If the hernia does not interfere with your operation of a commercial motor vehicle, then there is no problem.
If you have pain and are limited in some way, then you should take care of the problem before it does become a serious problem.
I had hernia surgery three years ago. There is some scar tissue which causes a little discomfort from time to time but in no way renders me unable to perform any tasks. Will this cause me to fail a DOT physical?
During the examination you will be questioned regarding the hernia surgery. Just let the medical examiner know how this effects you and whether it interferes with your daily activities. Some companies have restrictions, but this is a company policy not a DOT regulation.
I have a hernia that does not give me any trouble. The doctor I saw for my DOT exam says I have to get it repaired before my next exam. What if my regular doctor says that we can do watchful waiting instead of surgery? What are my rights under the law about this?
The DOT examiner is looking at a progressive condition, but if you will take a note from your primary doctor who has evaluated your condition and understands what you really do for a living, then the DOT examiner should consider your primary doctor’s opinion. However, the final decision to certify you, or not, remains with the medical examiner.
What if I have a limb impairment? Am I disqualified?
It is possible to apply for an exemption. FMCSA has a certificate program for drivers with limb impairments. FMCSA also has a special certification program for drivers with missing limbs. (see FMCSA Skill Performance Evaluation)

I’m a cdl holder with a dot expires in soon. I’m not currently driving commercially & haven’t since the surgery. I have returned driving personal vehicle.
I had a t10 laminectomy last August in Iowa. I did 3 weeks rehab in Omaha, Ne. Then I moved to Az w/family.
I continued physical therapy for “dead butt” syndrome.
Ok, I’m recovered/ing. I run, I pass mock exams & get a clear Babinski reflex. Indo jumping jacks, one legged bounding, etc.
So I want to go back to work which means I need to get a DOT exam.
My problem is the surgeon won’t release me, he hasn’t seen me since sept. I can’t get into my local neurologist until a June appt & my next pcp is late march.
On my own I scheduled & am paying for a Functional Capacity Evaluation (fit for work exam) but that’s still weeks away & after my med card expires.
So my current plan is take a dot exam, disclose the surgery from 7 months ago & the examiner can tell I’m recovered to drive.
Any advice about this?
Also, I’m curious what questions or scrutiny can I prepare for? I’m also a bit nervous I could lose my cdl entirely. Any feedback is appreciated.

It would be very helpful and may end up being a necessity for the CME. Medical release forms and a full return to work are needed for DOT/FMCSA. If you are ready to go, the medical examiner will not have any issues other than paperwork.
I have drop foot my left foot. I operate my vehicle with my right foot. Also, don’t most trucking companies have automatic transmissions, then I would drive a truck using right foot. Even though the drop foot is my left foot, would that disqualify me from getting DOT medical card? Do I need to get my spe certification? What are the steps I need to take and what order? I want to obtain a DOT medical card, so I can get my CdL class A.

If you wear a brace for the left foot, then you could be OK. It is not a disqualifier in its self, but will raise questions with the CME. We do not believe you will need a SPE certification if you are going to be driving an automatic.
i had triple bypass surgery 2 years ago. now I’m told that I need a stress test. Do you know the name and details of that test?

Your cardiologist will need to order the stress test, so get the information from them. If you are a class A cdl holder, you will need a stress test every two years and your medical card will be good for one year at a time. If you are a lesser class cdl holder or intra-state only driver, you will fall under your states guidelines and variances. Check with your local DLD.
I have drop foot on my right foot caused by nerve damage but I wear a brace and it doesn’t drop. I drive fine I feel for 20 years now. Can I pass a dot exam to be a truck driver ?

For a class A cdl, DOT/FMCSA is a bit restricted. But you should be fine. The CME will have to make the final decision.
I had a level 3 cervical spine surgery on C4, C5 and C6 in May. Doctor gave me lifetime restrictions of no lifting more than 30 pounds.

Hard to say. The CME has a series of things he must take into consideration regarding ranges of motion in the c spine and what restrictions have been placed on you by your physician. A note from your doctor as well as your job description from your company will be helpful in his/her decision making process.
I recently had an accident at home causing a tibial plateau fracture so severe it caused compartment syndrome. i was emergently taken to surgery to relieve the swelling of muscle and nerves. unfortunately, just over 2 months later I have no motor skills in my left foot. If this turns out to be a permanent disability, will it prevent me from obtaining a DOT card in order for me to return to work as a commercial driver?

If it becomes a permanent issue, there are steps that can be taken. For a class A cdl holder, you will need a skills performance evaluation. It will be needed before you go for your medical card. Lesser class cdl’s or intra-state only will fall under the states guidelines and variances. Do confirm with your local DLD.
I had my lower left leg amputated in january of This year… I’ve had a CD l and it’s still current. But my medical card expired in January so I need a new one anyway. I’m walking with a prosthetic now. And I have an automatic transmission Designation on my license anyway … I also have limited use of my left hand due to the accident but I can still drive.. Will I be able to pass the DOT physical with those extremity issues?

You will need to be tested by a specialist for a performance evaluation in the operation of a cvm. You will need this before going to a CME for your medical examination.

I’ve diabetes for quite a while and have neuropathy in my feet. My current doctor has not made issue of this but is now retiring and I need to see another doctor. How does this affect me moving forward if I come to see you?

For a class A cdl. You will need your diabetes exemption form and supporting medical documentation indicating proper compliance. The CME may request additional information to help aid in the decision making process and in determining your safe ability to perform the duties of a cmv operator.
I’ve been on disability due to back and neck injuries causing a complete physical/mental breakdown over a decade ago. I think I’ve reached the point in my recovery where I’m ready to try driving again, however none of my doctors want to assume liability by saying I’m able. I tried getting a physical by a return to work doctor, who said they would only do a comprehensive 16 page exam like I had before to get on disability (at my expense). I’ve worked really hard to get back to the place I felt safe and comfortable driving again. I took myself off the road from a high paying union job where I could return at any time and have lost over a million dollars from doing so. I can pass all the basic function tests easily and would never dream of driving dangerously.or taking a job requiring me to. At the same time though, I need someone to give me a chance though I am on meds for pain and anxiety which are not prohibited. I’m not going to lie, my MCSA-8375 looks like a horror story. How do I get a medical examiner to trust me and give me the chance? I’ll be using Ticket to Work to protect my disability.

The issue is not a question of trust, but a set of DOT/FMCSA rules, guidelines and regulations that must be met before passing a driver. If what you present with looks like a major risk factor, then no CME is going to take on that risk, just like your other physicians. If you look into intra-state only driving, you may find the regulations less strict than DOT/FMCSA.

I have a diabetic ulcer on my right foot so my doctor has me in a post-op (hard dole) shoe while driving. My employer says they’re not ok with this and won’t let me continue training until I no longer need the special shoe. Am I allowed to wear this specialized shoe or no?

It all depends on the class of your cdl you are working with.
I am recovering from an ulcer on my foot. The doctor has put a small portable wound vac on it to speed up healing.
Will I be able to pass a dot physical with the vac on? It doesn’t interfere with normal driving or walking.
Trying to figure it out as my card expires 1st week in January.

For a class A cdl, you may have to wait before you can do the examination. That will depend on the CME and his/her findings at the time of the exam.
I have painful trigger fingers across the width of both palms of my hands.

For a class A cdl, yes. You would be best to have the condition treated and corrected before doing the medical physical.

Class A cdl medical examination is going to require the ability to raise the arms to a functional level for the safe operation of a cmv.

It will raise questions. The situation will need to be determined by the CME as to whether you should do the examination now or wait until cast comes off. DOT/FMCSA for a class A driver, you may need to wait until the cast is off.
My husband is getting ready to have a shoulder replacement, his doctor told him after his rehab he will be better than before surgery but it will take a little time to build up to that (roughly 3 months). He is not an over-the-road guy, he runs freight locally between RI and Massachusetts in a tractor/trailer. His was told he HAS TO disclose the surgery when he goes to his DOT physical, which would be due right around the same time he expects to be returning to work.
Is this true? I’m not sure why he needs to disclose this if he returns to work with no restrictions.

There is nothing to hide here. The forms ask have you had ANY surgeries? As long as there are no restrictions or limitations, then there are no issues. Besides, when he has his physical, the CME will notice scars on his shoulder.

As long as you can perform the duties of a cmv operator, you should be OK. The CME may request additional information from you and/or from your physician. He/she may also require a medical release form from your doctor.
I have a class A license and have been through 4 surgeries on my left shoulder ending in a reverse shoulder replacement. I have been fired because of this permanent disability of 55% and a lift/pull/push restriction of 25 pounds. I haven’t been able to drive professionally since the accident in April of 2021. Two weeks after the accident my medical card expired and my license downgraded. All I have to do is get my my medical card renewed but I’m not sure I can pass it since I have a very limited range of motion and was released with a 55% disability rating. I also have disk degenerative disease in my lower back with several bulging disks and lower lumbar spinal stenosis making it incredibly painful to stand or walk for any length of time or short distances. So is it even possible to be able to get my medical card renewed with these issues or is my only option to fight social security and be stuck on disability for the rest of my life when I’m only 57?

For class A interstate driving, you will have a very difficult time passing the medical. Intra-state may be an option due to state requirements being less strict compared to federal regulations. Check with your local DLD for guidance.

I have knee pain on and off I can walk fine some days and others it hurts mainly when it rains due 2 authorities. I can perform my hub just fine. I drive an automatic semi..but even standards I know how 2 float the gears..does this disqualify me 4 dot or do I even have 2 mention it ?

I am on workers compensation due to an at work accident with my truck. I have to get both shoulders operated on. I have gotten one surgery already. My DOT Physical is coming up. Can I get a physicians note to postpone my physical?

I have had my CDL since ’07 and it is still in good standing. I now use a cane because of bad knees, but that does not prohibit me from driving. However, when I went to take my DOT Physical, I was informed, without even seeing what my limits are, that I would not pass. Is this legal?

I am currently on workers comp for a work related injury. My left leg was shattered at the knee last June. I am receiving PT 3X a week and have been put out of work until the end of March, but it will be at least the end of summer before I can go back. My DOT medical card expires February 6th. I won’t pass right now because of my leg. Is there an exemption/waiver/hold I can get for my DOT because I have a work related injury and am not driving?

None of these by themselves or combined are a disqualifying factor. If any of these create limitations in your abilities to operate a commercial motor vehicle, then there may be restrictions.
I have a CDL B license in the state of Michigan with a current DOT card that expires in 5 months. I had total knee replacement surgery 9 weeks ago and my doctor has given released me from his care and provided a back to work slip. Can I continue under my current CDL card or do I need to re-certify my medical card before I return to work? And if I do need a new card do I need to wait a specified amount of time?

Your cdl falls under your states guidelines and regulations, not federal DOT/FMCSA. Check with your local DLD. For class A cdl holders, once fully released, you would need a new medical card. Not sure at the state level. Once fully released, no waiting period.
I had a total left hip replaced. I passed my DOT physical. I had a Full release from my Doctor to return to work. The MRO said that he would not give me my medical card until I was post-surgery 90 days. I told the MRO I never drive I have a class A because of the safety sensitive part of my job. I am an Operations Manager. Can this MRO contradict my Doctors complete medical release.

I will assume that MRO (medical review officer) is being used to mean the CME (certified medical examiner) who performs DOT/FMCSA medical certification examinations.
The CME has to abide by DOT/FMCSA guidelines and regulations, which your medical doctor does not. There is only a guideline here to wait for 90 days, if the CME feels that there may be some level of risk to you and the public at large. Seems that your CME is being overly cautious and is not hearing what you are saying as a non-driver. But DOT/FMCSA is more strict on class A drivers than any other class cdl.
I had shoulder implant surgery and my arm does not go all the way up will that effect my dot medical card for a class A CDL or how far does the arm have to reach up?

It will depend on the class cdl you are testing for.
For class A, it will be determined by the medical examiner. Lesser class cdl is under the states guidelines and variances, so check with your state DLD.
I had colon surgery in October for what was stage one cancer. My surgeon cleared me for returning back to work with no restrictions after January 1st. My dot card expires in December. Is there anything I should be aware of before I try to get my dot card renewed. I am on fmla which expires in mid January

Once fully released to return to work, go for a new medical card and get a copy to the state ASAP. You should be fine, if all else is normal.
I broke my right leg on the job and am on workers comp at the moment, it has been 6 weeks now and my health card is expiring, I passed my dot physical but won’t certify me because of the leg, it is healing, will I lose my cdl because of this.

For a class A, over the road (OTR) driver, it is possible. It will require a skills performance certification though. A lesser class cdl falls under your states guidelines and variances.

Technically, no. Your results were entered into the federal data base. If no one checks it out, you may get away with it. But DOT stops you ???.

I’m waiting for dot physical appointment, I had both knees replaced last year in NY, this is my first attempt for class B cdl in florida. Should I be worried about passing?

A class B license fall under the states guidelines and variances, so make sure the examiner know you are going for a class B and not a class A. The states guidelines are different from DOT/FMCSA and are a bit more lenient.
I am out of work on FMLA because of a back injury. I’m am scheduled for surgery on my spine in July. There is no way I can pass a physical now, but I don’t want to lose my CDL. Thanks for direction.

Best to find the cause of the numbness and have it corrected, if possible. If the condition is being caused by a neuropathy, then there is an underlying condition which will be questioned by the CME.
What happens in Massachusetts if you can’t pass a DOT physical due to surgery recovery on my leg is there a leniency will they give you an extension.

Recently went to cdl physical for medical card, spoke with physician about arm surgery, they agreed that i dont have immobility issue for driving, but asked for medical notes from recent doc visits. My medical status is pending, should i be ok?

If clearance is needed by an Orthopedic Surgeon for a hand deformity, does the surgeon have to be certified to medically clear CDL drivers, or can any surgeon do it as long as the right paperwork is provided? Able to drive a hostler vehicle with no issues

Most state are lenient with situations like yours. Get a medical done as soon as you are released to return to work and get it into the state.
I have Class A CDL license. My left arm is paralyzed due to an accident in 2010. I learned how to do several things with my right arm (my dominant arm) including driving my manual transmission vehicle. I have driven a commercial vehicle since then and operated it fine for 2 yrs. Since my medical card expired, I haven’t driven for 10 yrs. Will I be able to obtain a medical certification to drive again?

You will need some supporting documentation before passing the medical certification examination. Check with your local CME to determine the documents that are needed.
I have a CDL hurt my knee had surgery now the doctor says no more climbing in and out of trucks well I want to keep my CDL do I have to keep a physical card current or can I just get one just say if I try drive again in the future. Just hate to loose them if had them for 30 yrs . Thks

Each state is different and most states require a current medical card to maintain a valid cdl. Best to check with your local drivers license department for the correct answer.
I suffer from poor circulation in my lower legs, just like my father did. As a result I’ve got Stasis Dermatitis which makes my condition look a lot worse than it is. I’m getting fitted for compression socks very soon which should clear the issue up. Even now my condition in now way hinders my functioning any pedals or any other ability to operate a motor vehicle. Will this disqualify me at the DOT physical?

Legally, by DOT/FMCSA regulations, no. Your medical card was invalidated at the time of the surgery. You will need to have a new medical certification examination to be legal.

Just had my fourth shoulder surgery replacement. Everything is finally healing up and doing well. I have 1 more month before being release from my doctors care. The new joint will cause me to not be able hold my arm straight up above my head. I can drive and still do things pretty much normal, just some things having to them a little different now. Will this effect me passing a D.O.T. physical in general.

As long as you can perform the duties of a cmv operator, you will be fine. When you get your release top return to work, watch for restrictions and limitations. The CME must take this into consideration.
I have had 4 back surgeries and l am fused all my lumbar is fused and have nerve damage in both of my legs and drop foot in both feet I have to where foot braces on both feet so l can kinda walk. Would I still qualify for my medical card renewal?

For a class A interstate cdl, you will have difficulty passing the medical certification examination. Lesser class cdl holders or intra-state only will fall under the states guidelines and variances.

Even if you took a new DOT/FMCSA medical, it would be null and void as soon as you had the surgery. Talk with your local drivers license department and explain your situation. They should have some guidelines to follow. Then, once fully recovered and a return to work, without restrictions or limitations, is issued, you can take a new physical and return to driving.
Will bilateral hip replacement prevent me from passing dot physical? Both hips are slid and I function normally.
I had gallbladder surgery on April 16,2019 laparoscopic and I’m perfectly fine. I just got scars which it’s very light. And I have to watch what I eat. Will this disqualify me? I am a Class B CDL driver for a public transit bus, 20 passenger. I recently had a back injury that requires me to now use a walker/ cane to walk. It does not impair my legs, feet just my back. I can still drive & would be able to perform the duties I have always had to do in driving. Since I was out of work for 6 months I would have to go get the medical exam to re-enter. I can not find anything that says I could not drive if having to use a walker/cane only that as long as I can physically perform the duties needed to safely drive, which I can. Can you tell me if using a walker or cane disqualifies me from getting my medical card to drive a public transit commercial vehicle? Thank you.

It will depend on the class cdl you are going to be striving for. Class A, interstate, will be your most difficult cdl to acquire. Lesser class cdl’s will fall under the states guidelines and requirements, not federal guidelines.

Good strength to hold a steering wheel and able to grip/hold tools if needed. Most CME’s do not go by measured grip strength and neither does DOT/FMCSA.

The only way to confirm or rule out a possible inguinal hernia is through the inguinal canal. He/she does not need to touch your testicles, but must go past them to get to the canal.
C6-C7: Right paracentral disc extrusion which contacts the right descending C8 nerve root at the right lateral recess. Mild central canal stenosis. There is indentation of the right ventral cord surface. Minimal right foraminal stenosis.
Symptoms: present in right arm, elbow, shoulder, numbness in thumb and pointer finger. Otherwise no loss of mobility.

For a class A cdl, running interstate, will be very difficult to pass the federal medical certification examination. Intra-state only and/or a lesser class cdl, will fall under the states guidelines and variances.

Your condition does not require an extension. Check with your state DLD and explain your situation. They should have policy in place for this.
I’ve always wondered this? What if you broke a leg or something to that extent that requires a long recovery, but your med card is due at that time. Here in Mass you need to recertification your med card with the registry every time you get one or they will downgrade your license and after a year you’ll have to do the written and road test again! Do they make an exception if your hurt at the time you need to renew it?

An employee of mine had his left leg amputated from the knee down and now has a prosthetic leg. He drives a company truck and needs a DOT physical and has never had issues getting one in the past.

If the driver is a class A license holder, then there will be questions and he will probably need an exemption form from DOT/FMCSA. That may require a specialist to ride with your driver to determine his physical capabilities.
After being struck in the face & shoulder by a trailer door (split lower lip open and bruised my jaw, tore out my bicep tendon (unrepairable), tore my bicep muscle, tore my rotator cuff. After treatment they found that I have 3-4 discs in my neck shifted & bulging- herniated – causing pain but say it isn’t a result of getting hit by the trailer door.
My question is “would I pass a DOT physical and be able to get my Medical Certification (card) with the herniated discs in my neck or would I be High Risk and not certifiable ?

If you can perform the duties of a CMV operator, without restrictions or limitations, then you may pass the DOT/FMCSA examination. The CME is going to be looking at all of your symptoms and abilities to make his/her final decision. Based on what your situation is right now, you will be considered high risk and may need to provide additional documentation from your treating physicians. Also, lots depends on which class CDL you hold.

Hi, my name is Chris and I have a question about medical card exempt. I fell out of a truck and broke my back and am now an incomplete paraplegic. I’ve gotten much gains back, but I use Afo’s because I have little movement below my knees. I still have a class A CDL. I contacted the DMV and that’s when they mentioned the medical card exempt with a skills test. What does that mean?

Not sure about your states requirements, so check with them if you are going to be limited to intra-state only. For Federal DOT/FMCSA your limitations would require an extremities exemption which must be supported by a Skills Performance Evaluation. You will need to find a certified SPE examiner in your state and demonstrate your driving skills with your limitations. The examiner can then determine what your actual capabilities are and make recommendations from there.
Ok thanks! I contacted FMCSA in Illinois and recieved Skill Performance Evaluation packet. This stuff is complicated! It’s nice to know there is a path back to the driver’s seat, even if it does appear to be difficult. I need the job first, then the SPE. I was hoping I could do this myself then present it to my prospect employer. I’m doubtful anyone will hire me just to go through all this especially with no guarantee of completing. So discouraged.

I have an above the knee amputation of my right leg. I recently filed for my business license and my motor carrier authority for a NON CDL hot shot trucking business which requires me to get a Dot medical certificate because I’ll be doing interstate commerce. my primary doctor filled out the medical waiver form to let the Dot doctor know that he thinks I am totally fit and strong enough with the microprocessor I’ve had and been walking with for almost 5 years already and I can do everything without mods I never Thad to get any kind of special drivers license and ride motorcycles without any mods I don’t let this disability stop me from doing anything. So I am qualified as a non CDL hot shot to only haul 26k pounds or less. and that’s combined truck trailer and load. I have everything done I just need to go get my physical done and get my liability and cargo insurance and I will be ready to work . Will this be an issue for me when I go for my physical even though I am doing non CDL hot shot trucking with my own 3/4 ton pick up truck and trailer ? I was told I wouldn’t have any problem because I am doing NON CDL trucking but I just wanted to get an opinion from u guys is all

Make sure the medical examiner is aware that you are a non-cdl holder and you fall under the states guidelines and variances, not DOT/FMCSA. Class A drivers exams are more strict, and would require a SPE, not so in your case.
If you get a drop foot as a result of a herniated disc, can that disqualify you from renewing your class b license? Have had hernia surgery and everything went well but drop foot has not recovered

Your ability to perform the duties of a commercial motor vehicle driver are going to be questioned, but not completely ruled out. The examiner will have questions and may request additional documentation to help him/her in the decision making process. If the limitations are extensive enough, they may request a waiver from the DOT/FMCSA. That decision will be based on the CME’s findings from your medical certification examination.

By DOT/FMCSA requirements, you should be able to perform the duties of the job requirements of a class A driver. That requires the bending of the knee. You will need medical documentation and a medical release from your doctor. The CME will have to take all the information into consideration before making a final decision.

I went for DOT physical, to get my CDL A for a new job and was told everything is good but said I needed to get a doctor to sign off on my injured left hand. I was in an accident in Nov 20 and shattered my left hand and broke my arm. I mentioned to the dr that it would sometimes get numb for a short time. I cannot find a dr to sign off on my form and my surgeon doesnt have any availablility until After Im supposed to start my new job. Can I go for another physical and not say that my hand used to go numb?

I hold a class A CDL. It expired and I needed to get it renewed. I was pulled off a porch by a dog 4 weeks ago and fractured two lower ribs. The Dr said he won’t pass me because of my rib. Stating it could puncher my lung if it got bump or hit with the seat belt. I feel he is being ridiculous, and because it is lower ribs puncturing my lung is almost impossible (I don’t have a medical background, I know anatomy specifically lungs, I was a respiratory tech) I can find nothing that says a broken rib disqualifies me. I will get with my regular PCP ASAP, but now I have to wait to start this job, because of him! I already quit my other job. I gave my 2 weeks and finished them out!!!! I’m a 60 year old woman, lifted 75 lbs the same day he did the physical, with no pain. I have no limitations and in pretty good shape for my age. What are your thoughts?

My coworker got his medical card for only 3 months. He doesn’t have any of the DMV medical restrictions especially Diabetes or High Blood Pressure. The doc in the box doctor said “You are too fat”. My coworker is 43 yrs old. We’ve both worked for a city here in California for 23 years and carry a class A license. He is a little heavier than me, I weigh 175lbs. I never heard of such a thing. Is that right or legal? If not, what recourse does my coworker have? He’s never had any accidents and has a clean record.

Have your coworker go else where for his DOT medical examination. Seems that a lot of the ‘doc in a box’ facilities have a conflict of interest and are in bed with sleep study groups.
They can give a 3 month physical solely based on a driver’s BMI being 40 or above. The purpose of the 3 month physical at this point would be for the driver to have a sleep study done. A DOT physician can request a sleep study be perform based on BMI alone and driver does not have to meet any other requirement for this to be requested if said driver has a BMI of 40 or above. There’s probably more to this situation than what’s been disclosed so it’s hard to give 100% guidance without full disclosure from the driver.

There is no set guidelines in the DOT/FMCSA regarding BMI of 40 as the sole reasoning for a sleep study. OSA should provide a multitude of signs and symptoms before a sleep study is requested. Get a second opinion somewhere else.
If they are required to get a sleep study done, don’t waste your time doing the at home study. If that study shows you to have sleep apnea of any level, they will require an in lab study. The in lab study sucks.

As far as DOT/FMCSA, an at home sleep study isn’t accepted as a final saying. If you have no issues with daytime sleepiness or difficulty staying awake, then we suggest a second opinion somewhere else.
I have been on SSDI for the last ten years. I have a spinal fixation device, a complete replacement Lt knee. I suffer chronic pain but is usually treatable with Naproxen. I have been released from SSDI as no longer disabled. But it comes with restrictions. Does that disqualify me from a medical card?

These conditions will raise questions with any CME. They will be asking for supporting medical documentation and medical releases from your treating physicians, but it is not an automatic disqualification.

It should not be an issue. You can’t pass a DOT medical certification examination now or until you are fully released from your doctors care. There can be no restrictions or limitations attached to the release either. Get with your local drivers license department and explain your present situation. You have injuries and are scheduled for surgical repair, so how do you maintain your cdl until you can go back to work. They should be able to guide you through the process with little or no hassles.
You should be able to maintain your CDL as long as your driver’s license is valid. You are just not able to operate a CMV before you get your certification.

Hi, I’m a retired veteran and I’m wanting to drive OTR. In 2006 I had a stent put into my Iliac artery because I was diagnosed with Peripheral Artery Disease and since then I’ve been taking CILOSTAZOL every day to keep my blood slippery to prevent blood clots. I also started taking ATORVASTATIN CALCIUM tablets to lower cholesterol.
Would this stop me from passing the FMCSA Medical Exam?

As a class A driver, your medications are going to raise questions. With supporting medical documentation that you have responded well and have been on these meds for quite some time will be very helpful in the CME’s decision making process. The cholesterol medication should not be the issue, but the potential side effects of Cilostazol will be questioned.
I’ve been taking this med since 2006 and have had absolutely no side affects.
When I first had the stent implant I smoked a pack of cigarettes a day which is why I had the problem to begin with. I have not smoked cigarettes for over 6 years now.

Based on your present situation, it is doubtful that you would be able to pass the FMCSA medical certification examination at this time. In three months, that should have changed and as long as you have a medical release form to return to work, without restrictions or limitations, you may be fine.

I am thinking about getting my class A CDL. Problem is, I have a broken leg that is not healing. Its been broken for over a year. I have a plate in the leg and can walk with a limp. I have had 3 surgeries to repair a massive rotator cuff tear. I am 3 months into rehab for the final repair, an encapsulation patch to replace the superspinatus. I still can’t lift my shoulder past shoulder height, have a lot of weakness and restricted range of motion, including laterally, and have very limited external rotation on that arm. Will this disqualify me?

Once you are fully released from your doctors care, you’ll need a return to work and a medical release form. Then if you can perform the duties of a cmv operator, you could be ok.
I already had fusion between c4,5,6 now the doctors are talking doing 6 to 7, will cause me to lose my privilege to drive commercial

As long as you can maintain some level of cervical motion, most of which occurs at the C1,C2,C3 levels, you should be fine. Once you are released to return to work without restriction, take a medical release form and any medical documentation you have with you to the DOT medical examiner. It all helps in their decision making process.
I have a Class B CDL, I was injured at work that required low back surgery. I drove a school bus route 3 times a day and also did trips until August, when I was told I needed surgery to repair my back my boss told me I was unable to drive because I couldnt pass my DOT physical;
my DOT physical expired in August. I am still under the care of my surgeon (also the work comp dr) and going to physical therapy 3 times a week. My boss called me today Dec 4th and informed me I am to return to work Dec 8th to drive 1 route a day because they need help and the work comp dr told my employer I can drive 2 hours a day while taking my meds and not lift over 25lbs.( I have NOT heard from ANY of my drs work comp or other that I can drive. I’ve had a CDL for 16 years and I’m positive I cannot drive without a current DOT physical and being a bus driver my State requires a yearly exam. I asked if the school is taking complete responsibility if “something” would happen. They did not answer. I know if I knowingly get behind the wheel I take that responsibility. I do not know what to do. I know I cant turn down work but I’m not sure what to do.

Even with a class B cdl you need a valid medical card. You are not physically able to perform the duties required to pass a DOT medical certification examination. And if something were to happen, it will fall on you and the company.

Your situation will be questioned extensively and you will need to demonstrate your abilities. The CME may request further information from your neurologist. If the condition does not interfere with your abilities to operate a CMV without risk to you or the public at large, then you may pass the DOT/FMCSA examination.

If the surgery is successful, then you should be able to continue driving. You will need a new DOT/FMCSA medical certificate and will need to pass all other sections of the physical examination. Good Luck, we hope it all turns out OK.
I’m unable to locate the specific ruling on Spinal cord stimulation implants. Understanding that opioid meds are an unfit for duty status but what about the implant. There’s surgeon and other doctors providing letters of return to work status with no limitations. Thanks.

It is not just the implant but what caused the need for any medications or implants to be needed. Your CME is going to request medical documentation to support his decision making process. Getting a letter to return to work without limitations is one step, but not a completed process. The surgeons may let you return to work, but they do not have the DOT/FMCSA guidelines and regulations to contend with. Just be prepared for a waiting period after the surgery and you may need to look into intra-state only driving for awhile.

I have Charcot-Marie-Tooth disorder (CMT), it effects my feet and have lost some feeling in my feet and have almost no reflexes in them, was diagnosed at a young age, I got my class A cdl at 18, I’m 22 now and considering having corrective surgery on my feet, it will only help with the pain of walking on the outside of my feet, I’m afraid if I have the surgery they will disqualify me for my medical card and revoke my cdl, any feed back will be much appreciated

It’s a tough call. If you do not have the feet corrected, then you may fail due to the inability to perform the duties of a CMV operator. Have the surgery, and you still could fail the DOT examination for the same reason. The surgery itself is not going to be an issue, it’s the ability to perform the duties of a driver.
I want to become an A class truck driver but I’m worried if I’ll fail the dot physical. In 2017 I broke and dislocated my left elbow. It has healed all the way but I can’t full straighten it out. The most I can is like a 150 degrees out of 180. I’m able to work and do everyday things of picking up, pushing, and pulling. Would I fail the dot physical just because it doesn’t extend fully, even though I can pick up weight and do everyday things we ease?

You should not have any real difficulty passing the DOT/FMCSA medical certification examination with a slightly reduced range of motion in your elbow.
If carrying a class B drivers license, and transporting passengers, can a driver continue to operate legally if he or she is wearing a boot on the non-driving foot?

I am a 47 year old T11 paraplegic (waist down). I have been confined to a wheelchair for 31 years. I do not suffer from pain and my upper body strength is not a problem. I have had a Class C California driver’s license since 1989 and other than the fact that I use hand controls I don’t have problems with driving. I take low dosage of gabapentin and duloxetine for neuropathy pain in my legs with no adverse side effects. Will my disability and/or medications disqualify me?

I have a Class A CDL and a hairline fracture on the top bone of my foot. My doctor gave me a boot to wear but it is an open toe boot. The company I work for requires steel or alloy toed shoes. My boss has told me about some form of steel toe “covering” that I could POSSIBLY put on my “boot”, but that he has to order it.So until then I will not have this “covering”. Is this going to get me into trouble for not wearing it/wearing it?

I have lymphedema in my left leg. I have been to doctors who don’t see any form of blockage (blood clots) in or around my leg. I currently drive paratransit (a small school bus) in my state of Georgia. I want to get my CDL to drive a full size bus (city bus), I drive with my right leg and have no issues with either, I even wear compression socks so I don’t get looks from people. Will this affect me if I were to go through to get my CDL class C? or any CDL? When I went for my Med card to drive Paratransit they had no issue but being that it is a bigger vehicle I am a little concerned.

Unless you try for a class A cdl, you would fall under your state guidelines and variances. Check with your local drivers license department first and then go from there.
My 2 yr DOT CDL A physical is due December 11, 2020. I may have to have surgery on my back. Can my DOT physical be extended until I am released from doctor to go back to work without loosing my CDL A? Looks like I will miss the 2 year date, maybe by a month or so. The moment you get out of the truck for the surgery you’re DOT card become invalid. Have the surgery and once fully released, without restrictions or limitations, go get a new DOT physical. As far as your cdl A, your license is state issued and you would need to contact your state drivers license department for further instructions regarding maintaining your license while being out due to the surgery. Most states have a policy already in place for such a situation.

I have recently undergone L4-S1 PLIF surgery with Stryker. I have previously had c3-5 fusion and still have a herniated disc in my neck. Post PLIF surgery I still have right foot drop with numbness and leg weakness. I was wondering if I would still be able to drive. I have been driving for 25 years

Once fully released, with a medical release form from your doctor, then the CME will have to make his final decision. As a class A driver, it’s going to be difficult to pass the DOT/FMCSA medical certification examination. But intra-state or a lesser drivers license and you would fall under the states variances and regulations, so all hope is not lost.
I have read the questions and comments I haven’t seen anything in regards to my question, please forgive me if I’ve overlooked! But I went to get my Dot Physical done today and the Doctor didn’t complete the physical due to the fact I’ve been having pain in my leg due to the fact that it maybe Sciatica Nerve damage, I don’t do anything for the pain I just deal with it!! The risk factors that the DOT/FMCSA placed on the CME’s causes the examiner to be leery. You have pain and you do not know what it’s coming from. The CME has unanswered questions regarding this and does not allow for good decision making. We suspect the examiner is wanting additional information before moving on. Find out whats causing the pain, get it taken care of and everybody will be in the know.

I went for a DOT Physical, and was not passed. The Doctor has requested an EKG, and a sleep test.
My Cardiologist performed an EKG, and says that I should be good to go. My Medical Dr has never required a Sleep Study. Is this neccessary and how should I proceed?

DOT/FMCSA does not require a sleep study unless there are signs of possible obstructive sleep apnea, OSA, If you have no difficulties with day time sleepiness, then go else where for a second opinion to get your dot medical card.
Steven B. That is not true. Is your BMI is greater than 40 a sleep study is required. Or, if your BMI is >33 and you have comorbidities a sleep study is required.

I am considering getting back into driving again. I’ve had a Cdl since 1987 and used it for 14 years prior to leaving trucking and doing something else for 18 years. I was diagnosed with cancer in 2018 and went through the usual treatments. Now in remission for over a year. The type of cancer I have is destructive to bone and a large tumor was discovered in the top of my right humerus. The bone was strengthened with a titanium plate and screws. This was done Dec ‘18, checked last Dec and no future appts are scheduled. I can lift my arm to a point that’s just short of straight up since the device has to go up into the joint. I’ve not been on anymore treatment medication as of the end of last year. I’m on my treadmill 90 min a day getting between 5-6 miles. Question is, will any of this limit my chances of taking a refresher and passing a physical, or should I just give up the idea altogether?

Never give up on a good idea. With some supporting medical documentation and a medical release from your doctors you should be fine, if all else is within normal ranges and limits. Good luck.
Thank you for the reply. I’m not one who gives up easily. There will be appts every few months to monitor blood and urine for signs of any problems. These could be stretched out to yearly. I will disclose this to the physician giving the physical.

The condition which the medications are being taken for will cause the most questions. Your driver is going to need a medical release form and supporting documentation to give to the DOT/FMCSA examiner. He will need all the information available to assist in his/her decision making process.
I was diagnosed with rheumatoid arthritis a few years ago. I take no medicine and have no loss of motion. I’m wanting to get my CDL but before I go any further I wanted to get an opinion before going any further. I have strong grip and no pain. Also avascular necrosis in my knees that doesn’t affect my day to day and forget it’s there to be honest which is why I ask as an after thought.

I wear a leather lace-up non-hinged Arizona Brace on my right ankle. I work for the Post Office in California and drive 10 miles a day on the job. Can I do a driving test at the DMV or is there some other means where I could obtain permission from the DMV to drive with the brace on?

Your condition already presents with possible restrictions, if it does not impair your ability to perform all the duties of a CMV operator, then you may be OK.
I’m helping with an inquiry into whether an inguinal hernia exam conducted by a PA as part of a CDL Physical was conducted appropriately. Rather than sitting in front of the standing patient, the PA describes standing to the patients right at 90 degrees before asking the patient to cough while applying mild pressure with his index finger to both discern the inguinal ring and curve around the inguinal ring and into the inguinal canal about an inch. Still standing, he then reaches across the patient to do the patient’s other side with the PA’s right hand by twisting his wrist to the right to allow his right index finger to depress the skin’s surface to allow entry into the inguinal canal at the same depth. Is there any clinical support for this technique as described in a standard CDL Physical?

DOT/FMCSA does not tell the examiners how to perform their examination. FMCSA expects the examiners to know their examination procedures and the guidelines/regulations of the FMCSA.
I live in tx and my medical card expires this month but I had surgery on my right foot to fuse my 4th metatarsal together I have a plate and 6 screws holding it together. Is that going to be an issue to renew because I still haven’t gotten the ok to go back to work. I’m in physical therapy now. What do I do when my med card expires and cant renew it?

Once you had the surgery your medical card was invalid. Once you have been released to return to work, without restrictions, you will be able to get a new medical examination and card.
I was diagnosed with a bulging disc affecting my sciatic nerve. VA verified with CAT scan and advised consider a new career path and recommend not to have surgery. The pain in my back is increasing while the sciatica is stabilized. The pain is affecting my sleep. I have a DOT physical due in early Jan. I am concerned if i will likely fail the physical.

If you are class A interstate driving, it is highly unlikely that you will pass the DOT/FMCSA medical certification examination. By FMCSA guidelines you are not ‘fit for duty’. A lesser class license may fall under the states variances, so check with your states drivers license department for more info.
I had my gallbladder removed in April of this year. I’m physically healthy and in no pain just i have scar tissue inside. With three small scars outside. Im looking forward and starting a career change in truck driving and I really want my CDL-A in NYS. Will i have an issue at the DOT office?

I was injured on the job. I got released on 8/27/19 with permanent restrictions. No over head reaching or lifting. 25lb weight limit with my elbows flexed to my side. I drive the metro bus. I pass the test, but was told that I could not get my dot card because I have permanent restrictions. I had rotator cuff surgery on my right shoulder. I can still drive. I am totally confused.

Your examiner is holding you to a class A drivers standard, you fall into the states variances. Go get a second opinion from an examiner that know the differences.

Hello…If a DOT driver is a diabetic or an unknown diabetic and he/she is spilling sugar in his/her urine, is it required to request his/her most recent AIC? We always perform a glucose finger stick if sugar is present in the urine, but if that number is under 200, is it required by DOT guidelines to get their A1C before determination is made? When is it required to request their A1C?

You should request a recent A1C when there is both a history of diabetes, of any type, and a sugar spill-over is present during your testing. DOT guidelines are letting you interpret them for the safety of the driver and the public at large. Support your decisions with proper supporting documentation.
IM a diabetic and I was off work on injury leaves and whenI I went to do my DOT physical they me I couldn’t get my certificate when I’m released back to work but when I was released back to work my Class A license was drop to a class C when I do my physical again how long will it take me to get my license up graded back to a class A how long do it take me to be able to drive Truck again

I had DOT physical exam on Friday but because I have limited movement in my LEFT FOOT actually they call foot drop, I was wounded in war and my nerve for foot is damaged and I have to see Physical doctor to clear and give approval and I call bunch off the doctors offices and receive lot off different answers so I’m really confused, anyway I’m mechanic full time and able to drive stick shift ( I couldn’t be mechanic if not able to drive stick shift) with no problem whatsoever, passing everything with no problems except my left foot, and I’m not sure that this doctor have no idea just because she said how I gonna be able to work with air brakes ( air brakes operate with same pedal as regular brakes ) I’m wondering now that I’m not be able to become truck driver what I always want to be, should I be worry or not for becoming truck driver

You will need a medical release form from your primary doctor explaining your situation and any limitations that it may create.FMCSA provides guidelines for the CME. The final decision falls on the CME and his understanding of the guidelines and how he weighs the possible risk and safety issues.

I live in Alabama and I’m getting my CDL B for the first time I’ve already have my permit and I took the pre-trip and driving test and I passed with flying colors!! But the Dr that passed me on the physical gave me a one year and have to take it again but I listed on the paperwork 3 times that I had my left foot amputated on Jan 2, 2015.and he did not check the box about needing to get a weaver. Now after I’ve done all that the AL Dot says I have to get a SPE waiver but for local hauls the lady at the federal office said she can’t give me one becauce I’m don’t go out of state. And I don’t have any problems with any task we do.And I wear my prosthetic all the time.. What can I do?

You fall under the states requirements and variances, not the FMCSA. FMCSA was established for class A long and heavy haul drivers. Go back and check with your local DLD for the best way to go from here.
I want a career change. I want to get a class A CDL license. I have no training. I want to go to a large trucking company or a school. The issue is I am missing fingers on my left hand. I was born this way. I can drive a car just fine. I’m want to drive an automatic truck. The trucking company I would like to work for stated that I would need a SPE test. Also, I was looking into going into a school. Both companies have no idea or are very much help on getting the SPE test. I have no training at all, so how am I suppose to get evaluated without training first? First I am thinking that I would have to pass the Dot physical, then get trained, then have someone come out and evaluate me for the SPE, but the paperwork is confusing. I just want to have the SPE waiver so that a major company would employ me.

Talk to one of the cdl driving schools. They should be able to guide you in the right direction.
I know truckers that are missing fingers
. Major carriers usually have onsite physicians for DOT exams. The physician will make a reasonable decision based on how your hands are, and might have you get a medical waiver okayed from other doctors
All should be well if your only missing one or two fingers.

I am a 50yr old Actively Employed Veteran Class A Regionally Dedicated Driver of 32yrs experience with a long history of degenerative disk disease, spinal stenosis, & formal narrowing.
Up until recently I have been able to manage my pain with Radio Ablation Therapy, Occasional Steroid Disk Injections, & A Regular Daily Physical Therapy Exercise Routine. (*No Medications other than physician prescribed & monitored moderately high dosage levels of Ibuprofen for Pain & Inflammation.)
I’ve recently received an MRI Lumbar Evaluation from my PCP, Pain Management Physician, & Neurosurgeon recommending an Open Lumbar Decompression (Single level) Laminectomy Surgery.
I have been assured that if I pursue this surgical option I will still be able to continue to drive at my current level with the added potential benefit of a 70-90% reduction in my current pain levels. They have projected a roughly 2hr surgery, followed by a three day hospital stay for observation, & an approximate 6 weeks off work for healing & Physical Therapy before returning to Normal Duty with no additional pain medications required.
I have been unable to find any type of information regarding this surgical procedure’s impact on a Class A Commercial Driver’s Career.
Any insight You & Your Colleagues can offer into to the Realistic Recovery Time, or More Importantly Potential DOT Physical Disqualification Risks to be considered pertaining to having this type of surgery would be Greatly Appreciated

All you will need, following the surgery, is a full medical release from your doctors, without any limitations. If they have you on any medications, then you may need to provide additional documentation from your doctors. If you can pass the DOT medical certification examination, then you are good to go.

WHAT? By federal DOT / FMCSA guidelines, you are required to have a waiver before being able to pass a DOT medical certification examination. The DOT officer is correct and you need to acquire a federal waiver.

No he can not. By DOT/FMCSA definition, you are not fit for duty as a class A driver. If there is a restriction to be placed on your license, then it would come from your states drivers license department and would probably limit you to intra-state only driving. But check with your DLD to confirm.
I recently went for a examination to get my CDL-A medical card from one of those box medical companies. I passed everything but he said he wouldn’t pass me because of tingling in my feet,he said it was peripheral neuropathy. I have complete feeling in both of feet and still wouldn’t give it to me. My personal doctor is certified DOT and said the box doctor is not reading the law right. The box doctor is not even certified DOT and just read me something out of a book with the rules. My company said I have to have the test done only from this box company and will not let me go to my own doctor. Is there any recourse that I can take,thank you.

If the box doc is not on the national registry, then his examination findings are not valid. Check with your DOT certified doctor and have him help you out. All valid medical examinations must be performed by a DOT certified medical examiner, period.
Hi. I m a CDL OTR driver, and my medical exam expires in a year. I had a surgery for achilis tendom ruptured on my left foot. And now I have a cast on my left foot under the knee. Doc said its ok to drive a vehicle, but not walk on it. So I am currently using walking sticks to walk. I was driving an auto transmission truck. Can I still legally drive OTR? Thanks

The surgery invalidated your current medical card and at present you could not pass a DOT medical card certification examination. You will need a full medical release from your surgeon, without restriction, before you can take a new DOT medical certification examination.
I have had a CDL in Oregon for over 50 years. About 6 months ago I separated my right shoulder and fractured the arm. I also have nerve damage in the right shoulder, so I can not lift my right hand above my head. I have good strength in the arm.

If the shoulder restriction is sufficient enough to limit your abilities to perform the duties of a CMV operator, then you will have difficulty passing the DOT medical certification examination. Class A FMCSA guidelines require you to perform the duties of an operator without restrictions.
I broke both wrists in an on job accident a year ago. I’m currently in OT trying to get grip strength back. My question is what is minimum strength requirements for DOT physical? I keep getting different weights when I call dot examiners office. Orthopedic dr as well as therapists say I’m more than ready but my strength isn’t up to dr requirements. .. trying to see if need to find another dot examiner….

There is no set grip strength via the FMCSA guidelines. You need to be fully released from your doctors, without any restrictions. And you must be able to perform the job description duties of a CMV operator.
I have a CDL but primarily don’t drive anything bigger than a 5 ton. I’m considering right ankle fusion. Would I lose my CDL or have a restriction put on my license because of limited movement and potential slower brake reaction time?

I would like to add that the cancer was only in my esophagus and not through the outer lining to surrounding areas or lymph nodes The small tumor responded extreamly well to treatment per exam.

I had esophageal cancer and had radiation and Chemo Aug through Oct of 2018 and an esophogectomy on Dec 14 2018, I am now cancer free and not in treatment. I had a CT scan in May and it was clean. My fed med examiner will only give me a card for 3 mos at a time because thats when I get labs done for my oncologist. It will go to 6mos my next time. I think this is compleate bullshit and can she do this? I am cancer free with no treatment and no effects that will hinder my driving abilities. What can I do?

Ok so I went and got my medical card so when I go to get my cdl permit I have it if they need it and on the the card it asks for your drivers licence number and if your a cdl applicant or holder I checked no for that since I don’t have it yet. So my question is when I get my cdl will my license number change if so do I have to get a new card? Also will I have to recheck the box that I am a cdl holder

I walk with a slight limp (left leg) due to an infection as a child. I work out six days a week, have a normal range of motion. I currently work with Pepsi as a sales rep. I am trying to move over to Frito Lay, I’ll be doing the same type of physical work (actually less physical) but driving a vehicle that will require me to have a DOT medical card. Will my limp keep me from getting the position?

The limp should not affect your getting a medical card. Just make sure you explain that you are not a class A long haul driver and that you fall under the states variances for your medical card.

I had a 5 way by pass open hart after recovery i got a stress test and a echocardiogram. Passed all test doc gave me the all clear to drive a truck. I passed my DOT Med exm and have been driving for a year, had to renew my card. the doc would not pass me with out a new Echocardiogram becuse my last one is 11 mouths old but my stress test is good for 5 years?

FMCSA requires a stress test every two years. ECG’s support the test results. The CME may request a new ECG to help in his decision making process.
I work for a trucking company. One of our applicants needs to get a Skills Performance Evaluation. Do you know how long the process takes? The doctor required it because he said he has diabetic neuropathy in his feet. Can he get a medical release from his neurologist and then go to a different clinic?

You could try for a second opinion with a medical release form from the neurologist. But you may still need the SPE exam. That one you will have to find a local examiner to perform the ride-along evaluation. Just a question, is your driver going to drive long haul semi or a lesser class license? It makes a difference.
I drive a 5- 6 ton 8- 12 passenger bus within the state of PA.
I have a workers comp injury since Feb of this year…which does not allow me as of yet to do wheelchairs…but I perform all other duties required.
I passed the DOT physical (as per her notes) today yet the Physicians Assistant is saying I have to get a Medical Report filled out after no restrictions are present.
But that isnt part.of the DOT physical.requorements…
Isnt that.illegal? Descrimination? Or something else?

We are guessing that you are not a class A cdl holder. Which puts you under the states variances and not FMCSA. The FMCSA guidelines were developed for over the road, long haul drivers. The PA is not differentiating FMCSA and the states requirements. If you are a class A holder, then the PA is correct and should not pass you for any reason due to your ongoing restrictions. The state may not require a repport. Check with your state DLD for clarification.
When returning from work after a surgical procedure Is it the Doctor Who performed the procedure who returned you to work or do you need to see a DOT certified physician? If your physician who performed the procedure Clear you to drive a school bus but states in the note that you cannot do any lifting is there anything that prevent you from returning to work?

The DOT examiner does not return you to work, but he does need to do a new physical, if your state requires it. Best bet is to check with your local drivers license department for the best answer.
I just had a left total hip replacement. I have a CDL license in the State of Illinois would I be able to return back to driving trucks or would a prosthetic hip disqualify me?

It is not a disqualifying factor. Just get fully healed with full recovery, without any restrictions, and you’ll be good to go.

Certain conditions and injuries have a set waiting time frame before a new medical examination should be performed. Other conditions fall under guidelines and do not have set waiting periods.

It will depend on the extent of the limp and any limitations that it may present during the examination.
I walk with a limp because I have AVN @20%; I also have AS accompanied with occasional uveitis. Gabapentin, methotrexate, folic acid, lotemax & prolensa are the meds I currently take. It will depend on the class drivers license you want to acquire. As a Class A driver you wooed not qualify. As a class B or less, you may fall under the state variances and be fine. Check with your state drivers license department for a better answer.

In louisiana yes cause i was drivikg big trucks after i got out of the military and i went tp the dmv in my home town cause i had just got my second award letter from the va for my military injuries and since my % was over 60% u get to get ur drivers license and they give u the handicap plates and also another one for a spare vehicle that u may b riding in or driving so…. But anyways back on topic they told me i had to pick one or the other i said shit i make way more a month sitting on my ass then holding the steering wheel and grinding gears did so i took the better offer and gave my cdl back up

With proper medical documentation, you should be able to get the medical card, if all else is within normal limits.
I am a above knee amputee right leg from a motorcycle accident in 2012.I wear a prosthetic and I have no problem getting around or driving.But I drive with my left leg.Obviously I can no longer drive a manual transmission.I plan on getting with a company that offers training.How do I obtain a medical waiver if I have never had a CDL?From what I have read,one must show that they can drive with no issues.I can not show that I can drive a tractor/trailer with no CDL.

You will need an exemption form from FMCSA. Once you start the process you will need a medical examiner to ride with you to determine if there are any potential safety risk. Once released by him and the exemption papers in hand, you can go for the DOT medical certification examination.
its a form CDL-10 : Certification of Physical Exemption 49 CFR Part 391/390 its a form I filled out check box # 3 The occasional transportation of personal property by individuals not for compensation nor in the furtherance of a commercial enterprise. went to Texas Department Of Public Safety , provided the form for the reason because I couldn’t get my Medical Examiners Certificate and got the restriction so I wouldn’t lose my License from 1/31/2019 through 1/31/2022, its gives me 3 years before my license expires to heal my work related injuries . I don’t know if the Company will allow this restriction on my license for the fact that am on light duty with restrictions because they need a medical card for there file , on the day it expired on 1/31/2019 . do you think they will allowed this till I heal or would they find a reason to terminate me because of my medical card and work related injury . department of public safety told me before I can drive commercial trucks need to get my medical card first and they’ll take off the restrictions . I hope this is the best move to save my CDL from getting down grade and I hope the company doesn’t find a reason to terminate me because of the changes I made . You will need to pass the DOT medical certification examination before you can drive, legally. To pass the medical examination, you must not be on light duty or other physical restrictions. Get healed from the injuries and them get a new medical card. That will save your cdl license.
they put a restriction on my license text : so I wouldn’t lose my CDL and do no test , exemption ? stating I wont lose my CDL ! I cant drive trucks yet till I get a release from restrictions , but it saves my CDL from down grading and once I heal then ill go get my medical card and change the restriction . I was only concern about not losing my CDL . longest I filled that form CDL – 10 : exemption is place on my CDL till I heal . then ill go get my medical card then turn it in to department of public safety and take off that exemption on my CDL then I can drive trucks once I turn in my medical card .

I was in in a motorcycle accident. I broke both bones in my left leg below the knee. They both were completely severed at the ankle. I have a titanium rod in the larger of the two bones along with 6 screws, two of them in my ankle. To date I can walk with a cane but I still have nerve damage (pain) in 2/3 of my foot and part of my ankle.
Is it worth even trying to get recertified?
Since most trucks are now automatic transmissions I would have to use a clutch

As a class A, OTR driver, you may not qualify. As an intra-state cdl driver, you may be fine. A lot will depend on the extent of your injuries and the restrictions the injuries impose. Check with a local DOT medical examiner for a more solid answer.
My company when I first started driving sent me to a Doc in a Box for my DOT exam. Which they paid for at the time. I passed the exam three years in a row. Then the fourth year I had a Doctor fail me cause I could not walk on my heel in my right foot. Sent me to the FMCSA for an exemption. FMCSA has in their form that I have to wear a brace. My regular Doctor has cleared me and also a Orthopedic Surgern has cleared me twice. Stating I do not need the brace. What do I need to do to keep from going through the FMCSA part of the paperwork every two years? I have full function while driving.

Go get a second opinion somewhere else and take your medical doctors releases with you. Once cleared by a new examiner, FMCSA would not require the brace, if it is not needed to perform the duties of a CMV operator.

I was diagnosed with osteoarthritis about 4 months ago. They have given me Mobic which from what I can tell will not disqualify me. I have slight bone degeneration of my right hip and very slight in my lower back. Also, I have bone spurs in both elbows, however while taking mobic I have no pain and full range of motion. I have applied for a few trucking companies to get my cdl and work OTR. Will I be able to pass a DOT physical if I have no grip impairment and no range of motion issues?

The medication will cause questions, so you need a medical release form from your prescribing doctor and a working prognosis. You will need to be on this medication for 30 days or more just to make sure there are no ill side effects.

80 degrees total or in either direction? FMCSA would require a greater range of motion than 40 degrees in any one direction.
Suffered a severe DE-Gloving of my left leg–shin to knee cap…related to operating a tractor mower at work– Leg required masive surgical repair and wound care follow up. The employer sent me to one of those I.M.E.’s and that quack caused more harm after being informed with documentation of Severe Hipo /Pelvis pain. Leg wound required the floatng back of the carnage and being secured with several hundred interior based dissolvable sutures , several exterior plastic sutures and large op-site dressing with a wound vac. This all happened in May 2018. To date.. nerve damage exists in the shin and knee , plus an MRI revealed a Labral tear of the posterior left pelvis. Pain and restriction of movement of the leg is constant . Surgery is scheduled… but my DOT Medical card expires this week.. I also must wear surgical support stockings due to the venous insufficiency. Do you think I’ll pass the exam ? Oh..the employer fired me before I was transferred into the Trauma Center.

FMCSA states that any injury that results in time off or any possible change in physical ability to perform the duties of a cdl driver requires a new DOT physical before returning to normal work duties.

As a class A driver, your going to have difficulties due to the facts that you do not even qualify to do a DOT medical certification examination. Your doctors note should be sufficient to answer any questions for the company.

I have Cerebral Palsy (I walk) but I have some coordination problems and some limits on range of motion. Very mild case compared to others. Went for a DOT physical, doctor said he had never dealt with a case like mine, and was so unfamiliar with the situation he didn’t feel comfortable certifying me til I saw a neurologist who was familiar with my condition and the limitations it may place on driving as a CDL holder. I do not even know where to find a qualified neurologist, and I am concerned having someone who doesn’t know me or my situation examine me will result in a denial simply because they dont know/ dont want to know about my personal situation. My primary care physician is also a DOT medical examiner, but the job I applied to sent me to this guy. What should I do next? Is there anyone out there who has CP who got their medical card?

The FMCSA, once upon a time, issued a guideline for neck and BMI for drivers. If those guidelines were followed it would have taken 60+% of the drivers off the road. So at this time there are no neck and BMI regulations. But some opportunist have used the initial FMCSA information to grease their pockets. If you have a true OSA situation then there are many other test and questions to be answered before requesting a sleep study. But if the test is being requested based on neck size and BMI, we would recommend seeking a second opinion elsewhere.

You have to have a full medical release, from your doctors, before taking your DOT medical certification examination. FMCSA does not have an exception or extension. Your state DLD just needs to know you are out with recovering injuries and therapy and will have an updated medical card as soon as you are released to return to work, without restrictions.

Hi my name is Steve and I was involved in a car accident resulting in my femur getting getting broke. After three months in the hospital I was told that I had drop foot as a result of my femur getting broken. That was three years ago and I am trying to get back to being a truck driver local equipment hauling .i have been told that I can’t do what I used to do in the heavy equipment hauling field.

One of our operators had prostate cancer and started taking chemo, he’s very weak and had to be sent home a couple of times. What are policies determining if this operator is fit to drive? I have a bladder osteomyelitis, I’ve had it for 8 yrs. And been able to drive with no problems passing my DOT physical. This time it was a problem. There was blood in my urine, protein, and bacteria in my urine. These will always be in there. Why is it this time I could not pass my DOT physical?

I have a driver who went for DOT exam, and his arm was sore and the Nurse Practitioner would not pass him because he could not pass the arm resistance test. She told him he had to come back in 2 weeks after his arm got better to see if he could pass it then. Can he go to a different doctor and if he passes that drs. exam, can that be done?

Short answer, Yes. FMCSA set specific guidelines for the medical examiner to follow. Part of the drivers job description, according to the FMCSA, requires a normal range of motion, without any limitations or restrictions.
I have been a truck driver for over 45 years. I just turned 65. Went in for my recert on 10/17/18 I have been carrying a cane for occasional use but I can walk without it. I made the mistake of taking it in with me to my physical and the physician asst. took one look at my cane and I knew I was in trouble. I walked across the room and back unaided. I bent over and I squatted down the only thing was on the squat I put my hand on the table to steady myself. On the form it says I meet standards but periodic monitoring required. That I’m qualified for 3 months if I have a SPE certificate. The company will not let me drive. What can I do?

Class A is long haul, interstate, and big rigs. Class B regs falls more under the states variances and may be a better chose. Your examiner must know the FMCSA guidelines and the state variances so he can make the proper evaluation for your medical certificate.

If you are a class A license holder and unable to perform the duties of a class A holder without restrictions, then the answer would be no. Once fully released to return to work, without restrictions, a new medical certification exam is needed and you are back in business.
Scenario-
Driver with current CDL gets a urinalysis result back showing positive for a legally prescribed medication. Driver gets note from prescribing physician staying they can drive even with the prescription (as per FMCSA regulations). Safety manager at driver’s company says go get a DOT Physical. Driver goes and gets physical that evening (with prescription information included on long form) and walks out with a 2 year DOT medical card.
A few days later, same safety manager calls and says driver is medically disqualified because said manager sent the original urinalysis results to the doctor who qualified the driver, and that the doctor stated they hadn’t seen that prescription and arbitrarily disqualified the driver without additional phone calls, messages, request to come back in, etc.
Ultimately, my question is this. While I know the doctor has the ability to disqualify even if the driver’s primary says they are good to go, can the doctor say AFTER giving a medical clearance that the driver is disqualified despite it supposedly being based on information that was included with the original qualification? Basically like saying “oops, I changed my mind” and not even notifying the driver.
Apologies for the lengthy explanation, but this fiasco had been ridiculous. Yes, this is a current situation still actively being played out and I’m just looking for opinions while I research as well.

You should check with your state DLD to determine if your medical certificate has been disqualified. If the DOT examiner had all the available information, then there should be no “oops”.

If you can perform the duties of a cmv operator, following a full recovery and full work release (without restrictions), then you can return to work as a driver. You will need a medical release form from your doctors and a medical release form from your prescribing doctor for your BP meds.
After 6 months off work, because my employer said they had no light duty for me, and 6 weeks of Physical Therapy for my knee symptoms only, Dr. Released me to Full Duty, but still has me doing another 4 weeks therapy to address numbness, tingling, loss of sensation in same leg as knee. This was on 9-13-08. Employer expects me back on 9-17-08 as soon as I complete my DOT physical that morning. . This doesn’t make sense in my mind. In 6 months time I’m only now starting therapy for numbness, tingling and loss of sensation in my leg. . .which I reported from day one. . . .but now I’m expected to back to work full duty on 9-17-18 after completing my Dot physical that morning. . .here’s the kicker. . .the same medical office that is handling my workers comp will be doing my DOT physical.

The doctors office will not pass you for a DOT medical certificate due to your WC and therapy. You, at present, do not qualify for the examination.
I am having surgery on my right foot for the second time in 12 years the first one they removed my little toe and bone all the way back to my ankle because of bone infection the bone is infected again this time they are removing infected metatarsal bone all the way up to the fourth digit I already have blance and trouble feeling pressure applying brakes how will this affect my cdl going forward

It should not effect your CDL, but does effect your medical certificate. You will need a new medical certificate once you have been fully released from your doctor.

If it does not limit your ability to operate a cmv safely, then you should be fine. Bring any supporting documentation with you, it helps the examiner in his decision making process.
I have a herniated disk in my neck, they initially wanted to perform surgery. After a few opinions, I found a doctor that chose to send me to Physical Therapy instead. I’ve been going to PT twice a week now for 3 months, next month in September I will only have to go once a week and once the month ends I will be done. My medical card expires within 4 days and I need to get it renewed. I am not taking any medications, my ROM is pretty much back to normal and I don’t experience any pain just occasional muscle tightness which my PT says can be alleviated by massage. I have started doing some light weight training also. Will mentioning my herniated disk prevent me from getting a 2-year medical card? In the meantime I have been terminated from my previous job because my FMLA expired, it’s a long story filled with lots of loopholes and nonsense. I just want to get back to work at a better company. Do I have anything to be concerned about or should I relax?

If you can perform the full ROM without discomfort or limitation, you should be fine.
I live in Minnesota and have a class B license and drive a flatbed straight truck. I have an interstate card because we cross into Wisconsin somtimes. I injured my midback two months ago putting my tarp away. I am still in physical therapy with work comp. I have been doing light duty work in our warehouse since the injury. My health card expired a few days ago and when I went to the dot doctor to recertify she disqualified me and said I need a skill performance evaluation because of my injury. I just need to know if this is true or is there an exemption in my case.

Not true. You do not even qualify for the DOT examination with injuries, work limitations and WC. Once fully healed and releaased from all doctors care, then go for your DOT cert exam.
Can rheumatoid arthrist be a reason for failing the dot physical. It’s is not aggressive or noticeable. I just take meds for it.

I had back surgery for a herinated disc in ‘98. In 2015 I had another herinated disc but I’m not on any medication. I lost 20 lbs and this helped a lot. Can this prevent me from CDL med card?

As long as you can perform the duties of a CMV operators job description, you should be fine. There will be questions as to the loss of the toes, ex. medical reasons or accident.

I recently saw a patient for DOT physical. Patient had lumbar discectomy roughly 7 weeks ago. Examination overall normal; good ROM, no spinal tenderness, and incision sites look like they are healing well. Patient only taking tylenol as needed for pain. Elevated BP on exam and so patient granted a 3 month certificate. Advised me that he had been cleared by his surgeon and has another follow up with the surgeon in 2 weeks. Since he is coming back in a few months anyways, I asked him to bring notes from the surgeon when he returns for BP check. If his exam was abnormal I definitely would have requested a note prior to certifying for 3 months. Was this appropriate? I find it is difficult as there really are no clear cut guidelines for MSK surgeries.

If the driver was able to perform the duties of a CMV operator, in your opinion, and only his BP held him to a 3 months card, you are fine in your decision.
I have dm2 which is now under control with oral medication. I want to renew my dot medical card. I have over the last year had ambutation of 2 picky toes and half of my right great toe. The most recent was a month ago which was my left pinky toe. I have letters from my podiatrist and endroconologist both stating that in their opinions there is no restriction on me operating a cmv. Make sure you are fully recovered and released from surgical care first. Also bring a copy of your last a1c results. It will help the examiner in his final decision making process.
I am a CDL B driver. 6 Weeks ago I had knee surgery to repair a torn Maniscus. My DOT med card expires on Saturday July 21st. I went to have a DOT physical this past Friday on July 13th, Even though I have no restrictions for driving, she made the determination that I have a restriction. If this was my left knee, I could understand it. She told me that I can have know restrictions, Yes, I am on short term disability and I am unable to perform the physical portion of my job. I passed every part of the physical exam and had I not been honest I would have past the exam. I am wearing a brace on my leg and all I had to do was taking that off before going into the Dr. office and she never would have known. I am annoyed because this is my livelihood. This seems very unfair as it is not a physical for my job it is a DOT physical to determine if I can operate a commercial truck safely!!! Is this something that they can actually disqualify me for my med card?

You are class B driver and thus you fall under the states requirements and not class A FMCSA regulations. Your examiner should know both the FMCSA and your states variances. Go for a second opinion and explain that you are class B not class A.
If the driver is on partial permanent disability due to a preexisting elbow injury which limits the weight of the arm to 25 lbs and no repetitive pushing, pulling, or lifting with the arm, would that disqualify the driver from getting a DOT medical card for a commercial vehicle?

In 2006 I had a bike accident causing a tri mal ankle fracture with non-union. Because of this I was granted disability and have not worked since. I have a strong aversion to opiates so I’ve treated this issue with marijuana (have my medical card). I’m only 40 years old and cannot do this anymore. Would abandoning my medical marijuana card make it possible to obtain my CDL, or does my injury, which has little to no effect on my driving, make it unlikely to get the license/pass the physical? A driver friend of mine told me that I could get licensed and pass the physical, but I would probably not be able to locate employment based on insurance companies refusing to cover me.

If your condition does not interfere with the safe operation and the ability to perform the duties of a cmv operator, then you should be able to pass the DOT medical certification examination. Any restrictions or medication will require a medical release form from your primary doctor.
I have scoliosis but no pain. also I’ve had my aortic root replaced and currently take blood pressure medication due to the surgery but my bp is always at normal range will this cause me to fail the physical

It may cause questions, but should not hold you back. Your bp meds will limit your medical card to one year and you will need a medical release for filled out by your primary doctor, without restrictions, once you are fully recovered snd released to return to work.

I have not seen anything close to my question forgive me if there is one and I didn’t see it – I got hurt on the job raising the landing gear on my trailer. I had a ruptured disc in my L4 . After surgery im still in pain and now after 3 MRI’s they doctor is saying I have nerve damage and nothing more can be done. He says I will have to take pain meds the rest of my life and thats all that can be done. I also have occasionally, usually at least once a day if not more, numbness from my right knee to my ankle and sometimes it goes across the top of my foot and into my toes. The numbness is not complete numbness but enough to where my leg feels like someone elses leg is attached to my body. The Medication he has me taking is Oxycodone and Gabapentin. – can I still operate a commercial vehicle under these conditions?

You will not pass a DOT class A medical certification examination to drive interstate. You might want to check with your local drivers license department to determine if you can drive cld intra-state only. Your medications and your condition may be acceptable under your states variances, but not the DOT/FMCSA.
Thank you for that reply, my situation updated today so I thought I would update my question and see if my situation as it is now would prohibit me from operating a commercial vehicle. I had another MRI performed and it shows no new rupture as the third one had showed. The Doc says my body probably absorbed the material. The pain is still there so he ordered a Myelogram/CT scan – I got the results of that today and it shows I have a bone broken between my L4 and L5, sorry I don’t know the technical term of the bone. The doc says the solution is a fusion. Doc says I may still have pain after the surgery and of course I will be stiffer. So with the pain still there plus the injuries to my shoulders and right foot I will need to be on pain meds the rest of my life. Doc says because I have been taking narcotics for so long due to the injuries I will most likely have to remain on them. So, will the fusion result in my not passing? Will the Meds cause me to fail? thank you

Pain, narcotics, and prolonged injuries don’t present a picture that says you are going to be driving anytime soon. It is highly unlikely that you will pass a DOT medical certification examination. That being said, wait until everything that could be done is done and then ask the question. Good Luck
FMCSA docs state you cannot drive a commercial vehicle if you are currently taking narcotic medications. Other medications that may have a tendancy to cause drowsiness are on a case by case basis and may be waived via a DOT Examiner decision. If you are on narcotic medications and you have an accident that injures or kills someone you could face criminal prosecution despite a doctor saying it is ok to drive. Keep that in mind.
Yes you can still drive, I take those meds and as well as other pain meds and still pass my physical to drive state to state!
My husband has cirrhosis. He is due for his dot physical but would not pass at this time. He is going to be evaluated to be placed on the liver transplant list.
If his medical card lapses will he have to have to start all over to get his CDL, after he receives the liver transplant.

I have recently been diagnosed with Complex Regional Pain Syndrome in my left arm resulting from a surgery to the ulnar nerve. I understand that there is no cure but it can go into remission and possibly result in a reoccurrence of the condition. Would this disqualify me from holding a CDL license?

CDL license is a state issued item, but your condition will raise some questions with the DOT medical certification examiner. If you can, provide a medical release form from your doctor to the examiner. This will help the examiner understand your situation and allow him/her to make the best possible decision regarding your medical certificate.

Would an individual who has ever engaged in self harm or attempted suicide in the past have a chance of passing the DOT Medical. Talking greater than 3 years in the past with zero relapse, was prescribed medication for depression and regular visits with a doctor but went off medication and doctor visits less than two weeks after the incident. I don’t have a family doctor or anything like that so I can’t have them say “he’s fine” “no future risk” exc.

FMCSA has some pretty strict regulations on self-harm patients, and their meds cause questions also. Make sure you can get medical release form from a doctor before you go for the DOT examination. Also check with the FMCSA regarding time frames from medication secession.

Uncontrolled ptsd is a safety issue. FMCSA wants the CME (examiner) to have as much information as possible to help him determine the safety of both the driver and the public at large. Controlled ptsd will need a medical release form from your treating and prescribing doctors. PTSD is not an automatic disqualifer.

I went to my attending doctor today and told him I passed physical without being cleared for work. You should not have passed a DOT medical certification examination with your present spinal conditions. Some one really dropped the ball here. Contact FMCSA and let them know your situation and report the CME that passed you.
I passed physical without being cleared for work clearance for lower back surgery dot doctor didn’t ask for work clearance. also had fusion c5-c6 on neck as well as ptsd. I am on workers comp waiting for back surgery on pinch nerves l/4 l/5 s1 waiting on approval from workers comp. my dot expires in 3 weeks, went today for recertification and the doctor said no because of previous injury and the surgery. Orthopedic surgeon said i can work regular duty just be careful. I am a spotter in the yard.

If being a spotter in the yard requires you to maintain your class A license, then you would not pass the DOT medical certification examination. Because you are on works comp and have physical limitation. FMCSA regulations do not allow for what could be seen as a safety risk to the driver and the public at large. It isn’t about what your orthopedic surgeon has to say about your work capabilities, it is what FMCSA guidelines for the DOT have to say.
I have a permanent weight restriction of 75lbs with a 35lb restriction over head because of tendon surgery (elbows ). I am able to move my elbows perfectly and no longer under a physician’s care, can I still get a DOT medical card?

According to the FMCSA guidelines, based on the job description of an over the road driver, you would have difficulty passing the medical certification examination. That being said, things have changed in the driving community over the past 10+ years and weight restrictions have shifted within the company requirements. You may pass the certification examination and not the driving job description of the company. If you know what company you will be driving for and you have a copy of the job description, you may pass the medical certification, but be prepared to answer questions about your restrictions. And check the FMCSA sideline for a job description for an OTR driver.

A lot will depend on the case conditions and any restrictions placed on the driver. Each situation is supposed to be evaluated case by case and final decision making should come from an understanding of the regulations and guidelines set forth by the FMCSA.

As long as there are no medical restrictions and a decent range of motion of the spine, you should be alright. The examiner will be looking for limitations that would pose as a safety risk.

I recently had an Umbilical Hernia repaired.
Went for a DOT EXAM in order to take the CDL TEST.
I Passed the Physical portion of the test. The Physician asked me to get a Relise Form signed by my DR.
The Dr signed the Release and Checked Off OK to Work and wrote, can not lift anything over 15lbs for 6 months.
I took it back to the Physician and was told I Failed and need to come back in 6 months.
From what I’ve read the FMCSA has nothing staying that the Restrication is an automatic Fail. It states that would be a Company Policy Issue.
I’m just trying to take the Written Test right now. Could easily be 4 to 6 months before I even get a License.
What can I do it go to show this Physician that it’s OK to Pass me and give me the DOT Certificate.

With nerve damage, depending on your restrictions, will cause questions but should be evaluated by the medical examiner. Make sure you take a medical release form from your primary doctor explaining your present condition and prognosis.

I am school bus driber and I had a total knee replacement in June. My surgeon gave me a letter on stating that I could return to work without restriction. The school I work for are asking for a letter from a DOT doctor that certifies me to go back to work. I am getting conflicting answers from a couple of DOT doctors. One said all I need is a letter from the surgeon specifically stating that I am not a risk to drive commercially, yet others are telling me that they cannot give me DOT clearence to drive without retaking a complete DOT physical. Which one is true or what is correct?
You need a whole new DOT physical because your knee surgery would disqualify you even if your medical card hasn’t expired yet. Anytime you acquire a medical condition that would otherwise be disqualifying, you’re technically supposed to report that and get a new exam when your treating physician clears you. Just be sure to bring your medical clearance letter to your exam.
I have had two neck surgeries since Nov . First was micro disectomy second was fusion of c6-c7 I am not sure I can pass a dot physical what are your opinions

Once you’re fully recovered and released to return to work, make sure you have a medical release form from your surgeon when you get ready to do your DOT physical, and you should be fine if all else is within normal limits.

They are both correct – yes you need to retake a DOT physical exam, and with proper documentation from the surgeon, and without restrictions, you should get a new medical card.

I had to get a D.O.T. physical as a pre-qualification for a “non-CDL” delivery position. I am legally blind in my right eye (20/200), but my only restriction for my regular driver’s licence is an outside rear-view mirror on the drivers side. The physician showed me where there is a “waiver” for that part of the test, but he didn’t know what the waiver was or how I could see if it would apply to me. Can you explain how I could get a waiver?

I have eye lens implants. One allows me to see distance and the other is for reading up close. Yet the dot guidelines say I have to have at least 20/40 in both eyes. Do I need to get a corrective lense for my reading eye to pass the physical?

Compression hoses will definitely be a plus. And you really need to reconsider a class A interstate driving career which has you sitting for up to 11 hours a day.

No, you must wait until you’re healed so that you can pass the medical examination. And, check with you local drivers license department to explain your situation, because the medical card on file will expire in the meantime.
By passing means what? I had the same rotator cuff surgery and my DOT doc said I had to lift 50lbs before I could pass the physical. At present I have a 5lb restriction but I drive a bus for a college and never lift anything.

If the limited ranges of motion do not allow you to safely perform the duties of a CMV operator, then you will not pass the DOT physical certification examination. You may want to check into intra-state only, but the same questions are going to arise.

A lot is going to depend on what class cdl license you are going to go for. If it is a class A, then you will raise may questions, because you will be a potential safety risk.
But if you are going to be state restricted, then you fall under the states variances and not FMCSA. If so, then you want to check with your local drivers license department.
Thank you for your response….I found out when my father suffered a heart attack they couldn’t figure out why so they did a biopsy on my aunt and myself since then I see a neurologist 2 times a year…. I have no heart problems as wel las no respiratory problem and hardly have any symptoms with the exception of a little tightening of leg muscle so I guess my next question would be if it hasn’t had a really bad affect would that matter?

Make sure you have a medical release form from your doctor explaining your condition. And explain the type of driving that you would be doing to the CME. With the proper documentation, you should be fine.
I had a Worker’s Comp. back injury approximately three years ago, I then obtained a attorney, There was no permanent injuries to my back I had approximately a year and a half of rehabilitation, which included physical therapy,and actual puncture. I had to see a physician who was a mediator, to determine the severity of my injury It was determined that I had 11% permanent disability due to my age. My primary physician informed the DOT doctor by letter that I was cleared to return to work with no restrictions. Hello there again Doc. What I was applying for was for my class B license, to drive a city bus, and my previous work I was a concrete Mason, I was cleared to return back to work with no limitations. I brought back a letter as the DOT Dr. required me to do, from my doctor . I went back to the DOT Dr. with a note from my doctor saying I can return to work with no instructions I gave the paperwork to the receptionist, she took it back to the doctor and when she returned she told me I did not pass with no explanation, The physician would not even come out and explain it to me. You are a class B driver and do not fall under FMCSA class A regulations.
So, check with you local drivers license department to find out what regulations or restrictions may apply to you. You’re being examined as a class A driver which is not appropriate for a class B driver.

Since you fall under your states drivers license rules and regulations, as a class C driver, we suggest that you check with the state regarding any visual impairment waivers or releases. If you can continue to perform the duties of a cmv operator and are not a safety risk you should be alright, if everything else is within normal limits. I have worked with a Class B license driving a bus for 4 years. I went today to get my DOT card renewed and the doctor said I did not have full range of motion when I turn by head to the left though I feel I do . She said she thought I had spasmodic torticollis and sent me to my family doctor and she said I was a little tight on the left side. She wants me to see a physical therapist to see if there is a test I can take to determine it. Is there anything in a DOT physical that says this is a criteria and also if I cannot work while getting better what are my options. Does the company put you on sick leave or are you considered unemployed.

There are range of motion guidelines with the FMCSA, but you are a class B driver, driving a school bus. You fall under the states variances and not FMCSA. As far as your other question, you need to check with the company policy for a situation such as yours.
In 2007 I slipped on ice and ruptured my right quadriceps tendon, after the repair surgery failed, I had surgery again 8 months later. I finally was cleared MMI, with restrictions of no crawling no kneeling. I fell last year and ruptured my left quadriceps tendon, and completely tore my right rotator cuff. After surgeries, and therapy I am at MMI with major work restrictions, no climbing, no lifting more than 10lbs. Can I pass a physical?

I have 2 lumps on the left side of my abdomen, and one one each thigh that I’ve had for over 15 years. A doctor checked them out years ago during a physical and said they were just fatty tissue. They don’t hurt unless I press directly on them, and they have never been an issue.
My question is, will this disqualify me if I don’t have a doctor’s release? I know they need to press on my abdomen for the physical, but as long as they don’t interfere with my duties, will I be ok?

You should be fine. If it is questioned simply explain yourself to the examiner. He may request a further medical examintion for a confirmation that they are simply fatty cyst. The CMEs are not supposed to do diagnostics, but to request further documentation from your primary doctor.
I have a B with tanker endorsement and before my injury I was driving interstate. I’ve had 3 surgeries on my right arm within the last 5 months. I was just released to work but I’m not allowed to use my right arm at all. I had part of my forearm muscle removed, tendons in my elbow repaired, and nerves repaired in my elbow and hand. My index and middle finger are probably going to be permanently numb. My question is will I be able to renew my med card before it expires in May? Any help would be greatly appreciated. Thanks.

If you plan to go back to interstate driving, then the examiner will want you to have a skills performance evaluation done before he will sign off on you. Another option may be to go intra-state only. That way you are home everyday and you will fall under the states variances and not FMCSA. Good luck.

You are a class B driver and fall under the states variances, unfortunately your examiners are trying to evaluate you as a class A interstate driver.
We do not believe that as an intra-state, class B driver, that you need a SPE exam. You need to explain to your DOT examiner that you are not being held to class A interstate driver standards. You are in your states rules and regs. Good luck

Once the fusion is done and you are fully healed you should be able to modify your movements thus allowing you to continue to perform your normal work duties. Squatting is only done to perform your normal work duties, but if you have a modified activity that still allows you to get the job done, then you should be fine.

Before you get started, you should look at the job description for a driver and make sure that you can perform the duties of a cmv driver. If you can do the normal duties, then there is no reason you should have any issues with the FMCSA. Rods are not a disqualification issue by themselves.

Hello, I recently had a DOT physical by my company’s industrial clinic and the determined I was not qualified due to my BMI. and high blood pressure. They want me to do a sleep apnea test and will not qualify me until this is done. I haven’t seen a personal doctor for several years and my knew doctor does not have enough history with me to sign off that it’s okay for me to drive until further tests are done. I have been out of work for a week now on safert hold.

About three months ago I was diagnosed with rheumatoid arthritis and have been off work since I drive for a bottling company delivering cases of pop and only reason I’m off was because of the physical demands NOT the driving my med card is up in july 2017 I have talked with employer about doing other jobs besides delivery and they said they will work with me! right after I was diagnosed from my family doctor I went to rheumatologist and I am on sulfasalazine and methotrexate and prednisone they seem to be working anyway?My concern is being able to requalify and I plan on getting the release form from rheumatologist before I go I know it will be the cme final decision if I am not driving! Andy, I kind of in the same boat however my card expires in 3 weeks, I live in Florida so I will let you know what happens to me. Florida DMV told me I could down grade to a class E for a small fee. Them when I get My release to work, and pass a DOT Physical I can go back to DMV and again for a small fee get my CDL A back with all endorsements and not have to retest, as long as it is with in a year from the down grade.. I’ll let you know if that is indeed the truth in a few weeks.

Well with my issue, (dermatomyositis) the DOT Medical officer told me it is disqualifying with an exemption, So I will need my Dr to sign off on disease and my medication list as well as being able to shoe MRO that i’m in good shape and my lower extremity muscles are able to support my body and complete the job description of a fuel hauler.. Right now that is not the case, so I did not pass which I fully expected, Dr said wait until you are sure and show definite signs of remission before attempting DOT physical again. So with that the state of Florida will allowed me to downgrade my CDL and I am able to reinstate it and will not have to retest at any time before it expires for a small fee. Good Luck to you..
This comment is for the state of Florida only. Speak with someone at your local office where you renew your driver license, you may find there is a document you can sign which allows you to keep your class A, even if you have no current medical card and your unemployed, but this is serious and not to be used for fraudulent work! Driving class A vehicle’s carry some serious responsibilities, please have yourself medically healthy and fit before jumping into a truck just because you want to.

With your doctors release form in hand and a brief explanation regarding the uses of the medications that you have been prescribed, you should not have any major issues. As long as you have supporting documentation and explain to the examiner the type of driving that you do, you should be good to go.

I suffered an accidental gun shot wound to my left shoulder when I was 18 resulting in a fused arm to shoulder connection. I’m 44 now, the fused part limits me from being able to extend my arm above my head, but not side to side or backwards. I can drive just fine. It is going to be tough for you to pass the DOT examination. You have pain issues, medication and possible surgery on the horizon and loss of strength in one leg. Based on the DOT/FMCSA medical certification examination, you probably won’t pass at this time. Get your health issues taken care of and fully healed and you could try again later.

I had a downhill skiing accident on December 26, 2015, four surgeries and 2 staff infections later, mid December 2016 my HR team will not clear me to get back to work…I’m a wine & spirit salesperson. Doctors have given them vague information regarding lifting weights 25-30 lbs. for 2-3 months, even though lifting 50-60 pounds is not an issue for me. I have a drop foot from medial nerve damage, but can walk wearing an orthotic when out for long distances or without the orthotic around the house. HR stating insurance liabilities for driving with this condition, even though we drive our own vehicles on the job. I have driven myself to the hospital twice now, which is 2 1/2 hours away since July 2016 and drive all around our valley without any incidents. My patient coordinator at the hospital recently suggested I may need to get an Independent Medical Exam or a Functional Capacity Exam. I contacted my local DOT, no clear answer and the Hospital staff at Occupational Health says they only deal with L&I (work related) cases. My question is: where can I go to get tested to validate whether I can drive my personal vehicle so I can get back to work and/or possibly validate my CDL B license so I might have the opportunity to drive commercial vehicles such as the local Link buses or school buses as alternative job possibilities in the future. Thank you very much for the site you’ve created and the information you provide to so many that are baffled by our medical professionals. Even though the staff that treated my tibia/plateau spiral fracture did a fabulous job of saving my leg and getting me healed, they’ve done very little to address the nerve damage in my lower leg and foot. With your present situation you will have some difficulty passing a DOT medical certification examination.
If you are a CDL holder you need to be fully recovered to even be seen to do the examination and you will need documentation to support your CME’S findings in the examination.
If you only drive on a standard drivers license then the best answer will come from your local drivers license department within your state.

If you are on disability then you are not fit to pass the DOT medical examination. Each state has a process in which you should be able to keep your cdl until you can pass the DOT examination once you are released to return to work. So check with your DLD to find out what those steps are.
I had a seismoid ectomy. The only last effect is intermittent mild pain that is relieved with otc meds like Advil. I currently drive a RR5000 which is a stand up fork lift for 12-16hour shifts 3 days at a time without issue. Will I still qualify for a medical card?

I have a CDL license in IL but I got a drop foot problem from an accident I was in I have a AFO flexible foot brace to wear. I have been getting my foot movement back slowly an now my doctor wants me to practice driving my pickup truck around to see how it works. If this works to drive my pickup truck I think he will let my go back to work an see how I do driving my semi. I have been trying to find out if I can drive a semi with my brace on til my foot heals up. Is it against the law to drive or would I have to have a doctors note to be able to drive with my brace. I also have a drop foot issue though with medial nerve damage, I also have an AFO flexible foot brace and progress is slow but steady. Keep doing your PT & exercises. I have a CDL B license which is cleared, but drive my own vehicle for work…but am still trying to get cleared by HR to return for work. I’ve adjusted my driving by moving my knee and leg to push on the gas pedal since I have limited movement in my foot/ankle…you can do it!
Hi so i just want to tell you my story right now, I may have a chronic ankle condition that causes my ankle to hurt and be unstable. The rest of my body is fine lol. i’m looking at my options in case i can not return to my current job. I was just a little concerned as I may need to do some heavy lifting, maybe i could use a dolly cart to push stuff. I am on restriction not to lift over 20-25 pounds.
If i am stuck with these restrictions can i still do this ???

It does not appear that you could qualify as ‘fit for duty’ to operate a commercial motor vehicle, as an over-the-road driver.
You could explore the likelihood of doing more regional work, but you will need to deal with a company that understands your physical condition and is willing to allow you to work with those restrictions.
I have partial drop foot I can push my ankle and foot just fine with no problem I can stand on my tippy-toes, My problem is with lifting my foot up the nerves will not allow me to lift my ankle up. I can lift my leg with no problem I can drive with no problem I have no problem pushing the gas or lifting my foot up to the break or back to the gas. Do you think this will stop me from being able to get my CDL or passing the DOT physical?

As long as everything else is in good operating order, you should be OK. The CME is looking for issues that would be considered a safety hazard. Also, have your doctor sign a medical release form for you explaining your situation and abilities, that will be a huge help.

If the splint limits your ability to operate a CMV safely, then you are disqualified until you can return to work without limitation and can be found, by a CME, to be medically fit for duty.
I have congenital heart disease that later resulted in VSD repair and two other surgeries for aortic valve replacement. I have been able to pass my DOT physical in the past. However due to a episode of overexertion I was sent to be checked out by my cardiologist. They found a abnormal heart pattern in a stress test (ST depression during exercise*No Arrhthyias noted*)and was then proceeded with a nuclear test which came with clean good results. Complications My Cardiologist listed Tachycardia, dyspnea, and presyncope. Prior to nuclear test results coming with clean bill of health results. My question. Can I pass a DOT physical with a letter from Cardiologist stating that I need to refrain from sustained heavy lifting, or resistance that requires bearing down and holding breath. (Valsalva)? I’ve seen Truck drivers with handicap stickers in windows before so I presume I can? Can I get some answers please?

A medical release form is going to be a must in your situation. Have your doctor explain your condition and restriction, if any. A lot will depend on the class and type of driving that you do. With all the information in hand before you go for your new DOT examination, that will be a huge help to the examining CME. He/she needs as much information as possible to make his/her best decision.

I have a Class A cdl. I live in New York. I had to have surgery on my abdomen to remove a bunch of scar tissue that was there. With that scar tissue it was difficult for me to eat without any type of pain. My doctor has cleared me to return to work. My company said that I must get another DOT cert before I can return back to work. My current cert is good for another 1 1/2 years. Company stated this is DOT regulation for any and all surgeries. Your company is correct. Any issue that takes you away from behind the wheel requires a new medical cert before you may drive again.
One of your drivers is going to have surgery on his shoulder, because of an on the job injury. His medical cert is going to expire when he is off for surgery. Right now he is certified category 1 – Non-exempt. Does he need to downgrade his cdl in the meantime since he will not be able to take a new physical until he is healed? If so what category should he be in 2 or 4? He has a minnesota cdl.

Yes, once off duty you must demonstrate a medical fitness for duty before you can go back to work. That means a new medical exam.
I’m a CDL-A in the state of MA and I do go out of state some times when required. I fractured my ankle on August 7, 2016 and my DOT card expires on September 26, 2016. I won’t be able to renew my DOT card because I won’t be cleared to return to work full duty until late October or even mid November. Is there any sort of form or waiver to submit to RMV for a self certification? I don’t want my license being down graded and risk losing my job!! I just don’t know what to do.

I have a driver who has been out on workers comp for a year. He had a neck injury. He went for his DOT and the ME told him he has to have a Functional Capacity exam before she will consider doing his DOT as he has been out for a year. The workers comp doctor has already signed him off as fit for work, should this FCE have been done first? Now he can’t drive but he isn’t receiving workers comp either. Would he have to get the FCE done himself?

If he can perform the duties of a CMV operator, then he’ll pass the examination. If he has normal mobility and is pain free, then the examiner should be able to determine medical fitness for duty.

I live in NY state, and have had my cdl B for about 10 years now, and I drive a school bus. Two years ago, I was diagnosed with diabetes II, insulin dependant. After that, they were giving me only 6 month cards, with physicians certifications. I just went in today and the nurse practitioner informed me that they no longer give medical cards to diabetics, and she refused to administer the physical, regardless of my cardiologists certification that he feels I am fit for duty. This is the ONLY company that the school goes through for their DOT needs. Is this true or is the nurse wrong??
Also, I have AFib with an implanted cardiac monitor in my chest, and I’m on antiarrhythmic meds for this condition. She told me that she believes that having any type of cardiac implanted device automatically disqualifies me from operating a CMV.

Nurse is incorrect.
You fall under state variances and should not be evaluated as an OTR driver. So check with your local drivers license department and get the fact straight before you take that information back to the examiner. Make sure you have medical releases from all of your doctors before you return to the CME for an examination.

As far as impact due to spinal fusion, you are looking for inabilities to perform the duties of a CMV operator. Can they do their job safely and can they continue to operate a CMV safely. Are there any chances of the driver becoming incapacitated due to the fusion.
If all is stable and you have a medical release form from the surgeon, then just follow the guidelines.

is there any restriction as to being denied your medical card for OTR if you have Fibromyalgia? I have been on a variety of treatments/injections/meds with little relief. Im currently on full disability but am wanting to earn my cdls. Specifically, I live in nerve pain, and am slow walking but capeable of doing the job required. I have rebuilt myself to cope and succeed with it for over 10 years.

You will want to check into the job description of a CDL driver. Make sure that you can perform the duties of a commercial motor vehicle operator.
Pain meds will be questioned and can be answered by the medical release form.
Physical ability has to be evaluated by the examiner.
So take plenty of information and your medical release forms with you.
You might also consider an intra-state only CDL. There are some variances within the state that may help you achieve your goals.
I am attempting to change careers, getting my Illinois CDL-A. Last year I was pff work on FLMA due to bulging and herniated discs (C4-5, C5-6 respectively). I currently have intermittent medical leave through my employer to cover my ansence during flare ups. My range of motion seems fine, but my current job as a casino dealer aggravates my condition due to tye repetitive movements. Will this be a problem in passing my Illinois DOT physical?

If you are going for a class A interstate license you will not be medically fit for duty as an interstate class A driver. Every time you would have a flare up you would need to have a new medical certificate to determine medical fitness for duty.
Now if you are going to drive intra-state only, you fall into your states variances. Contact your local drivers license department about state variances.

Three years ago i was suppose to have an acdf c4-c6. Now today i have to have my entire neck fused, yes c17, because of the constant jarring in the truck. I would tell you its not worth it bro.

Take the medical notes with you and show them to the CME up front, before you take the examination. If you are not going to pass you may as well know it up front.

I have a friend that I’m concerned about, he just jokes about it when asked. He is a diabetic with a Class A CDL, he told me his doc wanted him on insulin but he said he would loose his cdl so he’s taking the most expensive pills instead and he’s had laser eye surgery atleast 10 times this year to stop his eyes from internally bleeding(diabetic retinopathy) and he has peripheral neuropathy and I’m pretty sure the booze doesn’t help, Is it safe for him to be on the road?

I work for RTA in cleveland ohio, 3 days ago on tuesday my company asked me to see a dot doctor to be checked, I was not off work,I did not complain of pain in fact I drove my morning shift and was about to do my afternoon shift when the company ordered me to see a doctor at eastside occupational center in cleveland ohio, he reviewed my chiropractors notes and said I willl loose my cdl endorsement,I want to know what I can do at this point instead of being on disability which this doctor said I can go on, I prefer to keep my cdl and continue my driving job as a bus driver.

You are an intra-state driver and are not interstate.
Someone needs to understand that difference and rank you accordingly. Now if you unsafe due to injury or pain that’s one thing, but if are doing your job without any safety issues, then you should be able to drive.
So the first question would be, why did work send you for a DOT physical?
Second question would be what class driver are you and do you need endorsements to do your job?
My husband is non CDL holding but his company still requires a medical card. He injured his back at work in Feb and has been on light duty… driving only. Nothing else. His restrictions are no lifting over 20 lbs and push pull up to 40lbs. He has a herniated disc, and does is not on any meds for the injury. Can he get re-certified? or some sort of limited pass?

Medical certificates are to determine medical fitness for duty as a driver. Your husband should explain to the examiner that he needs a medical card for a non cdl holder. Then based on the finding of the examiner he will make the final decision.
My husband fell 2 wks ago cut above his elbow and got mersa in it. He went for company DOT and doctor said if he signed in they would fell him. We went back for wound check and it healing but that the elbow has bursitis (inflammation) doctor wrote a note saying he can return to work and that he is able to drive. Will they fell him on his test if he goes in for the dot physical?

If he cannot perform the duties of a commercial motor vehicle operator, then he will fail. Driving is only one part of his duties, so he must be able to pass as medically fit to operate a CMV with all of its duties and requirements.

The CME just needs more information from your neurologist and surgeon. Get a medical release form from both of them and make sure that you can perform the duties of a CMV operator.
I had my left kneecap shattered and had surgery. I am fully recovered (apprx 2 years) but cannot bend it fully. I can still walk normal and move around, but I cannot squat all the way down with both knees bent. I can still bend it somewhat but not all the way. Wearing a knee brace also helos it alot. I haven’t had any issues with it since. I wanted to know if I can still be qualified to pass a DOT physical with conditions if need be?

If you can manage the basic job description of a CDL driver, then you should be fine. As a class A driver you will need to perform some duties that may require you to kneel to do basic inspections of gear. Even if you use a brace to do so, you should be OK.

If you are presently unable to perform the duties of a medically fit for duty driver, then you will not pass the DOT examination. And if you are possibly going to have knee surgery in the near future, then you will want to talk with your local drivers license department and see what they recommend for you to do to keep your cdl while you are out with the knee replacement and recovery.

As a class A driver you do not fit the requirements, via FMCSA, as a medically fit for duty driver.
Your medical card was invalidated at the time of the accident and you need to have a new medical certificate, once the doctor releases you to return to work with out any restrictions.
I have my left leg amputated below the knee and use a prosthesis to walk. I get around fine and most people don’t even realize I have one unless I wear shorts or tell them. Anyway, I have missed out on a few decent paying driving jobs because when I was sent to have my drug test and DOT physical, the medical examiner said I would need a SPE certificate because of my leg. This job I was applying for wasn’t even one which required a CDL. The vehicle I would be driving was a typical Ford delivery van that anyone with a drivers license can operate but because of bureaucratic BS, and a DOT number on the van, I need an SPE certificate.
The problem is that I have to go have this evaluation done and none of the specialists offices I have contacted even know what I am talking about when I mention the SPE and the packet of paperwork from the drivers license office. I was told it takes 6 weeks to process the paperwork once it’s mailed to the Texas DPS in Austin by Concentra, the clinic that does the drug test and physical. Employers aren’t going to wait that long plus the time it takes to get an appointment and see the specialist. They are going to move on to the next candidate. I was working as a driver for another company which didn’t have a “magic DOT number” on their trucks before this but left due a cut on work hours from 40 to 30 (thanks Obamacare) and dismal pay.
My question: Is there a way to expedite this process or is there some kind of waiver I can get when a driving job doesn’t require a CDL?

It seem like someone is evaluating you as if you are going to drive interstate and big rig.
You need to have a CME evaluate you for an intra-state job and a lesser class license other than a class A. Your state should have a policy and guidelines that a local CME can use without the need for the SPE. You will have to explain what you are going to be driving and out to what distance. After that you should be OK.
I’ve had an Inguinal hernia for about 27 years . Hernia belt controls it except for squatting . Will this disqualify me on the physical exam ?

It is going to cause a lot of red flags, so be prepared to answer questions about it needs and usage. Get a medical release form from your prescribing doctor before going for you medical examination. The MD will need to explain why he has prescribed this medication, how long you have been on it and what conditions you are being treated for.

An injury that takes you out of the truck requires new medical examination/certificate. You won’t pass the DOT medical examination until you are fully healed and have a full medical release from your doctor. Since your doctor does not deal with the FMCSA and does not have to contend with the federal guidelines, he does not know this.
So get healed and get a medical release form from your doctor. Then you will be good to go.
I have not missed any work due to the injury. I can still do my job completely. I’m just concerned about going through weigh station or something like that and someone saying something.

Given todays driver requirements, you may do OK with the weight restrictions. You want to bring a doctors release form with you the time of the examination. Out of pain management and off all meds for awhile before you go for the DOT medical certificate. There will be questions about your medication usage, so be prepared and if you can get a medical release form from your PM doctor even better.
My fiance was in an auto accident 3+ years ago that rendered his left arm partially useless. He is able to lift the arm and move it around however the lower portion of his arm has very little muscle left and his hand is slightly contracted with limited movement of his fingers on that hand. He got his regular license back with documentation from his doctor that he is fully able to drive. He had his CDL prior to his accident and they were reinstated as well. He has applied for a job driving again and while seeing the DOT doctor that doctor asked if he could turn his left arm palm up. He told him “NO” then asked why. The doctor stated he wanted to make sure his arm was strong enough to drive. He told the doctor he was already medically cleared by his doctor that he could as well as he has the ability to lift 70-80 pounds with no problem. The DOT doctor now wants him to go out on a road test with a FEDERAL DOT Representative just cause he can’t turn his arm palm side up. Is this legal and if so why is he requesting this and whom would administer this driving test? Please help if you can. We reside in PA.

The CME is forced to follow the FMCSA guidelines and what he is requesting is FMCSA. Your doctor does not do DOT examinations and is not aware of their requirements. Your fiancé will need to have a skills performance evaluation to go back to cdl interstate driving. Your doctor cleared him for intra-state only cdl and a regular drivers license.
Hi, I got hurt at work and had L4-L5 spinal Fusion and am still off duty and recovering from the surgery.
My DOT physical expires in 3 weeks and i dont want to lose my Class A CDL liscense.
I dont believe they will pass me in this condition as i still can’t lift over 8 lbs, bend over, or twist.

You need to get further information from your local drivers license department. They should have some policy in place for situations just like yours.
You will not pass a DOT physical, so you need the information from the DLD so they don’t downgrade your CDL.
Change your over the road status to local driver in the state you live not required to have DOT physical until you get back on your feet

Not that I am aware of. One test gives results and a report. Not the other. A medical release form from the cardiologist will be very helpful though. If the treatment resolves the problem, then the release from the doctor is going to be required.
Applied for another CDL job. Previous job was on workers compensation part of time. When new employer calls for verification company states I was employed to the date I went on medical leave. While on workers comp aren’t I still considered employed by the company? And also can they come out and state I took a leave of absence?

I have a CDL class A. I had a handicap placard and ID card issued to me a while back as a result from ACL knee reconstruction. I’ve since had to surrender my handicap placard and ID because I hold a CDL. I am in no way hindered by this when it comes to driving, double clutching, getting in or out of truck. I just can’t walk long distances outside the truck. Why can’t I have a CDL and a handicap ID at the same time? It has no affect on my driving ability whatsoever. Would I seriously be disqualified from the Mvc medical unit over this?

FMCSA set the guidelines for a class A driver as medically fit for duty to perform the duties within the basic job description. You could back back and ask the FMCSA this question. As in all things, there are shades of gray within the black and white of each guideline, so ask away.
I have a unique situation I have not run into before with one of my drivers and I’m looking to find out how to help him. My driver in question has been out on workman’s comp for a sever injury he sustained on the job and has been out of work for almost 2 years now and may still be out for another year or more. Without getting into a bunch of detail he was hit by a set of dual wheels that came off another truck while he was on the side of the road and is just getting back to walking with crutches and cannot drive Inter or intrastate because he cannot pass a DOT physical to get a medical card to self certify. He has gotten the “We are going to put you down to a Class D license letter”. So my question is in the state of Massachusetts is there a waiver he can get or something so he doesn’t have to go through the tests all over again to have his CDL reinstated?

Only if you are unable to perform the duties of a CMV operator. CME’s are looking for medical fitness for duty and they have a basic job description for what a drivers job entails. As long as you can perform those duties, then you are good to go.

A class B cdl requirements will fall under the states guidelines and regulations, not DOT/FMCSA. Best to check with your local drivers license department for your best answer.
My son is a right below knee amputee due to birth defect. He learned to drive with his left leg because the Drs said that is what they recommend to their patients. There are no regulations in California about driving with left leg. My son is no 20 and an EMT and he was refused an ambulance drivers license. It was very hard trying to find out why but one clerk said it was probably because he drives with his left leg. He is extremely fit and has been a Firefighter Explorer. He had a hearing and the hearing officer talked to a FMCSA representative and they agreed my son should be certified but when the case went back to The Ambulance review board they again rejected him He has waited a year to reapply and now drives with his right leg and he has a much more advanced prosthesis so he is about to reapply he only would drive in California and we have been told that on the new DL-51 forms there is a box the Medical Examiner can check for a waiver for in state drivers with missing limbs. Is this true? He has been to a couple of different examiners and they were clueless about this whole situation and we don’t want to make a mistake and he get rejected and have to wait a whole year again.

Have the examination completed by the California examiner and he will mark that your son does “not qualify”. Then take that examination form to the state drivers license department and they will be the issuers of the needed waiver/exemption for him to “qualify” to drive in the state.

Any condition or illness that takes a driver from behind the wheel needs a new medical examination to determine fit for duty status. Not numbers of days, but ‘unable to drive’ for any reason.

If you will be able to perform the duties of a CMV operator and pass as medically fit for duty, then the only other thing you will want to do is bring a medical release form from your doctor stating that you are fully released and a brief explanation regarding your situation. That release form gives the CME the type of information that he will need to base his final decision on.
Hi all, not sure whether this is the correct place to get an answer to my question, but it is a starting point. My Dr thinks I have a torn rotator cuff (I saw your answer on passing a DOT for that, I’ve been working for 6 weeks after the injury, so it’s not a problem passing with this injury). My Dr has taken me off work for 2 weeks so we can get an MRI done and see what needs to be done. Our Safety director has stated that I will have to have another certification after my dr releases me to work again. Do you know whether this is a DOT regulation? My husband has been off for other reasons, and never had to re-cert afterwards.

FMCSA states that any injury or illness that takes a driver from behind the wheel has to have a new medical examination to determine fit for duty before returning to driving. It’s a FMCSA thing. In the past most of the examiners did not know that this was the law.
I’ve been out on workers compensation for over 6 months and medical card is due in a month. Drs do have me on full disability so obviously I can’t get qualified.will all drs on dot registry know that I’m on workers compensation.

With the broken arm, you can’t qualify as a fit for duty driver. So check with your local drivers license department and see what options they have to offer. In most cases they can extend a helping hand with situations like your. Also be able to answer questions like when will the cast come off and how long before you will be released to return to work once the cast is off and the arm is back to near normal.

You would invalidate your medical card the moment you went in for surgery.
Try checking with your local drivers license department and explain your situation. They should have a policy that will allow your CDL to stay while your are recovering from the surgery.

As soon as you are released from your surgeon and you can perform the duties of a commercial driver. So you will need to pass a new medical certification examination. Also bring a medical release form from your surgeon, that will help the CME in his decision making process.
I currently have a cel, I have been diagnosed with degenerative disc disease in my lower back, L4/L5 & L5/S1, the disc are ruptured and pressing on nerves that send extreme pain and numbness through my left leg. I am taking Oxycodine to control the pain to a point where I can function. My left foot feels as though it is a sleep all the time, my company says I have to come to work am I eligible to drive at this time? I don’t think so but I need to know so I can direct them to the proper requirement documentation.

You would not pass the DOT examination and would be disqualified as a driver. You would not qualify as medically fit for duty. Check the FMCSA website for drivers job description, you don’t qualify.

Not in and of itself, but you will need to demonstrate you medical fitness for duty. That may require a skills performance evaluation before being able to return to driving. If you can perform the duties of a commercial driver and the SPE is cleared, then you should be good to go.
I was in a roll-over accident in September. I’m in a neck brace until January. My DOT med. card is up in November what do I need to do. I will not be fit for duty until I am out of the neck brace in January.

Trying to find out if I should go through the process of getting a CDL, I’m currently LBKA (Left Below Knee Amputation) and also recently got diagnosed with type 2 diabetes (3 months) currently being controlled by medication, diet and exercise. Will any of this existing conditions hinder me from obtaining a CDL and if not what are the steps to follow? I have been doing a lot of research and believe I found most of the forms needed but not sure in which one comes first! Do I take the written test first and before the driving test get the medical exemptions (if I qualify!) or are there any other things I need to do? Any insight will be greatly appreciated and my thanks in advance.

You would need get information from the FMCSA, since you will be required to have a waiver for the missing limb. That should answer most of the questions. Also check with your local drivers license department and see what information they have to give you about their process.
i have degenerative disc disease in my neck i have c3 c4 c5 c6 fused and a titanium cage i had a second surgery last October to put a rod in because one of the discs did not fuse i am a school bus driver the dr at concentra pulled me off the road in september he said i didnt have enough range of motion in my neck to drive a comercial vehicle what is the percentage of range do i need to go back to work

About 70 degrees of motion in each direction would be a great start. The average person has about 90 degrees of motion in each direction. Also check into what your state wants with the use of your side mirrors, you are not driving interstate, so not sure why you would be held to interstate CMV driver standards.
I need cervical fusions for stenosis. My entire thoracic is fused because of scoliosis. Is it possible to have fusions of C4-5 and not put in rods if C6-7 do not have serious stenosis at this time? If the entire cervical spine is fused, can you drive? As you can tell, I am scared to death. Any help would be appreciated.

Your surgeon will have to be the decision maker on what needs to be done. Most of the cervical ranges of motion occurs at the C1 to C3 levels, so once you are fully healed, you may be able to return to driving.
My husband did not pass his DOT physical because the physician at Consentra said he had Peripheral Neuropathy. The physician stated he needed to see a Neurologist. He went to the Neurologist, which we had no help on where to find one, and the Neurologist gave him an exam and stated he was fit for Commercial Driving. He had an EMG done. A nerve conductivity test and his Neurologist still says he is fit for commercial driving. He also went to see a Physiatrist and she stated that he was fine to drive a commercial vehicle. Nonetheless this physician at Concentra will still not pass him. What credentials does she have to override what two physicians with this specialty have?

Peripheral Neuropathies This group of disorders consists of hereditary and acquired conditions where the nerves, including the axon and myelin or the myelin selectively outside the spinal cord, are affected. These conditions may affect the sensory or motor nerves individually, or both may be affected. Peripheral neuropathy may be a complication of diabetes mellitus. You should evaluate the sensory modalities of pain, light touch, position, and vibratory sensation in the toes, feet, fingers, and hands for signs of peripheral neuropathy.
The medical examiner at Concentra didn’t even touch him or evaluate him.
Recommend not to certify if: The driver has a diagnosis of a peripheral neuropathy. NOTE: Neuromuscular disorders represent a complex group of conditions. The severity can vary with the individual and in certain instances may be treatable or nonprogressive. Guidelines for reconsideration of the decision not to certify include: • Evaluation by a neurologist or physiatrist who understands the functions and demands of commercial driving. Specialist may recommend a simulated driving skills test or equivalent functional test. • Annual recertification that repeats specialist evaluation and driving test when indicated.
If the severity can vary with the individual and obviously it does, and he has done everything asked of him from this physician at Concetra, why can’t he get his DOT physical card? I have emailed the Medical Review Board with FMCSA as well.

So with all this information in hand, why don’t you just go somewhere else to get the medical certificate?
The physicians who have done all the background work for you must not be on the FMCSA’s NRCME registry, so the CME is left with his/her own evaluation. But it does require time and knowledge to evaluate each driver as they come in the door. Some of the ‘doc in a box’ places use the basic cookie-cutter approach and don’t even evaluate the drivers. Just take the easy money and screw the driver who needs a proper evaluation.
Answer: Take the available information and go somewhere else for the DOT certification.
Sadly she is on the National Registry. So I don’t think it’s fair.

Yes, you are not fit for duty, so you would not pass at this moment. Any injury that takes you from behind the wheel is an instant disqualifier. So once you are released from your doctors’ care you should be good to go.
If you won’t be fit for duty by the time your medical card expires, let your local DLD know so they don’t automatically downgrade your license.
I had Anterior Cervical Discectomy and Fusion C4 and C5 will this cause me to fail DOT physical? I am a road driver , driving doubles and also dock work is involved

This is not an issue unless the cervical surgery caused a marked decrease in your ranges of motion of your head and neck.
As an OTR driver you should be able to move your cervical ranges to be able to visualize at at least at 70 degrees.
i broke my arm on the job. it has come as far as it will come toward recovery but still missing function. it wont straighten and is shy by alot, maybe 30 degrees. my hand wont supenate more than about half way and i havent gotten much of my strength back. at last eval my grib in my right hand (the arm i broke and im right handed), is less than half as strong as my left in grip strength. and my pushing strength was less than half my left arm. pulling was better but still less than left arm by a chunk. will i be able to pass a DOT physical?

All of things will come into play to determine if you are medically fit to operate a commercial motor vehicle. If you feel that you can perform the duties of a commercial driver, then you should give it a go.
If you feel that you will not be able to do the jobs of a commercial driver, then you won’t pass the test. I would bring a medical release form from your doctors and that will help the CME to make more informed decisions about your capabilities and short falls.
You might also look into intra-state only and see what is available to you.

Until you have the surgery and know where you are afterwards, then no, not until you are fully healed and both you and your doctor are in a position to say you are fully recovered.
If you feel that you could drive once everything is finished, then make sure you have a medical release form explaining your situation and condition. Also note any medications and side effects. Have your doctor and surgeon fill out a medical release form before you try to take the DOT examination.
My question is: can I still qualify for a cdl B license if I have had several discs in my back fused. I am a municipal employee so I do not drive over the road. However, I spend long hours snow plowing and it is hard to get in an out of the truck. Other then that I seem to be able to drive ok. Thank you for any advice. I’ve been trying to find some info on back injuries but so far no luck.

If you have several fused vertebrae, your best bet is to get your medical doctor to fill out a medical release form for you and take a copy of your job description with you when you do the next examination.
You are a class B and are not being held to the same standards, but still must be able to perform the duties of your job. If you can safely do your job with your spinal condition, then you should not have any problems. Just take the needed information in with you.

If the neuropathy does not interfere with your ability to safely operate a CMV then you should be OK.

The question here is: Can you meet the physical demands of the job required of any commercial driver?
I am out of work with a broken hip with severe nerve damage. Some of the nerve damage is from two broken legs about five years ago, the perineal nerves were damaged, and made worse from my hip injury. Second, the femoral nerve was damaged, and the two nerves in the back of the leg were also damaged from the workmans comp injury and Icannot as of now work because I can’t walk a step without the femoral nerve firing and I lose total muscle control of my leg. I am scheduling a nerve block next week to help with the pain as I Am taking narcotics for pain control now. The doctor is giving no guarantees.
His second surgery would require a wire from a previously installed neurotransmitter attached to the femoral nerve to see if it can control the pain and muscle control loss, again no guarantees. My question is if I have the surgery to have wire attached to my femoral nerve in my right leg just above the knee does it disqualify me from getting my medical card if the surgery is successful. I know that if it isn’t successful I will probably be disqualified because the only relief I am getting is from the narcotic medications

There is no point here that would cause an instant disqualification. That being said, first see where you are after the surgery. If all is well, then be prepared to have your medical card limited, due to the examiner wanting to keep an eye on your situation. And yes you are right that if the surgery is not successful and you have to stay on the narcotics, you will not qualify. Good Luck with this.

If you can perform the duties of a CMV operator, then all you may need to do is a skills performance examination to determine that you can perform the duties of a driver. Any one that is performing the DOT examination is on the NRCME and has to operate under the same guidelines and regulations that everyone else is operating under.
I strained my right knee right when my med card is renewable and they failed me. the doctor did not even see me they took my money and the nurse told me they have to fail me and could not give me my med card and said they would turn it in to casc on my record. I have a Clean License with no probs at all. Some how his just dosent seem Right. What the Heck can I do about this. I see it as they stoled my money. You may have failed due to the fact that you could not perform the duties of a CMV operator, but you should not have been charged with out being seen. Lodge a complaint with the the BBB.
Had a physical done and passed. I have a pain pump implanted for many years. I no longer have needed the pump. No medicine has been in the pump since 2008. This dr wont renew my fed med card!

Had a physical and I’m diabetic. I got a medical card for 3 months doctor passed me on my neck measurement for sleep apea but said my bmi was a little high said I need a sleep evaluation. I answered the survey and my score was a 1 on it which was very low the doctor said. Do I need one and if I don’t can I go to another dot doctor and do another one thanks

Since there were no new rulings or regulations regarding BMI or neck measurements (according to the FMCSA), I would go somewhere else and have another DOT examination done, toward the end of your 3 month card.

You may qualify for an intra-state only card, but I would check with your local DLD to make that final decision. You would not be able to qualify for an interstate CDL/medical certificate.

You are trying to pass an examination that requires you to move around and under a vehicle that you also have to climb into. Does not sound like a good ‘fit for duty’ situation. You would probably have difficulty passing the examination.
I don’t move under a vehicle I pull up at docks and sit the freight at the back of the truck.
I have a medical condition called a drop foot. My right foot, It is only a slight drop I can pick it up a little. This is from an old injury. I can push down with no problem. I have been driving for 24 years with no problems. I do have a brace to hold it up but haven’t used it in many years.I see no safety issues. Should this stop me from getting mt DOT ?

I live in NYS and was recently in an automobile accident. I fractured my left knee and bruised some ribs. While trying to concentrate on recovering just a week into my injuries I get a letter fom NYS DOT saying my DOT physical expired and I no longer can drive commercially. I was disrtacted and let it lasps unknowingly. The good thing is that they said in the letter that if I can get a DOT certificate Faxed into them before the “effective” date of termination i’ll be good. OK great! So I go to my doctors office to get the physical done but he won’t give me a full two years on the certificate because of my leg still healing.He wants to see me back in a couple months so I fax and email in a certificate that only extends me one month past the “effective” date of my CDL termination. Will NYS accept this or am I in trouble of losing my CDL license and job because of this. Didn’t think having this injury would put me at risk in this way, it’s just bad timing and that’s all. If my injury happened in the middle of the two year DOT certificate would any of this be happening??? Probably not i’m guessing. Confused and nervous of losing my job. Thanks

This really is a question for your state Drivers License Department, and not a federal.
And the fact that you could have done this in the middle of your two year would still be a problem. So go to the state DLD office and ask them what you need for them to extend your drivers privileges until you are fully healed.
Doc: I am a forty year old class a local driver. I suffered a stroke at twenty five. Having state approval from neurology consult in 2008,open heart surgery to address ASD closure in September,2000. Recent doc demands that: I have cardiology consult every two years,SPE program for a mildly impaired left hand. The mandates I found about SPE are interstate driving and drivers with missing limbs! The testing parameters used by this occupational health doc are skewed and inaccurately portrayed my abilities. I am in fear of losing my dream job,having worked it two weeks already,as I am not a union member yet. This is a dock worker/driver job-also,I have a permanent fifty pound lifting restriction from my back surgeon.Bilateral wrist fractures and a t-11thru L1 lumbar fusion in December 2009. Look closely at your state information. You will find that you can be a class A driver and drive intra-state only. That would remove a lot of the interstate regulations from the medical examiner.
Just help the examiners with a medical release form completed by your primary doctor and check with the state DLD for the intra-state only regs.

I have been diagnosed with rhumatoid arthritis it effecting my hands mostly at this time. I did not pass DOT because of it. I have since started medicins that are helping an I functioning perfectly now. They say for me to b able get zz CDL back doctor has to sign paper saying my arthritis won’t progress well she won’t sign paper. Because everybody knows it will eventually. Just get a medical release form completed by your primary doctor. No one is going to say the condition will not progress. But, each year and each time you go for the DOT medical examination, you will need to be able to perform the duties of a commercial driver.
If I have a spinal cord stimulator implanted for pain control will this disqualify me from DOT driving? I want to avoid regular use of pain medications. I did a trial implant and it helped control the pain.

Long story short…..fell at work and developed an incisional hernia to a prior aorta bifemoral surgery. Im in a huge struggle with the insurance company. Naturally they want me back to work at all costs, including my health and well being. First hernia surgeon refused to operate due to causing more problems than I already had. Insurance company found a new surgeon that would operate, and he discovered during the surgery that the hernia was much larger than he anticipated and repaired it with mesh. I now have bigger troubles. Ilioguinal and genital femoral neauropathy from the fall, which magnified after the surgery. I am due for my DOT physical in January and wonder if these maladys are disqualifying? I am in alot of pain, but get some relief from using Lyrica and occaisionally hydrocodone(which I know I couldn’t use after going back to work.
I just wonder what to expect at my next physical. Thanks in advance.

Until you are completely healed and ready to go back to work, you are disqualified. Any illness or injury that takes you from behind the wheel requires a new DOT medical examination. So get healed and then see where you are. Your state DLD may want a note, if your driver’s license is up for renewal.

You will probably be limited to intra-state only with a pain pump. But that is not a positive. If you have the pump installed and have worked with it for a while, then you will know what the effects are and your doctor will be monitoring you the whole time. If, after he gets and understands the job requirements of a CDL holder, he feels that you can return to OTR, then get his medical release form completed. Take that with you when you go for your DOT examination and go from there.

California should have some policy in place that allows you to suspend your CDL portion of your license until you are healed. It may require you to re-instate your cdl portion once you have passed the DOT physical examination.
Example: In Utah you can surrender your class A and there is a small fee to re-instate your class A, once you can pass the physical.
So check to see if this is the case in your state.
If my DOT physical was flagged and not flunked am I still able to drive under my old. Card which was not expire until October 4th I was required to get two letters from a doctor which I did and now I have a new card but I’m wondering if my old card was still good as long as I was just flagged and not flunked

It’s no longer an issue. The new medical card has been input to the FMCSA data base, so you’re fine.
I have kidney disease and cant produce enough urine for a specimen on a drug test. Ive been turned down for a job for this although I don’t use drugs. What do I need to do to cleared medically to drive. I got a letter written from my doctor and tried to use that but that did not work. Is it possible to get some type of waiver or a way to take an alternative drug test like saliva,blood or hair?

The medical release form completed by your primary doctor should have been enough. But given your circumstances, you will do better with any other testing procedure. Be aware these test are more expensive and most companies are not willing to spend the money.
This is kind of similar to another question, but a little different. I am going to have surgery on my back one month before my DOT physical needs to be renewed. Is there an extension on the renewal process or will it lapse?

If you let your state DMV know what your circumstances are, there may be a way to grant a little grace period.
Also, even if you went and did the DOT card now, you would have to have a new examination done after healing from the back surgery. Good Luck.

Before you go, get a medical release form completed by your primary doctor, stating that he feels that you are capable of doing the job and you are only driving intra-state. The examiner needs to know this too, so have the medical release in hand when you go for your physical.

I would get a note from your doctor explaining the situation. I don’t believe you will have any major problems. But you will be asked to get a new medical card once you are released to return to work.
i have had multiple back surgeries, i was told by my doctor that having back or neck surgeries would throw up a red flag and i would not be able to pass the dot physical

It isn’t the surgeries that are the main question. It’s what are the limitations and restrictions brought about by the spinal surgeries.
Make sure you bring a note from your doctor stating what your limitations are. The doctor should understand what the job requirements are for a driver. So make sure you find a copy of the job description for the type of work you will be doing. Then let the physician determine what can be done.
What tests or deminations are used to disqualify a DOT driver for limb impariment? Specifically, driver can not lift over 10 pounds.
