Indiana CDL Requirements
All Indiana CDL holders are required to be medically qualified to drive a CMV by the means of a valid DOT medical certificate.
- Non-Excepted Interstate
You must meet the Federal DOT medical certificate requirements. - Non-Excepted Intrastate
You must meet the Indiana state medical certificate requirements and endorsements.
CDL Self-Certification
The self-certification process requires you to certify to the Bureau of Motor Vehicles the type of commercial motor vehicle you operate, or intend to operate, as well as acknowledge that you are required to meet the medical requirements for that CDL.
Do a Self Certification online.
- You will need to create a “myBMV” account, if you do not already have one.
- You do not need to submit your medical certificate when you self certify. This information is already on file from when you where issued your CDL.
You may use the automated system to verify that your medical certificate is on file by calling 888-692-6841, option number one (1).

MEDICAL EXAMINERS MUST CONTINUE TO ISSUE MEDICAL CERTIFICATES TO ALL DRIVERS
The FMCSA has requested that all medical examiners continue providing drivers with a paper copy of the Medical Examiner’s Certificate – Form MCSA-5876, so that Commercial Driver’s License (CDL) holders can provide a copy to the State licensing agency, and the non-CDL drivers can provide the documentation to their employers and Federal and State enforcement officials.
The Agency is currently completing efforts to put into place an electronic system to enable medical examiners to transmit the medical certificate information from the National Registry system to the State licensing agencies.
Until that system is completed, which compliance date according FMCSA is June 23, 2025, medical examiners must continue to issue paper copies of the medical certificates to drivers who pass the medical exam.

Commercial Drivers – You still need to carry a paper copy of your medical card, until FMCSA announces that this is no longer necessary.

I Had my left leg below the knee amputated. I will drive a semi that is automatic transmission and will not use my left leg in any manner other than exiting or barding the truck. Most likely, you will need a Skills Performance Evaluation [SPE] certificate before going for your class A cdl medical certification examination.

I had shoulder surgery in March. My Dr cleared me with a 20 lbs weight restriction and says I am cleared to drive. I was denied my Medical card because they say I cannot have any restrictions at all. Is this right?

Who ever did your medical certification examination is holding you to a class A cdl standard. You are not to be held to that standard. You fall under the states requirements and not federal standards. Find a medical examiner that understands the difference between state and federal regulations. Check with your local drivers license department for confirmation.

My cdl physical was due march 20 2024 I had to have open heart surgery on February 6 2024 so wasn’t able to due physical also there is a mandatory 3 month suspension from date of surgery but received 2 letters from state saying my license is suspended and 2nd letter says your medical certification status on the national commercial driver’s license information system (CDLIS) is not-certified and your commercial driving privileges are disqualified my mandatory 3 month suspension ends may 6 2024 can I get reinstated with a new dot physical with all proper heart test that is required plus all proper papers from cardiologist stating I am physically able to resume driving a commercial motor vehicle How much time do I have to do this been told I have 1 year Or do I have to start all over with all testing as if I were obtaining a cdl for first time I’m in Indiana.

Most likely with a new medical card in hand at the DLD and your driving privileges will be reinstated. Very doubtful they would make you start over given your situation. Don’t depend on some time frame, get the physical as soon as you are ready to return to work.
It says above that “All Drivers have to carry a paper copy of their CDL Physical”. Do they have to carry the whole physical or just the last page? Is it ok to shrink and laminate the last page?

It is always best to carry/have available for inspection, a copy of your medical. If you shrink it, it must be legible to the officer with his naked eye. Generally, just a copy of your medical card is sufficient. The last statement on the bottom of the medical examination form indicate that all the driver needs to have available, if an officer request, is a copy of your medical card. Take a picture and have it on your phone for backup.

Since your cdl is a state issued document, this question will be best answered by your local DLD. Our recommendation is to get it done and get it into the state ASAP.
If I take a new yearly DOT physical and forget to provide documentation for my CPAP, can I still drive on my old, not yet expired DOT physical until I get the documentation turned in?

It will depend, if the CME inputs your new information to the registry, then your old card is invalid. If he does not, then your old card is still valid.
I failed a drug screen but have completed all the necessary steps to get my CDL back except for the RTD negative test, which I think must be completed before I renew my medical license. My medical license expires soon and I am unable to pass a drug screen for my medical exam at this given moment. If I let my medical license expire for a couple weeks to ensure I am able to pass a drug screening, would I completely lose my CDL or just have to make some type of payment since it is past due?

Most likely you will not lose your cdl by waiting to pass the drug screen. Confirm with your local DLD.
If your CDL medical card expires and your doctor can’t sign off on it for a year (due to medical issues) what happens to your license? Is it still valid? Does it expire? Do you have to go through the entire testing process again? It’s hard to find information about this. Can you direct me to the best place.

This is going to be best answered by your local DLD.
You will be medically disqualified, but do not lose your license. You cannot drive a truck but you can continue driving a passenger vehicle. Need to verify how long you can be medically disqualified before you lose your CDL endorsement. Mine was for three months and I did not need retesting.
After the date on the physical gets older than 30 days, it will revert your class a to an operator….after a year you will have to retest.

Since this is not class A cdl, you will fall under the states guidelines and variances. best answer will come from your local DLD. You may be restricted to intra-state only, but that is only a maybe. Check with your DLD.
If your CDL medical card expires and you go to the doctor to renew, can they extend the Medical card for 45 days for a re-examine. Or is your medical card expired on the date it says.

There is no extension, card is invalid at midnight of the expiration date. A new medical certification card has a new date of expiration.

Just get in touch with the medical examiners office that you used. By law, they will have a copy and should be able to forward one to you.

It will depend on how long ago your medical card expired. Usually, a few weeks expired requires a new medical card. Months expired may have fees attached to reinstating your cdl. Years, may require starting all over.

It all depends on the class cdl you hold.
If a CDL driver is newly diagnosed with high blood pressure, does the driver self report that to DOT or what does the employer need to do? I can see there will be more frequent re-certs. But up front, do we (the employer) need to send for a new physical?

If the driver was recently placed on bp medication, then he will need a new medical card. He should be on these meds for at least thirty days and see that they are being effective, before the new DOT/FMCSA medical examination is done.

Unless your primary physician is on the national registry as a certified medical examiner for DOT/FMCSA, then the answer is no.

I have been on high blood pressure and warfarin for 14 years getting and have always been granted a 2 year physical certificate.
I just talked to an examiner, while scheduling a new physical and was told I can only get a 1 year certificate, because of the 2 medications I take. My BP runs 120/68 my warfarin has been dialed in for 30 years. Are these new rules?

No, these rules have been in place for years. When the DOT/FMCSA created the Certified Medical Examiners, (CME), the rules and guidelines were already there, but non-CME’s had been very lax or did not understand them. The CME’s have to follow the guidelines or be dismissed from the registry.
Actually no….I have been on high blood pressure medication for the last 15 years, and yes, every year you have to renew. It’s ridiculous because the meds keep your blood pressure under control. But yep….every year you gotta spend your hard earned money to keep someone else employed.
I’m in Indiana, and was told by a friend that he was recently pulled over and ticketed for not having a medical certificate. He was in a pickup truck, pulling a tandem trailer — not for hire — and under the 10,000/26,000 LB weight categories. I’m not sure who wrote the ticket (DOT or ISP). He may have been in KY when this happened — we are both involved in volunteer relief work for church.

States can be funny about pickup trucks and weight limits for a medical card. In some states, if a truck is capable, not transporting but hauling in excess of 26,000 lbs, then a medical card is required. This varies state to state.

It will transfer with you, make sure it gets into the IN drivers license department data base. Check with IN DLD for their process.
If your CDL physical requires you to return each year (not two years) due to blood pressure, etc…Is it a state requirement for your personal medical information be stated and submitted to your employer as to why each year.
I read, the medical examiner is not required to submit unless the employer requests it and a signature from the driver is obtained.

Unless the employer is paying for the medical certification examination, the CME should only provide you, the driver, with a copy of the long form. It is not stated on your medical card as to why you have a one year card. The company may have a written policy within the HR dept. that you provide a copy of your long form. A medical release form must be on file with the doctor to release any information about your medical status.
Even when the employer is paying for your medical, they can NOT have access to your records unless you fail or you give your permission.

DOT/FMCSA has extended their grace period until the end of August, for the moment. There may be another extension, but not known now. In most cases you will need to get your DOT medical certification examination done as soon as you get back and get it to your states drivers license department. That should avoid retesting.

Unless there was some missing information that was requested by the CME, then the entire physical must be redone. If the CME provided a 30/90 day card, then the entire physical must be redone according to DOT/FMCSA.

Most states require you to carry your card with you when ever you drive. It’s only ‘really’ important when you get stopped by a police officer or DOT officer.

My medical examiners certificate expired in February. I thought I had 1 year before I had to retest. The Indiana BMV has given 3 people 3 different answers on when my CDL will be taken if I don’t submit a new certificate. According to Indiana BMV, the fmcsa determines the length of time before your CDL is taken completely. They told me 1 year, my wife 6 months before a written test is required and 5 years before I need to start all over and they told one of her drivers he had 180 days and had to surrender his CDL since he lapsed.

CDL is a state issued document and is under the states control, not FMCSA. To avoid further complications, if you want to keep your cdl, get a new medical done and get it to your states DLD. If you don’t want to keep your cdl, then go to DLD and downgrade, before they do it for you.

Check with your local drivers license department. In most cases all you need is to update your medical card, but confirm that with your state.
Yes . This happened to me. All you have to do is print out the cell physical form or ask the BMV to Print you one out.
Take it when you Go get a CDL Physical and turn the completed paperwork into the BMV. And they will update it in the system.
If your CDL License was suspended. It will reinstate once it’s uploaded into the system.

I moved from Florida and was working for county government. All government agencies are exempt from the physical requirement in Florida. Planning on doing government roadwork here and was wondering if Indiana requires physicals for government class A drivers?

I fell in December of 2018 and suffered head trauma. In March of 2019 had a seizure due to the head injury. I’ve been on medication and seizure free. The state of Indiana requires for you to be seizure free for one year. Well it’s been a year got my medical card took my Dr. release disclosed everything to the examiner. Took my card to the bmv got a call that I need an exemption form from FMCSA . So I call for the fifth time four were previously to make sure I didn’t need a document or anything. So the answer was no , tell them my problem so it gets passed on to the medical safety dept. I will receive a e-mail which I did saying since I’m intrastate I fall under state regulations. Federal has nothing to do with it. Well I take a copy of two separate e-mails saying it’s state regulations so they say feel a affidavit saying I’m a intrastate driver and that will clear up the confusion and I will receive a letter which I got saying I need a exemption form from FMCSA. I know it’s a friggin joke. So back to the bmv they call back to the cdl and say oh the problem is my vision test wasn’t filled out go to the examiner and bring it back. So yes I have my card with me show to young lady as she tells the lady I’m looking at it now it’s written in I can hear oh yeah I see it. Well now it’s wait on another letter with information as to what they need. Yes your right it’s going to say we need a exemption form from FMCSA. Someone help please.

I live in Indiana and I had a physical prior to starting new employment in construction operating heavy equipment. I was only granted a 3 month because of risk of sleep apnea. My 3 months expired 3 months ago. Can I just go get a new exam? My employer won’t allow me to work without it.

Check with your local DMV to determine the process you will need to go through to maintain your cdl. Once you are released, without restrictions or limitations, get a new medical card. Your local DMV should be able to guide you through the process. And your meds are temporary, so it is unlikely that they will be an issue, once you have been released to return to work.

Check with your local DLD. You would fall under the states variances more so than FMCSA. If you need a medical card according to the state, then make sure the examiner understands your states laws and variances.
I have a 12000 GVWR truck in Indiana and do not require DOT numbers because we only haul our own property within the state. Does Indiana require my drivers to get a DOT physical? Hello I’m in in Indiana I just took my cdl physical yesterday for class B with air brakes I had leg injuries but I function ok the doctor approved me I told them about the cholesterol meds I was taking except I didn’t tell them about my hydrocodone script I was planning to maybe not take them and was not taking them during the day thought I might drop them because of the stigma with them. Except I rely appreciate being able to take them after the day is over to relieve the days pain and be able to function at home and sleep pain free at night how do I go back and
have them added I don’t want to be dishonest i can get a letter from my doctor. And what repercussions can I expect getting a job I function very well on them I don’t have any of the so called side effects from them they genuinely just take the pain away regards 🙂
If your company does random drug testing you are toast. Hydrocodone is a class 1 controlled substance. It is a derivative of codeine which shouldn’t be a big deal but it is.
My license has been downgraded causemy medical card expired. I just went and got a new physical and passed they now send it to the state on there own how long will it take for my license to be upgraded I’m in indana.

Just the medical certificate (but having a copy of your long form isn’t a bad idea for backup – take a photo of it on your smartphone if you have one!)
I carry CDL licenses in Inpls in…I just took my DOT phycial n was informed that bcuz my BMI was 2 points over I have to have a sleep study…. I was given a 90 day card to have the test done…my problem is having hard time finding a clinic also not sure of the cost n if insurance will pay…? Is what can I do if I can’t get the test done???

You’ll have to check with your local drivers license department. They may allow you, while you’re not driving, to change your self-certification to “intrastate excepted” so you don’t have to have a current medical card. Or they may have a period of time where you can downgrade your license, then reinstate your CDL again later. Only they can give you they details of how they can help you with their state process.

You need to have an interstate CDL medical card and drivers license. A lot depends on what class driver you are and why you have been limited to intra-state only.

Report and bring it back before this expires and he will give me the full year card w/o retaking the physical, I did and now they say the dot changed this and now I have to retake the physical, sounds like they make this up as they go, are they correct?

Based on the new FMSCA forms, there are no extensions. Each time is a new physical exam and a new certification that overrides the previous one. Your new card will be a full year starting from your new exam date.
I went for a dot CFL physical and told them I had mild neorupathy in my feet. It’s so mild I don’t take any medicine for it. The doc told me I was disqualified to get a CD’s. Is there an appeal process or can I look for a different Doctor?

Go get a second opinion elsewhere. Have your primary care doctor fill out a medical release form explaining your situation and have that in hand before your next DOT exam.
I live in TN which does not require a dot medical card for driving a box truck under 26000 lbs. I am going to work for a company across the state line in KY and the company is based in Indiana. Ky will not grant me a waiver because I live in TN and tells me to contact TN Dept of Licensing. TN tells me they can’t issue a waiver because they only require a dot medical card for CDL drivers which I will not be. Which state rules and what must I do to get a waiver that TN says I don’t need?

Sounds like your driving category will now be considered “interstate” rather than “intrastate”.
Interstate drivers fall under FMCSA regulations and need a federal medical card. You will also need to self-certify your driving category.
Not sure what waiver you are talking about. But, if you need a waiver for medical qualification, then as an interstate driver you’ll need a Federal waiver.
You should still be able to drive with a TN license, so talk again with the folks at TN drivers license department about self-certifying “interstate”.

I’m in Indiana and I just had a driver go for a re-certification physical .. He was told he needed to have a TB test before the paperwork could be filled out. Is this something new? I had drivers go just a few months ago and this did not come up. Also, our drivers are in a random pool to be drug and alcohol tested. Do they need to be drug and alcohol tested at the time of the physical also?

Sounds like someone is over reading the FMCSA guidelines. Urine testing at the time of the examination is for proper kidney function only. There is no drug or alcohol testing, unless the company requests it. And TB testing before you even see the examiner or fill out paperwork has nothing to do with FMCSA guidelines. After an examiner sees and evaluates a driver, the examiner may ask for further testing, but before hand???? Look for another facility to do your testing if this is what you are getting right now.
We are a business in the state of Indiana. We have single axle box trucks that we use to perform our service. Do our staff have to get dot physical? How old do you have to be to drive a 16ft box truck. Thanks

In most states, they are requiring a medical card to operate a commercial vehicle – that is any vehicle that is being used for commerce. But check with your local state drivers license department for a confirmation.
I am a vending route driver and drive a 26ft. box truck only in the state of Indiana. My company recently required me to take a DOT Physical. I am an Insulin Dependent Diabetic (Type II Diabetic). The physician who did my physical wrote temporarily disqualified on my physical form. She referred me to the FMCSA website and told me to apply for an Insulin Waiver. I read a paragraph on the website that said the FMCSA waiver/exemption does not apply to intrastate drivers. With that statement I then am confused of how to proceed with obtaining a DOT card. Does the state of Indiana offer a waiver program. If so where do I obtain the forms and information to apply for this waiver.

You really want to check with your state drivers license department. In most cases the state has what you need.
You are an intra-state only driver and your drivers license department will steer you in the proper direction. The CME is doing a DOT physical for an interstate driver and is not taking into consideration your needs or requirements.
I was told by my doctor today because my BP was 144/80 I would get a three month card, but because it expires after December 22 I would have to have a whole new physical..
I work for a doctor and we have many CDL licensed patients. Most are on suboxone, but for pain management and not opioid dependence. Can they be on suboxone even though its considered a controlled substance?

You need to have the doctor explain the need and usage of this medication and how long they have been using suboxone. Have there been any ill effects or side effects on the driver? Then if the CME feels that the risk is extremely low or non existent, then he may issue a one year medical card. The driver will test positive on his DOT drug screens and will need to explain to the MRO what drugs he is taking and have a written prescription from the prescribing doctor to confirm the facts.
What about the medication Gabapentin, is a driver permitted to use this medication and continue to drive and pass DOT?

It all depends on why this medication is being used.
And you would need a medical release from your doctor.

It all depends on why you are taking this medication. It has effects that could impair your ability to operate a commercial motor vehicle. So there are going to be lots of questions about its usage. A medical release will be required from your prescribing doctor.
My husband recently went in for his DOT physical. He is a type 2 diabetic and controls it with medication. He did not pass his physical because of quite a bit of sugar in his urine. According to his PCP one of his medications (farxiga) is the cause and the medication is doing what it is supposed to do. The doctor granted him a 3 month extension (his blood pressure was also high) and asked him to get blood work done because of his sugar. The doctor also stated he needed to see what his A1C was. Is it possible for him to get another extension and why does the doctor need to know what his A1C is?

With the new medications that are being used with the treatment of diabetes, we have no way to confirm or deny that the sugar is under control without a recent A1C. That’s why the CME is requesting one. As far as another extension, you’ll need to check with the original CME on that.
I have my CDL license I have had it for about 5 to 6 years now and I was recently told because of one of the medications that I’ve been done for over 10 to 12 years now its a narcotic and I am NOT able to renew my physical examination is there anything I can do to get an exception to this rule

Download a medical release form and take it to your primary doctor. Have him explain the needs and usage of the medications that you are taking. That answers most of the questions that the CME will have regarding a medication that you have been taking for years.

There are NO NEW RULES. So if you were driving before, just continue what you have been doing.

My son was told that he now has to see a PCP and get a sleep apnea test within 3 months because the female doctor he saw tonight said that he is overweight and must be falling asleep at the wheel. He has lost weight over the last 2 DOT ‘s he has had done and exhibits no sign of sleep apnea. I know this because I am a LPN. Can they make him do that?

Best bet is to get your medical card renewed a few weeks before the old one expires. Then submit your information to your state DLD right away. Then you’ll always be sure you’re okay, rather than play Russian Roulette with your CDL.
The processing time will differ from state to state depending how busy or backlogged their system is.
The states also differ on their downgrading rules.
You’ll have to check with your state drivers license department and even then you may not get a ‘days’ answer to your question.

They will not accept DOT physicals by nationally certified medical providers who are nurse practitioners, physician assistants or chiropractors as FMCSA allows. Is this true? If so, why does the state of Indiana have these professions on their certification listing for their state?
You are correct. In Indiana, professionals in these categories are already certified and listed on the NRCME database.
I would question the originator of this statement, rather than the state of Indiana.
They may have received confusing information. It’s true that the state of Michigan does not certify DCs, although they do certify APN, DO, MD, NP, and PA.
Your response is incorrect. I am an advanced practice nurse (NP) and have performed over 20,000 DOT physicals in the past 12 years. I am nationally board certified and am totally qualified to do these exams. Better check your facts sir.

I have had a CDL for nearly 30 years now. I drive a straight truck within the state of Indiana. I take Tramadol for moderate osteoarthritis and have taken it for approximately 10 years. I understand that it’s classification is changing to a narcotic. Will this disqualify my CDL? If so is there a waiver that I can get?

As a driver driving intra-state only, I don’t believe you will have any problems. The change is mostly going to affect over the road drivers in 18 wheelers.
I would just check with your state CDL/DMV and confirm that that is true for your state.
