical and get my medical card back?

      Your situation is going to raise questions and will require medical documentation from your cardiologist. That being said, with medical release forms from your doctors and the needed documentation, you may be able to return to driving.

    I need the waiver for my DOT physical until Sept 30, 2020. My current physical expires this week but my cardiologist appointment was rescheduled til later this month. I went to get my DOT physical done, but they refuse to sign off on it yet because I have not yet had my stress test. where can I get this waiver until I can get the stress test scheduled?

    I’m required to have an exercise tolerance test every two years. My last one was in March, 2018. If I go for a physical before the end of 2020 is the last test still valid or was it only good until March, 2020?

      FMCSA requires one every two years, we doubt that the CME will accept a report that is older than the required time frame. The CME will, most likely, require a new stress test.

    I have to many PVC’s (that do not bother me at all I am not out of breath, or tried and I do exercise ) but it has effected my heart, the LV is 40% and the right is 50% the cardiologist wants to do an ablation to try to stop the PVC’s how long after the ablation can I go back to driving? What will have to be done to get my CDL? Thank you for your time and I look forward to you answer.

      Your conditions are going to raise questions and will require supporting documentation from your doctors. As far as your cdl, that is state issued and you will need to check with your local drivers license department for the most accurate answers.

    HI I have been diagnosed with peripheral artery disease (PAD). I have 3 stents in one leg and getting another surgery for my other leg next week. I was released back to work a week after my first surgery. Went for my DOT physical for my CDL yesterday and they did not pass me. Stated I needed to wait 3 months after my surgery have a stress test done and if that is good they will pass me. Than said I needed to wait until my doctor releases me. They sounded very confused themselves as I am about this. Please help me understand what I have to do after my next surgery to be able to get my physical done to be able to drive truck again. Do I need to wait 3 months or can I go back after I am released. I explained to them this has nothing to do with the heart.

      After you next surgery and you have been completely released, without restrictions or limitations, from your doctors care, you will need a medical release form from your doctor. The CME may request a 3 months wait following your last surgery, due to the risk factors of a possible blood clot. But with proper documentation that may not be unnecessary. No stress test needed, this is not a cardiac condition.

    My question pertains to having a heart transplant, I’m about a 11 months post transplant and started having Minor rejection that can be fixed with some more meds. My question is when I’m rejection free can I qualify to drive commercially or am I permanently disqualified? I spoke with a DOT doctor who mentioned I had to be a year post with no issues.

      At least 1 year post-transplant; asymptomatic; stable on medications; no rejection; Consent from cardiologist to drive commercially.

        That’s great news. I had to give up my CDL with a
        Defilb but post 3 years with a Transplant. I miss
        Driving and yes at times it suck a lot but the open roads were beautiful
        I may go back into driving

    I’m looking into truck driving in the state of Georgia however I have a pacemaker/defibrillator that was installed 20 years ago as a preventative to a possible but not proven heart condition. I have not had it shock me or cause any problems in the entire time i’ve had it nor have I had any medical issues arise with my heart or body as of late and i’m looking to either have it “deactivated” or let the battery die, whichever one is the best option. If it is “turned off” and I went through the testing would it still pose an issue?

      Stress test are required every two years. If your test is done in July, then you will be good for your next two medical cards.

      More questions will be ask about the conditions which prompted the implant, than the implant itself. It will be up to the CME to make the final decision based on your answers and supporting medical documentation.

    Hi. I can’t be on a computer long enough to read ALLLLLLLL of the comments so, My question is, I have Paroxsysmal AFIB; it occurs whenever it wants to and then goes away the same way it came. AFIB lasts anywhere from 1 minute to 7 hours, I never know how long its gonna last. Does anyone here know what I mean? I did not know myself until I looked it up. I have Doctor appt in two days again…. I am about to enter driving school in two weeks. Don’t want to waste good money if I cant drive. I dont know where to go to get the Right answer. Can I drive a commercial vehicle? Thanks so much for your time

      Everything, right now, is going to be base on the final results of your testing with the cardiologist. Save your money for now and get the results done first. Then based on the proper treatments, medication or other interventions you may be able to drive.

    I had a stroke over 2 years ago. I have become medically qualified twice since then. Recently I was getting my med certificate renewed and the doctor said they could not qualify me because i was taking anticoagulant. Is that correct? Can someone that has had stents put in or a heart attack be on anticoagulant and be qualified? Whats the difference? I have no noticeably disabilities from the stroke and pass the DOT physical with flying colors.

      Your examiner is misunderstanding the FMCSA guidelines and regulation. Unless there are additional issues, you should be qualified for one year at the time. Make sure you have medical releases from your prescribing doctors with any additional documentation that will help the CME in his/her decision making process.

    Hello i had mitral valve replacement surgery in February 2020 got a mechanical valve put in and currently have afib also and its control by warfarin i got a echocardiogram done 8weeks after surgery results were normal and i had injection fraction of 58% currently am not driving right now but have cleared to go back to my non driving job do need medical clearance from my cardiologist and take another echocardiogram before i apply for CDL jobs and would i get a 1 year card or more also good bood pressure too

      The cdl will require a valid medical card. When you are released completely, without restrictions or limitations, you will need to bring a medical release form from your doctor, along with any supporting documentation from your test results.

    I have a driver who had a STEMI and we work under our vendors DOT number and they requires us to have DOT card but do not require us to have CDL’s? Trucks are just over 10,000 lbs. Is this STEMI a disqualifying event? If it is, how long does my driver have to wait to recertify? Will he have to have a stress test done? Only drives within WA State.

      The medical examiner must understand the difference between federal DOT/FMCSA requirements and your states variances for non cdl drivers that need a medical card. If the drivers condition has been stabilized and he has been released to return to work status, then he should renew his medical card. The examiner may need additional supporting medical documentation and a medical release. But your driver falls under the states requirements, which are less strict that class A cdl requirements.

    I was cleared by my cardiologist and a certified DOT doctor after a stress test but was disqualified by my company for an ischemia related to an occluded small posterior branch of my right coronary artery. My ejection fraction is 46%. Is this a DOT disqualification or a company disqualification?

      None of these conditions will disqualify you. The DOT examiner is going to be looking for any physical restrictions that may be present and limiting to your ability to operate a commercial motor vehicle, by FMCSA standards.

      Your situation will raise questions with the medical examiner, especially if you are a class A driver. If you are going to operate in NC only, then you will fall under the states variances and not FMCSA. Check with your local drivers license department for their guidelines and regulations.

      Can I get a dot card when you have CHF and your fraction rate is 35 to 40% my cardiologist said that it’s not an exact science but she says I’m able to drive in this DOT medical doctor says she needs more paperwork or should I go to another licensed medical aid examiner to get everything taken care of because I’ve been passing for the last eight years and then all of a sudden I’m having a problem this year with this doctor

        DOT/FMCSA guidelines want a fraction above 40%. If nothing has changed in the past eight years regarding your cardiologist, then you might try for a second opinion. You will still need medical documentation from your cardiologist so that the CME can make an informed decision.

    HI, I HAD AN ECHO DONE WITHOUT CONTRAST IN JAN 2020 AND ONE DONE IN MAY 2018 WITH CONTRAST, IT SHOWED THAT MY AORTA WAS 4.6 SO THEY WANTED ANOTHER ECHO DONE WITH CONTRAST BUT THE DRS OFFICE ORDER THE ECHO WITHOUT CONTRAST. THE DR SAID THAT THE TEST WAS HARD TO READ AND ORDERED A MRI, I DO HAVE PVC’S AND I HAVE A LOT OF THEM, THEY COME AND GO, I DID A MONITOR FOR A DAY AND IT SHOWED THE PVC’S WERE NOT THAT BAD THE CARDIOLOGIST PUT ME ON CARVEDILOL 3.15 MG TWICE A DAY TO HELP CONTROL THE PVC’S AND ITS WORKING, I DO NOT HAVE HIGH BLOOD PRESSURE ITS 110/65. MY QUESTION IS WILL THE MED CARVEDILOL TAKEN IN THIS LOW DOSAGE MAKE IT SO I CANNOT GET MY CDL? I AM ON A ONE YEAR FOR THE PVC’S.

      FMCSA recommends a one year waiting period following the transplant and will certify for 6 months at a time from now on. Make sure you can provide supporting medical documentation and a medical release form from your doctors when you go for the DOT/FMCSA medical certification examination.

    I have a Contractor who has been diagnosed with Symptomatic Sick Sinus Syndrome. He has had a pace maker (no defibrillator) installed and will be under a doctor’s care for a minimum of 3 months.

    After he is released from the doctors care, will he be able to be re-certified at all with a Pace Maker?

      Yes, he will be re-certifiable. He will need to provide a medical release form from his doctors, a return to work-without restrictions-and medical documentation regarding his present status, at the time of the medical certification examination for FMCSA.

    I had a triple AAA operation in 2017, my surgeon said at post operation i was ok no restrictions.
    He cleared me to go back driving , went to work they sent me for a physical that doctor found out i had to stay out for 90 day which i did, then went back driving. Now my med card is running out i went for a physical passed and this doctor wants the surgeon to sign off and me get checked out by the surgeon, Ihave not had any problems im getting checked again with ultra sound , Will I have to do this every time I get Med card physical. The doctor i went to this time said DOT is not clear about Triple AAA Thank you

      It will be up to the CME as to what he will want for supporting information and documentation. A medical release form from your surgeon and/or primary physician should be sufficient each time you go for your medical certification examination. The ultrasound is probably the single best verification that all is well and may need to be repeated yearly, just before going for your medical card exam.

      The CME may want a recent stress test to confirm that all is clear. If you had a stress test and have been seen by your cardiologist within the past couple years, you may use a medical release form from your doctor. With your pre-existing diagnosis the CME is going to be looking for supporting documentation.

      FMCSA guidelines were established for class A long haul drivers. You do not fall under FMCSA guidelines, but the states variances. You must find a CME that understands the difference and evaluates you as such.

    I currently have a Class A endorsement and hold a medical card. I have had angioplasty surgery and stents. My current job does not require me to need the Class A endorsement or medical card.       If your license is a class A cdl, then you fall under federal regulations and guidelines. Not clear as to a class A endorsement. You need to confirm your states requirements for the endorsements. If you carry a class A cdl, then you need a exercise tolerance test within a two year time span.

    My son had a defibrillator implanted because he had a one time experience of afib. The hospital insurance would not pay as they felt it was not necessary. The insurance company also told the hospital not to bill the patient. Can he still have a CDL in NJ?

      Just to be clear if I have a cardiac cath because I had SOB and had it checked out I would need to get a new dot medical card after being cleared by my heart Dr. This was not done as a emergency it was scheduled after a week from the the time my Dr did a nuclear stress test.

          CME is a Certified Medical Examiner with federal DOT/FMCSA. You can find a local CME in your area, either here on this website or by going to the FMCSA National Registry. Depending on the class drivers license you are working on will determine the requirements used by the CME. FMCSA guidelines were established for over the road, long haul and heavy haul drivers. Your state has its own variances for cdl’s that are not class A.

      The FMCSA guidelines state that a non-defibrillator pacemaker itself is NOT disqualifying however, the underlying disease may be. You will need a medical release form from your cardiologist and supporting medical documentation at the time of your DOT medical examination.

          If the defibrillator part of a pacemaker is “turned off” WHY would you say he is STILL disqualified? It would be as though he did NOT have one! What is the need for the defibrillator is no longer present? My husband’s health has improved since the time an ICD was installed and his doctor told him if the choice was made NOW, that they would NOT have installed it. So we were going to have it “turned off” so that he could get his CDL.

            An active defibrillator and an inactive defibrillator are two different sets of guidelines to follow with the FMCSA. The guidelines allow, with proper documentation, for a driver to operate a cmv with a defibrillator that has be turned off. The CME will need to follow the guidelines based on your husbands status.

    Hi. I have a doubt. It is not for the DOT exam; however, maybe you can still provide some guidance. I need a physical exam as I am applying to be a Chauffer for a private school bus company in Fl. I was diagnosed with arrhythmia many years ago and I have been on propafenone for years. My last electrocardiogram was normal, I have no symptoms and the doctor said that i am stable with a normal sinus. The doctor that did the exam did not felt comfortable approving my exam until I had another electrocardiogram. This is my first time applying to this type of job and will appreciate any guidance. Thank you in advance.

      Your examiner is applying class A, DOT/FMCSA guidelines, to a driver that falls under states variances not federal regs. FMCSA would like to have a new stress test every two years, for class A drivers. If you are a class A, then the examiner is correct in his request.

            If your LVEF is below 40% you will not receive a medical certificate. Your cdl is state issued, so you want to check with your state for the best answer.

        Not true read the guidelines, it’s five years from date of bypass. Then annually. I’ve had two separate bypass surgeries and heart attack. Your medical card willl be a yearly renewal.

          Not sure of the question, but 3 month waiting period following any by-pass surgery. FMCSA guidelines. And a CME who feels that the driver is a high risk may want a much longer wait period. He will be looking at a much fuller patient picture.

    Stent placment 10/18. EF at discharge was 55%. Passed stress test on 1/19 to stage 4 but echo showed EF to be 34%. Had new echo, PET scan and nuclear stress test 8/19). Passed all of them. EF back into 40%. Can I use the nuclear stress test or the test from January which shows I passed the excercise part even though it has the old EF. Insurance will not pay for excercise stress test. It seems I tcan’t get one on my own as they want physician order.

    I have brachycardia, I had a DOT physical in April 2019 where I informed the doctor. Due to mostly hypertension my physical is only good for 12 months. This month I was kept in the hospital for 24hrs because a ER doctor was concerned about my resting heart rate. Once my cardiologist came in, he said this was normal for me and released me. My company says I need a new DOT certification to return to work. Is this legal?

      You’ll have a three month waiting period before you can retake the DOT medical certification examination. Make sure you bring your latest test results and a medical release form from your treating doctors and a return to work, without any restrictions or limitations.

    Have stents places in 2006 and 2014. Was overweight. Lost 50 lbs. see doctor every year for heart health. Being forced to PAY for a stress test every other physical is quite costly. Had physical in July and Stress test in Feb. in July my next physical wants a stress test again. If I get one now in Sept. obviously I’d be ok for next July BUT would I still be good for the following July? As this would be still less than two years. I would need one before the following July.. no?

    I suffered a heart attack on June 6, 2019, and had 2 stents put in my heart, my doctors have released me to get back in the truck, it is now July 24, 2019, my safety director says I have to wait until August 8, 2019, before I can go back to driving, because of dot regulations, shouldn’t I be able to go back driving now that the doctors have released me?

      Your safety director is correct. FMCSA wants a given time frame following a full release, without restrictions, before returning to driving. Your cardiologist know hearts, but not the FMCSA guidelines. The medical certification examination must follow the guidelines.

          Clearance from your heart doctor will be a huge help, especially if the aneurysm is relatively small with no changes is the past few years. Bring any documentation from your doctor that you can, it helps the CME in his decision making process.

          Per 2002 Cardiovascular Conference Report Recommendation Tables, Page 37: s/p PCI, certification at least 1 week after procedure, approval by Cardiologist, tolerance to medications. ETT 3 to 6 months after PCI. page 36 notes certification at least 2 months post MI and post-MI ejection fraction >/= 40%.

    I was denied my medical certificate because I had an aortic aneurysm of 4.7 and AFIB. I had surgery to replace the defective aortic and also had a MAZE procedure done to stop the AFIB. I will not be released by doctors for 12 weeks what do I need to get my CDL back

      You will have to provide some supporting medical documentation. Get fully healed and released, without any restrictions, from your doctors. Then go check with a CME to determine the information he will need to proceed with a DOT?FMCSA examination.

    My dad is in a mess. He had lost his CDL in Alabama due to having an internal defibrillator. When he went to the license office to get the cdl took off they asked why. When he told them that he had a defibrillator they made note of it. He got contacted from Montgomery and said he had to get paperwork from his doctor that he is ok to drive his regular vehicle. He has been to his heart doctor and his regular doctor and neither wants to accept this responsibility. They are afraid of lawsuits if they give him the green light and he has a wreck. Any suggestions? He has not had any issues with his heart since his surgery about 3 years ago.

      As an OTR driver we understand the situation, but we do not understand the states position. A note from his physicians indicating that he has had no issues for the past three years should be sufficient for the state. No risk involved in telling the truth. Your doctors are over-reading some unwritten guideline.

      Medical documentation from the VA may be sufficient, but the CME may still require further documentation. The examiner is looking at risk factors and will require documentation to reduce any possible risk to you and the public at large.

    I was born with a bicuspid valve in my aorta. It was repaired when I was 12 and have had no problems ever since. I aquired my class A licence in August of 1998 and have never had a problem passing a physical until last week. I’ve been working out for over 30 years as well and have never had a problem either!     Since 2012 I have had two open heart surgeries. The last surgery I had in 2015 was to replace my tricuspid valve with a mechanical valve. Since my last surgery I have been living a normal life. I am about to start school to get my Class A CDL with the tankers and hazmat endorsements. Will this prevent me from getting my CDL?

      Since you are not going to fall under the DOT/FMCSA requirements, you should be fine, if all else is within normal limits. Make sure you explain to the examiner your situation and your drivers classification. He will need this information to make his final decision.

    in january i was hospitilized for a heart condition , it determined i had afib ,nonischemic cardiomyopothy,acute systolic chf , mitro regurge. was treated in hospital and had had low injection fraction 35 % was treated four days in hospital with meds and had echo, had cardio version after release from hospital ask for and received a letter allowing me to return to work with no restrictions still on meds though cant get recrtified by my companies dot doc . she says cant recert me all my numbers are good now ejection fraction well in yhe fifties is there any thing i can do i heard changing from interstate to intrastate might might help me get back working or any suggestion thanks

      Need to know why the dot doc is not qualifying you. Meds? Time frame for what condition? Present physical condition? Intra-state lets you fall under the states variances, along with the fMCSA guidelines. Check with your state DLD for that answer.

After 5 years I have had a stress test anually through my cardiologist, with no issues through the facility that issued me a D O T physical. In 2018, I performed my anual stress test, received my approval through my cardiologists office and went for my D O T physical only to have the Doctor giving the exame telling me I now have to have a echocardiogram as well or she would not give me a medical card. I disputed this issue and received the card. Is this a ongoing practice for medical facilities to add requirement or is the Doctor right?

    Recently I found out that EF had dropped to 35. My cardiologist found no blockages during a subsequent catheterization. My stress test was a little erratic but the dr. was unconcerned once he did the cath. Still in the state of Michigan I’m required to have an EF of at least 40. I fear I my loose my job if I cannot be licensed. Is there any thing I can do to bring the EF up or get an exemption.

    I had on 12/11/2018, thru 12/16/2018 8 minor heart attacks ,hich i though was a bad case of flu or bronchitis. I did talk with my manager about, and went going thru my work, on Thursday 12/13/2018, i did see my doctor for annual physical, was all good, but the discomfort as i was breathing ,did continue, since doctors said i was good, i got Friday the 14, off on personal day, but started to get harder and harder to breath, my wife want me to go to the ER, bit i did not went, thinking i had a flu, i did preferred wait until next monday morning, 12/17/18, to see my doctor, ER in waterbury ct, usually make you wait at least 5 hours, before you go to triage, and then they send you home to see your family doctors… its ridiculous.
    Sunday the 16 was miserable, and on the 17 ,i left home.to go to work, set up my load,and by 9 would see my doctor.. them one voice inside my head, believe you or not, said loud as someone, screaming at me. “” go.to the ER NOW, i tried to ignore, but it repeated, and i went… that place parking lot, saint marys hospital in waterbury ct, was full, i parked on handicap spot, it was the only one available, walked in but the ER was empty,, no one soul there, only the recepcionist, i asked for.help. saying that i was having a bad case of viruses, flu maybe, she looked at me and call the triage nurse, even that tooked about 20 minutes for the nurse call.me, and the place was ghostly empty. In 5 minutes i was on OR, with 95% of my heart stoped with blood cloth, and got 35% of hearth muscle damage,, i still.pf.work,, employed, but unable to get back to.work by doctors advise, ,the company i work.for, didnt got me any benefit, i had too beg the state for food stamps and health insurance…because the company says i am not qualified…30 years driving experienced, safe record, 1year with this company, and no assistance at all… my mortgage and bills are behind, my credit score is getting very low, my savings and my kid college fund i had to use, to pay bills and keep the house warm, i may have to give my car back to the bank, and maybe my home to.. i still.have my DOT medical card, valid, i only need a.doc release, but they will.not give it for at least another 3 month… i am.trying to learn other trade. But those medicines doesn’t help.much on concentration. I am.lost.not knowing what to do and what tomorrow will bring..even unemployment i cannot collect, because i need to be available to work..if.anyone can.advise i aprexiate

        I’m looking to start training for my cdl free of charge with a certain trucking company. I have coronary artery disease and have had two open heart surgeries with the last being back in 2006. I’ve had to have stents placed since then and in 2012 I had to have a pacemaker installed due to bradycardia. My ejection fraction is above 60 and I dont have angina unless I need another stent put in. I feel fine and can do anything, no restrictions from my cardiologist.
        So my question is will I be able to ho through training with this company?
        I’m positive that I will pass the cdl physical and I have clearance from my cardiologist to drive long haul which I believe is a class B license. I’ve been on disability for years now but I just cant survive on the amount that SSDI gives me per month any longer.
        My left eye has a partially torn retina since I was 12 years old but I can see perfect with both eyes and good enough to drive with only the left. My only problem with the left eye is reading normal print. My perifial vision is also fine in both eyes.
        Can you tell me if a company will give me a chance or will I be seen as a liability like all other jobs see me as? Hopefully I’ve included enough for you to give me an answer.

    I have an Lvad (left ventricle assist device) that was installed a year and 2 months ago. I’m currently back working at my full time job with a 50 lb lift restriction and a no-mandatory overtime restriction (so basically a 40 hrs max per week). I’m considering signing up for a CDL school as my current employer offered to pay for my CDL education. My only concern is whether I can get thru a DOT physical exam. What documentation will I need to pass the DOT physical? My health is actually better now than before the Lvad. Do I also need a list of meds etc?? If I can’t pass the DOT physical its not worth taking the course??

      Not sure which cdl level you are going for. As a class A driver, you won’t pass the DOT medical certification examination. As a class B intra-state driver, you will need to check with your local DLD for better information. So it depends on which class license you will be shooting for.

    Hi I have Valvular A-Fib and had a stroke in Oct/2018 which left n o residual effects. I take Metoprolol and Eliquis. Whats your opinion on my ability to pass the DOT physical to obtain my Class A CDL. Before I invest in the schooling and exams.

      The stroke put you on the side lines for a year and the medications are going to raise questions. After a year you will need to provide recent test results and medical releases from your doctors.

    I am 39 years and hold a Class A CDL in Utah. I was recently diagnosed with 3rd degree AV block and I go in for pacemaker implant at the end of January 2019. I have not gotten the results from the echo yet however they have scheduled me for the implant. My heart does stop for over 10 seconds at a time and I have never passed out although I have come close but never while driving. I am considering going to work for UDOT as a plowboy or going to work Trucking OTR. How will this pacemaker implant affect my career currently I am unemployed and under doctor’s orders do not drive at all at least until I get the pacemaker. Also how long after I do get the pacemaker can I get behind the wheel of a CDL requiring vehicle

      I know if you work for PADOT you don’t need a CDL physical only a CDL license, maybe you state is the same.

          I have the a similar condition… My CDL is out of Texas… I have, Ascending Thoracic Aneurysm, Angina Pectoris as well as Hypertension… My doctor monitors my aneurysm every 12 months via CT scan and echo cardioghram along with EKG. and the he has to sign off b4 I can get my med certs. Just another expense… What you have to worry about is a dissection of the aorta, tearing searing pain that starts in the chest moving into the neck shoulder and back. Clear sign of dissection and possible rupture.

      As long as you are following up with your cardiologist, and he signs a medical release form for you, you should have no issues. It would be a huge help to the examiners if you were retested every couple years by your cardiologist.

      You need to check with your local DLD. Things change from one drivers class to the other. As a class A, FMCSA guidelines say no to a new medical certification, Class B intra-state could be completely different DLD can answer your questions regarding your cdl.

    Have a CDL license in Missouri. My medical card does not expire till March 2019. I went to get it done earlier because I would not have to take off work again after having neck surgery. The new DOT doctor stated I needed to having clearance from my cardiologist for 7 years ago having a 5 way bypass. I have had multiple CDL renewals since then and only needed a note from my primary doctor. Why is it that now he is needing more information? Is my license still good till March 2019 even though the doctor said he was going to only give me 3 months to get it fixed?

      Dot requires a stress test every 2 years if you have had a heart attack. So you will need that and a clearance from your cardiologist

          You’ll need medical releases from your doctors, a stress test every two years, full recovery-without restrictions, and the ability to pass the remained of the DOT medical certification examination. With that you will receive a one year medical certification, each time.

      If the CME reported a 3 month card to FMCSA, like he is supposed to, then that’s what you have. FMCSA is much stricter on the examiners now that they are federally certified.

    I underwent aortic valve replacement surgery and after recovery renewed my dot med card, I took the stress test a passed by almost double the test minimum needed to qualify yay me, with that said I do now have to renew my dot card yearly now which I am good with, but do I need to do another stress test, dont find any info on it and it can take awhile to set up one, my last took 1.5 months to set up, just dont want to lapse licence waiting til last minute.

    I have an old PA CDL (I’m 62) and I’m now retired in PA and I wanted to work for Penn Dot for a few months in their winter maintenance program.(I worked there many years ago) My health: About 3 years ago I was diagnosed with Hypertropic Cardiomyopathy my cardiologist said at my last appoint. 3 months ago. I should have NO problems doing the job.So I applied and then scheduled my appointment and seen the DOT “nurse” . Anyways, my physical was good (I passed everything “else”) and she said at first IF my cardiologist writes a letter to clear me (regarding my heart condition) I’d pass.. Then later that day she called me and said that my diagnosis of Hypertropic Cardiomyopathy automatically disqualifies me. Note: My Cardiologist didn’t get back to her yet. So which is it? Can my cardiologist clear me or not? I question if she (the examiner) is correct or not. I got 2 different answers from her… Very Frustrating! ( The “nurse” meets me one time and knows more than my doc does about my general health and my condition) DOT needs to train their examiners better! SO HERE IS THE QUESTION DOES MY CONDITION: HYPERTROPHIC CARDIOMYPATHY AUTOMATICALLY DISQUALIFY ME in PA for a Class 2 CDL? and why? NOT everyone who has this condition is on deaths door. Some forms are less severe, which mine is.

      The CME is correct at the moment and can take other information into consideration. But it will require an evaluation by a cardiovascular specialist who understands the functions and demands of commercial driving to confirm or rule out a diagnosis of hypertrophic cardiomyopathy.

        You should be more specific. The FMCSA guidelines state that a non-defib pacemaker itself is NOT disqualifying however, the underlying disease may be.
        This is clearly outlined in Appendix D of the medical examiners handbook.

    I had a pace maker put in on 10/15/18, my cardio doctor cleared me to drive a school bus on 10/22/18 but the DOT doc says he will not clear me for 30 days 11/15/18 and he wont fill out my short term disability form as i am on a no pay status. Does he have the right to refuse to fill it out?

      The examiner can’t pass you on the examination for DOT, by FMCSA standards. And, it is not his responsibility to fill out any additional paperwork. That is out of his field of expertise.

        You are giving incorrect information. A driver can drive with a NON-DEFIB type pacemaker with documentation and a non-disqualifying underlying disease.

    I know after my mitral valve prostheses that i must be off for 3 months. but is that 3 months like from the 1st to the 1st, or is it 12 weeks, or is that defined by the examiner. there is 2 and 1/2 weeks difference in the 2 situations.

      Most likely not. Once a hearth condition comes into question, the DOT medical examiner must follow the guidelines more rigidly.

        I just Had a cardiac ablation done 2 weeks ago. my doctor says im ok to return to work but my company said I need to be off 30 days and then get recertified for dot medical card with a letter from my cardiologist stating I had no recurrence of afib. Is this true? I live in NJ

      You need a medical release form from your cardiologist, then it is up to the DOT examiner to determine your physical, mental and psychological abilities. Your cardiologist is correct, the examiner has the final say, not your cardiologist.

        I have had a diffibulater implant for 17 years, and never had any problems with it, or any problems getting my medical card renewed but this year I was disqualified even though I have a clearance from my cardiologist stating it’s ok for me too drive, I’m just really concerned about this since I drive for a living

    Hi,My problem is I had a bypass in 2000 I went and took my DOT Physical and passed But needed to have my two year stress test and echo witch I did, My cardiologist signed off that I passed all test fine.So the Dr that passed said said as soon as I bring the documents and clearance from my cardiologist he would sign it off.My problem is I cant see this Dr at the clinic so can i get another Dr at the same clinic that is certified to sign my certificate off?I will be driving Interstate and I am in California.The DOT physical is in the clinic computer.

    I recently May 2018 had a successful Cardiac Ablation performed at Duke Heart Center and I have had my first follow up which was good. I would like to get my CDL license back. What steps and forms need to be given to the Cardiologist and to the DMV before I can get my license back. My heart function is normal now and I am off many of the heart medications that I was previously on. Please advise.

    9 weeks ago I had my aortic heart valve replaced. My cardiologist has reviewed my return to work requirements and signed off. When I called the dot doctors office they said dot regulations require 12 weeks off.
    My questions are first is that right and second what is the regulation number. I’d really like to look it up.
    P.S. my license is out of Washington State if that helps.

      12 weeks from the point the cardiologist released you to return to work before the CME can perform the DOT medical certification examination. That’s a FMCSA regulation for the CME’s. You could pose your questions to the Federal DOT/ FMCSA website for regulation #’s.

      If it prevents you from performing the duties of a CMV operator, then yes you would fail. You will want to have a medical release form from your cardiologist with you. Make sure you look at the job description for a CMV driver before seeing your DOT medical examiner.

    I have a current dot med cert, I have since been diagnosed with congestive heart failure and will be receiving a biventricular ICD, I was hospitalized for 4 days and am currently being treated with meds…my question is do I surrender my cdl or does the state get notified and suspends it?

      Your medical card is invalid as of the hospitalization, your cdl is state issued and only they can answer your question of surrender. Most states have policies in place for situations like your, so check with your local DLD for the best answer.

    I am due to renew my CDL I had a heart attack May 31st. Cardiologists inserted a stent, said no heart damage. Second post op visit coming up. At the first post op visit the Cardiologist said all was good, no post op issues and will be good to go for CDL renewal. I plan to renew my license before it expires in August and I will bring the required Medical Release Opinion that will be completed by the Cardiologist. Will I have any issues with the DOT Physician examiner? I’m supposed to start driving locally (30 miles or less) in August. Employer is aware of stent and said I will only be driving locally and no lifting of any sort (great company). Should I bring in a note from the employer along with the Medical Release Opinion? I’m concerned. Thanks.

          If the murmur creates any known symptoms, then there will be questions and a medical release form will be needed from your cardiologist. If no symptoms, have your cardiologist pen a note indicating his awareness of the murmur and it’s lack of influence on your hearts performance.

        I know many school bus drivers in Texas with sleep apnea. Provided you take your compliance print out from your cpap machine, no problem with medical. Check with the relevant school district as some districts have their own rules.

      You will need proper documentation from your prescribing doctor. If you have been on this medication for some time, all the better.
      Start with a medical release form from your prescribing doctor. Have him explain your condition and his working prognosis.

        Based on an assumption that everything regarding your health else is well within normal limits, then all you should need is proper documentation from your doctors. Ask a CME what he/she will need before coming in for the DOT medical certification examination

    at my last dot exam I was told I have a-fib. Went to cardiologist , have done 2 nuclear stress tests, heart echo, I have no blockages or valve problems. He did tell me I would probley never get rid of a-fib. Curently on beta blockers and blood thinner. Also take blood pressure med. What do I need to get current medical card. Doctor said he didn’t see any reason I couldn’t drive Is he right?

          With a lot of questions, documentation, and a positive standing prognosis from your cardiologist indicating that he feels you are not a risk, along with the ability to fulfill the job description of a cmv operator, it’s probably not possible to pass the medical examination required to validate your cdl.

      Check with Mich. state drivers license department for the best answer. If you plan to drive class A, there will be lots of questions and request for supporting documentation before any CME will do a DOT medical examination, but it is possible.

    I went to renew my medical card 1 month after my heart attack, had 3 stents put in. Passed the test and had clearance from my cardiologist to return to work, but was told DOT regulations required me to wait 2 months before returning.

    When I was 9 years old, I had open heart surgery. It was an aortic valve replacement so I have a mechanical valve now. I am now 31 years old and have never had problems since the surgery. My blood pressure is usually around 125 over 80. But I do take Coumadin as a blood thinner. Can these issues cause me not to pass a DOT physical? I’m only asking because I will be enrolling in school in the next two weeks and I’m trying to prepare myself for the worst or best

      The medical examiner is going to need a medical release form from your prescribing doctor indicating the length of time that you have been on your medications and what likelihood these medication would/could have on your ability to safely operate a cmv.

    I have an employee who has been diagnosed with Myocarditis. I do not know all the ins and outs of how this affects his DOT physical. He does not currently drive for us, but wants to move into that position. Will this condition prevent him from obtaining a DOT medical certification from a qualified physician?

      The heart attack will cause the first series of questions and the non compaction condition will be a contributor to those findings. Once you have a medical release form from your doctor explaining your situation and the possible likelihood of a non risk prognosis, you may be able to continue driving. But you are going to be ask to provide all the supporting information for the CME to make the final decision.

    I am a local CDL A driver.
    I have had a bicuspid aortic valve since birth. The valve finally failed after years of monitoring.
    I had a mild event/heart attack (feb 4)the Dr then chose to replace my Aortic valve,(feb 13) this required cracking my sternum and opening my chest.
    I am schedule to get my updated DOT physical May 9th. (which is 90 day from surgery as required) I have a full release letter from Cardiologist.
    Am i required to take a stress test as well? And if so follow up stress test?
    And if…
    Do i get a temporary medical card pending stress test, or do i have to wait for results to be verified by DOT physical Dr?

    I am also of the understanding i should only need the Cardiologist release letter for the DOT physical. Is that correct?

      You have to be fully recovered, without restrictions, Medical release form from your cardiologist, with stress test results in hand before you can do a DOT medical certification examination. And you will need a new stress test every two years if you continue too drive.

    I had a MI back in Nov.2017 2 stents, and was just hospitalized Apr 2018 with Angina, they performed a cardio catherization and found no abnormaties or blockages. My cardiologist and primary care doctor botb released me to return to work, do i need another DOT physical since i just took one in December? The DOT doctor says i need to be off for 3 months even though ive been cleared to go back to work. This true?

        In this case he did not have surgery. The driver did not have his sternum cracked.
        The way i read the language says only if sternum cracked is 90 days off work required.
        Cath. Replaced valves in fact state you can return to work almost immediately couple weeks.

      Your medical card will have to be re-newed before returning to driving. You will need a medical release form from your prescribing doctor and from your cardiologist before presenting yourself to the DOT examiner. He will then make his decision based on his findings and the the other doctors opinions into consideration.

    What if I’ve have coronary artery bypass surgery?
    You will need to be off work for at least three months. You will need to get an Ejection Fraction and Exercise Tolerance Test, and be cleared by your cardiologist before returning to work. This is a disqualifying medical condition, so once it is resolved, and before resuming operation of a CMV, you are responsible for obtaining re-certification from a Medical Examiner.

    is a nuclear stress test acceptable for cardiovascular issues? i have had bypass surgery in 2014 and was unable to drive school bus because of 35% injection fraction. I had a nuclear stress test performed last year and test revealed a percentage of 47%. asked my doctor about it and they said that i was estimated at 35-45% . I can not perform a regular stress test due to having a defibulator inserted just for precaution.

      You drive a school bus and fall under the states variances. From a CME’s perspective, according to FMCSA, you can’t drive OTR, class A. But the state may allow you to drive school bus and the examining doctor will want a copy of your most recent test results for confirmation.

            Hard to say. Once you have fully recovered, have been released by your cardiologist and have a stress test in hand, then you can go for a new DOT medical certificate examination. The CME’s findings will be the final decision maker. Remember that the CME has to function off the FMCSA regs and guidelines and not your doctors opinion, but you will need to be released to return to work without restrictions.

        My husband is newly diagnosed w/ CHF w/ left bundle branch block. Ejection fraction is 25-30%. The cardiologist has put him on beta blockers, bp meds, and Entresto. Also low salt, low potassium diet with fluid restriction. He has had cardiac rehab and doing well but too soon to tell if EF has increased. He may need a pacemaker. He has a MD CDL with all endorsement and drives locally but within the DMV. Can he keep his CDL with a pacemaker?

          Once he has his ejection factors up to FMCSA’s guideline levels, then he should be alright if all else is within normal limits. Make sure you have a medical release form completed by his primary doctor with a full release to return to work. The pass maker should not be an issue, once all else is cleared.

    I have had a graft and s tenet installed for an abdominal aortic disection. Do I have to have a stress test for this and if so is a chemical stress test the same as a physical stress test. The vascular surgeon and my cardiologist are telling me that this kind of Stent does not meet the requirement for the stress test.

And are there any other tests that need to be taken in order to be compliant with the state of TN and the DOT and FMCSA?

    Will he be able to run interstate or INTRA state? Direction is needed please.

      Question one, yes, he will need to have a new medical certification examination once fully recovered and released by his doctors.
      Question two, yes, he will need to present his most recent stress test. He will also need a new stress test every two years for his medical card.
      Question three, there may be a request, from the CME, for further clarification, if there are further questions regarding your drivers over-all health conditions.
      Question four, all depends on his over-all health and findings from the CME.
      The rules change from fed to state and differ state to state. So there is not an answer here until all the findings are in.

    I had stents put in over 10yrs ago, now the doctor will not issue me a new DOT card unless I take a stress test, you want to know how hard it is to get heart doctor here from Washington and every time I get my name on the list see them, I get kicked off because they another person needed the appointment more than I do, they only come once a year for two three days a year, all I want to do is work and put my kids through college, I have no police record, never been arrested, I need the money from driving, I had a class b with PSX endorsemants, I need the money to renew TWIC , MMD cards, I live in Ketchikan, Alaska, the X is hazardous material which I drive a fuel truck and I need that TWIC card, please advise, my email is(personal information deleted by admin).

I am told by the people that give DOT physical that I need a stress test every year with my DOT required physical, or I will not be issued a DOT physical card. Is is true that a stress test is only required every two years. I drive in the State of South Dakota, only drive within a 100 miles of home base each day and home nightly. I have driven truck nearly 40 years, clean record, and this is my sole income that I rely on. I stress out about having a stress test each year as is getting harder because of a sciatic nerve problem and at times some knee issue. In very good health otherwise. Can you advise me if every 2 years is what is needed and even though it has been 10 years since I had a heart attack.

          If you have a class A license and only drive within the state, then you fall under the states guidelines as an intra-state only driver, not FMCSA/OTR guidelines. Check with your state to make the final determination. Stress testing requested by the FMCSA every two years, not every year.

    I recently got my DOT med card renewed. I drive a Schwans truck. I have had an implanted pacemaker most of my life. I spoke to the DOT doc about my condition and limitations. She cleared me. The past few years my cardiologist gave me a clearance letter as well to be safe. I recently moved from California to Texas so I don’t have an established cardiologist yet. My employer is now asking for a clearance letter from my cardiologist. I called the cardiologist that I saw last month and although everything was fine, because I have no history with him, he is requiring a stress test before he will sign off on me. I have never in my 35 years of having a pacer been asked to take a stress test. As a single dad of 4 kids, I can’t afford the $1000+ they are asking for up front. Any other way to get a letter. I’ve already been cleared by DOT doc and have a new medcard.

      You can go back to work pretty much as soon as you’re cleared by your cardiologist and you can perform satisfactorily on the ETT.

      Yes, as long as your cardiologist signs off. I’m a cal holder and been medicated for 5 years with afib…blood thinners and blood pressure meds (9 a day), I get cleared every year. Sometimes, it’s a little tedious with getting everything together, but it can be done.

    I had a anagram with a stent put in in may 2017
    I had no idea I needed a stress test until i received
    a notice from state of Indiana on October 30th.of 2017
    I went for my DOT physical and was told I needed a stress test.
    how often do i need a stress test in Indiana? can I get an extension
    on my time to get this taken care of. I’m running out of time it is very hard to get this scheduled
    in a short time.

    I recently had a percutaneous coronary intervention (PCI) and stent put in place due to a blood clot. I have been released by my cardiologist and family doctor, but now the company doctor is saying I must stay off for 2 months. if I have read the DOT regulations correctly, as long as I have been cleared my cardiologist and have been off for one week I can return to work. so can you tell me the real answer? why the big difference in a MI compared to a PCI?

    I have had SVT controlled by meds for over 20 years. Over 2 years ago during a stress test I went into Atrial Tach not fib at 230 Bpm with a very short run of WCT. I had a cadiac cath 2 years ago which was negative for any problems. 2 years ago I had an Ablation. All stress tests normal since. All EKGs normal since. 2 30 day event monitors with only PVCs. I have PVCs for over 20 years but of no clinical significance. I do take beta blockers twice a day and another HTN med. BP is very well controlled. Do you see any reason I would fail a DOT physical exam? I am about to start driving school but plan on getting physical first because if I don’t pass what is the point of spending the money. Thank you.

    My husband had surgery to implant a pacemaker. He lives in Ohio but drives long-haul for a company in Arkansas. Can he return to driving after he is cleared by his physician? Can he drive long-haul with DOT regulations?

        Fences states three months and then a recertification, as long as it is a Pacemaker and NOT an ICD. If it is an ICD rules change dramatically!

    My husband recently had a heart attack and a stent placement. His cardiologist released him after giving him an ejection fraction. My husband then went to the DOT doctor took and passed all of his physical requirements and he told him he had to be off 2 months before he could return to work. Now he is aying that he has to have an ejection fraction and his cardiologist is saying that it is not safe to do so

    My son is 22 and born with a heart defect. Had surgery at 3 months to repair it. In 2013 he received a defibrillator due to V tac, but medication has taken care of that.. He’s never had a heart attack, no heart failure, no fainting and has no restrictions. His doctors have written a letter explaining he is fine to do anything. The Cardiologist who help decide that having defibrillators means you should not have your CDLs are Cardiologist who deal with 50-60 year old people who acquire heart disease. Adults who are born with Congenital heart defects are a whole different ball game and Pediatric Cardiologist are the experts. The differences are major, he doesn’t have a sick heart. The likelihood of him ever being shocked is less than someone older having a heart attack at the wheel and dying. I want to fight them, just not sure what to do. Any suggestions?

      No interstate driving with a defibrillator! This is an FMCSA rgulation.
      For intrastate driving, you’ll need to check with your local drivers license department for their variance requirements.

    I am an interstate cdl class a driver and have been diagnosed with dialated cardiomyopathy and left bundle branch block. When 1st diagnosed my EF was 35%. My last echo said my EF was 41%, I have no other health issues except high blood pressure which is being treated with medication. Do I meet standards to get my medical card back or is there a fast food job in my future? Thanks for your help.

    My Dad (82 years old) drives a school bus and has been off work for the past year and a half. Due to being hospitalized for a spell that he had causing some disorientation. They were unable to find any reason for the spell that he had but his cardiologist claims he hears a slight murmur. So, will not release him to go back driving the bus. He has not had any repeat spells and healthwise he is doing very well. He has talked to his primary care physician and he says that he sees no reason why he can’t be driving. His blood pressure has been good and all his labs come back fine. So, my question is, Can the primary care let him return to work over what the Heart doctor says. From what my parents tell me, the heart doctor doesn’t want the responsibility of releasing him. From what I have been reading on this site a murmur doesn’t keep him from driving.

    Can a driver legally drive with a 30 day heart monitor? No diagnosis has been made. Documentation from dr stating that their was no heart attack and are no blockages and can report back to full duty. Even with that and the ct scan results from hospital the carrier is saying the driver has 2 options. 1- stay home 30 days with heart monitor or 2- get a new medical card stating that he can drive with monitor.       I have a brother-in-law who works for a company that teaches children how to drive, he is about to go into the hospital and have a defibrillator put into his chest, will he be able to go back to work and still teach children how to drive? He lives in Michigan.

            The medical examiner was contacted by my cardiologist and he furnished all medical findings and told them I was cleared to drive . M E still insists I have to be off 90 days as per D O T. I can’t find that rule .

    I’m 56 yr male and I just went to my personal doctor for my yearly physical. I have hypertension (high blood pressure) was 119/69 with my meds, type 2 diabetic (controlled with diet) 296 lbs
    My question is I had an EKG done in the office and showed a right bundle branch block-new finding.
    Doctor scheduled an urgent appointment with a cardiologist. Going in 2 days, will this disqualify my CDL? Is this something that can be treated and maintained so I can keep working?

      Any illness or injury that takes you from behind the wheel of the truck requires a new DOT physical. Once the cardiologist has cleared you you will need documentation from him to take to the medical examiner to help with the decision making process to qualify you for your new DOT certification.

      No, if the CME did a “pending information” report. You need to get that information back to the original medical examiner’s office for the report to be completed. If your current medical card has expired in the meantime you cannot drive until you have a new valid medical card.

    I am studying for my CDL permit and noticed that the Texas law school states “heart problems” as a disqualifying condition. Would congenital heart defects that have caused mitral and aortic valve prolapse be considered a problem. I am not on any medication for these conditions and have not seen a cardiologist in almost a decade.

    I have a heart transplant May 21 2016, prior to that I lived with a LVAD for over 6 years, which that heart was removed and new on transplanted. Saying this, been out of work force since 2007, So my year end heart transplant evaluation is done this month June 2017, so far all is looking well.

    I am on life anti-rejection, my infraction rate 59%, stress test came back great, other test came back fine, including biopsy which is 1R or 0R. I do have diabetes which I am type 2 on insulin, latest A1C was 6.3 my best A1C 4.9 was before transplant. However, transplant medication contributes to diabetes levels as well.

    I have been seen by endocrinology for past 10 years also, all this under Mayo hospital management.

    I am not able to chase bad guys anymore so I need to try something else, so thought about getting paid to travel looks like a good idea.

      Paid to travel? You could not pass the FMCSA/DOT certification examination at this point in time. If you are considering doing something like hauling campers around the country on less than class A license, then you will need to check with your local drivers license department to confirm your choices.

        you mentioned “at this point in time” what specific or is it several issues I would I need to address to work on getting, so I am able to pass in future? if in fact there is a future in trying?

        In my past dealing with bad guys I was required to have a class A for undercover work.

        However, it was in another state that I am not located in now.

        I am just trying to figure out if its worth pursuing. Paid to travel, here was my thought buy a truck and trailer and just travel get loads I want in the direction I want, have a nice cab with all the goodies, sorta like a motor home that makes money.

      I had arterial stents (5) placed in me on February 26th and today April 2nd was sent by my company for a DOT physical. When I went into the office they handed me a paper that with stents I would need a release signed by my Dr. and a copy of the stress test and cardio gram. I got all of those for them and then was told I could not get a physical until three months after the surgery. Is this true? My company is now also researching this as they had not heard of this.     It was recently found that I have a bicuspid aortic valve and a thoracic aortic aneurysm. My valve is in fine condition. My aneurysm is 4.5cm. Statistically, an aneurysm of 4.5 cm has a 1/2 of 1% (0.5%) chance of rupture. 5.5cm aorta has a 2.9% chance of rupture and is worthy of repair.

    Page 227 of the medical examiners handbook recommends not to certify driver if aneurysm is greater than 5.5cm. page 226 recommends not to certify driver if greater than 3.5cm. How is an examiner to make this decision based on my aorta diameter of 4.5cm and such a contrast by the handbook regarding thoracic aortic aneurysm diameters?

      CME’s are looking to ruleout high risk factors. Your best bet is to have as much documentaion available to support your situation. The medical release form from your doctor should be a big help. Have him explain what you have and its likelyhood of risk.
      FMCSA has given the examiners guidelines/recommendation to work with, so it all comes down to what and how the CME is reading and supporting his decision making process.

      FMCSA would like to have you on your new medications for at least 30 days before recertification. Depending on what class driver you are will determine what, if any, restrictions you might run into. Make sure you have a medical release form from your doctor before going for a recertification, if it is necessary to do so based on your drivers license class.

    I had a Pulmonary Embolism back in 2000. Dr. I saw today for my re-cert would not certify me unless I got a note from my primary saying I was ok to drive. Have had my cdl since 2011 and this has never been an issue Dr. said the laws have changed since last year but I cant see this being an issue! I live in NJ and cannot find any info to backup this Drs. claim. Could this really be true?

      As long as the condition does not pose any type of safety issue, then you should be able to return to driving. Before you go for your medical certificate examination, make sure you have a medical release form from your doctor and your cardiologist, with a working prognosis. The CME will need as much information as possible to make his final determination.

    Question: If I go in for a DOT physical and leave off any information (9 years ago I was diagnosed with an enlarged heart, however in the years since I have lost weight and gotten in shape) could I get in trouble? I haven’t seen my physician in regards to my heart for years, and I have had no problems with it since, so could I just omit the information regarding my heart? Will the doctor’s office performing the DOT be able to look up my past medical records?

      Honesty is usually the best policy. That being said, we would just have your primary doctor give you a medical release form stating that you have no present condition or symptoms. Just leaving the information off could be seen as lying on the federal forms that you signed as being truthful.

      You will need a medical release form from your doctor and a stress test every two years. FMCSA would like to see an echo included with the stress test, but the medical release from your doctor, with his working prognosis, should be all you need to support the CME for the examination. The CME is looking for any possible safety issues that may be underlying your test results and the information from your cardiologist should be sufficient.

    I have asymptomatic moderate aortic stenosis with a valve area of 1.2 and EF >55% and asymptomatic rate controlled a-Fib and have been on an anticoagulant (Eliquis) for over 20 months. I have a letter of assessment from my board certified cardiologist. I was told that a-fib with moderate stenosis was disqualifying. I read in the FMCS guidelines that if both conditions are satisfied and assessed then a determination would made as to how the two together would affect the safety to drive. I had the same condition last year with the same assessment from my cardiologist and was qualified. The same doctor at Concentra disqualified me this year..

      Concentra disqualifies everything they really should not be in the DOT program theytried to disqualify me for taking.coumadin saying.i needed 2 years worth of INR.checks come.to find.out only pilots.need the INR checks went to a an irgent care ru. Bu real medical doctors who ha e been.certifying pilots and drivers and found.out that fhey are doing a.DATA Base for themselves i have also told them they are not my attendei g pbysicians and DOT /Fmcsa did not require that much knowledge even pulled it up and showed them.and the PaS WERE LIKE IT DOESENT MATTER ITS OUR WAY OR NO MEDICAL CARD no actual doctors do the DOT physicals they are done by pbysicians assts only and somstimes.there are no doctors on site.

      Our focus is on requirements to pass the FMSCA medical certification.
      That being said, we cannot imagine that an employer should be responsible to pay for an employee health test, but we recommend you check into this with a party who is better qualified to answer such a question.

      It needs to be asymptomatic and you will need documentation from your cardiologist. Have your cardiologist fill out a medical release form for you stating your present situation and his working prognosis. The CME is looking to confirm that there isn’t a safety issue in the works.

          More than likely. You will want a medical release form from your doctor explaining your condition and a working prognosis. As long as the pacemaker does not have a defibrillator with it, you should be OK. Bring any documents that will help the examiner support his final decision.

    I had a heart attack on Feb 22nd ’17. I’m aware of the can’t drive for 2 months rule but does that mean I can’t work at all? My Cardiologist has already approved me to go back to work. I was wondering if I could work for my employer in some capacity that excludes driving?

    Had heart attack on 12/10/16. Had nuclear stress test and was re-certified on 2/13/16 and received medical card for one year. DOT doc said I needed to have another stress test halfway between my heart attack and my next physical which will be in February 2018. This means I should have another test in July 2017, however, it seems as though I would need one closer to being re-certified in Feb. 2018. I asked the doc about this more than once, but she insisted I needed one in July 2017. Is that correct? I don’t have the money to keep taking these tests and don’t want to get one in July and then be told I need another one right before the Feb. re-certification. Thanks for your help.

      FMCSA wants a stress test done every two years. When you get that done is up to you and your cardiologist, not anyone else. Check with your cardiologist and make sure you have a copy of the results and a medical release form from him too.

        So, if I’m understanding your answer, since I had a stress test in Jan. 2017, I do not need another one until Jan. 2019. Do I bring the results of that stress test to the DOT doc again for my Feb. 2018 re-certification? Also, I do have the release letter from my cardiologist for that Feb 2017 re-certification. Do I just need an updated release stating that I’m still able to work for that next DOT physical in Feb, 2018 along with the stress test results from Jan. 2017? The DOT doc was adamant about getting another stress test – what if they won’t accept the Jan. 2017 stress results again and insist on an additional one. I know this gets complicated, but I don’t want to get turned away because someone is insisting on something that I don’t even need to have. Once again, thanks for your help.

          You only need a stress test every two years and it is helpful for the examining CME to have the most up to date medical release form from your cardiologist with his working prognosis each time you go for your DOT exam.

    I went and had my DOT physical the Dr. said since I had a stint 15 years ago I needed to have a stress test. I have done that and now I’m waiting of the results. My question is if my medical card expires before I get my new one will my license be revoked and will I have to retest to get it back?

    My husband had coronary bypass graft surgery in Nov 2015. He had a echocardiogram and a medical release form from his cardiologist in April 2016. He received his medical certification at that time. I understand that he will need a yearly recertification with a medical release from his cardiologist and an exercise stress test every other year. My question is, as he had an echocardiogram and not an exercise stress test last year, will he need the exercise stress test this year or next year?

      FMCSA would like to see a exercises stress test every two years, so you will probably need one done before going to the next DOT physical. Also make sure you have a medical release form from the cardiologist, it helps the CME in his decision making process.

    my husband has had two heart attacks and had a pacemaker, defibrillator for almost three years now. he has a class b cdl. can he go back to driving? he also had a stress test about a year ago and it was good. his cardio doc says he is good.

      He will, most likely, be limited to intra-state only due to the defib unit. It falls under the states variances and not the FMCSA, so check with your local drivers license department to confirm.
      FMCSA will have him disqualified for interstate driving. Stress test results need to be available every two years.

      Pacemaker with no defibrillator should not be an issue by itself. If there are limiting restrictions placed on the driver, then it starts being an issue of ‘medically fit for duty’, there is no light duty for a class A interstate driver. Get a medical release form from you doctor explaining your situation and condition. You will be limited to one year at the time due to the pacemaker and may have request for additional information from the CME. Good Luck.

    I hold a Class A CDL and drive for UPS. I recently had a permanent pacemaker installed on December 16th, 2016. I have been diagnosed with Sick Sinus Syndrome with a high grade AV block. I have been doing some research trying to figure out if I fall into the 1 month or 3 month window for my medical release. I did not have a defibrillator installed. There seems to be some conflicting guidance amongst the FMCSA manual (2002) and the FMSCA Cardiovascular Disease Executive Summary (2007). Can you please shed some light on which category I fall into?

      Once you have a medical release form from your cardiologist and are fully healed, then you would fall in the 1 month wait period. For your type of condition, you will need 1 month post op. fully recovered and a medical release form from your doctors to return to work. Also, if you can, provide any other information that you can for the CME. All documentation is greatly appreciated.

      As an intra-state only driver, we suspect you can, but confirm with DLD.
      You would not qualify as an interstate driver with the ICD.

      I assume you mean heart transplant. If that is correct, then it is a one year post transplant before you can go for a new DOT medical certification and your card will be good for 6 months at a time. FMCSA wants a full re-evaluation every 6 months to keep an eye on you.

      Also when you go for the medical examination, make sure you have a medical release form from the cardiologist. Don’t try to do the physical before you are fully healed and fully released to return to work.         If this person is referring to a Ventricular Assist Device (VAD) which is a heart pump then if your physician approves you can drive Class C(auto)after recovery and assessment. Every heart failure situation is different but I know from personal experience that I wouldn’t want to or should not drive a truck with a VAD. Some people have to have a VAD for the rest of their life(destination therapy) and may regain strength to feel they could drive truck again. I had a VAD for five months(not fun) and then a heart transplant. I finally feel now(one year post transplant) that I could drive a truck if I wanted to go through all the hoops. I never had a class A license but had class A permit when I was younger. Bottom line: Consult your doctor 🙂

    I have had mitral valve prolapse since the age of 21, I am 49 now. I am not required to take any meds for this except aspirin 81 mg for blood thinning and cardizem if I have any palpatations, have had to take maybe 2 in the past yr. I have had a CDL since 2000 and have reported this condition every year. Suddenly today the examiner said I would have to be cleared by cardiology in order to retain the license. I have had no recent problems with this condition. What gives?? I live in Tennessee and this has never been an issue before. Insurance will most likely not pay for this.
