Arizona CDL Requirements

Arizona requires all CDL drivers, including those in the military, to maintain a current Medical Examiner Certificate.

  • Non-Excepted Interstate 
    You must meet the Federal DOT medical certificate requirements.
  • Non-Excepted Intrastate 
    You must meet Arizona medical certificate requirements.

 

Medical Certificate Update

Failure to maintain a current certificate on file with MVD will result in the suspension and revocation of your CDL.

To check on your Arizona medical certification, see CDL Medical Status Verification. Once medical certification is submitted please allow 10 days for processing prior to checking status.

A courtesy expiration notice is sent to the driver 45 days prior to the expiration date. It is the responsibility of the driver to provide a new Medical Examiner Certificate on or before the expiration date of the document currently on file.

Special Information Updates Pertaining To ALL STATES

MEDICAL EXAMINERS MUST CONTINUE TO ISSUE MEDICAL CERTIFICATES TO ALL DRIVERS

The FMCSA has requested that all medical examiners continue providing drivers with a paper copy of the Medical Examiner’s Certificate – Form MCSA-5876, so that Commercial Driver’s License (CDL) holders can provide a copy to the State licensing agency, and the non-CDL drivers can provide the documentation to their employers and Federal and State enforcement officials.

The Agency is currently completing efforts to put into place an electronic system to enable medical examiners to transmit the medical certificate information from the National Registry system to the State licensing agencies.
Until that system is completed, which compliance date according FMCSA is June 23, 2025, medical examiners must continue to issue paper copies of the medical certificates to drivers who pass the medical exam.

ALL DRIVERS MUST CONTINUE TO CARRY A PAPER COPY OF THEIR MEDICAL CERTIFICATE

Commercial Drivers – You still need to carry a paper copy of your medical card, until FMCSA announces that this is no longer necessary.

    is there a requirement (In AZ) for a Non-CDL holder that would require a medical card? (if a vehicle weighs between 10,000 – 18,000 lbs gvwr)?

    I was working my way up o maybe a truck driving school, when it was discovered that I needed to have a double bi-pass. Can I still get my CDL class A with endorsements etc. with the surgery under my belt? I have always had my V.A. Small Bus. Loan opion in the back of my mind to buy my own truck and know the importance of a good truck driving school. Thank ya’ll.

      It is do-able. You will need supporting medical documentation for the DOT medical examiner. Check with a local CME to see what documentation he will require.

    Just stay legal and on top of your required documentation and keep rolling.
    It’s not hard and if something that is your responsibility to keep up expires then it is your fault.
    Of course maybe your kids blame everything on someone else also.

    I have not taken a DOT exam since 2011. because I was not trucking a truck . The CDL part of my license was suspended but could drive a regular vehicle such as a car . Now that I’m ready to start back to work, I called the ADOT, CDL Department to see what was the requirements to get my CDL static back, and was told that as long as my license was not expired that I needed to take a physical and submit it to the ADOT, along with a $50.00 charge I pass the physical paid $80.00: walk the papers into the Mesa AZ CDL and was told that my CDL is revoked and was down graded to class D from Class A, and was told if I wanted to get my CDL back, that I would have to start all over from the beginning, Is this a true fact.

    P.S. I’ve held CDLs since 1974

      Each state is different and you have to get this answer from the state DLD. Go up to a supervisor and confirm the facts.

      Yes as it almost happened to me when I moved here from another state. You have to mail or deliver in person to the medical dept your medical exam report. I handed mine to a person in the comnercial DMV office and he didn’t mail it in. I received a letter that had a date of June 05 2013 which I received May 22, 2013. I was at the office in mid April for my CDL transfer from another state. The June 3rd date was the date that if they had not received a copy of a current medical which mine expired in October 2013 my license would be downgraded to a regular non comnercial license and I would have to take the CDL written and practical tests for every endorsement I had, meaning i would be starting over. So I am sorry and I think you lost your CDL. Never mind that a person can go buy a 34,000lb 39′ dual axle motor home with Air Brakes and tow a 33′ trailer with a combined length of 74.5′ and a weight of 52,000lbs. It really ticks me off that they can drive a combo heavier and longer then my dump truck with a regular license and have air brakes and we need an endorsment for them.

        Not all states do what happened to you. In most states you do have a short grace period, if there was a clerical error. All you would have to do is produce the medical examination and explain the error. Some states will charge a small reinstatement fee, for their error, go figure, but better than doing it all over again.

          In state of AZ if you don’t certify your dot card it gets suspended right? Can you reinstate cdl license by certifying it or do you have to start all over?

          Check with your local drivers license department on their process. Most states, once you get your medical card into the states data base, will re-instate your cdl.

      Two years without a card in Az will get your license (class A) revoked and you will start over. Happened to me.

    Is it mandatory in the state of Arizona if your body mass index (bmi) and the size of your neck is a part of the reason a doctor can order you to take a sleep test

      No, this is a misconception. There is no current ruling by FMCSA on sleep testing.
