All Alabama CDL holders are required to certify the type of commercial operation they’re engaged in.
Interstate Non-Excepted
If you choose Non-Excepted Interstate, you must, in addition to a completed self-certified application, provide the DPS with a current Federal DOT medical certificate.
Failure to submit the required forms to the Alabama Department of Public Safety will result in the cancellation of your commercial driver license.

Disclaimer
Your DOT Medical Card needs to be submitted before the current one on file expires. To avoid any overlap with your driving privilege, it is recommended to submit it at least 2 weeks prior to the expiration date.
Expiration:
If your medical card expires, and you haven’t renewed it, Alabama DPS will notify you that you are no longer medically certified to operate a CMV in non-excepted Interstate commerce. If a current medical examiners certificate and if applicable the variance (waiver) is not received within 45 days all your CDL privileges will be removed from your license.

MEDICAL EXAMINERS MUST CONTINUE TO ISSUE MEDICAL CERTIFICATES TO ALL DRIVERS
The FMCSA has requested that all medical examiners continue providing drivers with a paper copy of the Medical Examiner’s Certificate – Form MCSA-5876, so that Commercial Driver’s License (CDL) holders can provide a copy to the State licensing agency, and the non-CDL drivers can provide the documentation to their employers and Federal and State enforcement officials.
The Agency is currently completing efforts to put into place an electronic system to enable medical examiners to transmit the medical certificate information from the National Registry system to the State licensing agencies.
Until that system is completed, which compliance date according FMCSA is June 23, 2025, medical examiners must continue to issue paper copies of the medical certificates to drivers who pass the medical exam.
ALL DRIVERS MUST CONTINUE TO CARRY A PAPER COPY OF THEIR MEDICAL CERTIFICATE
Commercial Drivers – You still need to carry a paper copy of your medical card, until FMCSA announces that this is no longer necessary.

i had my ODOT physical done on 7-28-2021 but according to State Trooper it was not on file .if i Fax a copy how will i know they fix the problem
@ Jim
You will need to be in contact with your local authorities until you have a confirmation that the situation is resolved.
Why does the fax number to send medical card not work?????
Because they changed the way to submit the medical cards. You go online to alabamadl.alea.gov under commercial drivers to self certify. That is for Alabama, it probably varies from state to state.
Does my dot card expire at 12:01 a.m. on the expiration date?
@ Robert
12:01 am the next day.
I have been told that the DOT is currently closed in Alabama, and have a driver who have a valid medical card but cannot have it updated on his MVR due to this closure. Is there a grace period or extension / exception for this.
@ Richard R
It would seem logical that they would have more than one way to get the information into the DLD, but it is government. Would also seem logical that if they are closed that they would have to grant a grace period, but it is government. Best to look for any way in, including registered mail. Then you have proof that you did what you could do under their circumstances.
Got to alabamadl.alea.gov and go to the Commercial Driver section and it has a place to self certify the medical card.
You CANNOT self certify a medical card. You MUST mail or fax a copy.
I have a class a cdl permit, but I want to have a class b license. Do I have to retake permit test?
@ Terrell
Highly unlikely, but confirm with your local drivers license department.
How do you submit your medical certification by email?
@ jerry
Check with your local drivers license department. They should be able to guide you through their process.

does a non CDL driver have to have a medical card driving a f450 commercial truck in ALA. ?
@ David
Every state has different rules and requirements, best to confirm with your local drivers license department.
Can you get your CDL license renewed with a 3-month only health card
@ Eric
Each state is different. To the best of our knowledge, yes, but confirm with your local drivers license department.

Usually the company you drive for wants a copy and you need to get a copy of your medical card registered with your local drivers license department.
