Vermont CDL Requirements
If you hold a commercial driver’s license (CDL) or are applying for a CDL, you are required to certify the following information with Vermont Department of Motor Vehicles:
- The type of driving you are engaged in (interstate or intrastate) and
- Whether or not you are subject to or exempt from the Federal DOT medical examination requirements.
Interstate Non-Excepted
You must meet the Federal DOT medical certificate requirements.
Intrastate Non-Excepted
You must meet the Federal DOT medical certificate requirements.
All CDL holders must continue to carry all medical certification documents with them at all times even after completing the self-certification process with DMV.
Submit Your CDL Medical Certification to Vermont DMV
Submit the following two documents:
- CDL Medical Certification – download the form here from Vermont DMV.
- Copy of your valid DOT medical card (not the long form). Enlarge the copy to ensure that all information is legible. If your medical examiner’s certificate is only valid with a vision, diabetes or a skills performance evaluation waiver granted by FMCSA, you must also provide a copy of your waiver.

Keep Your Medical Certificate Current With Vermont Agency of Transportation
You must ensure that your medical documentation on file with DMV remains current and accurate. Failure to maintain current medical documentation on file with DMV may result in a suspension and/or downgrade of your CDL.
You may submit your new medical certificate by mail, fax, or email as described above.

Special Information Updates Pertaining To ALL STATES
MEDICAL EXAMINERS MUST CONTINUE TO ISSUE MEDICAL CERTIFICATES TO ALL DRIVERS
The FMCSA has requested that all medical examiners continue providing drivers with a paper copy of the Medical Examiner’s Certificate – Form MCSA-5876, so that Commercial Driver’s License (CDL) holders can provide a copy to the State licensing agency, and the non-CDL drivers can provide the documentation to their employers and Federal and State enforcement officials.
The Agency is currently completing efforts to put into place an electronic system to enable medical examiners to transmit the medical certificate information from the National Registry system to the State licensing agencies.
Until that system is completed, which compliance date according FMCSA is June 23, 2025, medical examiners must continue to issue paper copies of the medical certificates to drivers who pass the medical exam.
ALL DRIVERS MUST CONTINUE TO CARRY A PAPER COPY OF THEIR MEDICAL CERTIFICATE
Commercial Drivers – You still need to carry a paper copy of your medical card, until FMCSA announces that this is no longer necessary.

I have my CDL can I keep it without my med card? I don’t plan on driving for a year.If I change my mind do I go update my med card?

Most states require a valid medical card to maintain a cdl. If you let the medical card lapse for a period of time you may need to re-test. Best to confirm with your local DLD for guidance.
What do I need to bring with me when I renew my medical card?

If you have no health issues, then all you need is a smiling face, a way to pay, a need to pee, and picture ID.
I have an employee who will be on medical leave but her DOT physical is due during their recovery period. The employee may not pass the exam at the time of expiration, but will more than likely pass after recovering. Is there an extention that can be placed on her DOT medical card in the State of Vermont for such matters? I would appreciate any information.

A driver out on medical leave will not be tested for a medical card. Check with your local DLD for guidance. They should have a process in place for such an issue. DOT/FMCSA has no extension process.
My trucks are under cdl weight and are intrastate only. Do my drivers still need for medical cards?

Question…. Can you have sleep apnea and still keep your CDL license if being treated and compliant?
Why do the State Workers working on the Highway Department not have to have a medical exam like the rest of us have to???? Seems unfair that men with heart issues, high blood pressure and diabetes don’t have to have a yearly physical like the rest of us do. And there are a lot of those people working for the Highway Department and no exam is required. Why??????

I have a class A license but am no longer driving a commercial vehicle for my company. Do I need to renew my DOT medical card when it exprires or can I just change my status with the VT Department of Transportation?(stating that I am no longer driving a commercial vehicle but want to keep my class A license for later use) When I return to driving a commercial vehicle, I will then renew my medical card.

Some states may let you change status to Excepted, while you’re not driving a CMV. Some will not.
You’ll need to check with your local drivers license department.
You just need to notify the DMV !! I had to do this when I came off the road @ and my medical card expired !!
Is there a waiver process when employee fails medical exam for cdl because of type II diabetes.

Not that I am aware of. The FMCSA has a waiver process for drivers who use insulin.
A type II diabetes mush have his condition under control to drive. So a failed medical card due to diabetes not being under control only needs to get the situation controlled and then he/she would pass for another year.
What are the age requirements to have a dot card and driving between different states most important Vermont and New Hampshire.
