Hawaii CDL Requirements
All CDL holders in the State of Hawaii are required to have a valid DOT medical card.
Failure to keep Medical Certification current will result to immediate downgrade to a class 3 license. CDL status will be reinstated once current Medical Certification copy is submitted.
Interstate Non-Excepted
You must meet the Federal DOT medical certificate requirements.
Intrastate Non-Excepted
You must meet the Federal DOT medical certificate requirements.
Important: If you operate in both excepted interstate commerce and non-excepted interstate commerce you must choose non-excepted interstate commerce to be qualified to operate in both types of interstate commerce.
Submitting your Medical Certificate to HDOT MVSO Driver Records
Submit the following two documents:
- CDL Driver Self-Certification & Current Medical Certificate – download the form here from HDOT MVSO Driver Records.
- Copy of your valid DOT medical card (Medical card only, not the long form.) Enlarge the copy to ensure that all information is legible.

Special Information Updates Pertaining To ALL STATES
MEDICAL EXAMINERS MUST CONTINUE TO ISSUE MEDICAL CERTIFICATES TO ALL DRIVERS
The FMCSA has requested that all medical examiners continue providing drivers with a paper copy of the Medical Examiner’s Certificate – Form MCSA-5876, so that Commercial Driver’s License (CDL) holders can provide a copy to the State licensing agency, and the non-CDL drivers can provide the documentation to their employers and Federal and State enforcement officials.
The Agency is currently completing efforts to put into place an electronic system to enable medical examiners to transmit the medical certificate information from the National Registry system to the State licensing agencies.
Until that system is completed, which compliance date according FMCSA is June 23, 2025, medical examiners must continue to issue paper copies of the medical certificates to drivers who pass the medical exam.
ALL DRIVERS MUST CONTINUE TO CARRY A PAPER COPY OF THEIR MEDICAL CERTIFICATE
Commercial Drivers – You still need to carry a paper copy of your medical card, until FMCSA announces that this is no longer necessary.

I work for State Government in HI. My employer is requiring that non-CDL drivers who want to drive a F-350 (or similar rated) pickup truck need to get the HDOT Medical Card. This is for a off-road wildland fire application. I am having trouble finding what the circumstances are that would require any non-CDL driver to obtain a Medical Card are. Can you point me in the right direction? Everything I have been able to find lays out specifics for CDL drivers. Thank you

Most states require a medical card for anyone who is driving any motor vehicle (non cdl also) which is being used for commerce, (making money for someone). Best to confirm with your local drivers license department.

If the vehicle has a GVW of 10,000 lbs +, the drivers are required to have a medical card.

It is more about employment, not requirements. You can get it done when ever you want, but your job will need it for you to drive legally for them.

Yes, and go get it done ASAP, then get a copy of your new medical card to your local DLD right away. That will stop any action that would effect your cdl.

No. Grace periods for covid are over. Your medical card expires at midnight of the date of expiration.

I currently obtain a CDL in the state of Hawaii, but my current profession doesn’t require my CDL is there anything I can do to put my CDL on hold until I need it so I don’t need to do medical Examinations regularly?

My Medical Card has expired and I am trying to get a sleep apnea test done. It has been 30 days since it expired but I am having difficulty getting an appointment for the testing. I guess there are so many needing o do this test since it was implemented.
My question is: How long can I go without renewing my medical card before I am either downgraded to type C or lose my CDL?

I have a vehicle with a GVRW of 18,000 lbs.
DMV is telling me that I only require a Type 3.
Do I still need a PUC Medical Card?

Since DMV has given you the rest of the information they will tell you if you need a medical card. Check with DMV.

I’m visiting Hawaii as a driving instructor. Is it a requirement to have a DOT medical card to operate a vehicle over 10,000 lbs? You must on the mainland, but I’m being informed it’s not necessary here. I can’t find anything written down to corroborate that.

I’ve been driving for 13 years with a heart defibulator and this new doctor my company is now using to do our DOT tests will not pass me because of it . Is there a new law that I’m not aware about ?

Actually it is an old law. As far back as 2009, the FMCSA was stating that a defibrillator should not be certified to drive interstate CMV. But it really did not come to light until the NRCME came into existence.
You might try having your primary cardiologist complete a medical release form for you, stating that you have had this for a number of years and have had no issues with the unit and / or your heart. It might be a shot in the dark, but if the CME understands and can comfortably support his choice, then maybe. Also if you are driving intra-state only, this will effect the CME’s opinion too, maybe.

If it was done by a certified medical examiner, then there should not be a problem.

Are there any Federal or State statue that require your employer to keep, maintain and or pay for your medical examinations. All my past employers has paid for this examinations. My current employer will not pay for my PUC now. Did something change?

Each company has its own policy, you had one company that thought it was a good thing to pay for your examination and now you work for a company that has a different policy.
There are no state or fed regulations that a company must/has to pay.

Can you tell me what the blood pressure requirement is for get your PUC. I’ve always passed my renewal. But this time there is a new nurse and she just gave me three months and not my usual year. She says because my blood pressure is high. I can go anywhere else and my BP is good. So I’m just curious what the max BP is.

FMCSA does not have any type of extension. Their policy is to have you re-tested, if you have had an illness that took you out of driving for any period of time.

Check with Hawaii DLD/DMV how to keep your CDL while you are not in a position to have a current medical card.
