For your appointment, please bring:
- A list of medications taken, prescribed and OTC (if any).
- Your state issued driver’s license.
- A copy of your most recent medical examination certificate (Med card).
Notes to Drivers:
Medications: Certain medications you may be taking such as for anxiety, depression, nervousness, or other mental health problems or certain narcotic analgesics or opioid derivative drugs such as methadone or suboxone will require your prescribing doctor to complete FMCSA Form 5895 which is a CMV Driver Medication Form. You will be given this form if appropriate for you on the day of your scheduled DOT Physical. Once we receive this form back from your prescribing doctor, we will be able to make a reasonable assessment as to medical fitness for duty.
CPAP Users: Present guidelines suggest that compliance for
OSA is you be treated for at least 4 hrs. a night for at least 70% of the time (7 out of 10) to meet minimum compliance standards. Your Compliance Report sleep data must go back at least 90 days.

- Complete the Medical Examination Certificate and DOT physical forms

If you held a regular or special-issuance medical anytime on or after July 15, 2006, you are likely eligible to fly under BasicMed.

If you want a DOT physical done quickly, thoroughly and correctly by someone who can explain every thing he is doing, come to Dr. Russo. Since he communicated clearly what supporting MD documentation I needed to bring with me and since he did the necessary labs in-house, I was in and out in an afternoon. Because of his professional and courteous approach, I am telling my driving friends to come here get their DOT medical card.

Dr. Russo is a wonderful doctor for your DOT physical. He is very patient, he always explains what he is about to do while being very patient with you, he always takes the time to answer all of your questions and explain everything in detail. I have never had such a thorough DOT physical exam. I enjoy my visits to his office. He never keeps you waiting long in his outer office before calling you back. Everyone at the office is very kind and helpful.

i just would like say if you need a dot phy. & want to have a pleasant experience then this is the place for you this is one clean and organized dr.office on top of the staff being courteous. also when you come in for a dot physical the dr. russo doesnt leave you waiting he is ready and gets you on you way. i have no need to go anywhere else.
