For your appointment, please bring:
- A list of medications taken, prescribed and OTC (if any).
- Your state issued driver’s license.
- A copy of your most recent medical examination certificate (Med card).
Notes to Drivers:
Medications: Certain medications you may be taking such as for anxiety, depression, nervousness, or other mental health problems or certain narcotic analgesics or opioid derivative drugs such as methadone or suboxone will require your prescribing doctor to complete FMCSA Form 5895 which is a CMV Driver Medication Form. You will be given this form if appropriate for you on the day of your scheduled DOT Physical. Once we receive this form back from your prescribing doctor, we will be able to make a reasonable assessment as to medical fitness for duty.
CPAP Users: Present guidelines suggest that compliance for
OSA is you be treated for at least 4 hrs. a night for at least 70% of the time (7 out of 10) to meet minimum compliance standards. Your Compliance Report sleep data must go back at least 90 days.

- Perform a thorough physical exam

- Complete the Medical Examination Certificate and DOT physical forms

The new BasicMed program allows most “General Aviation” pilots to obtain a BasicMed exam in lieu of obtaining a 3rd Class Airman Medical Certificate performed by an FAA Aviation Medical Examiner.
More than 54,000 pilots are flying under BasicMed. You can join them.
If you held a regular or special-issuance medical anytime on or after July 15, 2006, you are likely eligible to fly under BasicMed.
